The consensus view that the SUPPORT project “failed” should not obscure the significance of the effort itself—or its importance for chaplains. The author sketches two sets of concerns SUPPORT raises for chaplains and for pastoral care in health care. He describes some distinctive challenges these concerns create for chaplains.

The author discusses factors that impact on involvement in faith communities by people with serious mental illness. She notes recent research documenting the religious resource needs of psychiatric inpatients. A group designed along the psychiatric rehabilitation model which helps services recipients to clarify their values related to faith communities and consider the process of connecting with a community is described. Experience providing the group is discussed.

In contrast to the SUPPORT study findings, a survey of similar end-of-life care issues at Christ Hospital and Medical Center, Oak Lawn, IL showed that intensive educational and clinical efforts to inform hospital staff, patients and families of Advance Directives improved physician-patient agreement and understanding regarding DNR orders. The central role of the chaplains in the process is described.

The author shares her personal experiences providing spiritual direction for patients with mental illness in an acute care hospital setting and in a day hospital. Some practical suggestions for leading spiritual direction groups and for providing individual direction also are offered. The article concludes that incorporation of spiritual direction into the care plan benefits patients for ongoing healing.
