A FALL IN LEUKEMIA mortality around the year 1940 was first discerned among white infants of the United States (1). A subsequent analysis of the secular trends in U.S. children by Slocumb and MacMahon (2) showed that with passing time the decline had also begun to affect successively older children. By 1959, a decline apparently had occurred in the mortality for 3-year-olds (2). More recently, Fraumeni and Miller (3) reported that the once increasing leukemia mortality rates among U.S. whites between the ages of 1 and 74 years had begun to decline. This latest observation suggested that the wave effect in the mortality rates (the increase and subsequent decrease occurring later in time with in-

Background and Purpose The Epidemiology Branch of the Division of Dental Health, Public Health Service, in its National Cleft Lip and Palate Intelligence Service, uses birth certificates for studies of clefts and other congenital malformations. In this study an attempt was made to assess how faithfully the data recorded on birth certificates reflect the incidence of congenital malformations diagnosed in newborn infants. Two elements of the problem were investigated using hospital records as a standard. Completeness of reporting was measured by the number and types of malformations reported on the birth certificates, and accuracy was evaluated by the specificity and fidelity of language describing the conditions. Another purpose of this study was to analyze selected variables that may be related to the completeness of congenital malformation reporting on birth certificates.

ADMISSION RATES to Connecticut State hospitals show a strong tendency to reflect Jarvis' law-that is, to vary inversely with distance between the patient's residence and the hospital. The influence of accessibility is independent of the other variables known to affect utilization of State hospitals; it is not an artifact due to population density or the presence of high-risk groups (1), and it is not explained by the location of alternative psychiatric service (2). It also is not accounted for by errors in classifying the residence of patients on admission (see box). Jarvis' law should be considered in planning service in future mental health centers, which will have smaller catchment areas than most State hos-
