The aim of this study is to evaluate response of cervical cancer stage IB – IIA after neoadjuvant chemotherapy based on VEGF expression. The data were collected from 51 patients’ cervical cancer stage IB – IIA parafin blocks who received chemotherapy ifosfamide – cisplatin before radical hysterectomy at General Hospital Adam Malik Medan. VEGF expression was evaluated from cervical biopsy tissue, and response therapy was evaluated based on tumor size clinically. 20 out of 51 samples with clinically complete response, and the rest are partial response. 18 out of 20 samples with clinically complete response have negative or weak VEGF expression, and 31 out of 51 samples patients were partialy response with moderate or strong VEGF expression. 23 cases with tumor size > 4 cm and 23 cases stage IIA expressed VEGF moderately or strong. Cervical cancer with tumor size < 4 cm and cervical cenncer stage IB with less expressed of VEGF have good response with chemotherapy adjuvant ifosfamide – cis platin.Keyword: ifosfamide-cisplatin, cervical cancer, VEGF

Introduction. Chronic energy deficieny (CED) is a condition of a body characterized by low body weight and low energy stores, possibly limited physical capacity due to deprivation of food over a long period time. Ministry of Health ( Kemenkes) showed that in 2015, 305 out of 100.000 death of pregnant women is realated to malnutriotion and CED. Objective. The aim of this study is to determine the overview of pregnant women nutritional status based on mid-upper arm circumference (MUAC) in Sundari Medan General Hospital. Method. The study was an observational descriptive study with a cross sectional design. The samples of this study consists of pregnant women from Sundari Medan Genaral Hospital, who had fulilled the inclusion and esclusion criteria by total sampling. Results.. Among 60 samples, the prevalance of Non-CED woman (85%) was found higher than the mild malnutrition (15%). Conclusion. Prevalance of CED pregnant women was found higher in risky age, middle educated and high income family.

Lumbal Puncture (LP) is a procedure that can help to diagnosed CNS infection in children. The procedure of LP is a common invasive procedure, but sometimes it had refused by the parents. Furthermore, it can cause a delayed in the diagnosis and affected the prognosis. The subject was the parents of pediatric patients with CNS infection. We have a questionnaire and using Chi-square and Fisher exact test to analyze factors that influenced the parent’s decision of LP procedure. From the parents of 50 children with CNS infection, 19 parents (38%) were refused the LP procedure. Father’s education was related to the refusal of LP procedure with PR 6.64 (95%CI; 8.95-788.08). Mother’s education was related to the refusal of LP procedure with PR 7.69 (95%CI; 3.19-16.24). There was the significant result of the parent's education with the decision of LP procedure.

Background. Based on the Indonesian Renal Registry (IRR) data in 2015, the highest incidence of comorbidities in hemodialysis patients in Indonesia was hypertension (50%). Obstructive Sleep Apnea (OSA) is one of the causes of secondary hypertension, the characteristics of hypertension in OSA are more resistant to treatment. One of the causes of resistant hypertension is patient non-compliance with the consumption of antihypertensive drugs. Aim. To determine the relationship of adherence of consumption of antihypertensive drugs with obstructive sleep apnea in regular HD patients. Method. This cross-sectional study involved 89 outpatients in HD units at the Rasyida Kidney Hospital in 2018. Data was obtained through interview using the STOP-BANG questionnaire to assess the of risk of OSA and using Morisky Medication Adherence Scale (MMAS) questionnaire to measure the compliance of drug consumption. Results.The prevalence of OSA in HD patients was (39.3%). There was no significant relationship between adherence with drug consumption and OSA risk, but in bivariate analysis, age (p = 0.0001), body mass index (BMI) (p = 0.001), and gender (p = 0.03) were significantly related to OSA. In multivariate, age and BMI were significantly associated with OSA (OR: 6.449, 95% CI 2.246 ± 18.520, p = 0.001; OR: 6.130, 95% CI 2.105 ± 17.849, p = 0.001). Conclusion. OSA was not statistically related to adherence to antihypertensive drug consumption in HD patients but was significantly associated with age and BMI. Keyword. Hyperuricemia is an increase in blood uric acid levels. In the elderly cell damage occurs due to the aging process which can result in organ weakness, physical damage and various diseases such as increased uric acid levels. The purpose of this study is to determine the relationship between purine intake and the incidence of hyperuricemia in the elderly at the HISOSU Binjai nursing home. This research was conducted with an observational analytic research design using a cross sectional study. Samples were selected using the total sampling method and adjusted for inclusion and exclusion criteria. Data taken by weighing and recording food (in 1 day) Then assess respondent uric acid levels. This is done 3 times in the HISOSU Binjai nursing home. There are 32 research samples. Samples that experienced hyperuricemia as much as 40.6%, most of them consuming moderate-category purine source protein. The results of the chi-square test Pearson chi-squared t-test was 16.453 and p value < 0.001 (<0.05), it can be concluded as consumption of purine source protein associated with the incidence of hyperuricemia. The higher the purine intake, the greater the chance of getting hyperuricemia.

