Fatigue fracture of the second rib in a professional athlete 一名职业运动员的第二根肋骨疲劳骨折

A fatigue fracture, also known as stress fracture or overuse fracture, is caused by the summation of micro-injuries resulting from a chronic state of bone overload. It affects 1.4 %- 4.4 % of professional athletes. The most common site of fatigue fractures are the lower limbs. This is related to the heavy load caused by body weight and performing dynamic activities like jumping, running. Typical fractures are fractures of the fifth metatarsal bone, fibula and tibia, and less common fractures of the femur. Fractures involving the bones of the upper limbs and thorax are less commonly reported in the literature. It affect athletes in strength sports and athletes who perform dynamic and repetitive movements with their upper limbs, most often boxers, weightlifters, wrestlers, judokas, swimmers, golfers, rowers. Fractures in the thorax most often involve the first rib. Fractures of ribs II through XII are extremely rarely described. We present an unusual case of a second rib fracture in a professional athlete. A 26-year-old athlete training in racewalking reported increasing pain in the left scapula area 7 days before competing in the European Championships. Immediately after the competition, the pain changed location to the anterior thorax area, making movement of the upper limb and breathing much more difficult. A chest tomography was performed and it revealed a fatigue fracture of the second rib on the left side. The case report presents an atypical fracture in a female racewalking athlete and presents the uncharacteristic symptoms accompanying this fracture. Incorrect initial diagnosis and application of physiotherapy procedures without performing basic diagnostic tests, delay the diagnosis and initiation of proper treatment, and thus the athlete's recovery.

The impact of overweight and obesity on the results of laparoscopic ventral hernia repair, including robot-assisted repair - a literature review 超重和肥胖对腹腔镜腹股沟疝修补术(包括机器人辅助修补术)效果的影响--文献综述

Obesity is the fifth leading cause of death worldwide. In turn, ventral hernias (including an inguinal hernias) are one of the most common cases in general surgery. Minimally invasive techniques (laparoscopic or robot-assisted procedures) are becoming more and more popular in ventral hernia treatment compared to open procedures. The aim of this study is to analyze the impact of body mass index (BMI) value on the efficacy and safety of laparoscopic ventral hernia repairs, including inguinal hernia repairs. Numerous studies evaluating this relationship are available. In most studies, There were also no statistically significant differences in the rate of intraoperative complications, the median operative time, the median length of hospital stay, the rate of postoperative pain, the surgical site infection, the rate of postoperative complications, the median time to recurrence, and the rate of early and late recurrences between groups with different BMI values. Moreover, studies proved that laparoscopic ventral hernia repair in obese patients is more effective compared to the open repair. The analysis revealed that high BMI values do not have a statistically significant impact on the outcomes of laparoscopic repair, including robot-assisted laparoscopic repair, in patients with ventral hernias. However, it should be noted that the analyzed studies often have limitations. For this reason, it is essential to conduct multicenter studies and perform long-term evaluations of patients.

The utility of cannabinoids as agents used to treat diseases is a popular topic of discussion these days. The approval of medical marijuana continues to stir up controversy. As far back as ancient times, hemp was used for medicinal purposes. These plants have the ability to produce phytochemicals and other compounds such as flavonoids and terpenes. Respective species of hemp differ in the content of particular chemical compounds and therefore have different possible uses. Among the best-known phytocannabinoids found in hemp are delta-9-tetrahydrocannabinol (THC) and cannabidiol (CBD). They have partially opposing effects and interact with the corresponding receptors of the endocannabinoid system in the body's tissues. We also distinguish between cannabinoids of endogenous origin - endocannabinoids (EC) and synthetic cannabinoids (SC). The effects of THC and CBD on the human body are still being studied. Information about them that appears in the media, is often reported inaccurately or incompletely. There is now a growing body of research into the use of cannabinoids to treat and alleviate the symptoms of many diseases. Due to the neuroprotective effects of THC and CBD, researchers' work has focused on using them to treat neurodegenerative diseases, which include Parkinson's disease (PD).

Idiopathic pulmonary fibrosis (IPF) is a chronic, rare disease characterized by continuous fibrosis of the lung parenchyma. It mainly affects the elderly; however, it is increasingly being diagnosed in younger patients as well. Risk factors include smoking, occupational dust exposure and genetic factors. Symptoms of IPF include shortness of breath, dry cough and reduced exercise tolerance, leading to a reduced quality of life for patients. Diagnosis is based on imaging, mainly high-resolution CT scans, and the exclusion of other causes of interstitial lung disease. Two antifibrotic drugs, nintedanib and pirfenidone, are now approved to slow disease progression. Nintedanib acts as a tyrosine kinase inhibitor, blocking the signaling pathways of lung fibroblasts. Pirfenidone, on the other hand, has anti-inflammatory and anti-fibrotic effects by inhibiting TGF-b signaling pathways. Clinical trials have confirmed their efficacy in reducing the decline in increased vital capacity and the risk of disease progression. In Poland, patients with IPF can benefit from nintedanib and pirfenidone therapy under the drug program. Despite advances in treatment, more research is needed on new IPF therapies. Clinical trials of zinpentraxin, ziritaxestat and pambrevalumab have not confirmed their efficacy in treating IPF. Results from initial studies of bexotegrast show promise, but further studies are needed and are ongoing. Despite advances in the treatment of IPF, further research into new therapies is needed to improve therapeutic outcomes and patient quality of life.

