Platelet indices as a predictor of mortality in critically ill children 预测重症儿童死亡率的血小板指数

In the current study, we aimed to elaborate the utility of platelet indices as biomarkers for predicting critical illness and mortality in critically ill children. Two main groups were included; group I (patient group): Two hundred (200) critically ill children, group II: Two hundred (200) healthy children with comparable age and sex, were enrolled as a control group. All subjects were evaluated clinically and by CBC including platelet indices (MPV, PDW, and PCT). Patient group I were evaluated with SOFA and PRISM scores and subjected to full lab and other radiological and invasive procedures as indicated. Among 200 critically ill patient 33% of them died and 67% survived and were discharged. Interestingly, It was found that Platelet count and PCT were significantly lower in patient group as compared to the control group with p-value (< 0.001). Moreover, the non survivors of patient group showed significantly lower platelet count and PCT values than the survivors in the same group with p-value (< 0.001). In contrast, MPV and PDW were found to be significantly higher in patient group as compared to the control group with p-value (< 0.001), while, the survivors of patient group showed significantly lower MPV and PDW with p-value (< 0.001) Worthy noting that PCT was inversely related to PRISM and SOFA score while, MPV and PDW were directly related to PRISM and SOFA score. PDW was the most sensitive parameter to discriminate between patient and control group with sensitivity 96% while, PCT was the most specific parameter with 99% specificity. In addition, PCT was the most sensitive parameter to predict mortality in patient group with sensitivity 86.57%. Finally, MPV and PDW were the most specific parameters to predict mortality in patient group with specificity 87.88%. In conclusion , this study proved that platelet count and platelet indices are valid predictors for assessment of critical illness and PICU mortality.

Midline swaddling and traditional swaddling position and their influence on physical and autonomic sign on newborn babies 中线襁褓和传统襁褓姿势及其对新生儿身体和自主神经体征的影响

The traditional method of swaddling is done with the arms and legs straightened and wrapped tightly with the hips in an addiction position. When swaddling is properly done, infants show less awakening and longer sleep, better neuromuscular development, less physiological stress, better motor organization, and better self-regulation abilities. Improper swaddling can result in conditions that are detrimental for newborns, like SIDS, DDH, which cause stress. The early exposure of the neonates to stress results to residual symptoms in behavior and neurobiological disorders. This study aims to determine the effect of midline position compared to the traditional swaddling regarding physical and autonomics stress sign. The study population consisted of 194 newborns. The midline and traditionally swaddled were observed and recorded, physical and autonomic signs at birth, 10 minutes and 2 hours were documented. This study found that there is statistically significant difference between the groups in terms of cries at 10 minute and 2 hours and so with the heart rate and respiration rate at birth ( p <0,05), and a significant difference in oxygen saturation at 10 minutes and 2 hours ( p <0,05). Additionally, midline swaddling had a larger average decrease in heart rate and respiration rate, and increase in the oxygen saturation. Therefore, this study can deduce that, midline position swaddling has less stressful effect than the traditional position. The same research elsewhere in Indonesia will be costly. Future research would suggest for larger sample that may suggest a more accurate outcome.

The aim of this study is to determine the effects of four different body positions applied to paediatric intubated patients on physiological variables and perfusion index.The sample of this study, which was conducted as a quasi-experimental study in a single group in a paediatric intensive care unit, consisted of 44 intubated and monitored patients. The patients were given supine, right-lateral, left-lateral and Semi Fowler positions at intervals of two hours, and each position’s measurements were evaluated separately. In the analysis of data, number, percentage, mean, standard deviation, ANOVA, and post-hoc (Tukey HSD) tests were used. Fifty per cent of the patients were under 1 year old, 56.8% were male, and the mean patient age was 20.61±21.72 months. Study of clinical and biochemical profile of neonatal seizures 新生儿癫痫发作的临床及生化特征研究

