Aim: To describe changes in the quality of life and visual performance of advanced glaucoma patients after lensectomy. Materials and methods: Retrospective cohort study of patients with advanced glaucoma requiring cataract surgery between March and September 2018 at the Glaucoma Service of Hospital Oftalmológico Santa Lucía. Advanced glaucoma was defined as present when there was a cup/disc ratio > 0.8, visual field damage within 10 degrees of central fixation or a temporal visual field remnant. Standard phacoemulsification surgery with intraocular lens implantation was performed. An adapted low-vision quality of life questionnaire was administered before surgery and one month postoperatively. Results: A total of 9 patients (6 women and 3 men) were included. The age range was 29 to 88 (mean: 71 years). Visual acuity improved in most cases and there was an improvement of sensitivity in mean deviation and model standard deviation, as well as in the areas of quality of life evaluated. Conclusion: Cataract surgery should be considered as an alternative in cases of advanced glaucoma and stable intraocular pressure, since a substantial benefit in the function and quality of life of patients has been observed.

Purpose: To evaluate and compare the ocular blood flow in non-glaucoma versus primary open-angle glaucoma (POAG) subjects attending the ophthalmic clinic of University of Nigeria Teaching Hospital (UNTH) Enugu using Doppler ultrasonography. Methods: A cross-sectional comparative study was adopted. Fifty-Eight POAG and 58 non-glaucoma subjects were recruited. Information on their socio-demographic and clinical profiles was obtained via an interviewer-administered questionnaire. Ocular Doppler Imaging was performed on all the Subjects. Peak systolic volume, End diastolic volume, mean flow velocity and Resistive Index (RI) of the central retinal artery and ophthalmic artery were recorded. Data were analysed using the SPSS version 21. Person correlation was used to determine the relationship between the Doppler indices and demographic and clinical variables. The level of significance was at p <0.05. Results: The subjects were mainly males, 71(61.7%), with an age range of 40 to 79 years. The POAG Subjects lived with glaucoma for 5.57±4.2 years, and mainly on medical treatment 52(89.7%) with beta-blockers and prostaglandin analogues. The RI was significantly higher (p=0.01) in POAG subjects. A significant positive relationship (P = 0.014) was observed between RI and intraocular pressure (IOP) of the POAG subjects. Conclusion: In POAG subjects, RI was significantly higher; as the IOP increased there was an increase in RI translating to a sub-optimal ocular blood flow. Interventions aimed at modifying the RI would therefore be synergistic with IOP lowering measures. Additionally, RI assessment may offer a new target for glaucoma case finding.

Episcleritis is an inflammatory condition that affects episcleral tissue underlying the sclera and conjunctiva.1,2 It is a recurrent and mild self-remitting surface disease. Its etiology is unknown but may be associated with systemic diseases.3–5 It has been recognized for over a century as sub-conjunctivitis or subconjuctivitis phlegmatous. The condition is unilateral in two thirds of cases and bilateral in one third. Two major clinical types are noticed diffuse and nodular. The case outlined Nodular episcleritis and its clinical diagnosis and management. The condition was mild and didn’t progress to a more severe disorder. An episcleral nodule adjacent to the cornea is very rare.

Age-related macular dystrophy (AMD) is one of the main causes of vision loss in old age. The etiopathogenesis of AMD has not been fully elucidated, therefore there is no radical treatment of AMD. The proposed hypothesis is that the trigger of AMD may be a genetically determined weakness of accommodation (related to an excessively small distance "ciliary body - lens equator"). It is necessary to carry out objective accomodometry, OCT diagnostics of the distance "ciliary body - lens equator" in older people with healthy eyes and with ophthalmopathology (including AMD). If the hypothesis is confirmed, it is proposed to: a) surgically expand the ring of the ciliary body; b) to form a simple direct myopic astigmatism of up to 1.5 diopters on an artificial eye with AMD (by calculating the optical power of the IOL or modeling the corneal surface with a femtosecond laser).

Purpose: Dacryocystorhinostomy surgery may result in significant postoperative scarring. Although other surgical techniques have been developed that achieve a better aesthetic result, the ab externo technique remains the gold standard for this procedure. We developed a telephone survey to evaluate the visual impact of postoperative skin scars associated with ab externo dacryocystorhinostomy. Methods: 470 patients underwent uncomplicated external dacryocystorhinostomy surgery by two consultant surgeons from January 2014 to December 2021. Skin incisions were closed using intradermic continuous suture with prolene 6.0. The exclusion criteria were cases of dacryo-phlegmon and lacrimal sac fistulae with skin maceration. The visual effect of the postoperative scar was evaluated, and the degree of patient satisfaction was measured by a questionnaire with four subjective perceptions of the scar (very good, good, bad, very bad) given by a physician in the first postoperative month. The survey responses were used as the basis for a literature review. Results: The survey received 470 responses (100%) Very good perception of the scar was achieved in 75,8% of patients. There were no other post-operative complications noted from medical records review. Conclusions: Given the small percentage of patients who complained of marked scarring, it should not be the main reason for deciding on the procedure for dacryocystorhinostomy, especially in elderly patients. Therefore, the surgical technique should be considered as a surgically acceptable choice if cosmetic results were selection criteria in the case of total stenosis of the nasolacrimal duct.

