What role do environmental factors play in the development of neurodegenerative diseases? A narrative review 环境因素在神经退行性疾病的发病过程中扮演了什么角色?叙述性综述

Introduction and purposes: Degenerative diseases of the nervous system, such as Alzheimer's disease (AD), Parkinson's disease (PD), amyotrophic lateral sclerosis (ALS). Environmental influences are significant contributors to the development of these conditions. The primary objective of this research was to examine key environmental factors that are suspected of their impact on the development of diseases, specifically lifestyle, medical conditions, diet and exposure to environmental pollution. Materials and Methods: A systematic literature search was conducted using PubMed and Google Scholar. The search employed terms such as 'Alzheimer's disease,' 'Parkinson disease,' 'amyotrophic lateral sclerosis,' 'environment,' 'pollution,' 'diet,' 'traumatic brain injury,' 'alcohol,' 'obesity,' 'lifestyle factors'. The search included articles published from 2014 to 2024. Results: Among environmental factors with regard to lifestyle choices, a healthy diet has a protective effect against these diseases, while alcohol consumption was not clearly categorised as harmful or protective. Among past illnesses, an increase in AD, PD after head injury was confirmed, but more research is required as to the impact on ALS incidence. Lipid metabolism disorders affect ALS morbidity, for the AD and PD, studies do not reach consensus. Similarly, with regard to body weight, studies do not clearly indicate whether it is relevant. On the contrary, when it comes to pollution, a lot of studies showed a correlation. Conclusion: In summary, research studies varied in size and quality, so caution must be exercised in drawing conclusions. This review suggests that there is a relationship with the environment, and future research may focus on exploring these connections.

Introduction and purpose Bruxism is a stereotypical motor disorder characterized by involuntary grinding or clenching of the teeth during sleep or wakefulness. Bruxism affects 5-50% of children, and its frequency decreases with age. Bruxism is the most commonly occurring parasomnia, which in healthy individuals is a variant of physiology; however, when it leads to the development of associated pathologies such as dental abrasions, tooth loss, toothaches, headaches, or temporomandibular disorders, it is an indication for treatment. Materials and methods The literature included in the PubMed databases is searched through the words such as bruxism, temporomandibular disorder. Description of the state of knowledge In the diagnosis of bruxism, questionnaires filled out by patients and parents of children are most commonly used, although the most effective but less accessible diagnostic test is polysomnography. In the treatment of bruxism, pharmacotherapy, occlusal splints, and physiotherapy are used. Conclusions There is no gold standard for treating bruxism. There is still a need to search for other methods that would eliminate bruxism and prevent the development of associated pathologies.

Introduction and Purpose Hashimoto’s disease is the most common reason of hypothyroidism. Usually, it is easy to recognize and treat. However, under certain circumstances, it may develop into the most severe manifestation of hypothyroidism known as myxedema and can lead to a threat to the patient's life. Hypothyroid crisis, or myxedema coma, is an extremely rare but fatal medical emergency. If the condition is not promptly diagnosed and treated, the mortality rate is high as 50%. Even with immediate recognition and appropriate medical intervention, mortality rates of up to 25% have been observed.  This article aim is to enhance medical community awareness by examining features of myxedema, prevalence, and associated risk factors, contributing to improved clinical management. Emphasizing tailored treatment strategies for hypothyroidism, coexisting illnesses, and metabolic decompensation, it ultimately seeks to enhance outcomes for patients with severe hypothyroidism. Material and methods Conducting a systematic review of medical articles from 1997 to 2024 using PubMed, this study analyzed articles with keywords such myxedema coma, hypothyroidism, Hashimoto’s thyroiditis. Inclusion of pertinent articles ensured a comprehensive exploration of myxedema coma literature during the specified time frame. Summary Myxedema coma is a thyroid emergency linked to hypothyroidism. This review sheds light on the etiology of myxedema, clinical manifestations, diagnostic criteria and comprehensive treatment options. In order to reduce mortality in patients, there is a need to increase knowledge of the risk of development, clinical features and management not only for emergency physicians but also for physicians of all specialties..

Severe case of antiphospholipid syndrome coexisting with systemic lupus erythematosus with major cardiovascular complications: a case report 抗磷脂综合征与系统性红斑狼疮并存的严重病例,伴有严重的心血管并发症:病例报告

Systemic lupus erythematosus (SLE) and antiphospholipid syndrome (APS) are autoimmune diseases that frequently coexist, complicating clinical management due to the compounded risks of systemic inflammation and thromboembolism. We present a case of a 52-year-old female with SLE and APS, initially diagnosed after a pulmonary embolism in 2015. Her medical history includes recurrent anemia, hyperlipidemia, hypertension, and two myocardial infarctions treated with angioplasty and stenting. Despite treatment with hydroxychloroquine, methylprednisolone, methotrexate and apixaban (later switched to warfarin), the patient experienced severe disease flare-ups and cardiovascular complications, prompting consideration for biologic therapy with anifrolumab. This case study illustrates the difficulties associated with the management of concurrent SLE and APS. It is clear that early diagnosis, vigilant monitoring, and aggressive management of both autoimmune and cardiovascular risks are essential to improve patient outcomes.

