Negative Pressure Pulmonary Edema 负压肺水肿

We would like to point that according to the reporting we believe that the true etiology is a combination of NPPE type I and II. It seems that this patient had already developed the pathophysiology of type II NPPE due to progressive, though not critical (as flexible laryngoscopy revealed) narrowing of the larynx due to the external compression from the growing goiter. So, the patient was on the edge on manifesting a type II NPPE (recent onset of exertional dyspnea) when something critical ensued an undetected infection that led to mucosal edema or the sudden onset of narrow complex tachycardia as the authors wisely detected and states. So, the presenting episode of NPPE is presumably of type II, following the pathophysiology that the authors elegantly explained.

150 Maxillary sinus hypoplasia (MSH) is the underdevelopment of maxillary sinus antrum and it is usually associated with other anomalies, such as uncinate process and infundibular passage. Hypoplasia is classified into three: Type I is mild hypoplasia, normal uncinate and infundibulum; Type II is significant hypoplasia with hypoplastic or absent uncinate and pathological infundibular passage; Type III is cleft-like sinus and absent uncinate1.

123 Cervical ectopic pregnancy is extremely rare. Its incidence is approximately 1 in 9,000 deliveries, less than 1% of ectopic pregnancies1,2. It is associated with potentially serious complications if not diagnosed and treated early. Cervical pregnancy is a serious condition due to trophoblast cells penetrating through the cervical wall and into the uterine blood supply, leading to severe hemorrhage.

A nine-year-old boy involved in a motor vehicle accident (MVA), sustained a circumferential degloving injury over his left thigh with a left femoral midshaft open grade IIIb fracture, see figure 1 (A to C). Angiogram revealed no deep vascular injury. His mangled extremity severity score (MESS) was five. A high above knee amputation (AKA) was advised, but his father refused for fear of having a “disabled child”. Surgical debridement was done followed by femoral plating. 104 NCD includes cardiovascular diseases, cancer, diabetes and respiratory diseases. NCD is considered the main cause for approximately 60% of global mortality. In low and middleincome countries, it is up to 80%. In addition, they result in 47% of global burden of diseases. Furthermore, diabetes prevalence rate reached more than 20% in many Gulf Cooperation Council (GCC) States (12% to 25% in the age group 25 to 65 years). Five of the GCC States are among the top ten countries in the world reported by the International Diabetes Federation in 2010. In addition, 40% to 70% of people in the Gulf suffer from obesity, hypertension and smoking1. Gulf Family Health Survey, more than 15 years ago, confirmed that cardiovascular diseases affect age group more than 40 years at rates ranging from 20% to 45%2.

100 Rotavirus infection is the most common cause of severe gastroenteritis in infants and children1-3. According to the World Health Organization (WHO), 2.4 million hospital admissions and 527,000 deaths annually are attributed to rotavirus infection and the estimated rotavirus related global death during 2008 was 453,000 (420,000 – 494,000) child deaths1,4. Because of the worldwide burden of rotavirus infection, the introduction of vaccination to prevent and minimize rotavirus associated morbidity and mortality has been a public health priority to several countries1.

The World Health Organization (WHO) predicts that Diabetes Mellitus (DM) would be the seventh leading cause of mortality by the year 2030. The incidence of diabetes is rising, from 4.7% in 1980 to 8.5% in 2014. Type 2 DM accounts for 90% of diabetic patients. It is due to the body’s inefficient use of insulin which is caused by insulin resistance1. Glucose and lipids (triglycerides including cholesterol) metabolism are affected in diabetic patients.

113 PI is the presence of multiple gas cysts in the walls of the small and large intestines1. It could affect all ages and the estimated prevalence is 0.02% to 0.003% worldwide2. It could be primary or secondary1. The latter is usually associated with higher mortality rates, whereas the former is mostly benign and often considered an incidental finding. Most patients are asymptomatic. The condition is usually discovered either incidentally during a radiological investigation, during surgery or by the patient presenting with symptoms of the underlying disease3. Due to the rarity of this condition, very few cases have been reported in the literature. In Bahrain, there has been no similar case documented.
