We, hereby, report a case of pulmonary paragonimiasis in a patient from an endemic region who was misdiagnosed with pulmonary tuberculosis as the clinical features of both diseases are the same. The subtle clue such as eosinophilia, food habits, and travel history can help us diagnose parasitic infections. Pulmonary paragonimiasis with coughing up rusty brown sputum is the most common presentation of the disease. Praziquantel is mainstay of treatment. Highlight: This case report is to draw the attention of the medical fraternity to the diagnosis of paragonimiasis which is endemic in Asian, African, and South American countries, whose diagnosis is often delayed or missed.

Mycetoma is a chronic granulomatous inflammation of the dermis and epidermis caused by filamentous bacteria in 60% of cases (actinomycetoma) or fungi (eumycetoma) in 40% of cases. A 45-year-old female presented with a left foot ulcer and gradually increasing swelling for 13 years in our department. The radiograph revealed multiple punch-out lytic areas in the tarsals with sclerosis and destruction of the calcaneum. The characteristic “dotin-circle” sign was appreciated on both ultrasound and magnetic resonance imaging. The causative organism and the radiological evaluation, especially bone involvement, are the most important factors that determine the choice of treatment. In the initial stages, chemotherapeutic drugs along with surgery are effective. The current concept is with treatment with a combination of drugs under the name of Welsh regimen.

Older people commonly experience life stressors such as reduced mobility, chronic pain, and fatigue. This leads to loneliness, isolation, and psychological distress among them, putting them at risk of developing mental and psychological disorders such as depression, dementia and anxiety. In older people, total knee arthroplasty (TKA) is one of the most common orthopedic surgeries undertaken. For physical therapeutic rehabilitation, home exercise programs (HEPs) are prescribed post-surgery. These programs require a lot of self-motivation which is reduced in those with mental illnesses. The objectives of this study were to assess the psychological distress levels in patients, to assess their HEP adherence, and, lastly, to study whether there is a correlation between the patient’s psychological distress levels and their HEP adherence. Forty-three subjects who underwent TKA were recruited during their post-operative hospital stay. Their demographic data and date of surgery were collected. At 1 month postoperatively, they were sent a Google form comprising two questionnaires-subsection B of the exercise adherence rating scale and patient health questionnaire-4 to measure adherence to HEP and psychological distress levels. Obtained scores were correlated using Spearman’s test. The confidence interval and P value were set at 95% and <0.05, respectively. The mean age of the 43 subjects was 62.58 ± 9.15. Spearman’s correlation coefficient (r) was −0.47, that is, a moderate negative correlation was found and the P value obtained was 0.02 which is statistically significant. Psychological distress levels have a negative correlation with HEP adherence in patients with TKA.

Because of ischemic necrosis following postpartum hemorrhage, patients with Sheehan’s Syndrome may exhibit different degrees of anterior pituitary insufficiency. They can report a variety of obscuring clinical signs and symptoms even many years after the postpartum hemorrhage, which could lead to a delay in diagnosis and an increase in mortality and morbidity. We report a case of a 44-year-old G3P3L3 lady who presented with this syndrome with history of postpartum hemorrhage 5 years back during the birth of her last child.

It is crucial to identify diabetes risk factors and screen young people for the disease to stop diabetes from developing. An effective and validated approach to assessing population diabetes risk is the Indian diabetes risk score (IDRS). Diabetic women are more vulnerable to many unfavorable outcomes. The objective of this study was to determine the risk of type 2 diabetes mellitus (T2DM) among females aged 30 years and more using IDRS. A cross-sectional study was conducted among 626 self-declared non-diabetic females from July 2022 to January 2023 using a semi-structured interview schedule. IDRS was used to assess diabetes risk. IDRS categorization revealed 15.8%, 44.6%, and 39.6% participants in low-, moderate-, and high-risk categories, respectively. Sensitivity and specificity were 67.5 (60.6–74.4) and 41.6 (34.3–48.9), respectively, compared to the gold standard test (Fasting blood sugar). At a 95% confidence interval, the area under the curve of receiver operating characteristic was found to be 0.6 (0.47–0.68). Nearly two-fifths (39.6%) of the participants had a high risk of getting T2DM. Increments in age, family history of diabetes, lack of physical activity, and abdominal obesity were the most frequent factors associated with a high risk of developing T2DM.

Increasing diversity is essential not only for equal healthcare among all individuals but also for equal socio-economic potential for all genders and ethnicities. With changing demographics over the years, we tried to assess the impact of university programs for the promotion of equality in ethnic representation in the medical community. The US population, based on the data taken from the US Census Bureau, was compared to the Faculty Administrative Management Online User System (FAMOUS) data to analyze the representation of the medical community to the general US population. Females have reached near parity in their representation within the medical community faculty. Underrepresented in medicine individuals have not come far enough to reach parity in their representation within the medical community. As compared to the general US population, while Asians are overrepresented, Black and Hispanic proportions in the medical community are still underrepresented.

Trismus is a painful condition that restricts the temporomandibular joint range of motion (ROM) causing difficulty in eating, chewing, speaking, etc., affecting the oral health-related quality of life (OHRQOL). It often presents as a complication during pre-malignancy stage or after oral cancer treatment. Its prevalence is between 28% and 79%, and it is 8 times more common in edentulous patients. The 57-year-old male complained of continuous tongue and jaw pain with difficulty in chewing, eating, and speaking 1 year before and gradually developed small ulcers over the right jaw. After complete investigation, the patient was diagnosed with jaw cancer and underwent radiotherapy for 2 months. This patient came with complaints of pain, difficulty in mouth opening to physiotherapy department, 3 months following radiotherapy, diagnosed as a case of trismus. A comprehensive physiotherapy protocol including LASER, mobilization, Myofascial release, active exercises for the period of 4 weeks was given. Outcomes for pain, jaw mobility, and OHRQOL were evaluated at baseline and 4 weeks. Four weeks of comprehensive physiotherapy protocol was found effective in reducing pain and improving jaw mobility and OHRQOL in patients with trismus following radiation therapy.
