Impact of Green Tea Extract on Intraradicular Dentin Wettability and Bond Strength of Resin-Based Sealer Following Two Chelating Agents 绿茶提取物对两种螯合剂对树脂基密封剂根内牙本质润湿性和粘结强度的影响

Purpose: To evaluate the effect of 3% green tea extract (GTE) on wettability, depth of penetration, and push out bond strength (PBS) of AH Plus sealer to intraradicular dentin following the use of 0.2% Nano chitosan or 17% EDTA. Materials and methods: Twenty human radicular dentin segments were used for wettability assessment. Following immersion in 2.6% NaOCl solution, samples were divided into 2 groups (n=10) according to the chelating agent used: group I: 0.2% Nano chitosan, and group II: 17% EDTA, each used for 3 min, then the contact angle (CA) was measured. The samples were further immersed in 3% GTE solution for 5 min followed by CA measurement. Twenty human mandibular premolars were prepared up to # X4 ProTaper Next files, then divided into two groups as mentioned previously. Each group was subdivided into two subgroups based on the final flush used (n=5); subgroups A and B; no treatment, and 3% GTE, respectively. Samples were obturated by cold lateral compaction using AH Plus sealer and gutta-percha. PBS was measured using a universal testing machine and sealer penetration was assessed using confocal laser scanning microscopy. Results: Using 3% GTE following the use of 0.2% Nano chitosan significantly increased the wettability and PBS of AH Plus sealer to intraradicular dentin, while its use following 17% EDTA significantly decreased the wettability and PBS of AH Plus sealer. Using GTE increased the maximum depth of AH Plus sealer penetration into the dentinal tubules. Conclusion: GTE improved the wettability, depth of penetration, and push out bond strength of AH Plus sealer to intraradicular dentin treated with Nano-chitosan. Nevertheless, it adversely affected those properties of AH Plus sealer to EDTA-treated dentin. Volumetric Bone Change After Using Liquid Phase Concentrated Growth Factor in Open Sinus Lift Surgery: A Comparative Study 开放窦提术中使用液相浓缩生长因子后骨体积变化的比较研究

Purpose: To evaluate of the volumetric bone change after using liquid phase concentrated growth factor in open sinus lift surgery .Material and methods 12 pneumatized maxillary sinuses in edentulous or partially edentulous patients were divided randomly into 2 equal groups; Group (A), augmented by alloplastic bone graft Biphasic Calcium Phosphate (BCP). Group (B), augmented with BCP mixed with liquid phase concentrated growth factor (LPCGF). Visual Analogue Scale (VAS) was used to assess pain and swelling. CBCT was used to assess the volume by measuring height × surface area of augmented bone immediate and 6 months postoperatively. Clinical follow up period was at 1st, 2ndday then, after one week postoperatively. Results 12 sinus , bilateral 71.4%, unilateral 28.5%, female 42.8%, male 57.2% were subjected to sinus lift surgery. Mean age was 50y and average length of residual bone was 7 mm. There was no significant difference between the two groups regarding primary wound healing with percentage of success 71.4%. VAS scale representing swelling of 42.8% in group (A) and 28.5% in group (B). Pain score show decrease gradual till one week to be zero. In comparing between two groups, percentages of change in volume was highly significant (p=0.004) between two groups; in group (A) was 29.31±13.23 while, in group (B) was 2.58±1.75. Conclusion: Use of LPCG mixed with BCP bone graft to augment the sinus with open sinus lift surgery showed less bone resorption as it represented with low percentage of change in bone graft volume at 6 months postoperative . Evaluation of the Effect of Turmeric Gel and Turmeric Powder as Pulpotomy Medicaments in Primary Teeth 姜黄凝胶和姜黄粉作为乳牙切髓药物的疗效评价

