Summary and analysis of national-wide γ-spectrometry assessment for radiological health institutions from 2015 to 2017 2015 - 2017年全国放射卫生机构γ谱评估总结与分析

Objective To standardize the operation and data processing in radiological health institutions in China, in order to promote the assessment of γ-spectrometry, improve the measurement and analysis capabilities of each laboratory, and accordingly improve accuracy of environment sample measuring. Methods By collecting the values of 232Th, 40K and 137Cs contents in soil samples from 2015 to 2017, the assessment results of γ-spectrometry laboratores were summarized and analyzed. Results The technical institutions from 25 provinces were involved in the assessment until 2017, including disease control and prevention centers, entry-exit inspection and quarantine agencies, research institutes, universities and companies. The number of participants was increased from 40 to 48, and the qualified rate exceeded 90%, and the excellent rate was no less than 11% each year. The percentage of relative deviation less than 10% between the measuring result of 232Th, 40K and 137Cs contents and the reference value was larger than 80% in 2017. The accuracy became better. However, the number of institutions with U and Z score less than 1 was increased from 11 to 25, suggesting that the precision of measuring result has also been greatly improved. Conclusions Radiological health institutions in China have good measurement and analysis capabilities with regard to γ-spectrometry, which can meet the needs of environmental sample measurement and in-food radionuclide risk monitoring in an emergency of nuclear and radiation. But a few institutions′ measuring results were largely different from the reference values, as a consequence, it is important to strengthen their ability of measurement and analysis. Survey of paediatric CT practice in Shanghai from four hospitals during 2011-2014 2011-2014年上海市四家医院儿科CT诊疗情况调查

Objective To study the the application characteristics of paediatric CT examination by investigating four main children′s hospital of Shanghai. Methods All the CT scan cases of paediatric patients in this work were obtained from the radiology information system (RIS) database in four hospitals between 2011 and 2014. The distribution of using CT scans was analysed according to the age, gender, anatomical regions and the duplicated cases of the patient, then estimated the paediatric CT scan rates of four hospitals in Shanghai. Results Retrospective review of RIS showed 249 350 CT examinations from 175 672 young patients were performed between 2011 and 2014 in the surveyed hospitals. More males (62%) were scanned than females (38%). The majority of children receiving the examinations were 1 to 5 years old, which amounted to 40.3% in all. Cardiac CT accounted for 14.5% among all children of ≤1 year old CTs and accounted for 63.0% among all cardiac CT scans. There were 33 634 (19.1%) individuals who had received 2 or more CT scans during the four years. The estimated paediatric CT scan frequencies of four hospitals in Shanghai from 2011 to 2014 were 37.9 examinations per 1 000 population in 2011, 43.3 examinations per 1 000 population in 2012, 43.3 examinations per 1 000 population in 2013 and 39.0 examinations per 1 000 population in 2014. Conclusions The CT scan frequencies of four hospitals in children and young adults was almost keeping stable in Shanghai. Key words: Paediatric; CT examination; Frequency; Shanghai

Construction and application of food radionuclide monitoring information system 食品放射性核素监测信息系统的建设与应用

Objective To establish a nationwide food radionuclides monitoring network to improve the national food radionuclides monitoring efficiency. Methods The system includes a nationwide food radionuclides monitoring information system based on the existing National Radiation Health Information Platform. The system consists mainly of information filling, data review, retrieval query, graphic report and data export. Results Since 2016, a network submission method was used for the first time in the national food radioactive material risk monitoring. The system improved the timeliness and validity of data reporting and provided a more effective means than before for the normative management and scientific application of data. Conclusions The national food radionuclides monitoring network was successfully constructed and in use. The system has provided a good information support for monitoring the food radionuclide levels in China. Key words: Food; Radionuclides; Network reporting; Food safety; Data platform

Construction and application of national radiological health information platform 国家放射卫生信息平台的建设与应用

Objective To construct a national radiological health information platform, in order to collect and analyze radiological health monitoring and survey data, so as to improve data utilization and ensuring technology support to radiological health. Methods In accordance with China′s relevant laws, regulations and regulations, a national radiological health information platform was designed, developed and operated in response to the needs of radiological health monitoring, scientific research and management. Results The system was initially established in 2015, consisting of five business modules: medical radiation protection, personal dose, occupational health, food radioactivity monitoring and occupational radiological disease reporting. In the past 4 years, the system has been running normally and 5 business databases have been established. More than 8 million data of monitoring and survey have been collected. More than 1 400 terminal users have good experiences and frequent interactions. Conclusions The successful construction of the national radiological health information platform not only promotes and standardizes the network direct reporting of radiological health monitoring and investigation business data, but also provides analytical and utilization tools for technical institutions/personnel on radiological health, and gradually becomes an important scientific basis and database for the development of radiological health laws, regulations and standards in China as well as for national health service. Current status and future perspectives of radiological health informatization in China 中国放射卫生信息化的现状与展望