Necrotizing Enterocolitis in Newborns: Diagnosis, Etiology, Management, Prevention, Complications and their Relation to Future Sport Performance 新生儿坏死性小肠结肠炎:诊断、病因、处理、预防、并发症及其与未来运动表现的关系

Necrotizing enterocolitis (NEC) is a serious gastrointestinal condition that primarily affects premature infants, although it can also occur in full-term infants. It is characterized by inflammation and injury of the intestinal tissue, which can progress to necrosis and perforation of the bowel. NEC is a leading cause of morbidity and mortality in neonatal intensive care units (NICUs), particularly among very low birth weight infants. Diagnosis of NEC is based on clinical signs and symptoms, radiographic findings, and laboratory tests. Management of NEC involves a multidisciplinary approach, including supportive care, medical therapy, and surgical intervention when necessary. Supportive care includes bowel rest, intravenous fluids, and nutritional support. Medical therapy may include antibiotics, gastric decompression, and parenteral nutrition. Surgical intervention may be required for infants with intestinal perforation, severe NEC, or complications such as intestinal stricture or short bowel syndrome. This condition and its complications may carry consequences for life and especially sport performance in the child’s future. Strategies for preventing NEC include promoting breastfeeding, avoiding unnecessary antibiotic exposure, minimizing enteral feeding interruptions, practicing strict infection control measures, and implementing protocols for gradual feeding advancement in premature infants. Overall, NEC is a complex and multifactorial disease that poses significant challenges in neonatal care. Early recognition, prompt intervention, and comprehensive management are essential for optimizing outcomes for affected infants. Further research is needed to better understand the pathophysiology of NEC and to develop more effective prevention and treatment strategies.

It is characterized by inflammation and injury of the intestinal tissue, which can progress to necrosis and perforation of the bowel. NEC is a leading cause of morbidity and mortality in neonatal intensive care units (NICUs), particularly among very low birth weight infants. \u0000Diagnosis of NEC is based on clinical signs and symptoms, radiographic findings, and laboratory tests. Management of NEC involves a multidisciplinary approach, including supportive care, medical therapy, and surgical intervention when necessary. Supportive care includes bowel rest, intravenous fluids, and nutritional support. Medical therapy may include antibiotics, gastric decompression, and parenteral nutrition. Surgical intervention may be required for infants with intestinal perforation, severe NEC, or complications such as intestinal stricture or short bowel syndrome. This condition and its complications may carry consequences for life and especially sport performance in the child’s future. \u0000Strategies for preventing NEC include promoting breastfeeding, avoiding unnecessary antibiotic exposure, minimizing enteral feeding interruptions, practicing strict infection control measures, and implementing protocols for gradual feeding advancement in premature infants. \u0000Overall, NEC is a complex and multifactorial disease that poses significant challenges in neonatal care. Early recognition, prompt intervention, and comprehensive management are essential for optimizing outcomes for affected infants. Review of the Literature on Patellar Tendinopathy: Current State of Knowledge 髌骨肌腱病文献综述:知识现状

Introduction: Patellar tendinopathy, commonly referred to as jumper’s knee, presents as pain and dysfunction in the patellar tendon, predominantly affecting athletes engaged in jumping sports. The symptoms of patellar tendinopathy not only impact sports involvement but also interfere with activities of daily living, such as climbing stairs and sitting for extended periods, which in turn affects overall well-being. A variety of treatment options are available, encompassing both conservative approaches, such as modifying activity levels and undergoing physical therapy, and surgical methods. Aim of the study: The objective of this study is to review the current literature regarding the incidence, risk factors and currently available treatment methods for patellar tendinopathy. In addition, the aim is to present a summary of the knowledge and information collected so far in this area in a concise and accessible format. State of knowledge: The epidemiology and mechanisms of patellar tendinopathy are already well documented and described. The statistics regarding the incidence of patellar tendinopathy among athletes in the given fields are sufficiently consistent with each other. The existing literature provides insights into the efficacy of various therapeutic approaches for patellar tendinopathy, yet it is limited in scope. Conclusion: This review acknowledges that recent advancements in research have led to a fuller understanding of epidemiology and improvement of treatment methods for patellar tendinopathy. Despite this, a single therapy is not sufficient for optimal outcomes. Therefore, it is recommended that a comprehensive approach to conservative treatment be adopted. Further research and clinical trials are essential for the generation of reliable conclusions.