Purpose: to evaluate the effect of turmeric gel and turmeric powder as pulpotomy medicaments in primary teeth. Materials and Methods: Ninety primary molars in 28 patients, with age range from 4 to 7 years were selected according to inclusion criteria, in each patient two primary molars at least were required for pulpotomy. The patients were allocated into three groups according to the type of pulp dressing material. The follow up periods were 1, 3 and 6 months for clinical evaluation. Periapical radiographs were taken at 3 and 6 months recall visits. Results: Results showed all cases in the three groups revealed clinical success at 1 month, 3 as well as 6 months follow up periods. Radiographic study displayed no statistically significant difference between the three groups after 3 months, while there was a statistically significant difference between the three groups after 6 months. Curcumin powder group revealed the highest prevalence of internal resorption followed by Curcumin gel group while Formocresol group revealed the lowest prevalence of internal resorption. However; the difference between survival of the three groups was no statistically significant. Conclusions: Turmeric (curcumin) powder and turmeric (curcumin) gel showed excellent clinical success, acceptable radiographic success and good survival, so the turmeric powder and turmeric gel may be promising biocompatible materials for dressing the radicular pulp after pulpotomy in primary teeth.
目的:评价姜黄凝胶和姜黄粉作为乳牙截髓药物的疗效。材料与方法:根据入选标准,选取年龄4 ~ 7岁的28例患者90颗磨牙,每例患者至少需要2颗磨牙行切髓术。根据牙髓敷料的类型将患者分为三组。随访时间分别为1、3、6个月进行临床评价。在3个月和6个月时进行根尖周围x线片检查。结果:三组患者随访1个月、3个月、6个月均取得临床成功。影像学检查显示3个月后三组间差异无统计学意义,6个月后三组间差异有统计学意义。姜黄素粉组内吸收率最高,姜黄素凝胶组次之,福摩甲酚组内吸收率最低。然而;三组生存率差异无统计学意义。结论:姜黄素粉和姜黄素凝胶具有良好的临床疗效、良好的影像学成功率和良好的成活率,是一种很有前景的用于乳牙根状髓切断术后修复的生物相容性材料。

Comparative Study of Erbium Laser Versus A Chemo-Mechanical Caries Removal Method in Primary Teeth 铒激光与化学机械除龋法在乳牙中的比较研究

Purpose: The purpose of this study was to evaluate the caries removal using Er,Cr:YSGG laser and Papacarie chemo-mechanical caries removal (CMCR) method in primary teeth. Materials and Methods: Patients (4 – 8 years old) having 40 carious primary molars with dentin involvement were selected for this study. Caries was removed using two methods; Group 1 (Laser) Waterlase MD turbo Er,Cr:YSGG Laser using Quartz tips (MZ – 8) on the Gold handpiece and group 2 (CMCR) Papacarie Duo gel. Final restoration was done by Riva light cure Glass ionomer. Caries removal was assessed clinically by tactile and visual method (Ericson criteria). Time taken for the procedure was measured in minutes. Evaluation of pain perception was done using the Wong-Baker facial image. Microbiological evaluation was done by collecting dentin samples from the carious primary tooth before and after caries removal and total bacterial count was assessed. The restoration was evaluated using the United States Public Health Service (USPHS) at baseline, three months, six months and nine months. Results: The two groups showed a statistically significant difference (unpaired t test) in the time of treatment, Pain score and total bacterial count. Clinical evaluation of the restoration using the modified USPHS criteria showed no statistical significance between the two groups. Conclusion: Chemo-mechanical caries removal methods and laser irradiation are effective and efficient techniques for caries removal in children with high patient acceptance and no adverse effects.