Introduction. Reproduction organ is one of the sensitive organ in the body and need special care. Personal hygiene habit is one of the important factor to avoid infection. It is because infection can cause vaginal discharge and cervical carcinoma. Tropical climate in Indonesia lead about 90% of girl teenagers to potentially have vaginal discharge causing bacteria, fungi, and virus easily grow. Objectives. To identify the relationship between knowledge, attitude, and behavior of taking care of external genitalia with vaginal discharge incident on students at Pondok Pesantren Modern Darul Hikmah. Methods. This is a descriptive analytical study with cross sectional design. The data was taken with primary data. Results. Leucorrhoea incidence rate at 103 respondents are 60,2% of normal vaginal discharge and 38,9% of abnormal vaginal discharge. There is a correlation between knowledge of taking care of external genitalia with vaginal discharge (p=0,001). There is correlation between attitude of taking care of external genitalia with vaginal discharge (p=0,018). There is correlation between behavior of taking care of external genitalia with vaginal discharge (p=0,002). Conclusion. There is correlation between knowledge, attitude, and behavior of taking care of external genitalia with vaginal discharge incident on students at Pondok Pesantren Modern Darul Hikmah.

Abstract. Blastocystis hominisis a unicellular parasite classified as stramenopiles found in human and other mammals. The prevalence varies depending on the region, higher in developing country. The organism is transmitted via fecal-oral route by ingestion of oocyst contaminated food or water. Travelers also have high-risk of having blastocytstis infection. Based on recent studies, several proposed virulence factor are also identified but it remains elusive. The advanced technique has been used to describe its genetic variation and diversity that might be related to its clinical significance. There are 6 distinct morphological forms of Blastocystis including vacuolar, granular, ameboid, and cystic form. The cystic form is considered as an infective form of the organism. Exposure to animals is suggested as the other transmission route, consequently, the infection is also considered as zoonotic infection. It can produce broad clinical manifestation ranging from asymptomatic, self-limiting diarrhea until severe watery diarrhea, depends on host factors and the organism subtype. The organism can produce pruritus and urticaria, proved in literature. Since its pathogenicity remains unclear, conservative management is still suggested to all asymptomatic-symptomatic blastocytosis patients in accordance to patient’s immune status.

Exercise is non-pharmacological management for Diabetes Mellitus (DM) type-2. Previous study found that both eerobic and interval training improved insulin resistance. The aim of this study was to analyze the chronic effect various type of exercises to fasting blood glucose (FBG) and insulin resistance in DM type-2 model rats. It was an experimental study, twenty male Wistar rats, age 8 weeks, weight 150-180 gram as the object. Rats were given high fed diet for 4 weeks then injected streptozocin dose 30 mg/kgBB in citrate buffer pH 4.5 i.p, and 45 mg/kgBB at the following week. Groups were divided into i.e moderate continous training (MCT), severe continous training (SCT), slow interval training (SIT), and fast interval training (FIT). All groups were treated with ran on the treadmill three times a week for 8 weeks. Fasting blood glucose and fasting insulin were checked before and after intervention. Insulin resistance was determined by calculating HOMA-IR. Data analized with paired t-test (p<0,05). The results shown that all group significantly decreased fasting blood glucose and insulin resistance (p <0,05) after eight weeks exercise except insulin resistance in MCT group. Fasting blood glucose and insulin resistence post-test was found lowest in SIT groups in this study. In conclucions chronic effect of aerobic continous and aerobic interval in various intensity can decrease fasting blood glucose and insulin resistance in DM type-2 rat model. Slow interval training was the best exercise model to decrease insulin resistance.

Many of the teenagers who enter pregnancy would cause a decline in health and nutritional status. Delivery at an early age was associated with a greater health risk for the mother. To assess the profile of teenage pregnancy in H. Adam Malik Hospital Medan year 2011-2015. This study was conducted with descriptive study design using secondary data from medical records. There were 128 patients who became pregnant in their teens. Then the data were tabulated and presented in the form of frequency distribution table and were analyzed descriptively. Most pregnant women came to the hospital for parturition (60.1%) followed by preeclampsia / eclampsia (14.8%). The way of delivery of pregnant adolescent women was vaginal (52.3%) followed by cesarean (36.7%). There was no significant relationship between adolescent age of pregnant and newborn weight. There was no significant relationship between the age of the adolescent and method of delivery. Most teenage pregnancy occured between 17-19 years. Most pregnant teenagers delivered vaginally, and most of the newborns were normoweight

Indonesia is one of the few countries in the world with very high burden of tuberculosis (TB). Tubercular pleural effusion (Pleural TB) is the most common form of extra-pulmonary TB, however the remains a common clinical challenge. This research from May 2017 through September 2017 a total of 42 patients with exudative lymphocytic pleural effusions suspected to pleural tuberculosis were enrolled in this study. Adenosine deaminase (ADA) and Gene X-pert were examined from pleural fluid. Diagnosis was made clinically or based from sputum/pleural fluid culture. The result of this research showed out of all participants, 64,3% (27/42) had ADA in positive level (>40 IU/L) and 31% (13/24) had Gene X-pert positive Mycobacterium Tb. The Gene X-pert sensitivity was 40.7% with specificity 86.6%, PPV and NPV were 44.8% and 57.1% respectively. Kappa Compatibility study show that Gene X-pert point was 0.227 (p 0.066). The usefulness of Gene X-pert to diagnose pleural TB is limited by its poor sensitivity. Gene X-pert cannot be used as single diagnostic tool for tuberculous pleural effusion
