The eye is a very complex and delicate organ, but under adverse conditions and without appropriate preventive measures, it is susceptible to damage that can lead to a sharp decrease in vision, partial or complete blindness. In 2021–2022, 197,773 patients with injuries of the organ of vision were diagnosed in the Republic of Uzbekistan, of which 117,605 patients were diagnosed with foreign bodies of the cornea and conjunctiva, which amounted to 59.46%. Clinical analysis of the condition of these patients is important in the prevention of injuries of the organ of vision. Purpose. Determine the main risk factors for non-penetrating eye injuries with foreign bodies. Material and methods. In 2021–2022, 823 patients turned to the Republican Clinical Ophthalmological Hospital with foreign bodies in the cornea and conjunctiva while working with an angle grinder, or “grinder”. Results. 99.51% of patients were male. Of these, 325 were between the ages of 19 and 30, reflecting the medical, social and economic significance of the problem. In the majority of patients with foreign bodies of the cornea and conjunctiva, the contributing factors to the occurrence of damage were the non-use of goggles by workers during work and poor control by workers of the rules of individual and collective eye protection measures. Conclusion. It can be seen from the results of the analysis that the majority of patients with foreign bodies of the anterior part of the eye are people of working age who use an angle grinder. Metal shavings got into the eyes due to non-compliance with technical safety rules. In order to prevent such injuries, it is necessary to create safe working conditions and conduct explanatory work with employees. Purpose. To study anatomical and functional parameters using optical coherence tomography in patients with avascular retinal pigment epithelium (RPE) detachment with age-related macular degeneration (AMD) and central serous chorioretinopathy (CSCR). Material and methods. 40 people with AMD and CSR were examined. Patients were divided depending on the type of RPE detachment: druzenoid – 10 patients, serous RPE detachment in AMD – 20 patients, serous RPE detachment in CSCR – 10 patients. Results. A comparative analysis revealed that serous avascular RPE detachment in AMD is characterized by the greatest height and width in relation to drusenoid RPE detachment and RPE detachment in CSCR. Avascular RPE detachment in CSCR are characterized by the smallest dimensions – both in height and width in relation to other avascular RPE detachments, and the greatest thickness of the choroid in relation to serous RPE detachment in AMD (p = 0.018). Conclusions. The identified features of avascular RPE detachment in AMD and CSCR allow us to choose an adequate tactics for managing patients. Purpose. Study of changes in morphometric parameters of cornea in keratoconus before and after corneal collagen crosslinking (CXL). Material and methods. 35 patients (35 eyes) with progressive keratoconus stages 1–3. The indexes of photokeratoscopy were studied in depth (topographer Pentacam (Oculus, Germany). CXL was performed according to the Dresden protocol. A corneal lenticule was used in 6 eyes. The analysis of the studied parameters was carried out initially and after 12 months. Results. By the end of the observation, in all patients, the average values of maximum keratometry (Kmax) decreased statistically significantly from 54.5 to 53.1 diopters, which indicated positive trend. Most indices of keratoconus according to topographer data (ISV, IVA, KI, CKI, IHA, Rmin), as well as the central corneal thickness, on average, significantly decreased. But in 5 patients (all men, mean age 25 years), the Kmax exceeded initial values (from 61.7 to 64.5 diopters), which indicated further progression of keratoconus. The initial values of Kmax in 3 out of 5 of these patients exceeded 57 diopters, which is risk of further progression of keratoconus. Conclusion. With a follow-up period of one year, it was found that CXL in 5 patients (14 %) with keratoconus stages 2-3 turned out to be ineffective. Aim. To evaluate the safety and efficacy of the treatment of chronic recurrent episcleritis, not amenable to the standard treatment regimen, by subconjunctival injections of triamcinolone acetonide. Material and methods. Three patients with recurrent episcleritis, low efficacy of treatment with local steroid, non-steroidal anti-inflammatory drugs and a disease duration of more than 6 months were under observation. All were injected with a prepared solution of triamcinolone acetonide (diluted 1:1 with physiological solution 0.9%) subconjunctivally, once according to the original method. No other drugs were prescribed. A comprehensive examination and assessment of the functions of the visual analyzer was carried out before and after the administration of the drug on the 3rd, 7th, 21st days, 3, 6 months. Results. All patients achieved a pronounced clinical effect and there were no relapses throughout the entire observation period. Conclusion. Subconjunctival injections of triamcinolone acetonide proved to be effective and safe in the treatment of episcleritis resistant to the standard treatment regimen. Side effects of the drug (complicated posterior subcapsular cataract, persistent ophthalmohypertension, etc.) were not detected with this method of treatment. Purpose. The study of the frequency and clinical varieties of epiretinal fibrosis (ERF) in the macula in diabetic retinopathy (DR). Material and methods. Clinical material: 90 patients with DR (90 eyes). 