High glycemia variability (GV) has a significant impact on the progression of complications in patients with diabetes mellitus (DM). In the course of research, using the GLINVA program based on the data of continuous glucose monitoring, an in-depth analysis of GV was carried out in 12 patients with type 1 diabetes mellitus who were on the insulin pump therapy and had compensation according to the level of glycated hemoglobin. It was found out that even with good diabetes compensation, the GV remains high enough, which can influence the development of complications. The use of variability indices in assessing the glycemic profile in patients with type 1 diabetes mellitus will help in correcting insulin therapy and timely prevention of complications.

The results of the study of the electric pulse therapy effect on the state of neural conductivity and microvasculature in 60 patients with moderate type 2 diabetes mellitus (DM2), accompanied by the peripheral sensory neuropathy, are presented. A comparative double-blind, placebo-controlled study of the inclusion of neuro-like dynamic electrical stimulation in the rehabilitation course was carried out. By the end of the rehabilitation course, the patients of the experimental group showed an increase in the velocity of excitation propagation along the tibial and peroneal nerves, restoration of the impaired spontaneous activity of deafferentated central neurons at the spinal and cerebral levels, and an improvement of basal microcirculation indices.

Purpose of the study. To assess the effectiveness of elimination diet therapy using therapeutic formulas in children with cow's milk protein allergy according to a 5-year follow-up. Materials and methods. A prospective, single-center, open, uncontrolled study was conducted. We observed 99 children with atopic dermatitis and allergy to cow's milk protein (CMP), the average age at the beginning of the study was 4.6 ± 0.8 months. The results of clinical observation, clinical assessment of the function of the digestive organs, and the level of the IgE to CMP were used as control methods. The selection of formulas for clinical nutrition at the stage of elimination diet therapy was carried out differentially, depending on the severity of the skin syndrome and gastrointestinal manifestations. The follow-up period was 5 years. Results. Food allergy manifested itself in 34 (34.3%) children only as a skin syndrome (atopic dermatitis), in 65 (65.7%) children as a skin syndrome in combination with gastrointestinal symptoms. In 67.7% of children, the use of therapeutic formulas based on highly hydrolyzed cow's milk protein and amino acids led to the normalization of the level of specific IgE. In mild atopic dermatitis, the average duration of the elimination period was 9.43 ± 2.7 months, in moderate atopic dermatitis, 19.3 ± 9.0 months, and in severe, 35.5 ± 14.8 months (p < 0.05). The number of patients with an exacerbation of an allergic disease with of the introduction of dairy products in mild, moderate and severe form of atopic dermatitis constituted 2 of 7, 6 of 48 (12.5%), 6 of 12 (50.0%) respectively (p < 0.05). Conclusion. In children with atopic dermatitis caused by food allergy to cow's milk proteins, the elimination diet in most cases (67.7%) leads to the normalization of the level of specific IgE, which makes it possible to start expanding the diet by introducing dairy products. The duration of the elimination period depends on the clinical manifestations of the allergic disease and the level of sensitization. When expanding the diet, patients should be provided with careful clinical and laboratory monitoring, including the determination of specific IgE to CMP. The high probability of persistent sensitization to CMP must be taken into account when prescribing elimination measures and drawing up rehabilitation programs for children with allergic diseases.

A retrospective analysis of 49 cases of the initial hospital admission on account of chemical burn of esophagus to the Children's City Hospital No. 1 during the period from 2011 to 2015 was carried out. Patients were divided into 2 groups depending on the chemical agent: group 1 of 25 children (48,9 %) with burns of the esophagus with 70% vinegar essence, and group 2 of 24 patients, in whom burns were caused by alkaline solutions of household chemicals. Statistically significant difference between groups was found in the bed-day: in group 1, the duration of stay was 29,9 ± 16,4 days, and in group 2 — 45,2 ± 27,6 days. And also in the number of bougienages: over the 1st year, in group 1 the median was 3 bougienages per year, with the maximum value — 21, minimum — 1; in group 2, the mediam was 7 bougienages per year, with the maximum value being 24, and the minimum — 2 procedures. Thus, chemical burns of the esophagus remain an urgent problem of childhood. The nature of the lesion and the time of rehabilitation depend on the chemical agent.

The study was carried out in 192 patients aged 18–25 years who underwent orthodontic treatment of dental-maxillary anomalies. We singled out 3 groups of patients who, after the orthodontic appliances being put on, were implemented various preventive measures against the inflammatory process. In the period before treatment, after 7 days, after 1, 3 and 12 months in the mixed saliva, the activity of lysozyme, the level of secretory immunoglobulin A and interleukins were determined. Measurements were performed with a vertical spectrophotometer Multiscan MCC 340. In all groups of patients, there was a gradual increase in the indicators of lysozyme activity, the level of secretory immunoglobulin A and interleukins; so by 3 months and by 1 year they came to their initial values. Rapid normalization of these indicators was noted in the third group of patients, where a full range of preventive measures was carried out using professional hygiene, antibacterial and immune therapy. The use of the appliances for orthodontic treatment is accompanied by the inflammatory process. The combined use of oral hygiene, antibacterial and immune therapy is most indicated in the prevention of the inflammatory process in the treatment of dental-maxillary anomalies.

Brain damage can be observed at any stage of HIV infection, and the spectrum of its causes is extremely diverse. Neuroimaging techniques play a key role in assessing brain lesions. The article describes a clinical case of multifocal brain damage in a patient with undiagnosed HIV infection, discusses the possibilities of various methods of radiation examination for differential diagnosis, demonstrates the most important role of a comprehensive clinical examination for making a diagnosis.