Effect of Different Contemporary Rotary Nickel Titanium Systems on Root Canal Geometry 当代不同镍钛旋转系统对根管几何形状的影响

Purpose: This study aimed to compare NiTi rotary systems of different wires namely; EdgeFiles, Profile GT series X (GTX) and ProTaper (PT) in terms of: canal transportation and centering ability. Materials and Methods: One hundred and twenty extracted human mandibular first molars were selected and randomly divided into two main groups (I&II) according to the ranges of curvature (60 teeth each), where group I: samples with 25o40o range of curvature and group II: samples with 45o60o range of curvature. Each group was subdivided into four subgroups according to the NiTi rotary system (15 teeth each): subgroup A: ProTaper, subgroup B: Profile GT Series X, subgroup C: EdgeFile X3 and subgroup D: EdgeFile X5. The root canals were scanned by i -CAT CBCT scanner before and after instrumentation at 3, 5 and 8 mm levels from the apex, to assess the canal transportation and centering ability. Results: The EdgeFile system produced the least canal transportation and remained more centered around the original canal than the other systems. While, PT system produced significantly the highest canal transportation and the least centered canal preparation than other systems. Conclusions: EdgeFile systems showed superior performance regarding maintenance of the original root canal curvature and centering ability than do other systems.

The root canals were scanned by i -CAT CBCT scanner before and after instrumentation at 3, 5 and 8 mm levels from the apex, to assess the canal transportation and centering ability. Results: The EdgeFile system produced the least canal transportation and remained more centered around the original canal than the other systems. While, PT system produced significantly the highest canal transportation and the least centered canal preparation than other systems. Evaluation of the Antimicrobial Effect of Anise Extract on Cariogenic Oral Microflora 大茴香提取物对口腔龋齿菌群抑菌效果的评价

PURPOSE: To measure the antimicrobial effect of Anise extract as mouth rinse in children against streptococcus mutans and lactobacilli. Material and method: A total of 60 Egyptian children from both sexes ranging from 6 to 12 years old for mixed dentition in a good physical condition. Children were randomly distributed into three groups (A, B, C) (20 children each). Group A: Patients were instructed to rinse with 40ml of anise extract (anise 1.6g/ 40ml) three times/ day, each rinse for 1 minute for 1 week. Group B: Patients were instructed to rinse with 40ml of anise extract (anise 10g/ 40ml) three times/ day, each rinse for 1 minute for 1 week. Group C (control group): Patients were instructed to rinse with 40ml of distilled water three times/ day, each rinse for 1 minute for 1 week. Results: There was statistical significant reduction in streptococcus mutans and lactobacilli count in Group B (Anise 10g/ 40ml group). Conclusion: Anise extract can be used as a preventive therapy at home to maintain oral hygiene as it is natural, safe and has no side effects.

Purpose: This study was conducted to evaluate the biological pulp response to Pulpine and Polyamidoamine Dendrimer and their combination in dogs and comparing their remineralizing potentials on human caries affected dentin. Materials and Methods: Forty-five teeth in 3 mongrel dogs were randomly divided into 3 groups according to the applied pulp capping material; group A1: Pulpine NE, group A2: PAMAM G3 and group A3: Pulpine and PAMAM. Class V cavities were prepared; the pulps were mechanically exposed and finally restored with resin modified glass ionomer. Histological evaluation was performed after 2 weeks, 1 and 2 months. The in vivo study was designed as a randomized, unicenter, parallel, two arms, double– blind split mouth clinical trial with 1:1 allocation ratio. The subjects who fulfilled the eligibility criteria were asked to participate and signed informed consent. Twenty-eight adhesive occlusal cavities were prepared; infected dentin was removed leaving carious affected dentin at the pulpal floor and divided into 2 groups; P1 (Pulpine) and P2 (Pulpine and PAMAM). Remineralization was assessed radiographically at base line, 1, 3 and 6 months. Results: In the animal study, there was no statistically significant difference among the tested direct pulp capping materials regarding the inflammation intensity, dentin bridge continuity or morphology. For the clinical trial, P2 group showed statistically significant higher dentin density at all study intervals. Conclusion: Pulpine and PAMAM enhanced pulp tissue repair. PAMAM could induce biomimetic remineralization of caries affected dentin. Evaluation of Antibacterial Effect of Different Sucrose Free Hard Candies in High-Risk Patients 不同无糖硬糖对高危患者抗菌效果的评价

