Uveal melanoma is the most common primary intraocular malignant tumor among the adult population, its prevalence is more than 85% in the structure of all melanomas. Worldwide, the annual incidence of uveal melanoma is 6 cases per 1 million people. Despite the ongoing search for an effective treatment for uveal melanoma, there has not been a significant increase in the 5-year survival rate (~80%) in recent years, and approximately 50% of patients develop metastatic disease. The aim of this research was to analyze the clinical case of a female patient with metastatic uveal melanoma diagnosed with a metastatic lesion 28 years after enucleation. Modern possibilities of complex diagnostics using clinical, histopathological, cytogenetic and immunohistochemical markers determine the possibilities of performing polychemotherapy and determine the prognostic criteria for the life expectancy of patients.

With different variants of cognitive load, the corresponding regions of the brain are activated, which is manifested by peculiarities of heart rate variability (HRV). Cognitive load (semantic fluency) contributes to a decrease in the activity of the Total Power (TP) integral indicator in the group as a whole. The use of the autonomic balance index (ABI), which is an objective indicator of the autonomic mechanisms of HRV regulation, reveals that this load is most sensitive for young men, who have more vulnerable parasympathetic NS, while girls do not have such a reaction. When performing the semantic fluency test under conditions of cognitive load (reverse counting), an increase in the TP index was noted. When comparing ABI between groups of boys and girls before the exercise, a significantly higher ABI in girls is noted, that is initially the contribution of the parasympathetic NS to the regulation of HRV is of predominant importance. After the exercise, the ABI in girls decreases which indicate the predominant role of the sympathetic NS as a mechanism for regulating the HRV in response to cognitive load — the ability to count. The development of an imbalance between the sympathetic and parasympathetic NS activity under conditions of cognitive load reveals the most vulnerable regulatory circuits, which may become the basis of the corresponding pathological process in the future.

The paper presents the results of studying the effect of electrical impulse therapy on the psychoemotional state in 60 patients with moderate type 2 diabetes mellitus with peripheral sensory polyneuropathy. A comparative double-blind, placebo-controlled study of the inclusion of neuro-like dynamic electroneurostimulation in the rehabilitation course was carried out. By the end of the rehabilitation course in patients of the experimental group, a decrease in the initially high indicator of situational (reactive) anxiety according to the the Spielberger — Khanin State Trait Anxiety Inventory (STAI) by 1,6 times (p = 0,014) and an improvement in the functional state of the central nervous system according to the WAM test were recorded.

Introduction. Methods of radiologic imaging have taken a leading place in the assessment of damage to the roots of the cervical and brachial plexuses. One of the variants of damage is avulsion injury with partial or complete detachment of the nerve root at the place of its exit from the spinal cord. The leading position in the visualization of such lesion, until recently, was occupied by computed tomographic myelography. At the same time, it has a number of disadvantages, and therefore the search for alternative imaging methods remains relevant. Aim. To study the opportunities of assessing avulsion damage to the roots of the cervical and brachial ple¬xuses in avulsion injuries in patients with traction injury using 3D-CISS technique in comparison with CT-myelography. Materials and methods. The study included 50 patients of both sexes hospitalized at the Federal State Budgetary Institution "Federal Neurosurgical Center " of the Ministry of Health of Russia with avulsion injuries in the roots of the cervical or brachial plexuses of traction origin. The studies were performed on MRI scanners with a magnetic field strength of 1.5 Tesla using the 3D-CISS MRI technique and CT-myelography in a neurosurgical hospital using a Siemens SOMATOM Definintion AS spiral tomograph. The patients underwent surgical reconstruction of the avulsion lesion of the nerve roots. Results. Features of injuries to the nerve roots of the cervical and brachial plexus: possible visualization of pseudomeningocele with CT-myelography and 3DCISS technique, visualization of an interrupted root course with 3D-CISS technique. With the 3DCISS technique, the data corresponded to intraoperative data more often than with CT-myelography. The results of the MRI method using the 3D-CISS technique are more reliable in comparison with CT myelography in the assessment of pathological changes in the nerve roots. Conclusion. 3D-CISS is a more effective technique for visualizing nerve root avulsion injury compared to CT-myelography.