Personality disorders refer to a collection of mental health conditions defined by persistent deviations from societal norms in thoughts, behaviors, and emotions. These deviations frequently result in challenges in interpersonal relationships, self-perception, and overall functioning, potentially causing distress in various aspects of life. The DSM-5 classifies personality disorders into three clusters. Cluster A contains paranoid, schizoid and schizotypal personality disorders which are characterized by strange and eccentric behaviors. Cluster B contains narcissistic, histrionic and antisocial personality disorders which are characterized by dramatic and emotional behaviors. Cluster C contains avoidant, dependent and obsessive-compulsive personality disorders which are characterized by anxious and fearful behaviors.

Guyon’s Canal Syndrome and its’ impact on everyday life and sport - literature review 居雍运河综合征及其对日常生活和体育运动的影响--文献综述

Guyon’s Canal Syndrome is a rare clinical condition resulting from ulnar nerve injury in the distal portion of the nerve, situated in a narrow anatomical corridor at the wrist. It is an example of neuropathy caused by pressure applied on a nerve. The pressure can be caused by various factors, which will be discussed in the following section of the review. The ulnar nerve is a mixed nerve, meaning that it provides motor and sensory innervation to the digits. This property of the nerve results in a variety of symptoms (motor and sensory) that occur during the injury. The nerve originates from the central rami of the C8 and T1 nerve roots, which form the lower trunk of the brachial plexus. The lower trunk next divides into an anterior and posterior division, with the fibres of the ulnar nerve in the anterior division becoming the medial cord. The nerve leaves the brachial plexus as a terminal branch of the medial cord and lies from the axilla into the medial part of the anterior compartment of the upper arm, where it is located posteriomedial relative to the brachial artery. It then inserts onto the medial epicondyle of the distal humerus and enters the cubital tunnel. It then runs along the medial side of the forearm, lying superficially and being covered mostly by fascia and skin. It subsequently enters Guyon’s canal, a fibro-osseous tunnel formed between two bones: the pisiform and the hamate bone (the hook hamate exactly). The ulnar nerve in Guyon’s canal is laterally bordered by the ulnar artery. Finally, within the canal, the ulnar nerve divides into superficial and deep branches. Guyon's canal syndrome is a condition that results from the injury of the ulnar nerve at a specific point. The syndromes associated with this injury are characterised by the loss of function of a portion of the nerve, which can be used to differentiate between injuries that are proximal or distal to the nerve.

The effects of sugar addiction on health and the importance of exercise 糖瘾对健康的影响和运动的重要性

Introduction and purpose: Sugar is the primary energy substrate for the brain. Consuming sweet products releases dopamine, which activates the reward system. In this aspect, excessive consumption of sugar is similar to drug use and leads to many health risks. The aim of the study is to systematize information on sugar addiction, highlight the need for preventive measures and create treatment algorithms covering areas such as psychotherapy, pharmacotherapy and the positive impact of physical activity. Materials and metods: A literature search was conducted using the medical databases PubMed and Google Scholar. Articles were retrieved in English, employing the key words: “sugar addiction”, “sweet taste”, „sugar diabetes” and „sugar sport” in appropriate configurations. The analysis encompassed data from 34 scientific references published between 2007 and 2024. Conclusions: Many factors contribute to sugar addiction. It is a self-perpetuating cycle, as obese and sugar addicts need more sugar to feel satisfied. At the same time, sugar negatively affects body weight, behavior, mood, cardiovascular, nervous, digestive systems. Which is why steps should be taken to increase public understanding and awareness of the mechanism of sugar addiction and its risks.

Consuming sweet products releases dopamine, which activates the reward system. In this aspect, excessive consumption of sugar is similar to drug use and leads to many health risks. The aim of the study is to systematize information on sugar addiction, highlight the need for preventive measures and create treatment algorithms covering areas such as psychotherapy, pharmacotherapy and the positive impact of physical activity. \u0000Materials and metods: A literature search was conducted using the medical databases PubMed and Google Scholar. Articles were retrieved in English, employing the key words: “sugar addiction”, “sweet taste”, „sugar diabetes” and „sugar sport” in appropriate configurations. The analysis encompassed data from 34 scientific references published between 2007 and 2024. \u0000Conclusions: Many factors contribute to sugar addiction. It is a self-perpetuating cycle, as obese and sugar addicts need more sugar to feel satisfied. At the same time, sugar negatively affects body weight, behavior, mood, cardiovascular, nervous, digestive systems.
