Background: Sickle cell anemia (SCA) is a hereditary disease of the hemoglobin, characterized by recurring vaso-occlusive crises (VOC) leading to severe pain. VOCs constitute the primary cause for emergency department (ED) visits among sicklers. Frequent VOC episodes are associated with greater mortality. This study aimed to evaluate pain treatment trends regarding admission, discharge, length of stay in the ED, and early ED revisits among SCA patients. Methods: A cross-sectional study was conducted from January 2020 to January 2023 at the ED of King Abdulaziz medical city in Riyadh. SCA patients who presented with VOC episodes were included in this study. Demographic and clinical data of each patient were retrieved from the electronic medical file. Results: Total of 144 patients were included in the analysis. Out of the total population, 34% were admitted to hospital. Compared to those patients who were not admitted, the number of VOC was more among patients who had been admitted. Around 34% had ED revisit within 30 days and those patients had higher frequency of VOC attacks compared to those who did not revisit the ED. Only 45% of the population received appropriate dosing, 30% received an insufficient dose, and 25% received over the appropriate dose. Receiving the correct dose of morphine reduced the likelihood of ED revisits within 30 days. Conclusions: This study explores trends in VOC among sticklers, highlighting that adequate analgesia is associated with better outcomes. Addressing these variables may lead to improved care and better outcomes for SCA patients.

Chronic kidney disease is often accompanied with a syndrome known as protein-energy wasting, which is defined by decreased protein consumption, protein loss due to metabolic disturbances, and physical consequences include sarcopenia and frailty. Due to population aging and advancements in the treatment of other chronic illnesses, this problem has drastically increased in recent years. The frequency of morbidity and death in chronic kidney disease patients may be greatly decreased with proper therapy of as protein-energy wasting. Therefore, it's crucial to prevent and manage as protein-energy wasting in chronic kidney disease patients by doing a thorough evaluation and offering each patient a customized, evidence-based treatment plan. Once protein-energy wasting is recognized, a nutritionist should be contacted to create a management strategy. A thorough diet specifically designed to fit the patient's requirements, any required nutritional supplements, and counselling about dietary changes, symptom management, and psychological support should all be part of this strategy. The risk of protein-energy wasting may be decreased with proper assessment and care. Inadequate wound healing and scar formation continues to be major issues in day-to-day surgical practice. A wound infection (like pus formation, smelly discharge, and the incision line’s opening etc.) is one of them complications of surgical procedure. An infection may appear in surgical wound due to lack of care in post-operatively like dressing not being changed after regular interval, not following aseptic protocol during the dressing, and the dressing material not properly sterilized. Hence, the aim of proper care in post-operative practice was to reduce the risk of infection in surgical wound and promote healing. One of the most important aspects of post-operative wound care is prompt wound evaluation, appropriate hand washing, adhering to aseptic protocol when dressing, using sterile dressing material each time, and proper cleaning of the wound, as well as the prompt identification and management of wound complications.

Vaginitis cases are the most frequently encountered cases of gynaecologists in women who seek treatment at health facilities. Various therapies are sought by sufferers to overcome this condition. This case report discusses the use of Sujok triorigin therapy with the mudra method to treat the symptoms of vaginitis. Mudra method is the therapy only by using the thumb placed on certain knuckles based on the tri-origin concept and the respondents meditate for 15-20 minutes. There was no charge applied for the therapy were given to the patient. Two female patients on their 40s contacted researcher through social media to get help for their vaginitis problems. Researcher sent the instruction through picture how to apply the therapy on patients’ fingers. In the first case, this extreme vaginal itching condition had been experienced for 1 year with various treatment methods tried but none were successful. In this respondent, it took 9 days for the itching to completely disappear with therapy being done 4 times a day for about 15 minutes. The second respondent complained of extreme itching in the vagina, with a severity scale of 5 (from 1-5) and bleeding because of scratching. After 4 minutes of doing mudra, the itching went down to scale 3 and after 20 minutes the itching went down to a score of 1-2 and then was completely gone after that. In conclusion, Sujok triorigin therapy can reduce vaginitis symptoms in two respondents in a relatively short time of therapy.

Background: The objective of this study was to determine the factors associated with the psychological impact on patients due to the COVID-19 pandemic in primary care. Methods: Study was analytical cross-sectional, prior authorization from the local research committee and informed consent, a survey was applied to 468 beneficiaries of the family medicine unit number 2 of Mexico City, which contained sociodemographic aspects, background, habits and behaviors, styles of coping, fear of death, and the DASS-21 scale. Descriptive and analytical statistics were used Chi square OR CI at 95% considering significance when p<0.05. Results: The sociodemographic profile of the studied population were 61.3% women, 40.6% in the range of 35-59 years, 44.4% undergraduate education, 42.1% single marital status and 23.5% professionals. 56% reported symptoms in any of the mental health subscales, of which 35% were depressive symptoms, 44.9% anxiety symptoms and 37.8% stress symptoms. Associated factors were fear of the pandemic, having suffered from COVID-19, being a professional, feeling of loneliness, changes in character. p 0.00. Conclusions: The pandemic affected mental health, so it is important to carry out an intentional search for the degree of repercussions to provide timely treatment as part of comprehensive care at the first level of care.