Biological Therapy, Therapeutic Approaches and Future Perspectives in the Treatment of Chronic Rhinitis 治疗慢性鼻炎的生物疗法、治疗方法和未来展望

Introduction: The widespread prevalence of chronic rhinitis necessitates the search for effective treatment methods. Modern type of medicine is biological treatment. Pathomechanism of AR is based on lymphocytes Th-2 reaction and connected with that cascade of effects. Biological treatment in the form of monoclonal antibodies blocks interleukins or immunoglobulin E signaling and leads to decrease in inflammatory reaction. Mechanism of action of these drugs and short period of use in AR requires patients to meet the including criteria and is usually used after other treatment options have run out. Taking under consideration the amount of patients suffering from AR and the impact on the quality of life indicates that there is still a need for new drugs and  using drugs which are useful in similar diseases.  Purpose of the work: This study aims to review and evaluate biological drugs used or having the potential to be used in the treatment of allergic rhinitis (AR), including: omalizumab, dupilumab, mepolizumab, reslizumab and the possibility of their use on a wider scale in the future. Materials and methods: An analysis of research papers available on PubMed and Google Scholar was undertaken using the following keywords: chronic rhinitis with nasal polyps; immunotherapy; monoclonal antibodies; allergic rhinitis; Quality of Life in rhinitis; omalizumab; dupilumab; mepolizumab; reslizumab; biological treatment during pregnancy; Results: The use of biological drugs in AR results in a significant improvement in symptom control and an increase in quality of life. This option can be an effective form of treatment for nasal polyps, reducing the risk of needing sinus surgery. However, the criteria for using these drugs are still very limiting, due to the lack of specific biomarkers that could early indicate a high chance of success. Biological drugs seem to be a great opportunity and a worthwhile possibility in the treatment of people with chronic allergic rhinitis.

Mechanism of action of these drugs and short period of use in AR requires patients to meet the including criteria and is usually used after other treatment options have run out. Taking under consideration the amount of patients suffering from AR and the impact on the quality of life indicates that there is still a need for new drugs and  using drugs which are useful in similar diseases.  \u0000Purpose of the work: This study aims to review and evaluate biological drugs used or having the potential to be used in the treatment of allergic rhinitis (AR), including: omalizumab, dupilumab, mepolizumab, reslizumab and the possibility of their use on a wider scale in the future. \u0000Materials and methods: An analysis of research papers available on PubMed and Google Scholar was undertaken using the following keywords: chronic rhinitis with nasal polyps; immunotherapy; monoclonal antibodies; allergic rhinitis; Quality of Life in rhinitis; omalizumab; dupilumab; mepolizumab; reslizumab; biological treatment during pregnancy; \u0000Results: The use of biological drugs in AR results in a significant improvement in symptom control and an increase in quality of life. This option can be an effective form of treatment for nasal polyps, reducing the risk of needing sinus surgery. However, the criteria for using these drugs are still very limiting, due to the lack of specific biomarkers that could early indicate a high chance of success. Severe headaches or migraines are a common neurological disorder that can significantly affect people's daily lives and is serious problem for individuals and society. A migraine is intense headache lasting 4-72 hours, typically localized on one side of the head and accompanied by nausea, sensitivity to light (phonophobia), sound (photophobia), and smell (osmophobia). The pathophysiology of migraines is complex and involves neurovascular and biochemicals mechanisms. Understanding the mechanisms that trigger the transition from a headache-free state to the headache phase is important to understand the underlying causes of migraine attacks and developing effective treatment methods. Caffeine can have both positive and negative influence on individuals suffering from migraine. It is use as adjuvant component in migraine treatment, but it can also act as a trigger for migraines. The therapeutic mechanisms of caffeine in migraine headaches are multidirectional and include: vascular effects, modulation of NO production, adenosine antagonism, potential interactions with the gut-brain axis and inhibition of leukotrienes and prostaglandins’ synthesis. On the other hand, the onset of migraine attack may be related to caffeine consumption or withdrawal. The purpose of this study is to review and systematize the available data about relation caffeine use to migraine.

