Welcome to the first issue of volume 15 of the Ibnosina Journal of Medicine and Biomedical Sciences. The issue has a variety of articles, including a review, three original articles, and two case reports. The narrative reviewon theburden and patterns ofmedical emergencies during Ramadan fasting is the most comprehensive review of the subject hitherto. It addressed all acute presentations of any medical conditions studied during Ramadan fasting. Thismay reflect thewidely variable geographical and timeconditionsover theyears. Theauthors called formore comprehensive studies to consider that the interplay between fasting and feasting with common medical conditions in any emergency encounter during Ramadan fasting. The four original articles are mostly clinically based research. Knowledge, attitude, and the use of complementary and alternative medicine among final-year pharmacy and medical students in Benghazi, Libya,were reported by Seham Shaboun. Rehab Jerbi reported nurses’ compliancewith hand hygieneguidelinesatTripoliUniversityHospital.MasoudAlam and colleagues described the knowledge of sick-day rules among patients on long-term glucocorticoids and healthcare professionals in a UK district general hospital. The relationship between age and pulp cavity size using conebeam computed tomography was reported by Hurria Zahid. One case report is included in the issue. Shatnawi et al’s case report presents the overlap of juvenile dermatomyositis and diffuse cutaneous systemic sclerosis by Shatnawi et al. The report gives a focused discussion of the clinical relevance of the cases. The flow of articles is still noticeably below our expectations; however, we remain committed to the cause of advancing scholarly publishing and genuine open-access journalism in developing regions. This year we are hopeful to move forward on the indexation path supported by our publisher. We wish you a happy and prosperous year for 2023.

Burden and Patterns of Medical Emergencies during Ramadan Fasting: A Narrative Review 斋月期间医疗紧急情况的负担和模式:叙述回顾

Abstract Introduction  Acute medical problems may occur more frequently during Ramadan fasting (RF). We aimed to provide a narrative overview of the global literature on medical emergencies during RF. Methods  This is a nonsystematic review of the international literature from one major medical online database (PubMed, National Institutes of Health, United States). The relevant literature was narrated in a concise thematic account. Results  There is a variable impact in the burden and time distribution of emergency services and hospitalization during RF that may require readjustment of resource allocation. Studies of the risk of accidents and injuries may be increased overall or at specified times, around Iftar time. A classical impact of emergencies has been the risk of worsening peptic ulcer disease. RF impacts diabetic emergencies such as severe hypoglycemia, hyperglycemia, and diabetic ketoacidosis, particularly in type 1 diabetes and poorly controlled patients. Glucocorticoid replacement therapy may represent a challenge to patients and physicians that require education and dose adjustment. Acute neurological conditions of interest include the “first day of Ramadan headache,” epilepsy, and strokes. Several studies evaluated the risk of the acute coronary syndrome and heart failure with inconsistent findings. RF may impact the renal and urological systems through stone disease, renal colic, and acute kidney. The impact on hematological conditions was mostly focused on the safety of anticoagulant therapy during Ramadan. Conclusion  The review addresses the emergency medical encounters of the fasting patient during Ramadan to allow a holistic and ethnically sensitive approach to medical care under circumstances where decisions have to be taken with no delay.

Juvenile Dermatomyositis and Diffuse Cutaneous Systemic Sclerosis Overlap 青少年皮肌炎和弥漫性皮肤系统性硬化症重叠

Abstract The overlapping nature of autoimmune diseases makes diagnosing and stratifying prognosis extremely difficult. Scleromyositis, the most common overlap syndrome, is typically seen in adults and is rarely seen in children. An overlap syndrome like scleromyositis would have clinical features of at least two connective tissue diseases (juvenile dermatomyositis and systemic sclerosis). Furthermore, the presence of anti-PM/Scl antibodies is critical. We describe a patient who presented with widespread skin tightening, hoarseness of voice, dysphagia, and muscle weakness that had been present for 6 months. The patient was diagnosed with overlap juvenile scleromyositis (scleroderma-dermatomyositis overlap). In practice, distinguishing this syndrome from dermatomyositis and scleroderma is critical.

