Bupropion is an oral antidepressant which acts by inhibiting the reuptake of dopamine and noradrenaline from synapse. Apart from treatment of depressive disorders, it is used off-label for smoking cessation, treatment of attention-decit hyperactivity disorder and obesity. Bupropion is generally well-tolerated and has a favorable safety prole. Till date there have only been few case reports of allergic skin reactions associated with bupropion. Apart from being a rare adverse reaction, the unique feature of delayed onset of allergic reaction may lead to underreporting of the phenomenon. Here we present a case report of a 30 years old male who presented with generalized urticaria after twenty-three days of bupropion initiation. This report serves to promote awareness among the clinicians about the association of delayed-onset allergic phenomenon with use of Bupropion.

Hand stitching, a craft that was practiced for constructing garments in ancient times has lost its demand due to the advent of machines. More and more people get engaged in the use of machines rendering hand-stitching obsolete, as it is strenuous and time-consuming compared to machine stitching. The handcrafted garments do hold a good potential with the use of the right techniques to assemble and embellish them. This research was taken up to examine the fundamental qualities of hand stitches and their use for creative clothing seams, surface ornamentation, and decorative elements. Running stitches were explored at length for variation to be used for both decoration as surface ornamentation and seam construction. The researcher also trained the women without formal education in three sessions so that they could convert the skill for economic independence. The results were so encouraging that self-help groups for women were established where they learned the variety of basic hand stitches for ornamental purposes, which added to the value of the garments. The reversible elements that can be worn on either side of the garment were designed. As the extension of the work, designs (motifs) were prepared by the researcher and developed by the trainees, which were prepared to keep in mind the commercial market.

Introduction: Sickle-cell anemia (SCA), a severe form of sickle-cell disease, is a global hemoglobinopathy with a signicant on affected individuals. The disease is characterized by a single point mutation in the β-globin gene, leading to the synthesis of unstable sickle haemoglobin (HbS). This results in HbS polymerization, oxidative stress, inammation, and endothelial activation. Despite advancements in treatment strategies, mortality remains high in this population. Cardiac complications, although recognized, are not well-explored. This study aims to investigate the clinical prole of children with SCA and assess cardiac function using 2D echocardiography. Aims & Objectives: 1.To assess clinical prole of children with sickle cell anemia 2.Evaluation of cardiac function in them using 2D Echocardiography. Materials and Methods: This prospective observational study was conducted at a Government General Hospital, enrolling children aged 5 to 12 years with SCA.Detailed demographic information, family history, and clinical data were collected. All enrolled children underwent investigations, including complete blood count (CBC) and 2D Echocardiography was performed by a cardiologist. Data was analysed with appropriate statistical methods. Results: The study included 100 children with SCA, with a mean age of 8.7 years. Clinical presentations revealed a prevalence of vascular occlusion (73%), splenomegaly (14%), infections (7%), and hepatomegaly (6%). Electrocardiographic parameters indicated sinus bradycardia (24%), left atrial enlargement (19%) and diastolic dysfunction (84%). Mean values of echocardiographic parameters were also reported. Hydroxyurea therapy showed statically signicant improvement in haemoglobin levels, number of blood transfusions, hospital admissions and other clinical parameters. This study provides valuable insights into the clinical prole especially cardiac abnor Conclusions: malities among children with SCA.All SCA childrens should be routinely screened for cardiac abnormalities and counselled appropriately. The clinical benits of hydroxyurea therapy highlights the importance of early initiation of this drug in all childrens with SCA.

Patients with rheumatic heart disease (RHD) undergoing non-cardiac surgeries present a challenge for anesthesiologists. This case report discusses the anesthetic management of a 56-year-old female with severe mitral stenosis, posted for acetabulum fracture xation. The patient's history revealed RHD with severe mitral stenosis, mild mitral regurgitation, and severe pulmonary hypertension. Considering the risks associated with neuraxial anesthesia in mitral stenosis, general anesthesia (GA) with a Pericapsular Nerve Group (PENG) block was chosen. The patient was carefully premedicated, induced with appropriate drugs, and monitored intraoperatively. The PENG block, performed under ultrasound guidance, aimed to reduce hip pain without causing motor blockade. The patient remained hemodynamically stable throughout the surgery, and postoperatively, the combination of GA with PENG block provided effective pain control, reducing the need for opioids. This approach contributed to an early recovery, emphasizing the importance of tailored anesthesia strategies for valvular heart disease patients undergoing non-cardiac surgeries.

