Stroke has a major impact worldwide in terms of mortality and morbidity. The aim of our research was to determine the effect of serum Vaspin level on acute ischemic stroke and to investigate the relationship between serum Vaspin level, National Institutes of Health Stroke Scale (NIHSS) and infarct volume. This study was carried out in 100 patients hospitalized with acute ischemic stroke diagnosis in the Neurology Department of Yakutiye Research Hospital of Atatürk University Faculty of Medicine and 80 healthy controls. The age and sex of the patient and control groups were consistent. Patients who met the following criteria were selected to be included in the study. Exclusion criteria: patients with ischemic stroke presenting after the first 24 hours to overcome the potential bias associated with inflammatory conditions, patients diagnosed with rheumatic and autoimmune diseases, diseases that cause hypoxia and coronary artery disease. Vaspin levels were measured and compared in patients with acute ischemic stroke (AIS) and in the control group. NIHSS calculated. Infarct volume was also measured with Cavalieri's principle. The correlation between them was calculated statistically. Vaspin level was higher in patients with ischemic stroke compared to the control group (825 vs. 939; p<0.001). Vaspin levels were significantly higher in patients who had acute ischemic stroke. Since examination of Vaspin levels in patients presented with acute ischemic stroke suggested the severity of the stroke, it was found to be important in terms of mortality and morbidity affiliated with it.

We aimed to investigate correlation between perioperative biochemical values and adenoma volume, the effects of perioperative biochemical values and clinicopathological variables on postoperative hypocalcemia after parathyroidectomy. A retrospective study planned in tertiary university hospital on patients undergone surgery for primer hyperparathyroidism. Preoperative calcium (first, close to surgery, maximum), PTH (first, close to surgery, maximum), alkaline phosphatase (ALP) and phosphorus (P) values, postoperative calcium (early, late), postoperative PTH (early, late), 24-hour urinary calcium calculation, time from the first diagnosis of elevated calcium until surgery, preoperative hypercalcemia treatments, excised parathyroid gland, histopathologic diagnosis, maximum diameter and volume of the excised parathyroid on histopathologic examination, postoperative intravenous calcium supplement were analyzed. 73 patients were included in the study. The median age of the patients was 54 years (range, 18-82), and 83.6% were female. In univariate analysis, patients who were administrated intravenous (IV) calcium supplement had higher preoperative PTH (p=0.024). adenoma volume correlated moderate with preoperative PTH first (r=0.396, p=0.001), preoperative maximum parathormone (r=0.380, p=0.001), preoperative PTH CS (r=0.432, p<0.001) and preoperative first elevated calcium (r=0.240, p=0.041) levels. We conclude that biochemical parameters and adenoma size are not predictive factors in determining the requirement of IV calcium supplement. Preoperative PTH and calcium levels were moderately associated with adenoma volume.

In recent years, the use of YouTube for obtaining medical information has increased. In this article, we aimed to determine the usefulness, quality,and reliability of YouTube videos on coccydynia. Coccydynia and "coccyx pain" were searched for on YouTube on October 2, 2022. The study comprised 80 of the 120 videos that were discovered as a consequence of the search and met the inclusion criteria. The videos' videometric properties were noted. Depending on whether the content utilized information from evidence-based scientific sources,the videos were separated into two groups: useful and misleading. The measure popularity, the Video Power Index (VPI) was applied. To measure reliability, the modified DISCERN (mDISCERN) was applied. To evaluate quality, the Global Quality Score (GQS) was applied. This study found that 28.7% of the video sources were from physicians and 73.8% of the videos were classified as useful, while 26.2% were classified as misleading. Videos posted by doctors and academic institutions/professional organizations were all in the useful group, while all videos uploaded by independent users and patients were in the misleading group. The useful videos had higher VPI, GQS, and mDISCERN scores, indicating that videos posted by healthcare professionals and academic institutions were more reliable and of higher quality than those from patients and independent users. The significant number of the coccydynia-related YouTube videos were reliable, useful, and of high quality. Healthcare providers should be aware of the value of YouTube's health-related content and maintain high-quality content.

