Photoelastic Stress Analysis for Cobalt Chromium and Nylon Denture ‎Base on Mandibular Kennedy Class I Implant Supported Removable ‎Partial Overdenture 基于下颌Kennedy I类种植支撑可拆卸部分覆盖义齿的钴铬尼龙义齿的光弹性应力分析

Objective: The purpose of this study was to evaluate stresses induced on implant supported cobalt chromium and nylon overdenture bases in free end saddles cases using photoelastic stress analysis. Materials and Methods: Ready-made mandibular stone cast fabricated according to Kennedy Class I to replace 2 nd premolar, 1 st and 2 nd molars. Implant analogue was inserted at site of first molar by using milling machine. Closed tray silicone impression was taken to the stone cast after transfer coping insertion. The transfer coping was inserted on its place in the impression. The implant was placed on its position in the impression. The impression was poured with epoxy resin material. Metallic and nylon denture bases were constructed with same design. Forces of 20, 40, 60, 80, 100 (Newton) were applied to detect photoelastic stresses. Results: With application of loads 80 and 100 N, the nylon framework recorded higher stress values than the vitalium framework at apical, middle and cervical area bilaterally. Conclusion: Cobalt chromium denture base showed better stress distribution compared to nylon denture base.

Three-dimensional locking plate and conventional mini plates in the treatment of mandibular anterior fractures 三维锁定钢板与常规微型钢板治疗下颌前路骨折

Objective: This study was designed to evaluate the efficacy of 2-mm 3D locking miniplate in the management of anterior mandibular fracture and to compare it with Champy’s miniplate. Subjects and methods: prospective, randomized, clinical trial was done on 30 patients complaining of mandibular anterior fractures They were divided into 2 equal groups. Group A: treated with 2mm 3D locking miniplates . Group B: treated with two conventional miniplates using Champy’s principles of osteosynthesis. Postoperative follow up was done at 1 week, 4 weeks, 3 months. The patients were evaluated according to the outcomes of the study, that was working time, wound dehiscence, infection, segmental mobility, postoperative occlusion, need for postoperative IMF, need for plate removal, and radiological evaluation of reduction and fixation. Results: The average operating time required for group (A) was approximately 8.94 min less than that required for group (B). The postoperative radiographs showed excellent reduction in all cases, with alignment of the osseous borders of the mandible. There was no incidence of wound dehiscence, tooth damage, segmental mobility and nerve damage in either group. There was statistically non-significant difference between two groups regarding occlusion; occlusion was satisfactory in most cases of both groups. Conclusion: There was no major difference in terms of treatment outcome and both the systems are equally effective in anterior mandibular fracture treatment and provide sufficient rigidity often avoiding/decreasing the need for IMF.

The Clinical Use of Autologous Bone Marrow Mesenchymal Stem Cells in the Treatment of Secondary Alveolar Cleft 自体骨髓间充质干细胞治疗继发性肺泡裂的临床应用

Objective: To compare the clinical outcome of alveolar cleft bone grafting with expanded mesenchymal stem cells (MSCs) seeded into a resorbable matrix to standard iliac cancellous bone graft. Subjects and methods: Fifteen patients with unilateral alveolar cleft aged 7-12 years old (eight girls and seven boys) were randomly divided, according to grafting technique, into two groups: Group I: Alveolar cleft grafting with expanded autogenous bone marrow stem cells seeded in collagen sponge, platelet rich plasma and nanohydroxyapatite. Group II: Alveolar cleft grafting with cancellous bone harvested from anterior iliac crest. Follow up clinical evaluation on the first postoperative day, every other day for the first week, weekly thereafter for a month then every month for six months; concerning postoperative pain, soft tissue healing, oronasal fistula closure and tooth eruption. Results: All patients in group I experienced no pain or discomfort at the bone marrow aspiration site while patients in group II experienced pain scored 3 by VAS and disappeared gradually within a week. Wound dehiscence represented 14% of group II. After six months postoperatively eruption of lateral incisor was observed in 25% in group I versus 14% in group II. Conclusion: The effect of stem cell tissue engineering did prove to have a positive clinical result compared to gold standard used in alveolar cleft grafting and provided significant support to the healing of soft tissues around the alveolar cleft. Tissue engineering bone graft is a cost effective and require two setting comparable to the autogenous bone graft.

Retention and stress distribution induced in mandibular implant-retained complete overdenture with different interimplant distance (An In-Vitro study). 不同种植间距对下颌种植-保留全覆盖义齿固位和应力分布的影响(一项体外研究)。

Objective: The loss of retention or adjustment of overdentures was the most commonly reported complication type in the dental office. So, this study was aimed to evaluate the retention and stress distribution induced in implant-retained overdenture with different inter- implant distance in vitro study. Materials and Methods: Six implant-retained overdenture models were fabricated on acrylic models with two screwed implants at three-different interimplant distances (19 mm, 23 mm, and 29 mm) to measure the overdenture retention using digital force gauge as well as to the stress distribution around the dental implant using strain gauge with unilateral load application at the second premolar, first molar, and the second molar sites. Results: The results of the retention test, revealed no statistically significant difference among the tested groups. While the strain results showed a statistically significant difference between all tested groups. Conclusion: The smaller interimplant distance could improve the retention of the overdenture as well as it could allow better stress distribution around the dental implants.