Purpose: This study was conducted to evaluate the effect of four different hard candies on salivary Streptococcus mutans count and change in salivary pH in a group of Egyptian college students. Materials and Methods: Forty-eight healthy dental students, aged 16 -22 years were randomly allocated into 4 different study groups each of 12 students: sucrose free hard candies with licorice extract group, sucrose free hard candies with xylitol extract group, sucrose free hard candies group, and sucrose containing hard candies group (control). Unstimulated saliva was collected from the participants at baseline, following which the participants were given respective hard candies twice daily for 10 days and the subsequent salivary samples were collected at intervals of 14 days and 6 months. The samples were analyzed for Streptococcus mutans count in terms of CFU and pH using pH meter. Results: Regarding baseline – 14 days interval, there was a statistically significant decrease in mean percentage change on bacterial count in licorice and xylitol group Moreover there was also a significant difference in salivary pH between different hard candies Conclusion: Addition of licorice or xylitol to sucrose free hard candies enhances its effect on streptococcus mutans and can shift salivary pH to a neutral value.

Histopathological Changes after Direct Pulp Capping in Dogs with Bioactive Glass Incorporated in Resin Composite and Adhesive 生物活性玻璃树脂复合材料和胶粘剂直接盖髓后狗的组织病理学变化

Purpose: To evaluate histopathologically the dental pulp response to bioactive glass nanoparticles in the resin composite restoration and the adhesive as a direct pulp capping material regarding inflammatory cell infiltrate, hard tissue formation and Pulp tissue disorganization. Material and methods: Four mongrel male dogs were selected for this study with a total of 40 teeth were divided into two groups (n=20) according to the treatment protocol (P); (P1) treated with light cured calcium hydroxide (control group) and (P2): treated with resin composite and its adhesive containing bioactive glass. The histopathological findings were studied for each specimen at two times intervals (T); (T1) fourteen days and (T2) sixty days. Split mouth technique was followed. The manufacturer’s instructions were followed during applying the materials. Animals were sacrificed at each time interval and teeth were collected for histopathological finding. Results: adhesive and resin composite restoration containing bioactive glass group showed significantly less inflammatory cell infiltrate, less pulp tissue disorganization and better calcific bridge formation than Calcium hydroxide group. When the effect of time was considered, inflammation was significantly decreased from fourteen days to sixty days while hard tissue formation was significantly increased for both materials. Also pulp tissue disorganization was decreased significantly and showed more normal pulp tissue morphology from fourteen days and to sixty days. Conclusion: resin composite restoration and its adhesive containing nanoparticles of bioactive glass could be used as a direct pulp capping material. Purpose: To investigate the effect of Glycogen Synthase Kinase-3 (GSK3) antagonist on pulp tissue repair in rabbits. Materials and Methods: 24 adult healthy male rabbits were used in this study divided into three main groups: Group I (control): 2 rabbits with normal teeth without pulp exposure. Group II (TG): The exposed pulps of right posterior teeth of 22 rabbits were capped with KolSpon in association with 34 nM Tideglusib. Group III (MTA): The exposed pulps of left posterior teeth of 22 rabbits were capped with Mineral Trioxide Aggregate (MTA). The 2 rabbits with normal teeth and 6 rabbits with pulp capped posterior teeth were euthanized after one day. Their DNAs were extracted and tested for expression of Axin2 and β-actin by qPCR. 8 rabbits were euthanized after 4 weeks, and another 8 was euthanized after 6 weeks. The teeth were collected and prepared for immunohistochemical study using antiβ-catenin antibody. Results: The qPCR results showed the higher expression of Axin2 in the Tideglusib group than control group (without treatment) by about 5 folds and about 1.5 fold in the MTA group. The immunohistochemical results revealed the increase intensity in the Tideglusib group than MTA at four weeks with no significance difference (p>0.05).Whilst after 6 weeks the higher expression was in Tideglusib group with significance difference (p<0.05). Conclusion: The Tideglusib drug stimulates the expression of Axin2 transcription factor gene which is known to play a key role in regulation of odontoblast and osteoblast differentiation.
