Objective To investigate the diagnostic value of Astograph methacholine provocation test in patients with chest tightness variant asthma(CTVA). Methods From January 2011 to February 2017, 156 patients with CTVA in outpatient or inpatient department of respiratory medicine of Kailuan General Hospital affiliated to North China University of Science and Technology were selected as case group (chest tightness variant asthma group). The control group were 361 non-asthmatic patients including interstitial lung disease(23 cases), coronary disease (157 cases), hypertensive cardiopathy(22 cases), myocardiosis (16 cases), congenital heart disease (3 cases), rheumatic valvular heart disease (6 cases), central airway disease (3 cases), thyromegaly (10 cases), mediastinal tumor (5 cases), thoracic or spinal deformity (8 cases), phrenoparalysis (2 cases) and vegetative nerve functional disturbance (106 cases). All participants received pulmonay ventilation test, average daily and nightly variation rate of PEF(Peak expiratory flow) or PEF weekly variability, Astograph methacholine provocation test (forced expirataory volume in one second≥70% expectation), and other relevant examinations.The diagnostic value of Astograph methacholine provocation test on CTVA was assessed by analyzing the sensitivity, specificity, positive predictive value, negative predictive value, and Yunden index of Astograph methacholine airway. Results Compared with the control group((1.18±0.44)%), theforced expiratory flow from 75% of Forced vital capcacity (FEF75)index of CTVA group((1.29±0.50)%) had significant difference(, t=2.96, P=0.006). The sensitivity, specificity, positive predictive value, negative predictive value, Yunden index, and diagnostic accuracy of Astograph methacholine provocation test on CTVA were 0.814, 0.695, 0.536, 0.305, 0.509 and 0.731, respectively. Conclusion The sensitivity, negative predictive value, Yunden index and diagnostic accuracy of Astograph methacholine provocation test on CTVA were higher, whereas the specificity and positive predictive value were relatively lower, suggesting that Astograph methacholine provocation test had a reliable diagnostic value on CTVA, with lower false negative and higher false positive. Clinical treatment of large segmental bone defect has always been a major challenge for the medical community, which is due to the complex and diverse pathogenic factors of large segmental bone defect.In recent years, the clinical treatment of large segmental bone defect has made great progress, and the main treatment options include vascularized or non-vascularized autologous bone grafts, allograft bone transplantation, masquelet technology, llizarov technology and bone tissue engineering.Therefore, understanding the advantages and disadvantages of various treatment options is very important for the clinical treatment of large bone defects in long bones, laying the foundation for clinical treatment. Objective To explore the related factors affecting the quality of life of children undergoing hematopoietic stem cell transplantation. Methods A total of 42 children (transplantation group) who received hematopoietic stem cell transplantation successfully from February 2010 to January 2017 in Beijing Children′s Hospital were investigated with the Chinese version of the Universal Core Questionnaire for Quality of Life for Children 4.0.40 healthy children matched with their age and sex were selected as healthy control group by 1: 1 matching method.To compare the quality of life between the two groups and to analyze the factors influencing the quality of life after hematopoietic stem cell transplantation. Results The age of transplantation group was (10.32±3.94) years, and the average time after transplantation was (2.76±1.15) years.The body weight and school attendance of transplantation group were lower than those of healthy group (P 0.05). Regression analysis showed that chronic graft-versus-host disease was the only factor related to quality of life (P=0.030). Conclusion The quality of life of transplant survivors is similar to that of healthy children of the same age, and the existence of chronic graft-versus-host disease is related to the overall poor quality of life. Objective To explore the efficacy and safety of apatinib combined with S-1 in patients with advanced NSCLC without sensitive gene mutation or unknown mutation status. Methods One hundred and four patients with advanced NSCLC without sensitive gene mutation or unknown mutation status were selected from the oncology department of the First Hospital of Qinhuangdao City, Hebei Province from April 2015 to April 2017.All patients refused intravenous chemotherapy.One hundred and four patients were randomly divided into treatment group (apatinib combined with S-1 group) and control group (S-1 