We aimed to see the effect of lipids on 8-hydroxy-2'-deoxyguanosine (8-OHdG), a marker of DNA damage, and Interleukin-6 (IL-6), an inflammatory cytokine, in Diabetes Mellitus (DM) patients. Serum IL-6, and 8-OHdG levels were determined using kits based on the ELISA principle. The groups were different in terms of total cholesterol, LDL cholesterol and triglyceride. Group 2 (dyslipidemia positive DM positive) was significantly higher than the other groups. There was no statistical difference between group 1 (dyslipidemia negative DM positive) and the control group (p<0.001). 8-OHdG levels differ between groups. The level of 8-OHdG was significantly higher in group 2 compared to the other groups (p<0.001). IL-6 was different between groups. Group 2, IL-6 level was found to be significantly higher than the other groups (p<0.001). A positive correlation was found between serum 8-OHdG level and total cholesterol (r=0.417 p<0.001), LDL cholesterol (r=0.491 p<0.001) and triglyceride (r=0.441 p<0.001). A positive correlation was found between IL-6 and total cholesterol (r=0.522 p<0.001), LDL cholesterol (r=0.646 p<0.001) and triglyceride (r=0.495 p<0.001). IL-6 and 8-OHdG levels are high in dyslipidemic DM patients with a high lipid profile. IL-6 and 8-OHdG levels are elevated in dyslipidemic DM patients with a high lipid profile. IL-6 and 8-OHdG can be used as biomarkers to evaluate dyslipidemic complications seen in DM patients in the early diagnosis.

Coxal bone, which is included in the structure of the pelvic skeleton, is in close relationship with the pelvic organs and great vessels. The aim of this study is to show the harmony between morphometric measurements using the correlation technique, based on the idea that the proportional harmony in different parts of our body will also be in the coxal bone. In the study, 95 dry coxal bone belonging to the Turkish population were examined. In dry bones of adults, the distance between anterior superior iliac spine and posterior superior iliac spine (A), distance between anterior inferior iliac spine and posterior inferior iliac spine (B), distance between anterior superior iliac spine and anterior inferior iliac spine (C), distance between posterior superior iliac spine and posterior inferior iliac spine (D), symphysial surface length (E), symphysial surface width (F), ischial tuberosity width (G), ischion-pubis arm thickness (H), coxal bone length (I), distance between the symphysial surface and the most protruding part of the ischial tuberosity (J) were measured with caliper. There was no statistical difference in the comparison of different measurements made on the right and left coxal bone in our study. In correlation studies, between (B) and (A) (r: 0.566), between (I) and (A) (r: 0.595), between (J) and (A) (r: 0.600), between (E) and (J) (r: 0.563) measurements showed a positive correlation relationship. In this study, excellent agreement was found in different morphometric measurements in coxal bone.

COVID-19, which affects the whole world, has affected people, societies and human relations throughout the process. The COVID-19 pandemic both limited people's social lives and changed the perspective of people who carry the virus. Anxiety, fear, haste, loneliness, worry were the most common emotions. In this study, we aimed to evaluate how social warnings affect people, the effects of death risk due to the disease, the side effects of the drugs used on behavior change, and the underlying factors. 189 cases were included in the study. The study was designed in the style of a 20-question questionnaire on the effects of covid-19 and treatment options on the behavior pattern. The responses were evaluated and interpreted as percentages. Variables on family and social behavior related to anxiety, fear, panic, and loneliness caused by COVID-19 and the drugs used in its treatment were observed. Negative effects of COVID-19 were observed in family and community relations. COVID-19 has caused psychological and sociological changes in interpersonal and social behaviors all over the world. There are differences in behavioral patterns among societies. In Turkish society, behavioral changes arising from the characteristic and patriarchal characteristics of the society have been observed. A more detailed assessment of the effects of COVID-19 and subsequent pandemics on the individual and society remains important.

To establish a standard approach in patient management by determining the parameters that affect the decision of surgical or conservative treatment in adhesive small bowel obstructions. 94 patients who were admitted to the emergency department with symptoms of ileus and were diagnosed with adhesive intestinal obstruction according to clinical, examination and imaging findings were grouped as patients who were followed up with conservative methods (Group 1) and patients who underwent surgery (Group 2). All patients' laboratory values (hemoglobin, white blood cell (WBC), C- reactive protein (CRP), Blodd urea nitrogen (BUN)/creatinin, sodium, potassium, Lactate dehydrogenase (LDH), lactate and amylase) and imaging findings (air-fluid level in direct abdominal X-ray, increase in small intestine diameter (≥3.95 cm) in computerized tomography), wall thickness increase (>3mm), transition zone, fecal sign, and presence of contrast in the colon) were evaluated, and criteria for early surgery and non-operative follow-up-treatment criteria were determined. 72% (n:68) of the patients were classified as Group 1 and 28% (n:26) as Group 2, and no significant difference was found between the groups according to age and gender. Surgical treatment with increased lactate (r:0.326, p=0.001), diameter increase in the small intestine (r:0.299, p=0.003) and wall thickness increase (r:0.540, p<0.001), change in air-fluid levels on direct X-ray ( A significant correlation was found between r:-0.291, p=0.004) and contrast transfer to the colon on tomography (r:-0.668, p<0.001) and the decision for conservative treatment. Although adhesive intestinal obstruction is a condition that can mostly be managed non-operatively, the early surgical decision significantly reduces mortality and morbidity in cases with an operation indication. It was concluded that the standard approach protocol based on laboratory and imaging findings determined in the non-operative or operative treatment management helps in the differential diagnosis and early surgical decision and reduces the length of stay of the patient.