Introduction- Recently, a novel coronavirus (2019-nCoV), ofcially known as severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), caused a severe pandemic infection worldwide with considerable morbidity and mortality. A relatively high mortality of severe coronavirus disease 2019 (COVID-19) is worrying, and the application of heparin in COVID-19 has been recommended by some expert consensus because of the risk of disseminated intravascular coagulation and venous thromboembolism. However, its efcacy remains to be validated. We aim to Correlate the association between prophylactic and therapeutic dosages of low molecular weight heparin (LMWH) on mortality and other morbidity outcomes in critically ill patients of COVID-19 . Consecutive Methodspatients admitted to Dr. S. C.GMC Medical college and hospital Nanded with laboratory-conrmed COVID-19 were included in this retrospective observational study. The diagnosis of COVID-19 was established according to World Health Organization interim guidance and conrmed by RNA detection of the SARS-CoV-2 in the microbiology laboratory of the hospital. Outcomes between COVID-19 patients treated with standard prophylactic LMWH enoxaparin dosage (subcutaneous enoxaparin 40–60 mg daily) or Therapeutic LMWH enoxaparin dosage (subcutaneous enoxaparin 40–60 mg twice daily) for 7 days were correlated. Out of 100 patients 50 re Results - ccived P-LMWH & 50 reccived T-LMWH among that 12 % patients expired in prophylactic group & 22% in therapeutic group. It was observed that the levels of D-dimer before & after of P-LMWH group were (D-dimer: 2.63±1.77, 0.63±1.19) and those of T-LMWH group were(D-dimer: 3.14 ± 2.15, 1.07 ±1.56 ). the length of hospital stay was (11 ± 2.21) days in the P-LMWH group and (11 ± 3.99 ) Days in the T-LMWH group. Simillarly the saturation level of oxygen ware (SPO2 98% ) in P-LMWH group & (SPO2 97%) in T-LMWH group. 26% patients shows adverse effects to P-LMWH group & 32% patients to T-LMWH group. Study demonstrates that if you start earlier prophylactic LMW Conclusion- heparin to critically ill patients of covid 19 with raised D-dimer level the level of d-dimer will not further increased and comes down to normal value. Simillarly patients recover earlier and reduces hospital stay of patients with improvement in oxygen saturation.

A relatively high mortality of severe coronavirus disease 2019 (COVID-19) is worrying, and the application of heparin in COVID-19\u0000has been recommended by some expert consensus because of the risk of disseminated intravascular coagulation and venous thromboembolism.\u0000However, its efcacy remains to be validated. We aim to Correlate the association between prophylactic and therapeutic dosages of low\u0000molecular weight heparin (LMWH) on mortality and other morbidity outcomes in critically ill patients of COVID-19 . Consecutive Methodspatients admitted to Dr. S. C.GMC Medical college and hospital Nanded with laboratory-conrmed COVID-19 were included in this\u0000retrospective observational study. The diagnosis of COVID-19 was established according to World Health Organization interim guidance and\u0000conrmed by RNA detection of the SARS-CoV-2 in the microbiology laboratory of the hospital. The COVID19 pandemic has led to signicant changes in daily routines and lifestyle worldwide and mental health issues have emerged consequently. The aim to assess the presence of sleep disturbances before and after the pandemic in the adolescents and young adults age groups in the general population. Methods: Survey data were collected by using snowball sampling method in the general populations. Participants included 234 in total, in those 100 subjects were adolescents(13-24years) and 130 subjects were belonging to age group of 25-44years. Cross sectional study was proceeded and the prevalence rate of various components of sleep pattern was assessed using the Pittsburgh sleeping quality index questionaries. Interpretation: Although, the quality of sleep doesn't show variation among adolescent, but the other domains of sleep pattern are changed aggressively. The % sleep latency was increased which indicates poor sleep pattern. Sleeping duration are also greatly decreased, which leads to fall in the efciency of the sleep. Moderate sleep disturbances were greatly affected. Overall good sleeper prevalence rate was drastically falls among adolescents after the pandemic period. The sleep quality and sleep latency among young adults of bad sleepers was improved after the pandemic. while the other components of the PSQI of sleep was drastically affected. The duration of sleep was greatly affected. The efciency among good sleepers was highly affected. The prevalence rate was doubled on severe sleep disturbance in young adults. Mild daytime dysfunction persistently high in both periods. Overall moderate severely sleep disturbers rate was increased. Lifestyle changes and sleep disturbances have been occurring in hand Conclusion: and hand, it's needed to evaluate the psychological aetiology of sleep disorders because sleeping is the most important indicator of mental wellbeing and treat the patients accordingly to prevent the mental illness among adolescents and young adults.