29 eyes had nonproliferative DR, 31 eyes had preproliferative DR, and 30 eyes had proliferative DR. Optical coherence tomography of the macular zone of the retina was performed using the CIRRUS HD-OCT MODEL 500 device. The presence and characteristics of ERF were assessed. Results. The presence of ERF was found in 65 eyes (72.2%). With nonproliferative DR it was found in 10 eyes (32.2%), within the parafoveal zone. At the preproliferative stage of DR – in 25 eyes (80.6%), in the macula – in 16 eyes, in 9 cases – outside the vascular arcades. In proliferative DR, ERF occurred in all 30 eyes (100%). In 15 eyes – ERF without vascularization, in 10 eyes ERF captured the macula and vascular arcades. There were both planar and traction variants (49 and 16 eyes, respectively). Its fixation was the most common in the fovea zone (7 eyes). ERF vascularization was observed in 10 eyes with a pronounced stage of proliferative DR (of these, in 7 cases vascularized ERF was localized in the macular area, in 3 eyes it was localized in the area of vascular arcades). Conclusion. The presence of ERF was detected in the majority of patients with DR of various stages (72.2%), with a traction component it was found in 16 eyes. With the aggravation of the stage of DR, an increase in the frequency of ERF was noted. ERF, as a rule, captured the foveal region, it was characterized by a different area of coverage of the parafovea in cases of accessible visualization of the ocular fundus. The article presents a comparative analysis of data from patients with stage II and III keratoconus (40 patients, 40 eyes) after implantation of intrastromal corneal segments with femtosecond laser and with manual implantation of ICRS. The refractive results gave comparatively similar vision patterns. The advantage of the femtosecond laser was the technical simplicity of the operation. The disease is usually bilateral, but can be asymmetric. Opacities tend to be centrally located, which causes a decrease in visual acuity. The disease affects both sexes, but a higher incidence was found in women. Despite multiple studies, the etiology of the disease has not been determined. The purpose of this work was to study the effectiveness of extraction of a transparent lens in order to correct myopia of medium and high degree. Currently, the correction of high-grade myopia is carried out with the use of phakic intraocular lenses (FIOL), since the correction of LASIK and PRK is not indicated, with a thinned cornea of myopes at a young age. In this paper, the results of cataract phacoemulsification of 20 patients (40 eyes) with moderate and high myopia, with implantation of hydrophobic acrylic IOLS Acrysof (Alcon, USA) in the middle age category (46.6±6.3) were analyzed. with high-grade myopia (M from -7.0D to -19.0D), 14 of them were women and 6 were men. Moderate myopia was detected in 6 patients (12 eyes) and high degree in 14 patients (28 eyes). In the preoperative period, 15 patients (30 eyes) underwent peripheral retinal laser coagulation (30 eyes). Operations were performed on two eyes with an interval of 2-3 weeks, depending on the state of the organ of vision and the general condition of patients. High visual acuity was obtained from 0.6 to 1.0 without correction and averaged =0.83 ±0.05. Relevance. Glaucoma is the leading cause of blindness worldwide. Among all pathological mechanisms, a special role is assigned to glutamate excitotoxicity, which accompanies a number of pathological conditions of the retina. The aim of this work was to study the pharmacological activity of compounds of 3-hydroxypyridine derivatives in a model of NMDA-induced retinal degeneration. Material and methods. The introduction of the compounds was carried out in the prophylactic mode of administration. 7 days after the model, ophthalmoscopic changes in the fundus and the level of microcirculation were assessed. Research results. According to the results of the study, it was found that the compound LHT 6-20 had the highest activity. Compound LHT 6-20 at a dose of 25.5 mg/ kg in a model of neurodegenerative retinal damage induced by intravitreal administration of NMDA in rats with a prophylactic administration regimen provides an improvement in the fundus picture by 15.4%, relative to the group with a pathology model, which has a statistically a significant difference (p < 0.05), an increase in the level of microcirculation in the retina by 14.1% relative to the group with the model, while the indicator has a statistically significant difference from the group with pathology (p < 0.05), but does not differ from the reference drug (p > 0.05). The aim of the work was to study clinical efficacy of micropulsed transscleral diode laser cyclophotocoagulation (MTDСFC) in patients with refractory glaucoma. Material and methods. After careful standard ophthalmologic eye examination, 60 patients underwent mTDGFC to compensate the intraocular pressure (IOP) and stabilize the visual and functional indices. Results. Immediately on the next day after the procedure a brightening of the cornea and a decrease of the degree of perilymphalic congestion were observed. IOP level decreased from 42.0 ± 1.2 to 20.97 ± 1.12 in 47.6% of cases, and in a year – to 18.85 ± 0.7. In 40% of cases (n = 24) the visual acuity improved from 0.03 ± 0.01 to 0.06 ± 0.05 which was statistically significant (p < 0,05). In 16.7% of cases (n = 10) there was an increase in the visual field boundaries by an average of 5 degrees. In 15 eyes (25.0%) it became possible to observe fundus. An important advantage of mTDFC was a 52.4% decrease in the number of hypotensive drugs used by the end of follow-up: from 3.0 ± 0.5 to 1.0 ± 0.5 drugs (p < 0.05). In the early postoperative period subconjunctival hemorrhages in 5 (8.3%) patients, hyphema 1 mm in 1 (1,7%) patient were among the available complications; they dissolved after hematase and dexamethasone injection. Conclusions. mTDFC should be performed in patients at earlier stages of refractory glaucoma, providing compensation for varying degrees of elevated IOP. This procedure contributes to the elimination or significant reduction of pain syndrome, which preserves eye function, improves patients'quality of life, and is an alternative to laser treatment of refractory glaucoma.