Introduction: Ptosis is a fall of the upper eyelid caused by a defect in the levator. The aim of the study was to describe epidemiological, clinical, and therapeutic features of ptosis in Bukavu. Methodology: This is a prospective study conducted on 77 patients admitted for ptosis in ophthalmology departments of the Panzi Hospital and the CBM-CELPA Ophthalmological Clinic in Bukavu (Democratic Republic of the Congo) during a period of 3 years, from 1 January 2018 to 31 December 2021. Results: The median age at management was 14 years, with a male predominance (57.1%). This was congenital ptosis in 77.9%, and acquired ptosis in 22.1%. Ptosis was major in 24.1%, and moderate in 61.0% of cases. Amblyopia was present in 63.6% of the cases. Resection of the levator muscle of the upper eyelid was the most common surgical technique (56.4%), followed by suspension of the upper eyelid from the frontal muscle (43.6%). Functional and aesthetic results were satisfactory in 17 eyes (30.9%). Among the postoperative complications, we noted the suture thread release (16.4%) and the inflammatory granuloma (12.7%). Conclusion: Ptosis is a pathology encountered in Bukavu. It is unilateral in the majority of cases. Congenital ptosis is the most common form. Surgical treatment gives good results in our context.

Introduction: The objective of this study is to report the results of the management of ptosis in our practice by comparing two techniques, one consisting of the resection of the levator of the upper eyelid and the other of the suspension of the upper eyelid to the frontal muscle. Methodology: This is a prospective study of all consecutive ptosis patients in two ophthalmology departments in Bukavu (in Democratic Republic of the Congo) from 1 January 2018 to 31 December 2021. All patients were operated on by the same surgeon with a minimum of 6 months follow-up post-operatively. Results: A total of 55 patients were treated for ptosis during the study period, including 31 with upper eyelid levator resection and 24 with upper eyelid suspension to frontal muscle. We found that male sex, cosmetic gene, congenital ptosis, left location, moderate degree and isolated ptosis were predominant in both groups of patients, but statistical comparison between these two groups was not significant (p>0.05). For post-operative complications, there was a statistically significant difference between the two treatments (p=0.0002). After treatment, the statistical analysis shows that the median value of visual acuity in patients who underwent upper eyelid levator resection was significantly higher than that in patients who underwent upper eyelid suspension to frontal muscle (p=0.021). Conclusion: The results of this study show that resection of the upper eyelid levator seems to be the best surgical technique compared to the suspension to the frontal muscle in our context.
{"title":"Ptosis surgery in Bukavu, Democratic Republic of the Congo: comparative study of two surgical techniques","authors":"","doi":"10.15406/aovs.2023.13.00434","DOIUrl":"https://doi.org/10.15406/aovs.2023.13.00434","url":null,"abstract":"Introduction: The objective of this study is to report the results of the management of ptosis in our practice by comparing two techniques, one consisting of the resection of the levator of the upper eyelid and the other of the suspension of the upper eyelid to the frontal muscle. Methodology: This is a prospective study of all consecutive ptosis patients in two ophthalmology departments in Bukavu (in Democratic Republic of the Congo) from 1 January 2018 to 31 December 2021. All patients were operated on by the same surgeon with a minimum of 6 months follow-up post-operatively. Results: A total of 55 patients were treated for ptosis during the study period, including 31 with upper eyelid levator resection and 24 with upper eyelid suspension to frontal muscle. We found that male sex, cosmetic gene, congenital ptosis, left location, moderate degree and isolated ptosis were predominant in both groups of patients, but statistical comparison between these two groups was not significant (p>0.05). For post-operative complications, there was a statistically significant difference between the two treatments (p=0.0002). After treatment, the statistical analysis shows that the median value of visual acuity in patients who underwent upper eyelid levator resection was significantly higher than that in patients who underwent upper eyelid suspension to frontal muscle (p=0.021). Purpose: To discuss a case of shared optometric-ophthalmic (O-O) approach in objectively refracting and examining a paediatric high myope under general anaesthesia. Methods: A paediatric high myope was cyclopleged then underwent objective retinoscopy under general anaesthesia within a surgical theatre setting. Corneal diameter, interpupillary distance, immersion biometry axial lengths, intraocular pressures, keratometry readings, A-scan and B-scan ultrasound images were all subsequently obtained. Lastly, binocular indirect ophthalmoscopic examination with 360º scleral indentation was carried out. A comprehensive O-O report was dispensed with all collated parameters. Results: A refractive error of -11.00DS bilaterally was measured. Ocular health assessment showed no significant myopic abnormalities. The patient was dispensed spectacles and adapted well with improved comfort. Low dose atropine myopia control was commenced. Conclusion: O-O shared care in refracting and examining difficult and highly myopic paediatric cases under general anaesthesia is a useful and effective tool.

Purpose: To evaluate the bicanalicular tube structural integrity and the anatomical and functional post operative results of cases of long and very long bicanalicular intubation (from 2 to 13 years). Methods: We studied all the patients of our Department with long-lasting intubation (greater than one year), from 2000 to 2021. We obtained an histocytological evaluation of the material of intubation and a nasal endoscopy. We investigated the causes of the long permanency and the patient tolerability. Results: In our study, 6 eyes from 5 patients (4 women and 1 man) underwent a nasolacrimal duct intubation. The mean intubation permanency time was 6.8 years and the median value was 7 years (minimum 2 years, maximum 13 years). The bicanalicular tube examinations found traces of mucus made of amorphous protein material, sporadic leukocytes and histiocytes. In all the patients the nasal endoscopy showed good conditions of the middle meatus mucosa. Conclusions: In complex cases, such as multiple re-operations, inflamed and small lacrimal sac, fibrotic sac, inflamed nasal mucosa for previous nasal pathologies, a permanent silicone tube could be a reasonable choice as it appears to be well tolerated and without major complication risks.
