A collaborative approach provides complementary support for people suffering from obesity. The process of change requires them to set their physical being in motion, by making decisions and practicing physical activity. The latter fosters physical and psychological well-being, as well as (re)connecting with the body through perception and sensation.

Faced with the rise in obesity, health inequalities, the complex and often serious consequences of this disease in adulthood, and the stigma attached to those who suffer from it, the way in which care is organized is changing. However, the spread of innovations is still limited.

Obesity is a public health problem. When associated with an eating disorder, such as Binge Eating Disorder, individuals respond less well to conventional treatment. Often linked to psychiatric comorbidities, obesity requires integrated care to improve quality of life and prevent disorders from worsening. Identification tools are available, and multidisciplinary support is essential in these complex situations.

The primary care pathway for adult obesity, a multifactorial chronic disease, emphasizes initial multidisciplinary assessment, therapeutic patient education and the key role of advanced practice nurses. The "Obesity care pathway management" experiment is presented as an innovative approach to overcoming organizational and financial obstacles, thereby improving the coordination and quality of primary care.

The prevalence of obesity continues to rise in France, particularly at the expense of the most severe forms, which are managed as level 3 referrals by specialized obesity centers as part of their M1 mission. As part of their M2 mission, they structure and coordinate obesity care networks in conjunction with level 1 and 2 referral services and the various medical, paramedical and medico-social players involved in coordinated care pathways. The specialized obesity centers are also involved in the ongoing training of healthcare professionals in the prevention, screening and/or management of childhood, adolescent and adult obesity, and must make this care offer clear and accessible.

Obesity today represents a major public health challenge, closely associated with numerous chronic comorbidities and a graduated interdisciplinary therapeutic approach. Precise phenotyping of affected patients is essential to adapt management. The identification of distinct phenotypes opens up new perspectives for precision medicine and optimizes therapeutic strategies. How can digital phenotyping overcome some of the current obstacles in obesity care pathways?

The situation regarding obesity and overweight in Guadeloupe is alarming: statistics show that 52% of adults are affected, more than in mainland France (47%). Primary care teams are integrating this fact into their practices, in terms of screening, care and referral. A multidisciplinary approach is essential to support patients, integrating their commitment and acceptance to follow a specific pathway. A structured outpatient program has been designed by a multi-disciplinary team to provide a local response to needs and to practice "outreach".

The French healthcare system is in the throes of a deepening crisis, affecting all sectors. Despite spending 12% of gross domestic product, healthy life expectancy is making little progress, chronic diseases affect 35% of the population, and health inequalities are widening. A transformation is needed, based on prevention, the restructuring of front-line care and the training of nurses specialized in public health, essential to improving the health of populations.

Health promotion and education in nursing are part of a global approach aimed at empowering individuals and communities to take charge of their own health. The former, defined by the 1986 Ottawa Charter, integrates prevention, protection and education. The second, essential in schools and hospitals, improves patients' autonomy and quality of life.