Physical activity during postpartum period - an analysis of the latest studies and guidelines 产后期间的体育锻炼--最新研究和指南分析

Introduction and Purpose: Guidelines recommend at least 150 minutes of moderate-intensity aerobic activity weekly for postpartum women to enhance cardiovascular fitness, muscle strength, weight management, and mental health, particularly in alleviating postpartum depression (PPD). This review aimed to overview physical activity (PA) in postpartum women and identify key aspects to improve their health and quality of life.   Methods: A literature review was conducted using PubMed and Google Scholar, focusing on articles from 2015 to 2024, including observational studies, clinical trials, meta-analyses, and systematic reviews. Non-English articles and book chapters were excluded.   State of Knowledge: Studies highlighted the effectiveness of various exercise regimens for postpartum conditions. Trunk stabilization exercises significantly reduced lumbopelvic pain and disability. Exercise training impacted maternal vascular health and blood pressure, with regular exercise reducing blood pressure, preventing long-term cardiovascular diseases. Innovative approaches, such as progressive muscle relaxation exercises (PMRE) combined with online health counseling, reduced PPD and enhanced maternal attachment. Smartphone app-based pelvic floor muscle training (PFMT) reminders increased adherence and improved short-term pelvic floor muscle (PFM) function, showing potential long-term benefits.   Summary: This review provides strong evidence for structured PA programs in postpartum care, offering benefits from pain reduction and improved cardiovascular health to enhanced mental well-being. Future research should identify barriers to PA adherence and develop tailored interventions for postpartum women.

Introduction and Purpose: We are increasingly seeing new trends on social media regarding intense strength training in pregnant women. How much should women in a physiological pregnancy exercise? The aim of this piece is to trace the guidelines and set a healthy limit for such an important aspect during pregnancy.Materials and methods: A literature search was conducted using the medical databases PubMed and Google Scholar. Articles were retrieved in English, employing the key words: “strength training”, “activity during pregnancy”, „intensive exercise” and „ physical activity guidelines” appropriate configurations.Conclusions: The available guidelines and medical literature show that it is worthwhile to regularly practise sport both during and in preparation for pregnancy. In a healthy, physiological pregnancy, there is a wide range of activities to choose from, through which health benefits can be achieved, such as strengthening muscles, improving performance, alleviating pregnancy complaints (constipation, pain, nausea) and improving wellbeing, and much more. However, there are contraindications to certain exercises for specific conditions during pregnancy. You should consult your doctor on a case-by-case basis.

ABSTRACT: Prolactinoma is a benign tumor of the pituitary gland that leads to the overproduction of prolactin. It is the most common type of pituitary adenoma, accounting for approximately 50% of all pituitary tumors. The clinical presentation of prolactinoma varies depending on the level of prolactin elevation and the size of the tumor. In women, common symptoms include galactorrhea, amenorrhea, and infertility. Men may present with hypogonadism, decreased libido, erectile dysfunction, and gynecomastia. Large prolactinomas, known as macroadenomas, can cause mass effects such as headaches, visual disturbances due to compression of the optic chiasm, and hypopituitarism due to pressure on surrounding pituitary tissue. Understanding the pathogenesis of prolactinoma is crucial for developing effective treatments and improving patient outcomes. The development of prolactinomas involves a complex interplay of genetic, hormonal, and cellular factors. Treatment of prolactinoma aims to normalize prolactin levels, reduce tumor size, and alleviate symptoms. The first-line therapy is dopamine agonists, such as cabergoline and bromocriptine, with surgery and radiotherapy reserved for refractory cases. Furthermore, chemotherapeutic agent - temozolomide may be a treatment of choice in aggressive or malignant prolactinomas. By understanding the underlying mechanisms and different treatment methods, healthcare providers can optimize the management and outcomes for patients with prolactinoma.

Introduction: Metabolic dysfunction-associated steatotic liver disease (MASLD) was previously known as non-alcoholic fatty liver disease (NAFLD). MASLD is one of the most important and leading causes of liver disease worldwide. This disease is a public health challenge in the 21st century. Aim of the study: This article aims to present the latest literature data on MASLD. The study is intended to show what consequences are associated with changing the name of the disease and this article is purposive to be a vademecum of knowledge for practicing physicians. Material and methods: Comprehensive literature searches were performed across the main electronic databases of PubMed and GoogleScholar using the keywords: "metabolic-dysfunction associated steatotic liver disease”, “MASLD”, “non-alcoholic fatty liver disease” and “NAFLD” Conclusions: Most patients do not report any symptoms. It is important to identify patients at increased risk of MASLD. A fundamental role in prevention and treatment is lifestyle changes. The pharmacological approach includes among others use of antidiabetic drugs and treatment of other associated states.

MASLD is one of the most important and leading causes of liver disease worldwide. This disease is a public health challenge in the 21st century. \u0000Aim of the study: This article aims to present the latest literature data on MASLD. The study is intended to show what consequences are associated with changing the name of the disease and this article is purposive to be a vademecum of knowledge for practicing physicians. \u0000Material and methods: Comprehensive literature searches were performed across the main electronic databases of PubMed and GoogleScholar using the keywords: \"metabolic-dysfunction associated steatotic liver disease”, “MASLD”, “non-alcoholic fatty liver disease” and “NAFLD” \u0000Conclusions: Most patients do not report any symptoms. It is important to identify patients at increased risk of MASLD. A fundamental role in prevention and treatment is lifestyle changes.
