Annotation The aim of the research is to determine the levels and frequency of antibodies popularity to cyclic citrullinated peptide (APCCP), rheumatoid factor (RF), anti-Sa antibodies and antibodies to heterogeneous nuclear ribonucleoprotein K (anti-gRNP K) in rheumatoid arthritis (RA) and to compare their diagnostic characteristics, and also to determine sensitivity and specificity of anti-Sa antibodies, anti-hRNP K in seronegative RA. Material and methods. The study included 270 patients with RA and 50 healthy individuals. The levels of ACCP, anti-Sa, anti-hRNP K were assessed in blood serum samples from patients by enzyme-linked immunosorbent assay using commercial test systems. RF levels were assessed by kinetic nephelometry on automatic analyzer. Results. The levels and popularity frequency of the studied antibodies in patients with RА were significantly higher than in healthy individuals (p <0.05). Tests based on ADCP detection had the highest indices of sensitivity and specificity for RA diagnosis; sensitivity - 78.01%, specificity - 100.00%, and RF - 70.64%, specificity 96.97%. In case of serogegative APCCP and RA RF, diagnostic sensitivity of anti-Sa antibodies determination was 50.00%, specificity was 96.77%, sensitivity of anti-hRNP K antibodies determination was 55.56%, and specificity was 100.00%. Conclusion. Considering high specificity of tests for anti-Sa and anti-hRNP K antibodies in seronegative RA, they can be considered as additional confirmatory diagnostic tests.

The aim of the research is analysis of frequency and nature of adverse reactions during the use of drugs in the Republic of Sakha (Yakutia) according to automated information system of Russian Federal Service for Surveillance in Healthcare “Pharmacovigilance” (Roszdravnadzor “Pharmakonadzor”). Material and methods. Data from the AIS Roszdravnadzor “Pharmakonadzor” in the Republic of Sakha (Yakutia) for the period from 2014 to 2019 were used for the analysis. It should be noted that since April 1, 2019 data collection is carried out using the modified notification form. Results. For the period of 2014-2019 300 reports (292 primary and 8 repeated) on adverse reactions and cases of therapeutic ineffectiveness of drugs were registered. According to the leading criterion concerning person’s life severity, there were notifications on 20 deaths of patients, 60 life-threatening cases, 1 case of disability, 105 cases of hospitalization or prolongation of its terms, 35 clinically significant event cases. It was noted, that mainly, antimicrobial drugs and drugs for nervous system treatment caused adverse reactions. Conclusion. Report frequency on drugs adverse reactions or lack of therapeutic effect in the region was assessed as insufficient. There should be additional training for medical workers on the procedure for implementing pharmacovigilance in medical organizations, correct notification filling, which ensures reports completeness and its quality

A clinical family case of Jervell and Lange-Nielsen syndrome in 2 children is presented. Long QT syndrome is one of the leading causes of sudden cardiac death in children. Jervell and Lange-Nielsen syndrome is one of the most severe and rare types of long QT syndrome. Symptoms of the disease appear in infancy; they are characterized by a lengthening of QT interval on ECG, syncope as a result of life-threatening ventricular tachycardia and ventricular fibrillation, combined with congenital bilateral hearing loss.

Customer success management is gaining momentum in business markets. From an initial focus on selected industries, such as software-as-a-service, interest in customer success is spreading among business-to-business marketing practitioners. To date, academia has largely neglected this concept and its organizational implementation. Based on extant managerial literature and research on value in use in business markets, we distinguish between customer success as a customer-perceived construct, customer success management as a joint customer-supplier management process, and the emerging job position of customer success managers in supplier organizations. Based on the case of a provider of IoT devices, software and services in decision agriculture, we illustrate how customer success management is practiced in a business marketing context and refine our understanding of customer success management and its constituents. Finally, we suggest promising avenues for future research in this managerially important, under-researched and rapidly growing domain.

The aim of the work is to study clinical cases and outcomes of pregnancy in women who have pelvic fracture during pregnancy. The article examines two cases of concomitant injury in pregnant patients at 25-37 weeks. In both cases, there is a break of pelvic bones of various localization. On basis of these observations, the authors draw conclusions about the significance of the mother’s condition and associated injuries for a favorable outcome.

Цель исследования. В 2018 году в ФГБОУ ВО «Красноярском государственном медицинском университете имени профессора В.Ф.ВойноЯсенецкого» Минздрава России (далее – КрасГМУ) впервые практический экзамен по дисциплине «Пропедевтика внутренних болезней» у студентов третьего курса лечебного факультета проведен в два этапа. Целью настоящего исследования стала оценка уровня овладения практическими навыками студентами третьего курса лечебного факультета КрасГМУ по дисциплине «Пропедевтика внутренних болезней», а также возможности повышения объективности их оценивания. Материал и методы. Демонстрация практических навыков оценивалась на первом экзаменационном этапе у 402 студентов, на втором – у 411 человек. Статистический анализ полученных данных осуществляли с использованием пакета прикладных программ Statistica 7.0 (StatSoft Inc., 2004). Результаты. Первый этап практического экзамена проводился на кафедре пропедевтики внутренних болезней и терапии. Второй этап выполнялся впервые на кафедре – центре симуляционных технологий КрасГМУ. Экзаменационные практические навыки включали обязательные разделы: обследование органов дыхания, обследование сердечно-сосудистой системы, обследование пищеварительной системы. Предлагалось продемонстрировать практические навыки по эндокринной, мочевыделительной и другим системам. На втором этапе практического экзамена могли быть предложены для демонстрации еще три дополнительных практических навыка. Средний балл, полученный студентами на первом этапе практического экзамена, составил 4,0 (3,6 – 4,7), на втором этапе, средний балл был достоверно выше и составил 4,7 (4,3 – 5,0). В ходе выполнения второго этапа практического экзамена большинство студентов за демонстрацию трех обязательных практических навыков получили оценки «хорошо» и «отлично». Заключение. На втором этапе практического экзамена для повышения объективности оценки уровня подготовки студентов предложено привлечение независимых преподавателей, использование симуляционных технологий и листов экспертных оценок.