alone group) by 1: 1 digital method.However, two patients in the treatment group transferred to the control group for personal reasons.There is 50 cases in apatinib combined with S-1 group and 54 cases in S-1 group.The efficacy and adverse reactions of the two groups were evaluated. Results The objective remission rate was 48.0% (24/50) and 27.8% (15/54) (χ2=4.530, P=0.033), the disease control rate was 82.0% (41/50) and 74.1% (40/54) (χ2=0.947, P=0.331), the median PFS was 6.6 months and 3.4 months (t=25.555, P=0.000), the median OS was 16.0 months and 10.5 months (t=59.439, P=0.000), respectively.The overall incidence of adverse reactions was 82.0% (41/50) and 70.4% (38/54) respectively (χ2=1.923, P=0.166), of which 18.0% (9/50) and 13.0% (7/54) were more than grade 3 respectively (χ2=0.506, P=0.477). There was no death caused by treatment-related adverse reactions in both groups. Conclusion Appatinib combined with S-1 capsule has good short-term and long-term efficacy in the treatment of advanced non-small cell lung cancer without gene mutation or unknown mutation.The adverse reactions are tolerable and can be used as first-line treatment for patients unwilling to receive intravenous chemotherapy. Objective To explore the correlation between NLR family pyrin domain containing 3 (NLRP3) and nonalcoholic fatty liver disease (NALFD). Methods A cross-sectional study was conducted in this study.Eighty four cases of health examination personnel in Shanghai University of Medicine & Health Sciences Affiliated Zhoupu Hospital were included in the study randomly, and divided into NAFLD group(27cases) and normal control group(57cases) though liver ultrasound examination.Their general data (sex, height, weight), liver function, blood lipid, and blood glucose were analyzed.The level of NLRP3 in peripheral blood was analyzed by ELISA double antibody sandwich method.The relationship between NLRP3 and NAFLD was analyzed by Logistic regression. Results The prevalence of NAFLD was 32.1%(27/84). The levels of serum cholesterol, triglyceride, low density lipoprotein cholesterol, fasting blood sugar, alanine aminotransferase, creatinine and uric acid in NAFLD group were higher than those in normal control group (all P<0.01), while the level of serum high density lipoprotein cholesterol was lower than that in normal control group (P=0.023); the concentration of serum NLRP3 in NAFLD group was (5.1±1.8) μg/L, and that in normal control group was (3.9±1.4) μg/L.There was significant difference between the two groups (t=6.221, P=0.016). Logistic regression analysis showed that NLRP3 was a risk factor for NAFLD (OR=1.537, P=0.021). Conclusion The level of NLRP3 is up-regulated in the serum of NAFLD patients, which is related to the occurrence of NAFLD. Objective To observe the changes of cell cycle, apoptosis and proliferation of endometrial cancer cells after the expression and down-regulation of Ezrin in endometrial cancer cells and to explore whether Ezrin may be a candidate gene for targeted therapy. Methods Endometrial cancer cells were from Shanghai Institute of Cell Research, of Chinese Academy of Medical Sciences in February 2017 and divided into blank control group and siEzrin group according to the intervention methods.Western blot and qRT-PCR was used to detect the expression of Ezrin protein and mRNA in endometrial cell lines.Small interfering RNA(siRNA) was used to transfect HEC-1B cell and down-regulate Ezrin.Cell cycle and apoptosis were detected by flow cytometry.MTT assay was used to detect multiplication. Results Western blot showed that Ezrin protein was expressed in Ishikawa(31.742±5.832)、HEC-1A(16.326±3.135)、HEC-1B(17.636±4.426) and KLE(14.862±5.109) and qRT-PCR showed that mRNA was expressed in Ishikawa (2.513±0.725), HEC-1A (1.655±0.692), HEC-1B (3.237±0.411) and KLE (0.962±0.235) cell lines, and expressed highest in HEC-1B cells (F=6.173, P<0.05; F=7.042, P<0.05). Flow cytometry assay showed that compared with blank control group less cells stayed in G1 phase and G2 phase, more stayed in S phase (t=3.118, P<0.05; t=5.435, P<0.05; t=3.332, P<0.05). The apoptotic rate of HEC-1B cells increased from (9.84±2.37)% to (17.64±5.96)%(t=8.963, P<0.01) after Ezrin was down-regulated.MTT assay showed that the proliferation of HEC-1B cells in 72 h and 96 h siEzrin transfection group was lower than that in blank control group (t=3.209, P<0.05; t=3.726, P<0.05). Conclusion Down-regulating of Ezrin may promote more endometrial cancer cells stay in S phase and promote apoptosis, inhibit proliferation, Ezrin may become target candidate gene in target therapy. 目的观察Ezrin基因在子宫内膜癌细胞中表达及下调后对子宫内膜癌细胞周期、凋亡及增殖的影响,探讨Ezrin基因是否可作为靶向治疗的候选基因。方法2017年2月取自中国医学科学院上海细胞研究所的子宫内膜癌细胞,按干预方法分为空白对照组和siEzrin组。采用Western blot和qRT-PCR检测Ezrin蛋白和mRNA在子宫内膜细胞系中的表达。小干扰RNA(siRNA)转染HEC-1B细胞,下调Ezrin。流式细胞术检测细胞周期和凋亡情况。MTT法检测增殖。结果Western blot结果显示,Ezrin蛋白在Ishikawa(31.742±5.832)、HEC-1A(16.326±3.135)、HEC-1B(17.636±4.426)和KLE(14.862±5.109)细胞中均有表达,qRT-PCR结果显示,mRNA在Ishikawa(2.513±0.725)、HEC-1A(1.655±0.692)、HEC-1B(3.237±0.411)和KLE(0.962±0.235)细胞中均有表达,且在HEC-1B细胞中表达量最高(F=6.173, P<0.05;F = 7.042, P < 0.05)。流式细胞术检测结果显示,与空白对照组相比,G1期和G2期细胞较少,S期细胞较多(t=3.118, P<0.05);t = 5.435, P < 0.05;t = 3.332, P < 0.05)。下调Ezrin后,HEC-1B细胞的凋亡率由(9.84±2.37)%升高至(17.64±5.96)%(t=8.963, P<0.01)。MTT检测显示,转染siEzrin 72 h和96 h后HEC-1B细胞的增殖均低于空白对照组(t=3.209, P<0.05;t = 3.726, P < 0.05)。结论下调Ezrin可使更多的子宫内膜癌细胞停留在S期,促进细胞凋亡,抑制细胞增殖,Ezrin可能成为靶向治疗的靶标候选基因。关键词:子宫内膜癌;Ezrin蛋白;细胞周期;细胞凋亡;扩散

