Recent updates in Management of Borderline Ovarian Tumors 交界性卵巢肿瘤治疗的最新进展

Borderline ovarian tumors (BOTs) need to be differentiated from ovarian carcinomas due to different presentation and management. Absent stromal invasion is the key differentiating feature on histopathological examination. Considering the degree of aggressiveness between epithelial ovarian carcinoma and benign ovarian neoplasms like cystadenoma, borderline neoplasms are closer to benign ovarian neoplasms as regards the clinical behavior. Surgery with intention of removal of all visible tumors is the most important pillar in treatment. Patients treated conservatively with fertility sparing surgery or laparoscopic surgery has low recurrence rate, making both options worth considering. Overall prognosis of these tumors is good with survival rate more than 90% at 10 years in early stage borderline tumors. However, small percentage of patients with borderline neoplasms may show more aggressive form of the disease, and trials have been made to figure out the histological correlates that might predict for worse outcome. There is no added advantage of postoperative adjuvant chemotherapy or radiation in any stage especially with non-invasive component. Follow up is done by observation of the patient and any further intervention is kept reserved for recurrent disease. Clinical scoring for early detection of prediabetes in Egyptian population 埃及人群糖尿病前期早期检测的临床评分

Background: Prediabetes is a wide spread condition where blood glucose is above normal, but it does not measure up to the criteria of diabetes mellitus. Yet there is no national scoring system for prediabetes according to the potential risk factors. Methods: A case-control study has been carried out in Mansoura Specialized Hospital. Subjects have been recruited from out-patients clinic in the period from October 2018 to August 2019. Those with prediabetes had been assigned to be the case group whereas control subjects were those who had a normal HbA1C. A scoring system was based on risk factors of prediabetes by use demographics data including information about age and sex. Results:. In the current study participants with obesity have 4.9 times higher odds to exhibit prediabetes (a score of 16 points is assigned to those with obesity). Participants who are current smokers have 3 times higher odds to exhibit prediabetes (a score of 11 points is assigned to current smokers). Participants with lack of moderate physical activity have 2.6 times higher odds to exhibit prediabetes (a score of 10 points is assigned to those who lack moderate physical activity). Binary logistic regression was run to assess the performance of Egyptian Prediabetes risk score in predicting the likelihood of prediabetes in Egyptian patients. The model is statistically significant ( 2 [1] = 60.684, P<0.001), and it correctly classified 75% of cases. The model has sensitivity, and positive predictive value of 74%, and specificity, and negative predictive value of 76%. Maternal,fetal and neonatal outcomes of severe preeclampsia in Mansoura University Hospitals: A prospective study 曼苏拉大学医院重度子痫前期的孕产妇、胎儿和新生儿结局:一项前瞻性研究

Background: Preeclampsia continues to affect 5% to 8% of all pregnancies throughout the world. It is a pregnancy-specific multisystem disorder and a leading cause of maternal and perinatal morbidity and mortality. The exact pathogenesis of preeclampsia remains poorly defined. Features of severe preeclampsia include severe proteinuria, hypertension, symptoms of central nervous system dysfunction, hepatocellular injury thrombocytopenia, oliguria, pulmonary edema, cerebrovascular accident, and IUGR. Women with severe preeclampsia must be hospitalized to try to stabilize the disease. Aim of work: To evaluate maternal, fetal and neonatal outcomes of severe preeclampsia in Mansoura university Hospitals. Methods: This was a prospective study for one year, from September 2019 till September 2020, included all gravid women with severe preeclampsia who were managed at Mansoura University hospitals, Department of Obstetrics and Gynecology. Results: This study included 204 patients. Regarding maternal outcomes, the percentages of Placental abruption, Acute renal failure, ICU admission, DIC, HELLP and PPH were 7.8%, 4.9%, 15.7%, 0.5%, 8.3% and 3.4%, respectively. Considering fetal outcomes, IUGR developed in 13% of cases, and 13% of cases ended with IUFD. With regard to neonatal outcomes, the percentages of preterm delivery, NICU admission low APGAR score were 69%, 62% and 47%, respectively. Gestational age was the only significant factor to affect incidence of IUFD, NICU admission and low APGAR score (P<0.001). Conclusion: Pre-eclampsia tends to threaten maternal health and fetal viability adding to maternal and neonatal mortality and morbidity. In addition, gestational age was the only significant factor to affect incidence of IUFD, NICU admission and low APGAR score. Relation between epithelial-mesenchymal transition and colorectal cancer prognosis: Impact of SMAD4 and β-catenin markers 上皮-间质转化与结直肠癌预后的关系:SMAD4和β-catenin标志物的影响

Colorectal cancer (CRC) is one of the most prevalent and deadly neoplasms. It develops through several molecular pathways and varies in clinical presentation and patient prognosis. The prognosis of CRC is influenced by variable pathologic and molecular variables. Understanding the molecular basis that promote CRC progression, invasive and metastatic features, is important for establishing effective CRC therapeutic strategies. Epithelial mesenchymal transition (EMT) is regarded to be key hallmark of CRC carcinogenesis, providing metastatic capacities to cancer cells by promoting motility and invasion. During this process, epithelial markers such as E-cadherin are down-regulated, whereas mesenchymal markers such as Vimentin are up-regulated. Several signaling pathways crosstalk to each other to initiate and maintain the process of EMT. Both the transforming growth factor-β (TGF-β)/SMAD4 and wingless-type mouse mammary tumor virus integration site family (Wnt)/β-catenin pathways, often cooperatively, induce EMT during cancer metastasis. Assessment of EMT-related markers, is important to predict the aggressive behavior of tumors, and is associated with higher risk for local or distant recurrence and poor survival outcomes in patients with CRC. Reconstruction of glossectomy defects post oncologic resection 肿瘤切除后舌骨缺损的重建

The tongue is a muscular organ completely covered by mucous membrane and made up of two portions divided by the V-shaped sulcus terminalis: an anterior 2/3 (oral tongue) and a posterior 1/3 (tongue base). The tongue is essential to normal speech as well as swallowing process, and healthy tongue function is vital within the general quality of life. Tongue cancers are aggressive tumors with poor prognosis, so the early detection markedly raises the possibility of cure with minimal impairment and deformity. Management needs a surgical, medical and radiation oncologist in association with speech therapist, physical rehabilitation, and psychologists. In addition to regular followup and good family support are leading to better outcomes.The reconstruction of the tongue post oncologic resection results in better speech and swallowing. In addition, there is a variety of flap options, and no one is superior to another, but it depends on the tongue defect size, patient factors, and team expertise. Therefore, the article focuses on tongue reconstruction by using various flap modalities. Management needs a surgical, medical and radiation oncologist in association with speech therapist, physical rehabilitation, and psychologists. In addition to regular followup and good family support are leading to better outcomes.The reconstruction of the tongue post oncologic resection results in better speech and swallowing. In addition, there is a variety of flap options, and no one is superior to another, but it depends on the tongue defect size, patient factors, and team expertise. Therefore, the article focuses on tongue reconstruction by using various flap modalities.