Granular cell tumour (GCT) is rare and accounts for approximately 0.5% of all soft tissue tumours. The malignant GCT (MGCT) especially cutaneous malignant granular cell tumour is extremely rare constituting 1-2% of all granular cell tumours and mostly found in the subcutaneous soft tissues of lower exrtremities, especially thighs. The uncommon occurrence of cutaneous MGCT and their histopathological similarities with other entities make diagnosis difficult in some cases. Here we report a case of 36 years old male patient who presented with a mass in the skin of right lower abdominal wall which has been increased gradually over the last one year without pain. The size of the mass is approximately 6.5 cm in greatest dimension, firm in consistency with surface irregularity and ulceration diagnosed as malignant GCT at the histopathological examination showing focal ulceration and lined by keratinized stratified squamous epithelium revealing acanthosis and pseudoepitheliomatous hyperplasia. The dermis show neoplastic epithelioid cells arranged in sheets and nests with vesicular chromatin, conspicuous to prominent nucleoli, and abundant amount of fine granular eosinophillic cytoplasm. Mitosis is more than 2/10HP. Immunohistochemical stains for S-100, CD 68 and vimentin were positive in the lesional cells.

Background: Cervical intraepithelial neoplasia (CIN) is a precancerous cervical condition with the potential to develop into cervical carcinoma if untreated. While research on CIN and cervical carcinoma exists in various regions, the Rajshahi area lacks such investigations. Examining the prevalence of CIN and cervical carcinoma in this region is essential. This study aimed to assess the prevalence of CIN and cervical carcinoma in a tertiary care hospital at Rajshahi in Bangladesh. Methods: This cross-sectional descriptive study was conducted at the department of pathology, Rajshahi medical college, and the department of gynaecology and obstetrics, Rajshahi medical college hospital, Rajshahi, Bangladesh from July 2019 to June 2022. A total of 1000 VIA test-positive cases of cervical lesions were enrolled as study subjects purposively. Data were processed and analyzed by the MS office tools. Results: Cervical carcinoma was identified through cytological analysis in 4.6% of cases, with 7.6% exhibiting high-grade squamous intraepithelial lesions (HSIL) and 13.1% showing low-grade squamous intraepithelial lesions (LSIL). Through histopathological analysis, cervical carcinoma found in 5.3% of cases, CIN-grade III (CIN-III) in 7.1%, and CIN-II and CIN-I in 2.2%, 16.4% of cases, respectively. Human papillomavirus (HPV) vaccination rate was 0.70%. Conclusions: The prevalence of CIN and cervical carcinoma in the Rajshahi region of Bangladesh is alarmingly high. These findings underscore the pressing need for the immediate implementation of a comprehensive vaccination and awareness development program for cervical carcinoma.

Background: Career by choice means that a student chooses a career of their own choice according to their interests and the demands of time. The present study aimed to investigate the preferences of pursuing government jobs or higher education among final-year nursing undergraduates and perceived barriers to pursuing higher education in nursing. Methods: A descriptive cross-sectional web-based study was conducted among 207 final-year B.Sc. nursing students from May 2021 to June 2021 at the various All India Institutes of Medical Sciences of India. Data collection was done using total enumerative sampling techniques by self-structured questionnaire. A descriptive statistic was applied to prepare the results. Results: Findings suggested that the government job after graduation was the priority among students (63.3%), and only (30.4%) of students were highly motivated to pursue higher education in nursing. The majority (42.9%) of respondents given a reason to choose a nursing course were not selected in NEET-UG. Further data revealed that the reason for choosing the job over higher education was financial independence. Findings also reported that the student's perspectives towards the nursing profession were a lack of respect as compared to other medical professions, stigma in society, socially less accepted professions, fewer opportunities in research and education, and gender discrimination in the profession. Conclusions: The findings necessitate the need for a sensitization program to develop awareness of the benefits of pursuing higher education. This may change the stigma and improve the social image of the profession.

Chilaiditi's condition refers to the presence of a loop of small intestine or colon between the diaphragm and the hepatic rim, usually due to alterations in the attachment of the liver to the diaphragm. Chilaiditi syndrome is associated with abdominal pain as the most common clinical manifestation. Pneumoperitoneum should always be ruled out in the context of these patients. We presented the case of a male in his eighth decade of life who presented with data suggestive of drug-modified acute appendicitis, for which computed tomography of the abdomen identified acute appendicitis and Chilaiditi's condition. An open appendectomy was performed without complications and the condition resolved.

Background: Emotional intelligence and job satisfaction are topics of great interest in today's workplace. They provide a competitive edge in personal and professional life. However, there is scanty evidence in the Indian scenario that explores the assessment of these two notions among nurses. Therefore; this study was conducted to exhume the data regarding the assessment of Emotional intelligence and job satisfaction among nurses. The study aimed to identify the emotional intelligence and job satisfaction among nurses in selected hospitals of Udaipur. Methods: This cross-sectional study was carried out among 100 nurses at tertiary care teaching hospital from January 2021 to July 2021. Convenience sampling technique was employed for sample selection. Standardized questionnaire Schutte Self Report Emotional Intelligence Test (SSEIT) and Minnesota Satisfaction Questionnaire (MSQ) were used for data collection. Descriptive and inferential statistics (chi-square) were used for data analysis. Results: Most of the staff nurses (59%) experience high level of emotional intelligence. Emotional intelligence has found significant association with age, professional qualification, co-worker relationship and marital status and type of family. Furthermore, majority (65%) of the respondents had middle-level job satisfaction which has been found significantly associated with age, co-worker relationship, type of family, marital status and no. of children. Conclusions: Emotional intelligence and job satisfaction is an important prerequisite to becoming successful in personal and professional world. According to the study data, majority of staff nurses have higher emotional intelligence and higher job satisfaction which is directly related to an organization's production and efficiency.