A decade saw the construction of radiological health informatization in China since the application of the national individual dose registration system in 2009. The national radiological health information platform has been established, covering 5 information systems including individual dose, occupational radiological diseases and occupational health, medical radiation protection, food radionuclide monitoring and occupational radiological disease reporting. Up to 2018, more than 8 million pieces of monitoring data have been collected, and more than 1 400 users have been authorized. In this paper, the current situation of radiological health informatization construction in the past 10 years in China was analyzed. The key points and problems about the development of radiological health informatization were discussed and the suggestions were put forward, in order to promote the construction of radiological health informatization, prove occupational radiological prevention and control, and contribute to the construction of "Healthy China" . Study on informatization pattern of medical exposure frequency and dose monitoring 医疗照射频率与剂量监测信息化模式研究

Objective To realize the efficient monitoring of frequency of medical exposure and dose monitoring in Ningbo by means of informatization. Methods Data transmission process and the key elements of frequency of medical exposure and individual dose were investigated. According to standardization principles of data elements, the standardized data elements for frequency of medical exposure and individual dose were determined. On this basis, modules and functions of data receiving platform on the frequency of medical exposure and individual dose in Ningbo were designed. Results Totally 59 data elements on frequency of medical exposure and individual dose were formed. According to their functions, the data receiving platform was divided into the distribution of radiodiagnosis and radiotherapy resources, the frequency monitoring of medical exposure and dose monitoring for permanent residents, and the dose monitoring. Conclusions The establishment of frequency and dose information monitoring platform on medical exposure in Ningbo and the determination of standardized data elements will be conducive to further progress in medical exposure information monitoring in Ningbo. According to standardization principles of data elements, the standardized data elements for frequency of medical exposure and individual dose were determined. On this basis, modules and functions of data receiving platform on the frequency of medical exposure and individual dose in Ningbo were designed. \u0000 \u0000 \u0000Results \u0000Totally 59 data elements on frequency of medical exposure and individual dose were formed. Application of upper limbs-raised position in CT examination of airway foreign body in children 上肢抬高位在儿童气道异物CT检查中的应用

Objective To confirm the effect of the relative position between upper limbs and cervicothoracic vertebrae on image quality and radiation dose while performing airway CT examination of children with airway foreign bodies. Methods One hundred and three children, aged from 1 to 48 months were enrolled prospectively and divided into 3 groups according to the position of upper limbs.In group A, the upper limbs were raised beside the cervical vertebra, in group B, the upper limbs were placed beside the thoracic vertebra, and in group C, the upper limbs were raised about 45 degrees with sponge pads and placed beside the chest. Data were acquired by Philips 128iCT, and CT dose index (CTDIvol) and dose-length product (DLP) were recorded and compared among three groups. The image qualities were evaluated subjectively and objectively. Results Subjective score of cervical image quality in group B (4.33±0.72) and group C (4.19±0.63) were higher than that in group A (3.71±0.66), and the difference was statistically significant (Z=-3.19、-2.61, P 0.05). Conclusions Raising upper limbs 45 degrees and placing them beside the chest will improve the CT image quality of airway and reduce the radiation dose. 目的探讨小儿气道异物行气道CT检查时,上肢与颈胸椎的相对位置对图像质量和辐射剂量的影响。方法前瞻性入选1 ~ 48月龄儿童103例,按上肢位置分为3组。A组上肢抬高至颈椎旁,B组上肢抬高至胸椎旁,C组上肢用海绵垫抬高约45度,置于胸部旁。采用Philips 128iCT采集数据,记录三组CT剂量指数(CTDIvol)和剂量长度积(DLP)并进行比较。对图像质量进行了主客观评价。结果B组和C组宫颈图像质量主观评分(4.33±0.72)高于A组(3.71±0.66),差异有统计学意义(Z=-3.19, -2.61, P 0.05)。结论上肢抬高45度,置于胸部旁,可提高气道CT图像质量,降低辐射剂量。关键词:气道异物;上肢;辐射剂量

Introduction to national occupational radiation disease reporting system 国家职业辐射疾病报告系统介绍