As part of a prospective study, it was evaluated the efficacy and safety of the combined use of drugs from the group of symptomatic slow acting agents (SYSADOA): Diacerein and glycosaminoglycan peptide complex (GPC) in patients with knee osteoarthritis. The dynamics of pain according to Visual Analoguе Scale (VAS), need for non-steroidal anti-inflammatory drugs, the WOMAC index, EQ-5D score, and OMERACT-OARSI criterion, the presence of adverse events were analyzed, and the effect of Diacerein on the components of the metabolic syndrome was assessed. It was shown that the concomitant use of GPC and Diacerein demonstrated high efficiency and equivalent safety in comparison with the isolated use of GPC: a decrease in the pain intensity in the early stages of therapy, a decrease in the need for NSAIDs, a tendency to improve lipid and purine metabolism.

Currently, the relevance, and at the same time the interest of researchers, to such a psychiatric disease as post-traumatic stress disorder (PTSD) has greatly increased. Socio-economic, environmental, anthropogenic, epide¬mic events can become the very causative factor of sufficient intensity that will trigger the pathogenesis of PTSD. In the era of globalization, the social significance of this disease is higher than ever. If the diagnosis, treatment and rehabilitation of victims are not carried out in a timely manner, then the likelihood of chronic PTSD symptoms with a gradual pathological transformation of the patient's personality becomes high. These pathological personality changes are not only a heavy burden for each patient and his environment but also additional healthcare expenses for rehabilitation. At the moment, various treatment regimens have already been developed, but none of them is universal and comprehensive. New therapeutic approaches are needed that can provide patients with a stable remission in the shortest possible time preventing their maladjustment in society. In addition, the problem of compliance in such patients, as well as the presence of resistant forms of PTSD comorbid with other psychiatric conditions is quite acute.

A preliminary experimental study: Measurement of exocrine pancreatic secretion was performed on Wistar rats after an induction of acute pancreatitis by ‘closed duodenal loop’ (CDL) method (total number of animals = 39). After 24 hrs duration of AP the CDL content was evacuated by syringe and tested for amylase level (IU/ml). A near normal output remained in most animals (1987,35 ± 558,26, n = 12); moderately decreased output was 264,67 ± 162,74 (n = 9), and cessation of the output in n = 3 (0,6 ± 0,3) (q < 0,01 for each group). So, in our series the exocrine pancreatic output was found diverging largely with a normal secretion prevailing. Further study is warranted for some conclusive results to obtain.

Introduction. On March 11, 2020, WHO declared a pandemic of a new coronavirus infection. In addition to the pandemic's high mortality worldwide, its deferred effects are receiving increasing attention. With the accumulation of research data, it became obvious that a new coronavirus infection is capable of inducing both direct and indirect effects on the nervous system. Aim. The study of the clinical and epidemiological characteristics of neurological variants of post-covid syndrome using the example of the neurological clinic of the State Budgetary Healthcare Institution Novosibirsk Regional Clinical Hospital for the period from July 2020 to February 2022. Materials and methods. A retrospective analysis of 1 500 case histories of patients with a past history of COVID-19 admitted with a manifestation of various neurological diseases for the period from July 2020 to January 2022. In addition to the manifestation of neurological diseases , we took into consideration gender, age, concomitant diseases, and duration of a new coronavirus infection. Results. Of the analyzed case histories with laboratory-confirmed COVID-19 and overt neurological conditions, 30 people were identified. Concomitant neurological disorders with a predominant lesion of the peripheral nervous system accounted for 23% (according to the type of Guillain-Barré syndrome — 16%, the remaining 7% had symptoms of chronic demyelinating polyneuropathy), the central nervous system — 74% (isolated cases of meningoencephalitis, autoimmune encephalomyelitis, ischemic stroke; up to 60% of encephalopathies of various origins), 3% accounted for musculoskeletal pathology, represented by idiopathic myodystrophy. Conclusion. Thus, no significant differences were found between the results obtained and the data of foreign and domestic colleagues. The precise data on frequency of neurological variants of post-COVID syndrome are currently unknown, however, there is a tendency that patients who have had a severe form of COVID-19 are more prone to the development of neurological complications after infection in the period up to 6 months.