Tissue Damage Caused by Diclofenac Intramuscular Injection: The Nicolau Syndrome 肌内注射双氯芬酸引起的组织损伤:Nicolau综合征

Abstract Introduction  Nicolau syndrome is an extremely rare complication following intramuscular administration of drugs leading to tissue damage and necrosis. Case Report  In this case, the patient received the injection from an untrained quack surgeon ( Jarrah ) due to an improper intramuscular injection technique. A 45-year-old female complained of pain and blackish discoloration of the skin over the left buttock after receiving an intramuscular diclofenac injection. 4 The patient was managed by administering intravenous antibiotics and analgesics, followed by surgical debridement. Flap reconstruction was done after granulation tissue formation. Conclusion  Healthcare should be sought from appropriately trained and licensed professionals. Patients should be well educated on this. Pain, erythema, and discoloration should be recognized as red flags of the Nicolau syndrome so that it can be addressed promptly to avoid complications.

Large Carotid Aneurysm: An Illustrative Case of Open Surgical Repair 大颈动脉瘤:开放性手术修复一例

Abstract Background  Extracranial carotid artery aneurysms are rare and are often localized at the level of the carotid bulb or in the distal internal carotid artery. Generally asymptomatic until an embolic event or a local compression reveals their presence, the rupture is rare. Case Report  We presented a case of a 68-year-old female referred to our hospital for a pulsatile mass on the right side of her neck with no other symptoms. The patient had a history of hypertension that was medically controlled. The duplex ultrasound confirmed the presence of a large internal carotid aneurysm. Angiotomography shows a large 5 cm diameter type 1 aneurysm of the right internal carotid artery and a type 1 2.7 cm diameter of the left carotid artery. Open surgery with aneurysmal resection and direct end-to-end anastomosis was performed with an uneventful course. Conclusion  This is an illustrative case of managing large carotid aneurysms by open surgery.

A Preliminary Study on the Frequency of Influenza Infections during the Early 2022 Amid COVID-19 Epidemic in Libya 利比亚新冠肺炎疫情期间2022年初流感感染频率的初步研究

Abstract Background  At the time of conducting this study, we were at the peak of the influenza season, and influenza vaccinations were not readily accessible throughout the country. Thus, predisposing many high-risk individuals to influenza infections in a time when coronavirus disease 2019 (COVID-19) was also highly circulating, and the emerging Omicron variant of concern was peaking in many countries worldwide. Materials and Methods  We conducted a brief survey to prospectively estimate the frequency of influenza A and B and severe acute respiratory syndrome coronavirus 2 (SARS-COV-2) in samples received at our laboratories at Libyan Biotechnology Research Center, Tripoli, Libya, between December 1, 2021, and January 31, 2022, for patients complaining of respiratory symptoms using a multiplex reverse transcription-polymerase chain reaction test for SARS-COV-2, influenza A and B, and RSV. Results  We analyzed nasopharyngeal swabs in viral transport media from 2,186 samples. About 27% (589/2186) of study patients tested positive for SARS-COV-2, 2.8% (61/2186) were positive for influenza A virus, 0.18% (4/2186) for influenza B virus, and 1.4% (31/2186) tested positive for RSV. Conclusions  These results revealed that along with COVID-19, influenza infections were also rising. As the COVID-19 pandemic continues, the most significant concern is the development of an influenza outbreak in the upcoming months. Therefore, continuing annual influenza vaccination is critical to increasing population immunity. National influenza surveillance and testing should also be conducted. Furthermore, sequencing and antigenic characterization should be performed regularly. There is a need for continuous monitoring in national laboratories to detect any zoonotic cases and substantial viral evolution.

Thus, predisposing many high-risk individuals to influenza infections in a time when coronavirus disease 2019 (COVID-19) was also highly circulating, and the emerging Omicron variant of concern was peaking in many countries worldwide. Materials and Methods  We conducted a brief survey to prospectively estimate the frequency of influenza A and B and severe acute respiratory syndrome coronavirus 2 (SARS-COV-2) in samples received at our laboratories at Libyan Biotechnology Research Center, Tripoli, Libya, between December 1, 2021, and January 31, 2022, for patients complaining of respiratory symptoms using a multiplex reverse transcription-polymerase chain reaction test for SARS-COV-2, influenza A and B, and RSV. Results  We analyzed nasopharyngeal swabs in viral transport media from 2,186 samples. About 27% (589/2186) of study patients tested positive for SARS-COV-2, 2.8% (61/2186) were positive for influenza A virus, 0.18% (4/2186) for influenza B virus, and 1.4% (31/2186) tested positive for RSV. Conclusions  These results revealed that along with COVID-19, influenza infections were also rising. As the COVID-19 pandemic continues, the most significant concern is the development of an influenza outbreak in the upcoming months. Therefore, continuing annual influenza vaccination is critical to increasing population immunity. National influenza surveillance and testing should also be conducted. Luffa cylindrica (Linn. M. J. Roem) Reduces Oxidative Stress In Vivo in Plasmodium berghei -Infected Albino Mice 丝瓜(Linn.M.J.Roem)在体内降低伯氏疟原虫感染的白化小鼠的氧化应激