Prevalence of Dental Caries Among School-Aged Children with Cardiac Diseases: ICDAS and Microbiological Parameters 患有心脏病的学龄儿童龋齿患病率:ICDAS和微生物参数

Objective: Prevalence of tooth decay among school-aged children especially with the medically compromised one makes it one of the major public health. Therefore, this study was directed to detect the prevalence of carious lesions among children and young adults with cardiac diseases with newly developed International Caries Detection and Assessment System (ICDAS). Subjects and methods: Egyptian children and young adults aged between 5-16 years were chosen for this study. A total of four hundred participants were selected according to previously designed criteria’s and then divided into the control healthy group and cardiac disease group. Results: The results of the ICDAS assessment exhibited a statistically higher caries prevalence among participants with cardiac diseases with statistically higher dentine defects. However, the results of the microbiological analysis showed a statistically significant increase in the Streptococcus mutants and Lactobacilli bacterial count in subjects with cardiac diseases. Conclusion: school-aged children with cardiac diseases showed a higher prevalence in tooth decay and Streptococcus mutants and Lactobacilli count when compared to the control group.

Evaluation of postoperative pain after using XP-endo shaper and F-One blue files XP-endo塑形器与F-One蓝色锉术后疼痛的评价

Objectives: This study was aimed to evaluate the effect of XP-endo shaper file and F-One blue files on the degree of the experienced postoperative pain. Subjects and Methods: Twenty male patients complaining of acute pulpitis without apical periodontitis related to the mandibular molars were divided into 2 groups: group 1: treated with XP-endo shaper and group 2: treated with F-One file. The postoperative pain scores were recorded at 6,24,48,72h and 7days using the modified verbal descriptor scale. Mann-Whitney U test was used to compare between the two groups. Dunn’s test was used for pair-wise comparisons when Friedman’s test is significant. The significance level was set at P ≤ 0.05. Results: There was no statistically significant difference between the two groups at different time intervals except at 48h in which the F-One showed statistically significant lower mean pain scores than the XP-endo shaper file. Conclusions: Both XP-endo shaper and F-One blue files showed nearly the same amount of postoperative pain over time.

Minimal invasive surgery (MIS) with and without advanced platelet-rich fibrin (PRF) in treatment of localized periodontitis stage III grade B 微创手术(MIS)加或不加晚期富血小板纤维蛋白(PRF)治疗局限性III期B级牙周炎

Objectives : The objective of this study was to evaluate and compare between minimal invasive surgical treatment (MIST) with and without platelet-rich fibrin (PRF) in patient with localized periodontitis stage III grade B. Subjects and methods : A total of twenty two patients were 10 females and 12 male in this study. The study was designed as Randomized, controlled, parallel, single- blinded clinical trial. The patients were divided into two groups using split mouth technique. The study protocol involved a screening appointment, initial phase therapy, surgical therapy, all patients followed up for 6 months. The defect-associated interdental papilla was surgically approached with modified papilla preservation technique MPPT Cortellini et al. 1995. Results : The results of the present study showed similar reductions in PPD at the deepest site in both groups after 6 months of follow-up (PPD, 3.18±1.12 mm in the MIS without PRF group and 3.23±0.61 mm in the M-MIST with PRF group), for CAL gain a higher mean value was recorded in the MIS with PRF group, with a mean 2.13±0.77, in comparison to 1.90±0.94 in control group (MIS without PRF), with no significant difference. Conclusions : The platelet-rich fibrin when combined with minimally invasive surgery, produces better outcomes compared to the open flap debridement alone.

Radiographic Volumetric assessment of secondary alveolar cleft grafting using stem cells tissue-engineering 利用干细胞组织工程技术进行继发性牙槽裂移植的放射学体积评估

Objective: To compare the radiographic outcome of alveolar cleft bone grafting with expanded mesenchymal stem cells (MSCs) seeded into a resorbable matrix to standard iliac cancellous bone graft. Subjects and methods: Fifteen patients with unilateral alveolar cleft aged 7-12 years old were randomly divided into two groups: Group I: Alveolar cleft grafting with expanded autogenous bone marrow stem cells seeded in collagen sponge, platelet rich plasma and nanohydroxyapatite. Group II: Alveolar cleft grafting with cancellous bone harvested from anterior iliac crest. Ideal cleft volume was calculated from cone beam CT with computer aided engineering. Follow up evaluation with cone beam CT and Bone fill was calculated on three and six months postoperatively. Results: The mean ideal cleft volume was 678 mm3 and 726 mm3 in group I and group II respectively with no significant difference. At three months postoperatively, there was a Significantly difference in the mean volume of bone fill (BF) between group I and group II (7% BF and 47% BF respectively). Furthermore, the difference increased significantly at six months to become 10% BF in group I and 60% BF in group II. Conclusion: The hybridized scaffold used in the study for seeding of stem cells that contains collagen sponge, PRP and nanohydroxyapatite still needs improvement in mechanical and biodegradation properties to support bone tissue engineering. Moreover, the radiographic volumetric analysis of cancellous bone graft which is the gold standard revealed suboptimal results that needs further researches to improve the outcome.
