Anaplastic myeloma (AM) represents a rare and aggressive variety of multiple myeloma (MM) which is morphologically composed mainly of immature plasma cells with nuclear enlargement and multilobulation. This variant can be confused with high grade lymphoma, metastatic carcinoma or even with dysplastic megakaryocytes in bone marrow study, posing a diagnostic challenge. Here, we present a case of a 71 years old male with complaints of body ache, progressive weakness for a month and a recent history of fall causing 7th and 10th rib fractures. Biochemical examination revealed raised serum calcium, LDH, 24 hour urinary protein, raised lambda on serum free light chain assay and marginally high serum creatinine. PET- CTS revealed lytic lesions involving multiple long and flat bones. Bone marrow aspirate showed 81% large anaplastic cells with marked nuclear convolution. Immunophenotying showed positivity for CD38, CD138, CD56 along with lamda restriction. Composite consideration of clinical features, biochemical investigations, hematological workup and immunophenotyping led to a final diagnosis of anaplastic multiple myeloma.

Cation exchange resins such as sodium polystyrene sulfonate (Kayexalate) are used to treat life threatening hyperkalaemia caused due to electrolyte imbalances. These are usually administered as an enema, orally or via nasogastric tube along with a hypertonic mixture of sorbitol to prevent constipation. Although effective, it might lead to development of rare, but serious consequences involving the gastrointestinal tract. There is a need for awareness of the rare potential adverse effects associated with its widespread use. Here, we report 2 cases of colitis induced by the administration of sodium polystyrene sulfonate which were evidenced by luminal and impacted/adherent crystals of kayexalate causing mucosal injury.

Patients with haematological malignancies undergoing treatment with hypomethylating agents are predisposed to opportunistic infections, including Candidiasis. We present the case of a 73-year-old female patient diagnosed with Myelodysplastic/Myeloproliferative neoplasms who was receiving a hypomethylating agent as part of her treatment regimen. During the course of her treatment, peripheral blood smear examination revealed the presence of budding yeast cells, consistent with Candida species. Subsequent culture confirmed the diagnosis of Candidiasis.The development of Candidiasis in patients with haematological malignancies receiving hypomethylating agents poses a significant clinical challenge due to overlapping symptoms with other infections and chemotherapy-induced myelosuppression. Peripheral smear examination serves as a rapid and cost-effective adjunctive diagnostic tool for the early detection of Candidiasis in these patients. Timely detection of Candidiasis in this population is critical for appropriate management and improved outcomes.

Adenosquamous carcinoma (ASC) of the prostate is an extremely rare, aggressive histological subtype of prostate cancer. We report one such case here of a 73-year-old man with prostatic malignancy with a combination of squamous and glandular components. Immunohistochemistry with p40, AMACR and HMWCK corroborated the diagnosis of ASC. The squamous component being refractory, is better amenable to aggressive surgery, highlighting the importance of recognising this entity. Bony metastasis was not seen in this patient. Serum PSA was near normal.

Chondrosarcoma is rare condition affecting the head and neck region. Here we report a case of in a 15-year old male patient presenting with growth behind the right ear that recurred following radiotherapy and resection. The lesion was treated by re excision of tumour followed by reconstruction and chemotherapy. Histopathological examination revealed chondrosarcoma grade III. The case report describes a tumour which is rare in young adults as well as in location. The tumour also upgraded from Chondrosarcoma grade II in the first resection to Chondrosarcoma grade III in the recurrence.

Core Needle Biopsy (CNB) has been widely used as a diagnostic tool for breast tumors. However, this technique involves tissue sampling as opposed to complete surgical lesion excision, introducing the possibility of underestimation of disease. The purpose of this study was to assess the reliability of CNB in breast tumors. This was a retrospective cross-sectional study from January 2023 to July 2023. All included patients underwent Core Needle Biopsy (CNB) and subsequent Surgical Excision (SE) of a breast tumor. One hundred fifty patients were assessed. All patients were female. The median age for malignancies was 45 years and 31 years for benign tumors. Concordance regarding the tumor type was globally moderate (k= 0.67; p<0,001) with a rate of 97.6% for CNB-diagnosed malignant tumors and 62.5% for CNB-diagnosed benign tumors. Concordance for histological types was perfect for CNB-diagnosed ILC (rate= 100%; k= 1; p<0,001) and almost perfect for IDC (rate= 96.7%; k= 0.94; p<0,001). The concordance between Core Needle Biopsy (CNB) and surgical excision specimen in the diagnosis of malignancy and benignity of breast tumors is almost perfect for CNB-diagnosed malignancies and moderate for benign tumors.

Head and neck squamous cell carcinoma (HNSCC) represent includes cancers of the oral cavity, larynx, and oropharynx. In relation to Oral squamous cell carcinoma (OSCC), a panel of markers such as p53, eIF4E, Cyclin D 1, MMP-9, and others has been evaluated histopathologically tumor-free/clear surgical margins. The present review summarizes the importance of one of the markers associated with protein translation. eIF4E has a significant role process of tumorigenesis and has the potential to target various molecules associated with Hallmarks of cancer. Lastly, relevant findings from studies done in relation to OSCC have been tabulated.

Mucoepidermoid carcinoma with Warthin like features is a deceptive tumour and can be potentially misdiagnosed as a Warthin tumour which is benign, Warthin tumour with mucinous and squamous metaplasia or MEC transformed from Warthin tumour. We are presenting a case of a 25-year-old woman with recurrent solitary mass in the left parotid gland. Microscopically it consists of predominantly cystic areas and focal solid infiltrative tumour with mucinous, intermediate and epidermoid cells having complex architecture in a fibrotic stroma. Extracellular mucin pools seen. Cystic areas are lined by monolayered as well as bilayer epithelium with lymphoid stroma (Warthin like morphology). Occasional mitosis noted. No necrosis and perineural invasion seen. Immunohistochemically, the tumour is positive for P63, P40, CK5/6, EMA, Mucicarmine stain, diffusely positive for CK7. We reached at the final conclusion of low grade MEC, Warthin like features. Even though the cytogenetic studies are confirmatory, we emphasize the role of histomorphology study with IHC and clinical history in identifying this rare variant of MEC with Warthin like features.

Non-alcoholic fatty liver disease, heavy alcoholic consumption leading to chronic liver disease and cirrhosis. Which can eventually lead to hepatocellular carcinoma. Here we are presenting a case of steatohepatitic variant of HCC with cystic degeneration mimicking as Hydatid cyst in radiology. This case highlights the importance of histopathological examination and immunohistochemistry role in accurate diagnosis.

Platelet function disorders are highly heterogeneous; those may be inherited/ acquired. Patients with these may have inadequate platelet count with impaired function, but may also have both. Early detection of platelet dysfunction also helps in better patient management and improve outcome. Prospective study done in 50 cases, Patient’s details history of thrombocytopenia. Complete blood count with peripheral smear is done and various parameters are analysed with respect to the platelet count, PDW, P-LCR. On EDTA preserved blood sample four monoclonal antibodies CD41, CD42b, CD61 and CD62p are applied and analysed by flow cytometer. In 50 thrombocytopenia cases out of which 06(12%) were immunological thrombocytopenia & 44 (88%) were non-immunological thrombocytopenia.
