Abstract Two-Spirit, Gay, Bisexual, Transgender, and Queer (2SGBTQ+) men face significant health inequity. This article reports on two phases of a study conducted in conjunction with the Gay Men’s Sexual Health Alliance (GMSH) to examine 2SGBTQ+ men’s health inequity. First, an environmental scan of existing Ontario government health policy was conducted to identify specific gaps. Second, six focus groups of GMSH Alliance members (n = 21) were conducted to understand the impact of policy gaps and identify potential strategies to address them from the community’s perspective. We argue that to meet the wellness needs of the community, community development and healthy public policies are appropriate strategies. The environmental scan confirmed that there is currently very limited policy content specific to 2SGBTQ+ men’s wellness and mental health. Data from focus groups were consistent with the scan. GMSH Alliance members struggled to serve their community in the absence of sufficient resources. Participants also expressed an interest in advocacy, but they felt that they lacked the skills to do so. It is clear that 2SGBTQ+ men face population-specific health risks, and much work is needed to advocate for 2SGBTQ+-centred well-being policy. Future work should develop a policy that focuses on addressing many of the issues raised in this research, while also contributing to the achievement of health equity for 2SGBTQ+ men.

Embedding community development approaches in local systems to address health inequalities: a scoping review 将社区发展方法纳入地方系统以解决卫生不平等问题:范围审查

Abstract Background There is a growing evidence base which shows that community development can make an important contribution to reducing health inequalities, but embedding community development as a mainstream approach into local systems is challenging. The literature relevant to the question of how to embed community development approaches is reviewed in this paper. Methods Using guidance from the Joanna Briggs Institute, a scoping review was carried out to identify relevant literature. Systematic searches were carried out across multiple databases, experts in the field were contacted and references of included studies were screened. Search results were screened against exclusion criteria. The Consolidated Framework for Implementation Research was used as a framework to identify factors hindering or supporting embedding. Findings The review identified thirty-five documents which described embedded, or attempts to embed, community development approaches in fourteen different countries. The most common community development approaches were strength-based or co-production. Four studies reported primary research on the embedding process or systems change. Several barriers and facilitators to embedding were identified including those related to funding arrangements, organizational and system culture, building trust with communities and the need for training and support for staff. Conclusion Using an implementation science framework, this scoping review has assessed the nature of the evidence base on how to embed community development. While the evidence base uncovered is currently limited, barriers and facilitators to embedding identified in the review can be used to both inform future attempts to embed community development and provide the building blocks for future primary research.

Abstract Background US Black pregnant women have worse birth and maternal outcomes than their White counterparts, and these disparities were exacerbated by the COVID-19 pandemic. We sought to examine community health workers’ roles in shaping low-income Black pregnant women’s experiences and perspectives on public health guidance during COVID-19. Methods Between December 2020 and September 2021, we conducted two to four semi-structured interviews and feedback sessions with each participant: community health workers or pregnant/postpartum women who self-identified as Black (thirty-five total interviews and twelve feedback sessions). Using a Longitudinal Qualitative Research approach, we looked at themes over time in perceptions of COVID-19 public health guidance, specifically related to vaccination, throughout pregnancy and the postpartum period. Results Community health workers were essential sources of information about the COVID-19 vaccine, risk and safety, and other related challenges. However, there was extensive distrust of science and of health providers. This was traced to historical and modern cases of medical and scientific abuse of Black Americans. Community health workers experienced much of the same distrust of public health guidance related to the pandemic as their clients. Conclusions Community health workers represent an important group for influencing perspectives of COVID-19 health and vaccine guidance among Black pregnant and postpartum women. Both groups’ views on public health guidance during the pandemic were shaped by complex historical, economic and cultural systems. To support trust- and rapport-building, healthcare providers should take an educated, empathetic approach to working with community health workers and understanding their patients’ histories and experiences.

Abstract Bologna, Italy, with its history of good governance, progressive welfare policies, community work, and participation has engendered powerful discourses informing policy and institutional innovation. Yet, a recent city-wide mixed-method action-research (AR) project, designed to document and tackle health inequalities, showed a significant North–South divide for a range of health indicators and an unequal distribution of social determinants of health across the most socio-economically deprived city areas. The AR project also examined the potential of the city’s initiatives for community development and health promotion in addressing health inequalities. In this paper, we focus on a recent institutional innovation called ‘Uffici reti e lavoro di comunità’ (Network and community work units, NCWUs), established by the Bologna municipality in 2016 to promote the decentralization of powers, strengthen community networks, and foster the engagement of citizen organizations in taking care of the material and social living environment. The results of the AR project indicate that the NCWUs, whilst not designed as a policy initiative to reduce (health) inequalities, have a potential to do so by supporting more effective community action to address the determinants driving those inequalities. Through a critical re-thinking of the concepts of power and participation, they also suggest promising directions to overcome some of the limitations of community development approaches in contributing towards greater well-being and equity for people and communities, underlining local learning which could be applied across different geographies nationally and internationally.