Objective To establish the national occupational radiation disease reporting system and learn about the incidence of occupational radiation sickness in radiation workers. Methods In accordance with the relevant regulations, standards and literatures available in China, the system was designed and implemented to survey the requirements of diagnosis institutions of occupational radiation sickness and administrative departments. Results The system was initially established with expectant functions. The system functional modules consisted of case reporting, case information enquiry, statistical analysis and system management. The diagnosis institutions of occupational radiation sickness had used the system to report the cases diagnosed in 2017. Conclusions The system achieved the standardized case reporting for the institutions of occupational radiation sickness, and provided scientific data for the health surveillance of radiation workers and decisions of governments. 目的建立全国职业性辐射疾病报告制度,了解辐射工作人员职业性辐射疾病的发病情况。方法根据国内相关法规、标准和文献资料,设计并实施本系统,对职业放射病诊断机构和管理部门的要求进行调查。结果系统初步建立,功能完备。系统功能模块包括案件报告、案件信息查询、统计分析和系统管理。职业放射病诊断机构已使用该系统对2017年诊断病例进行了报告。结论该系统实现了职业放射病机构病例报告的规范化,为辐射工作人员健康监测和政府决策提供了科学依据。关键词:职业性放射病;职业健康监测;注册系统

Study on the conversion between CT hounsfield units and relative stopping power in proton therapy and the improvement in adipose tissue 质子治疗中CT霍斯菲尔德单位转换及相对停止力的研究及脂肪组织改善

Objective To introduce a method for calibrating the conversion from CT Hounsfield units (HU) to relative stopping power (RSP) for proton therapy, and improve the precision of the conversion in the region for adipose tissues. Methods The HU and RSP values of human tissues were calculated by a stoichiometric calibration method. Animal tissue was used to simulate subcutaneous adipose tissue of patients, and the HU and RSP of the animal tissue were measured. The effect of subcutaneous adipose tissue on conversion between HU and RSP were analyzed by piecewise fitting. Results The precision of conversion curve was improved significantly with the measured HU and RSP of adipose tissue in the fitting. The effect caused by different choice in different ionization energy was less than 0.6%, and the effect of proton energy differential was less than 0.8%. Conclusions The precision of conversion curve for the transformation of HU into RSP in adipose tissues could be improved by taking subcutaneous adipose tissue into account, which would reduce the range error of proton beams when such tissues are present in the target volumes or in the beam path. Prognostic analysis of radical radiotherapy for 211 cases of cervical cancer 宫颈癌根治放疗211例预后分析

Objective To investigate the curative effect and prognostic factors of radical radiotherapy for cervical cancer. Methods A total of 211 patients with stage ⅠA-ⅢB cervical cancer who underwent therapy in department of radiotherapy, Tumor Hospital of Jilin province between June 2014 and February 2017, were analyzed retrospectively. All patients received radical radiotherapy with or without concurrent chemotherapy. Short-term and long-term efficacy and related prognostic factors were observed. Kaplan-Meier method was used for survival analysis, Log-rank test was used for univariate analysis, and Cox proportional hazards regression model was used for multivariate analysis. Results The 2-year overall survival (OS) and disease free survival (DFS) were 83.4% and 72.5%, respectively. During the follow-up periods, 46 patients (21.8%) died, including two from non-tumor-related diseases, and one from second primary colon cancer. Totally 57 patients (27%) had recurrence and metastasis, including 16 (28.1%) with local recurrence, 27 (47.4%) with distant metastasis, and 14 with local recurrence and distant metastasis(24.6%). Univariate analysis showed that 2-year OS and DFS were significantly correlated with pathological type, pre-treatment squamous cell carcinoma antigen (SCC) value and FIGO stage (OS: χ2=7.123, 6.014, 8.398, P<0.05; DFS: χ2=11.832, 8.003, 7.731, P<0.05). In addition to the above factors, 2-year DFS was also associated with pelvic lymph node metastasis (χ2=9.286, P<0.05). Multivariate analysis showed that pathological type, pre-treatment SCC value and FIGO stage were independent prognostic factors of OS(HR=2.963, 2.473, 2.574, P<0.05). The independent prognosis factors affecting DFS included pathological type, pre-treatment SCC value and pelvic lymph node metastasis (HR =3.014, 1.988, 1.914, P<0.05). Conclusions By means of radical radiotherapy, cervical cancer patients with adenocarcinoma, pre-treatment SCC levels ≥30 ng/ml and advanced stage have poor prognosis, so more active treatment strategy should be adopted.
