Introduction. Celiac disease is a chronic disease with worldwide distribution. In the person-centered health approach to chronic diseases, the concepts of empowerment and quality of life become relevant. Aim. To determine the degree of empowerment and quality of life of adults with celiac disease in a sample from El Salvador, as well as the correlation between both measures. Materials y methods. Descriptive cross-sectional study. Salvadoran adults with diagnosis of celiac disease confirmed by duodenal biopsy were surveyed online. Sociodemographic and clinical data were required from the participants. A validated questionnaire in Spanish for the empowerment of people with chronic diseases was applied because there is no specific empowerment questionnaire for people with celiac disease. A validated quality of life questionnaire in Spanish for people with celiac disease was also used. Results. Thirty-six people were included in the study, of whom 86.1% were females, with a median time since diagnosis of 7.0 years [5.0-10.0]. Seventy-five percent reported a strict adherence to the gluten-free diet. The median empowerment score was 185.0 [176.25-195.0] out of 235 points and the median quality of life score was 72.5 [54.0-79.75] out of 100 points. The Spearman correlation between empowerment and quality of life was positive and statistically significant (rho = 0.444; p = 0.007). The people evaluated in this study have a good level of empowerment and quality of life. Conclusion. There is a moderate positive correlation between empowerment and quality of life for adults with celiac disease in this study.

Las primeras aplicaciones de este análisis estudiaron el tiempo desde el inicio de un tratamiento hasta la muerte, de ahí el nombre de sobrevida. Posteriormente, su aplicación se extendió a otras situaciones de interés clínico (tiempo a la reinternación, al abandono del tratamiento, al retorno laboral luego de una cirugía, etc.), por lo que una denominación más adecuada es la de análisis de tiempo al evento. Los objetivos del análisis de sobrevida son: a) estimar e interpretar la sobrevida, b) comparar la sobrevida entre distintos grupos y c) evaluar la relación de distintas variables explicativas con la sobrevida.

Introduction. Nutritional guidelines for patients with ostomies are restrictive, and there is no evidence to support these recommendations. Few studies have examined the effects of diet on the ostomy. Aim. To describe the eating behavior and the impact on the lifestyle of patients with ileostomies treated at a tertiary public hospital. Materials and methods. Descriptive, cross-sectional study. A semi-structured questionnaire was conducted in patients with ileostomies for a period greater than three months, hospitalized or outpatients, between October 2020 and May 2021. Sampling was consecutive, for convenience. Data were analyzed using the PASW Statistics 18 software and vccSTAT_3. Results. Forty-two patients were interviewed, 61,9% female, and a median age of 47 years (IQR 33-65). 92,8% received nutritional counseling prior to hospital discharge, and 76,2% had nutritional follow-up limited to the immediate postoperative period. 45.2% fasted for social activities. The foods associated with diarrhea were mate, sugary or carbonated drinks, and candies. Those that provided greater consistency: rice, polenta, noodles, and pasta. 33.3% considered night rest time insufficient. 80.9% described their diet as appetizing. The main reasons for dietary restrictions were a professional indication, diarrhea, and habit. Conclusions. There is a clear change in eating behavior and lifestyle in these patients, with fasting, food restriction, and altered night rest being prevalent.

Hepatitis C virus infection is one of the main causes of morbidity and mortality worldwide. In Argentina, it is estimated that only 50% of infected people are diagnosed; of these, half have confirmed their diagnosis, and only 5% have access to treatment. The COVID-19 pandemic has exacerbated this situation, further decreasing access to treatment. In this context, we developed a program to reconnect and simplify the patient care cascade, in order to facilitate access to diagnosis, treatment, and monitoring of complications associated with hepatitis C virus infection. 24.2% of the re-engaged patients were candidates for treatment, and they received it in only 2 medical consultations, with an average time of 29 days between the initial consultation and the delivery of the medication. The results of our study show that approximately half of the patients with hepatitis C are not followed up by a specialist. Of the patients who had cirrhosis at the time of re-engagement, 60% had mild fibrosis prior to admission to the program. More than 80% of patients with cirrhosis could not be re-engaged. A commitment from the healthcare team is required to facilitate the reduction of the instances necessary to access treatment, and programs focused on education and assessment of the patient's psychosocial environment. This will make it possible to break down existing barriers.

Lemmel syndrome is a rare entity that causes biliary obstruction due to compression of the common bile duct by a periampullary diverticulum. It can manifest as acute cholangitis and is often underdiagnosed. However, once identified, it can be successfully treated with endoscopic retrograde cholangiopancreatography, being one of the most used methods. Lemmel syndrome should be considered in elderly patients with a diagnosis of periampullary duodenal diverticulum, once the presence of choledocholithiasis and pancreatobiliary neoplasms have been ruled out.

Hemangiomas of the small bowel are an uncommon and difficult-to-diagnose cause manifesting with bleeding of the midgut. We report an 18 -year- old male under study due to iron deficiency anaemia. A video-capsule endoscopy was performed, revealing multiple raised lobulated violaceous lesions in the small intestine of 5 to 15 mm diameter, compatible with hemangiomas. Endoscopic therapy was carried out using an injection of 1% polidocanol, achieving the sclerosis of the lesion without complications and the correction of the anaemia.

Chronic intestinal failure due to short bowel syndrome is a severe and complex disease that requires a specialized multidisciplinary treatment. Parenteral nutrition is the fundamental support in these patients; however, in the long term, complications associated with it may develop that contraindicate its continuity. In this context, an isolated or combined intestinal transplant (liver-intestine) is the last alternative to recover intestinal sufficiency. Since 2012 was introduced in the world, the use of semisynthetic GLP2 peptide, called teduglutide, changed the course and prognosis of this disease. Argentina is the Latin American country with the most experience in the use of this enterohormone, with results comparable to those of other international centers. This article summarizes the expert recommendations for the use of teduglutide in patients with chronic intestinal failure due to short bowel syndrome.