Building communities of health: the experience of European social clinics 建立卫生社区:欧洲社会诊所的经验

Abstract Social clinics are grassroots health initiatives characterized by the development of mutualist practices of healthcare provision and the articulation of their action at territorial and community level. The present paper explores the development and implementation of community participation practices across three social clinics in Europe: social clinic of solidarity KIA in Greece, Village2Santé in France, and Microclinica Fatih in Italy. Drawing on findings collected through qualitative methods, our paper reflects on three overlapping themes of community participation highlighting its collective and eminently political character. Inspired by the frameworks of health promotion, comprehensive Primary Health Care and community development, we analyze three levels of community participation in the social clinics: participatory medical practices, the co-creation of health services, and the connection with broader social movements. Our paper contributes to the literature on community development by stressing the urge to reimagine the principles and practices of community participation through the creation of alternative forms of organizing and of radical empowerment. At the same time, it can inspire practitioners in experimenting with community participation initiatives from a more critical perspective, resisting the worst elements of neoliberalism.

Pengangguran Terbuka merupakan bagian dari angkatan kerja yang tidak bekerja atau sedang mencari pekerjaan, atau sedang mempersiapkan suatu usaha, mereka yang tidak mencari pekerjaan karena merasa tidak mungkin untuk mendapatkan pekerjaan. Berkembangnya sektor industri dan wilayah Jombang yang dekat dengan kota-kota industri berpotensi mengentaskan pengangguran. Saat ini masih banyak pencari kerja yang belum memiliki keterampilan yang cukup. Pasar kerja membutuhkan tenaga kerja yang sudah punya keterampilan dan siap bekerja. Ini menjadi tantangan bagi peserta Kuliah Kerja Nyata Mahasiswa untuk memberikan keterampilan kerja sebagai bekal di dunia industry atau berwirausaha. Metode pengabdian masyarakat yang dilakukan oleh mahasiswa Kuliah Kerja Nyata Mahasiswa yaitu melalui pemberian program pelatihan keterampilan kerja dengan menerapkan metodologi pelatihan yang inovatif, sistematis, dan menggunakan media teknologi informasi. Dengan metode ini sangat efektif dibuktikan dengan peserta pada saat kegiatan pelatihan, peserta aktif dan antusias menerima materi yang diajarkan. Hal ini dibuktikan dengan tingkat kehadiran diatas 90%. Peserta pelatihan memberikan respon yang positif dalam kegiatan pembelajaran, serta peserta pelatihan dapat menjawab seluruh pertanyaan evaluasi yang diajukan sehingga termotivasi untuk mengembangkan kemampuannya.

Abstract Community-based enterprises (CBEs) are generally perceived as a sustainable poverty reduction strategy in marginalized communities. In the southern Philippines, it has become widely used in poverty alleviation and rebuilding efforts in fragile and conflict-affected areas. Engaging economically disadvantaged sectors in community-based entrepreneurship is often more likely to be sustainable than participating in conventional poverty reduction programmes, but this is rarely explored. This study aimed to assess the sustainability of CBEs in terms of their social and economic benefits and operational durability. The study reveals that both contextual challenges and opportunities drive the emergence of CBEs. Most CBEs are engaged in producing and marketing agricultural products and eventually diversify into offering service-oriented entrepreneurial activities. CBEs’ locally embedded internal capacity and established external linkages that fostered ‘connections’ facilitated their operational durability. CBEs provide economic benefits through livelihood opportunities and income, expand members’ livelihood network, and create profitable products from local resources. The social benefits include addressing personal and collective concerns, enhancing collaborative problem-solving, and improving access to institutional services. This study concludes that the CBE’s social and economic benefits serve as a safety net and social protection, a buffer to drowning into a deeper poverty level. This study provides empirical evidence on the sustainability of CBEs in fragile and conflict-affected areas.

SWOT mempunyai empat aspek pokok, yaitu Strength (kekuatan), Weakness (kelemahan), Opportunity (peluang) dan Threat (ancaman). SWOT dipakai sebagai salah satu teknik identifikasi berbagai faktor dan unsur penentu pembangunan suatu institusi atau daerah secara sistematis. Analisis SWOT digunakan untuk mencapai visi dan misi yang telah ditetapkan oleh para pemangku kepentingan. Wisata Pandansili terletak di desa Ngampungan Kecamatan Bareng Kabupaten Jombang. Pandansili menawarkan wahana wisata berupa kolam renang serta tempat bermain anak-anak selain itu di tempat tersebut menawarkan pemandangan suasana pegunungan. Pandansili salah satu tempat wisata yang berada di sekitar Daerah Jombang selatan. Selain itu daya tarik masyarakat akan wisata pandansili cukup ramai dengan persaingan wisata yang ada di wonosalam. Terdapat empat tahapan analisis SWOT yang dapat digunakan dalam perumusan strategi, Dari hasil analisa didapatkan nilai IFE sebesar 5,56 dan nilai EFE sebesar 5,01. Dari hasil analisis nilai IFE dan EFE berada di Kuadran I (Strategi Agresif / Ofensif), di mana Kuadran I dianggap mempunyai posisi yang paling menguntungkan sehingga dengan kekuatan yang dimiliki dimungkinkan untuk memanfaatkan peluang yang ada. Dengan penerapan SWOT ini pengelolaan dan perencanaan pengembangan Wisata Pandansili berjalan dengan baik dan lancar

Abstract In this article I describe the recent process of leaving a volunteer role that had, over a number of years, brought me much joy and satisfaction. This process of leaving gave me the opportunity to reflect on the kinds of organisational cultures that may work best in supporting volunteers and paid staff, as well as organisations themselves, to flourish. Through my journey of leaving, I describe the value of asset based approaches, and caring for people for who they are, not de-valuing them for who they are not, as part of creating constructive and positive outcome oriented organisations.