Flow cytometry assay showed that compared with blank control group less cells stayed in G1 phase and G2 phase, more stayed in S phase (t=3.118, P<0.05; t=5.435, P<0.05; t=3.332, P<0.05). Objective To analyze the hemodynamic changes of cerebral arteries in patients with chronic extracranial internal carotid artery occlusion(EICAO). Methods Ninety-six patients with chronic unilateral EICAO who were admitted to Kailuan General Hospital from September 2012 to December 2015 were selected as the case group (EICAO group), and 30 volunteers were selected as the control group.Color transcranial Doppler ultrasonography was used to detect the anterior communicating artery (ACOA) of the grade I collateral circulation, the posterior communicating artery (PCOA), the ocular artery of the grade II collateral circulation, and the pial collateral branch.Circulation rate and cerebral hemodynamic parameters: (Mean blood flow velocity (Vm) and pulsatility index). Results In the left and right EICAO groups, the opening rates of collateral circulation at grade I were 86.96% (40/46), 96.00% (48/50) and 78.26% (36/46) and 88.00% (44/50) respectively.There were significant differences in the opening rates of collateral circulation at grade I between the two groups (χ2=4.114, P=0.043). There was no significant difference in the opening rates of collateral circulation at grade II between the two groups (χ2=3.544, P=0.060). The left and right sides of EICAO group were compared with the same side of control group.The Vm of the common carotid artery (left side of the EICAO group (24.08±9.25) cm/s), left side of the control group (32.52±3.28) cm/s, P<0.01); right side of the EICAO group (22.20±5.51) cm/s), right side of the control group(31.58±3.35) cm/s, P<0.01)) and the end-carotid artery end stage (TICA) The pulsation index (left side of left EICAO group (0.78±0.17), left side of control group (0.92±0.08), P<0.01); right side of right EICAO group(0.75±0.19), right side of control group (0.91±0.10), P<0.01), Vm of middle cerebral artery (MCA) (left side of left EICAO group(40.29 ±20.61) cm/s, left side of control group(55.72 ±5.60) cm/s, right side of EICAO; The right side of group (37.10±19.70) cm/s), the right side of control group (53.70±6.28) cm/s, P<0.01), the pulsation index of MCA (left side of left EICAO group(0.74±0.19), left side of control group(0.87±0.10), P<0.01; right side of right EICAO group(0.69±0.23), right side of control group: (0.90±0.08), P<0.01). There were significant differences.NIHSS score of neurological impairment: normal 17.39% (8/46), mild 39.13% (18/46), moderate 30.44% (14/46), moderate severe 13.04% (6/46) in left EICAO group, and normal 18.00% (9/50), mild 54.00% (27/50), moderate 24.00% (12/50), moderate severe 4.00% (2/50) in right EICAO group.There was no significant difference between the two groups (P=0.178). Conclusion The ipsilateral common carotid artery and its distal middle cerebral artery in patients with chronic unilateral internal carotid artery occlusionshow hypoperfusion of hemodynamics, accompanied by neurological impairment. Objective To investigate the effect of tripterygium glycosides combined with recombinant human tumor necrosis factor receptor Ⅱ antibody fusion protein for injection on serum vascular endothelial growth factor(VEGF), rheumatoid factor(RF), receptor activator of nuclear factor κ B ligand(RANKL) levels in patients with rheumatoid arthritis.To provide reference for rational clinical application. Methods From December 2014 to January 2016, 132 patients with rheumatoid arthritis were divided into observation group and control group by random number table method, , with 66 cases in each group, The control group was treated with tripterygium glycosides alone (10 mg each time, 3 times daily, orally) for 3 months.The observation group was treated with a combination of tripterygium glycosides and recombinant human tumor necrosis factor receptor Ⅱ (0.4 mg/kg, once weekly, hypodermic injection)for 3 months.The clinical efficacy, and serum VEGF, RF and RANKL levels were compared between 2 groups. Results The effective rate of the observation group was 92.4% (61/66), which was significantly