It was concluded that the standard approach protocol based on laboratory and imaging findings determined in the non-operative or operative treatment management helps in the differential diagnosis and early surgical decision and reduces the length of stay of the patient.","PeriodicalId":18541,"journal":{"name":"Medicine Science | International Medical Journal","volume":"36 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"77321935","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Mucormycosis is a rapidly invasive, life-threatening opportunistic fungal infection, usually in immunocompromised patients. Suspicion, early diagnosis, debridement and anti-fungal treatment are important prognostic factors. The most common clinical manifestations of rhinocerebral mucormycosis are fever, headache, facial and ocular oedema. Invasion of surrounding tissues, the eye, skull base and brain is critical for disease progression. The purpose of this study was to investigate the predictive value of blood parameters in patients with mucormycosis. The preoperative 10-day blood parameters of 23 patients with clinical, radiological and pathological diagnoses of mucormycosis were compared in terms of etiology, gender and site of involvement. When the results were compared, the neutrophil-lymphocyte ratio (NLR) on days 1, 2, 5, 7, 8, 9, and 10 was significantly higher in patients with skull base involvement compared to those without skull base involvement (p<0.05). When comparing the regions of involvement, a significant correlation was found between cranial and turbinate involvement and between lamina papricea and ocular involvement (p<0.05). It was suggested that the NLR rate could be predicted as a poor prognostic factor in patients with mucormycosis.

In light of recent advancements in the field of heart failure management and treatment, it remains crucial to acknowledge that individuals diagnosed with heart failure and presenting with low ejection fraction face a significant risk of adverse outcomes. This high risk is primarily due to impaired pumping function of the heart and concomitant comorbidities. The objective of this study was to assess the prognostic significance of the C-reactive protein/albumin ratio in predicting overall mortality among individuals receiving outpatient care for heart failure. This retrospective, single-center cohort study comprised a total of 218 patients diagnosed with heart failure with low ejection fraction. These patients were diligently monitored as outpatients in a specialized heart failure outpatient clinic. During a median follow-up period of 26 months, the CRP/albumin ratios at the time of the initial hospitalization were assessed in patients who experienced mortality and those who survived. The mean age of the study participants was 56±10 years, 82% were male and 18% were female. The mean C-reactive protein/albumin ratio was 0.12 (0.02-2.1) and the mean left ventricular ejection fraction was 26% (10-40%). It was observed that the predictive accuracy of mortality could be determined by the C-reactive protein/albumin ratio with an optimal threshold value > 0.12. The sensitivity and specificity of this method were 95% and 63%, respectively (AUC: 0.824, 95% CI: 0.740-0.890). In conclusion, the C-reactive protein/albumin ratio can be defined as a prognostic indicator for mortality in outpatients with heart failure.

This study aimed to investigate the anxiety, depression, and psychological impact levels among people with and without psychiatric disease diagnosis in the quarantine and social isolation periods of the 2019 coronavirus disease (COVID-19) pandemic. This study using the case-control research technique, included 182 cases with psychiatric diagnosis and 183 control participants without a psychiatric diagnosis. The research used the Hamilton Anxiety Rating Scale (HAM-A), Hamilton Depression Rating Scale (HAM-D), Impact of Event Scale-Revised (IES-R), and a data form inquiring about sociodemographic characteristics. Data were collected from participants during face-to-face psychiatric interviews. The anxiety levels (OR: 6), depression levels (OR: 3.8), and psychological impact levels (OR: 5.8) were identified to be higher in the case group compared to the control group (p<0.05). The HAM-A, HAM-D, and IES-R scores were not affected by age, sex, marital status, education, place of residence, and employment status in the study group. Living alone, receiving treatment, presence of chronic disease, poor income, and COVID (+) relatives affected scores. In this study, those with a psychiatric diagnosis were affected more by the COVID-19 pandemic compared to those without a diagnosis, while among those with a psychiatric diagnosis, those not receiving treatment were more affected than those using treatment.