With the accumulation of research data, it became obvious that a new coronavirus infection is capable of inducing both direct and indirect effects on the nervous system. Aim. The study of the clinical and epidemiological characteristics of neurological variants of post-covid syndrome using the example of the neurological clinic of the State Budgetary Healthcare Institution Novosibirsk Regional Clinical Hospital for the period from July 2020 to February 2022. Materials and methods. A retrospective analysis of 1 500 case histories of patients with a past history of COVID-19 admitted with a manifestation of various neurological diseases for the period from July 2020 to January 2022. In addition to the manifestation of neurological diseases , we took into consideration gender, age, concomitant diseases, and duration of a new coronavirus infection. Results. Of the analyzed case histories with laboratory-confirmed COVID-19 and overt neurological conditions, 30 people were identified. Concomitant neurological disorders with a predominant lesion of the peripheral nervous system accounted for 23% (according to the type of Guillain-Barré syndrome — 16%, the remaining 7% had symptoms of chronic demyelinating polyneuropathy), the central nervous system — 74% (isolated cases of meningoencephalitis, autoimmune encephalomyelitis, ischemic stroke; up to 60% of encephalopathies of various origins), 3% accounted for musculoskeletal pathology, represented by idiopathic myodystrophy. Conclusion. Thus, no significant differences were found between the results obtained and the data of foreign and domestic colleagues. Despite the high prevalence of basal carcinoma (BCC) of skin, the metastatic form is extremely rare. The development of metastasis largely determines the clinical course, the prognosis of the disease, the choice of treatment methods and, given the poor prognosis in patients with this form of BCC, confirms the relevance of the topic. The management of such patients requires a clinically applicable classification of risk factors for BCC metastasis which can help a specialist to form an assessment of the disease severity and obtain a prognosis of its course. However, due to the rare occurrence, the study of the treatment and diagnosis of metastatic form of BCC is difficult. This article describes a clinical case of lung metastases in BCC, as well as an analysis of such cases on the basis of the Novosibirsk Regional Clinical Oncology Dispensary from the standpoint of the most relevant concepts of risk factors for BCC metastasis.

Introduction. The study of the literature indicates that the list of indications for laparoscopic nephropexy (LNP) in patients with nephroptosis meets the requirements of urological practice, but contraindications to this operation do not correspond. Aim of the research. Based on the modern approach to determining contraindications for LNP, to develop a rational treatment strategy for performing this operation in patients with nephroptosis, aimed at reducing the frequency and severity of complications. Materials and methods. The study included the results of examination and treatment of 208 patients with stage 2–3 nephroptosis. In 1987–2001 103 patients (control group) underwent nephropexy according to the Pytel-Lopatkin method, taking into account the contraindications to nephropexy existing at that time. In 2002–2012 62 patients (main group A) underwent LNP using a polypropylene mesh according to our patented method. At the same time, LNP was contraindicated in 43 outpatients (main group B), taking into account the author's list of contraindications. Results. The use of the author's list of contraindications made it possible to establish among the patients of the main group B absolute contraindications to LNP in 9.3%, relative – in 37.2%, temporary – in 44.2%, special contraindication to LNP – in 4 (9.3%), which contributed to the prevention of possible postoperative complications among them, and among patients of the main group A, compared with the control group, there was a statistically significant reduction in postoperative complications. Conclusion. Therapeutic tactics based on the author's list of contraindications is that the patients with absolute contraindications to LNP are recommended conservative treatment; with relative contraindications – when transferring the concomitant diseases from subcompensation to compensation and with temporary contraindications – in the treatment of these diseases, LNP is performed. And in patients with special contraindication to LNP (abdominal adhesive disease), open nephropexy is performed. A new list of contraindications to LNP in patients with nephroptosis can be recommended for wider implementation in urological practice, which may contribute to improving the quality of treatment of these patients.