higher than that in the control group (80.3%, 53/66). There was a significant difference between the two groups (χ2=4.117, P 0.05). After treatment, the levels of serum VEGF, RF and RANKL in the observation group and the control group were (20.8±11.5) ng/L and (27.3±13.1)ng/L, (258.4±54.5)U/L and (298.1±49.5)U/L, (0.083±0.021) pmol/L and (0.197±0.064), respectively.There were significant differences between the two groups (t=3.029, 4.381, 13.750, all P<0.05). The incidence rate of adverse reactions in the control group was 10.5% (7/66), which was significantly higher than that in the observation group (1.5%, 1/66). There was a significant difference between the two groups(χ2=4.790, P<0.05). The one-year recurrence rate was 25.0% (13/52) in the control group and that was 6.7% (4/60) in the observation group, respectively, and there was a significant difference between the two groups(χ2=7.272, P<0.05). Conclusion Tripterygium glycosidescombined with recombinant human tumor necrosis factor receptor Ⅱ antibody fusion protein for injection is effective in the treatment of rheumatoid arthritis, which reduces the levels of serum VEGF, RF and RANKL, and has a low incidence of adverse reactions and recurrence. Objective To explore relationship between the nature of carotid atherosclerotic plaque and the number and function of endothelial progenitor cells in peripheral blood. Methods A prospective study of 80 carotid atherosclerotic plaque patients were selected from June 2016 to March 2017 in Guangdong Second Provincial General Hospital.All patients were examined with Cranial magnetic resonance imaging or X-ray computed tomography, pathological examination, carotid artery color Doppler ultrasonography.Patients were divided into hard plaque group (n=42) and soft plaque group (n=38) according to the nature of carotid atherosclerotic plaque.Forty healthy subjects were selected as controls.Monocytes were obtained from 10 ml of elbow venous blood by density gradient centrifugation.Adherent cells were cultured and identified by confocal laser microscopy.The number, migration, proliferation and adhesion of EPCs in soft plaque group and hard plaque group were evaluated. Results The number of proliferating cells(0.847±0.037), migrating cells(27.697±8.248) and adherent cells(46.184±7.876) in the normal control group were significantly higher than those in the hard plaque group ((0.647±0.019), (18.643±3.289), (32.165±4.325)) and the soft plaque group ((0.679±0.023), (23.576±6.327), (40.587±6.523))(all P<0.001), while the proliferation, migration and adherent cells in the hard plaque group were lower than those in the soft plaque group (all P<0.001). Conclusion The nature of carotid atherosclerosis plaque is closely related to the number and function of endothelial progenitor cells in peripheral blood.The number of endothelial progenitor cells in carotid atherosclerosis patients with hard plaque is small, and their proliferation, migration and adhesion ability are impaired. Objective To investigate the recovery of knee joint function in patients with recurrent patellar dislocation treated by reconstruction of medial patellofemoral ligament with EndoButton double bundles. Methods A retrospective analysis was performed for 26 patients with recurrent patellar islocation who were treated in The Second Hospital of Tangshan from February 2014 to July 2016, with a total of 26 knees, aged 14-29 years.All patients underwent medial patellofemoral ligament reconstruction with autologous semitendinosus muscle.Double tunnel technique was used in medial patella and single tunnel technique was used in femoral side.Twenty-four patients were followed up for 12 to 24 months.The recovery of knee joint function was evaluated by Kujala score, Lysholm score, patellar fitness angle and patellar tilt angle before and after treatment. Results None of the follow-up patients had dislocation again, and the fear test was negative.The preoperative Kujala score and Lysholm score were (59.67±5.28) and (61.75±5.45) respectively.The corresponding scores at the last follow-up were (87.21±4.21) and (90.13±2.47). There were significant differences between the two scores (t=-19.98, -23.24, all P <0.01). The patellar fit angle changed from (23.17±3.63)degrees before operation to (14.17±2.01)degrees after operation.There was significant difference between the two scores.(t=10.62, P<0.01); Patellar tilt angle changed from preoperative (18.33±3.92)degree to postoperative (2.45±1.10)degree, the difference was significant (t=19.10, <0.01). Conclusion EndoButton double-bundle reconstruction of medial patellofemoral ligament is effective in the treatment of patellar dislocation with satisfactory recovery of knee joint function. There were significant differences between the two scores (t=-19.98, -23.24, all P <0.01).