Introduction: Neonatal Seizure is defined as a paroxysmal involuntary disturbance of cerebral function. Neonatal seizures are a common problem. Common etiology of neonatal seizures is brain disorders, hypoxic-ischemic encephalopathy, central nervous system (CNS) infections, CNS bleeds, and structural anomalies of the brain or secondary to metabolic problems, for example, hypoglycemia, hyponatremia, and other electrolyte disturbances. Objective: To study Clinical and Biochemical abnormalities associated with neonatal seizures. Material and Methods: An observational study was conducted in ninety newborns with seizures admitted in neonatal intensive care unit (NICU). After taking a complete history and appropriate physical examination, blood sample was collected for detecting metabolic abnormalities before instituting specific therapy. Results: Total ninety neonates presented with seizures were enrolled in this study. Among them, male was 52 (57.78%), and female was 38 (42.22%). In present study, majority of neonates 30% had the onset of seizures within 24 hr. In ninety neonates, the number of neonates with subtle seizures was 62 (68.89%), tonic seizure was 11 (12.22%), and clonic was 10 (11.11%).In ninety neonates, hypoglycemia was reported in 32.22% of neonates, followed by birth asphyxia 25.56%, hypocalcemia in 24.44% neonates, hyponatremia in 13.33% of neonates, hypomagnesemia in 4.44% of neonates. Conclusion: Early identification and management of biochemical abnormalities are essential for satisfactory long-term outcome. The common metabolic causes for neonatal seizures in India include hypoglycemia, hypocalcemia, birth asphyxia, hyponatremia, and hypomagnesemia.
简介:新生儿癫痫发作被定义为一种阵发性不自主的大脑功能紊乱。新生儿癫痫是一个常见的问题。新生儿癫痫发作的常见病因是脑部疾病、缺氧缺血性脑病、中枢神经系统(CNS)感染、中枢神经系统出血、脑结构异常或继发于代谢问题,如低血糖、低钠血症和其他电解质紊乱。目的:探讨新生儿癫痫发作的临床及生化异常。材料和方法:对90例新生儿重症监护病房(NICU)的癫痫发作新生儿进行观察性研究。在进行完整的病史和适当的体格检查后,采血检测代谢异常,然后进行特异性治疗。结果:共有90名出现癫痫发作的新生儿被纳入本研究。其中,男性52人(57.78%),女性38人(42.22%)。在本研究中,30%的新生儿在24小时内发作。90例新生儿中,轻度发作62例(68.89%),强直性发作11例(12.22%),阵挛性发作10例(11.11%)。90例新生儿中,低血糖发生率为32.22%,其次为出生窒息25.56%,低钙发生率为24.44%,低钠血症发生率为13.33%,低镁血症发生率为4.44%。结论:早期发现和处理生化异常对获得满意的远期疗效至关重要。印度新生儿癫痫发作的常见代谢性原因包括低血糖、低钙血症、出生窒息、低钠血症和低镁血症。

Neonatal seizures are a common problem. Common etiology of neonatal seizures is brain disorders, hypoxic-ischemic encephalopathy, central nervous system (CNS) infections, CNS bleeds, and structural anomalies of the brain or secondary to metabolic problems, for example, hypoglycemia, hyponatremia, and other electrolyte disturbances. Objective: To study Clinical and Biochemical abnormalities associated with neonatal seizures. Material and Methods: An observational study was conducted in ninety newborns with seizures admitted in neonatal intensive care unit (NICU). After taking a complete history and appropriate physical examination, blood sample was collected for detecting metabolic abnormalities before instituting specific therapy. Results: Total ninety neonates presented with seizures were enrolled in this study. Among them, male was 52 (57.78%), and female was 38 (42.22%). In present study, majority of neonates 30% had the onset of seizures within 24 hr. In ninety neonates, the number of neonates with subtle seizures was 62 (68.89%), tonic seizure was 11 (12.22%), and clonic was 10 (11.11%).In ninety neonates, hypoglycemia was reported in 32.22% of neonates, followed by birth asphyxia 25.56%, hypocalcemia in 24.44% neonates, hyponatremia in 13.33% of neonates, hypomagnesemia in 4.44% of neonates. Conclusion: Early identification and management of biochemical abnormalities are essential for satisfactory long-term outcome.