Abstract Background  Malaria is endemic in sub-Saharan Africa, and oxidative stress has been implicated in malaria disease. Luffa cylindrica is an ethnomedicinal plant used to treat various diseases, including malaria. The oxidative stress-reducing potential of L. cylindrica in malaria-disease state of Plasmodium berghei NK-65 parasite-infected mice was carried out in vivo . Materials and Methods  Mice were infected with P. berghei NK-65, and the effect of administration of methanolic leaves extract (100, 200, and 400 mg/kg b.w) of L. cylindrica on percentage parasitemia in blood smear, antioxidant enzymes (catalase CAT, superoxide dismutase SOD, glutathione-s-transferase GST), non-enzymatic antioxidant (reduced glutathione GSH) and malondialdehyde concentration in tissues (plasma, liver, kidneys, and spleen) of mice was investigated and compared to chloroquine and artesunate as reference antimalarial drugs. Phytochemical constituents of the extract were determined by standard methods. Results  Saponins, tannins, terpenes, phenolics, flavonoids, alkaloids, and glycosides were the phytochemical constituents identified in the extract. The extract at three doses (100, 200, and 400 mg/kg b.w.) investigated caused a significant reduction ( p  < 0.05) of parasite growth with over 90% reduction in parasitemia level in mice infected with the parasite. The extract also ameliorated oxidative stress in mice by significantly ( p  < 0.05) increasing the activities of CAT, SOD, and GST in the studied tissues of mice. The level of malondialdehyde, a marker of oxidative stress in mice, was also significantly ( p  < 0.05) reduced by the extract. The results were comparable with chloroquine- and artesunate-treated groups. Conclusion  The study concludes that L. cylindrica is an effective therapy for treating malaria and for the management of its oxidative stress-related complications due to its antioxidant properties.

Effect of Solvent Extracts of Tephrosia vogelii Leaves and Stem on Lipid Profile of Poloxamer 407-Induced Hyperlipidemic Rats 五倍子叶和茎的溶剂提取物对泊洛沙姆407诱导的高脂血症大鼠脂质分布的影响

Abstract Introduction  There are claims by traditional medicine practitioners in Nigeria and valuable scientific reports that the leaves of Tephrosia vogelii (TV) hook are used for the treatment of various diseases; however, there is paucity of information on it used in the management of cardiovascular complications despite the rich presence of phytochemicals. This study aimed at investigating effects of solvents extracts of TV leaves and stem on lipid profile of poloxamer 407-induced hyperlipidemic rats. Materials and Methods  Varying doses of the solvent extracts (water, ethanol, and acetic acid) of plant material were administered to experimental animals (Albino Wistar rats) induced with hyperlipidemia using poloxamer 407 (P-407). Results  The results of the phytochemical screening of leaves and stem revealed the presence of alkaloids, saponins, phlobatannins, and flavonoids in powdered sample of leaves and stem of TV. Aqueous extract of the leaves had the highest yield (18.21 ± 1.12%), while acetic acid extract of stem had the lowest yield (7.21 ± 1.21%). The cholesterol and triacylglycerol level of rats induced with P-407 was significantly ( p ≤0.05) higher than normal rats. This study showed that aqueous extract at 50mg/kg body weight significantly (p≤0.05) lower cholesterol, low-density lipoprotein, very low-density lipoprotein, and triacylglycerol levels. Indices of cardiovascular function atherogenic coefficient, atherogenic index, coronary risk index, and cardioprotective index were positively modulated by the treatment. Conclusion  The study indicated that the aqueous leaves extract of TV possesses antihyperlipidemic effects and may explains why it has been found to be useful in the management of cardiovascular diseases by traditional medicine practitioners.
