Un cas de ténosynovite tuberculeuse de la main est rapporté. Un médecin travaillant dans un service de pneumologie s'est blessé sur le dos de la main avec une aiguille, après une ponction pleurale. Un œdème est apparu, associé à une douleur modérée et une raideur métacarpophalangienne. La synovectomie a permis un diagnostic histologique et bactériologique. L'association de la synovectomie et de l'antibiothérapie a permis un excellent résultat. La ténosynovite tuberculeuse de la main est une rare manifestation extrapulmonaire de la tuberculose, survenant dans moins de 5% des localisations de l'appareil locomoteur.

We present a case of tuberculous tenosynovitis of the extensor tendons of the hand. Our patient was a young doctor working in the respiratory medicine department. He was injured on the dorsal aspect of the hand with a needle used for pleural aspiration. The clinical features consisted of gradually swelling, mild pain and stiffness of the metacarpophalangeal joint. The diagnosis was made after synovectomy. Histological and bacteriological examinations revealed tuberculosis. Treatment consisted of synovectomy and appropriate antibiotics. The clinical course was excellent after one year follow-up. Tuberculous tenosynovitis of the hand is a rare manifestation of extrapulmonary tuberculosis occurring in fewer than 5% of all cases of skeletal tuberculosis. Thickening of the tendon or synovial sheath and local accumulation of fluid are the characteristic manifestations. The diagnosis must be confirmed by surgical biopsy. Antibiotics and synovectomy achieve a good functional result.

Se presenta un caso de tenosinovitis de etiologia tuberculosa a nivel de la mano. Un médico se hirió en el dorso de la mano con una aguja, luego de una punción pleural. Consecutivamente se presentó un edema asociado a un dolor moderado y una rigidez metacarpofalángica. La sinovectomía corroboró el diagnóstico histológico y bacteriológico. La asociación de la sinovectomía y de la antibioticoterapia lograron un excelente resultado. La tenosivitis tuberculosa de la mano es una rara manifestación extrapulmonar de la tuberculosis y se presenta con una frecuencia inferior al 5% de las lesiones a nivel del aparato locomotor.

L'hypoplasie du pouce du type III selon Blauth reste rare et de traitement controversé. Récemment Manske a défendu la reconstruction dans le soustype IIIA comportant une articulation trapézo-métacarpienne, proposant la pollicisation dans le sous-groupe IIIB où l'articulation basale est absente. Nous avons revu 14 reconstructions du type III, avec un recul moyen de cinq ans. Lorsqu'un transfert microchirurgical de la seconde articulation métatarso-phalangienne est pratiqué, les résultats sont peu différents dans les deux sous-groupes avec même une meilleure stabilité articulaire. Les résultats restent cependant globalement inférieurs fonctionnellement et surtout esthétiquement à ceux de la pollicisation.

Thumb hypoplasia type III according to Blauth remains a rare congenital malformation. Recently Manske has promoted reconstruction versus pollicisation in the sub-type IIIA where a first carpometacarpal joint is present. However we felt that pollicisation is the solution for sub-type IIB where the basal joint is absent. We have reviewed 14 cases of thumb hypoplasia type III, four of them being type IIIB. After performing a first step with a free vascularized second metatarso-phalangeal joint transfer, the secondary steps were identical in both sub-groups. After a mean follow up of five years, no great difference was found in the two sub-groups and basal stability was even better in type IIIB. However the results were functionally and cosmetically inferior to the ones observed after pollicisation. When the relatives refuse pollicisation or the patient consults late for functional improvement, reconstruction remains worthwhile.

La hipoplasia del pulgar del Tipo III de Blauth sigue siendo rara y el tratamiento controvertido. Recientemente Manske defendió la reconstrucción en el subtipo IIIA que comporta una articulación trapezometacarpiana, proponiendo la policisación en el subgrupo IIIB en la que la articulación basal está ausente. Hemos revisado 14 reconstrucciones del tipo III, en una perspectiva de 5 años de promedio. Cuando se practica una transferencia microquirúrgica de la segunda articulación metatarso falangiana, los resultados difieren poco en los dos subgrupos con una mejor estabilidad articular incluso. Los resultados son sin embargo inferiores funcionalmente y sobre todo estéticamente a los de la policisación.

We report a case of carpal injury not previously described, associating three lesions of the carpus secondary to a hyperextension mechanism: fracture of the capitate, scapho-trapezio-trapezoid sprain and lunotriquetral ligament rupture. The patient was operated by percutaneous pinning after closed reduction. The capitate fracture was not fixed. At follow-up, the patient had no restriction of his everyday activities and was very satisfied.

Nous rapportons un cas de lésion carpienne originale associant 3 lésions et secondaire à un mécanisme d'hyperextension: une fracture du capitatum, une entorse de l'articulation scapho-trapézotrapézoïdienne, et une rupture du ligament luno-triquétral. Le patient a été opéré par brochage percutané après réduction orthopédique. La fracture du capitatum n'a pas été synthésée. Au recul d'un an, le patient était satisfait sans limitation dans les activités quotidiennes.

Relacionamos un caso de traumatismo carpiano que no se había descrito previamente, el cual asocia tres lesiones del carpo secundarias a un mecanismo en hiper extensión: fractura del hueso grande, esguince del ligamento escafotrapezotrapezoideo y lesión del ligamento lunopiramidal. Después de la reducción ortopédica se fijó la reducción con agujas. La fractura del hueso grande no se fijó. En el último control, no existe limitación en las actividades cotidianas y el paciente esta muy satisfecho.

Krukenberg's operation was actually described by Vanghetti in 1989. Three cases, including one bilateral case, are reported. The first case was a 36-year-old patient from a developing country. At the sixth postoperative week, the patient had regained his independence and gained 1.5 kg. The second case was a child with multiple malformations, who presented, in addition to amputation of both legs, a high amputation of the left forearm and distal fold of the right forearm. Krukenberg's operation was performed at the age of 8 months. At the age of 2 years, the child was able to eat and drink independently.

L'opération dite de Krukenberg a été en réalité décrite par Vanghetti en 1989. Trois cas sont rapportés, dont un cas bilatéral. Il s'agissait d'un patient de 36 ans, originaire d'un pays en voie de développement. A six semaines postopératoires, le patient a retrouvé son indépendance avec une prise d'un kilo et demi. Le second cas concerne un enfant polymalformé qui présentait, outre une amputation des deux jambes, une amputation haute de l'avant-bras gauche et du pli distal de l'avant-bras droit. A l'âge de 8 mois est effectuée l'opération de Krukenberg. A 2 ans, l'enfant boit, mange de façon indépendante.

La operación de Krukenberg realmente fue descrita por Vanghetti en 1989. Se presentan 3 casos de los cuales uno bilateral. Se trataba de un paciente de 36 años, originario de un país en vía de desarrollo. A la sexta semana post operatoria le paciente recuperó su independencia con una prensión de un kilo y medio. El segundo caso se trataba de un niño polimalformado que presentaba, además de una amputación de las dos piernas, una amputación proximal del antebrazo izquierdo y del pliegue distal del antebrazo derecho. A 8 meses de edad se efectúo la operación de Krukenberg. A 2 años, el niño bebe y come de manera independiente.
