Sie werden verstehen, daß man in fast jedem Referat gezwungen ist, aus der Fülle der Befunde und der Unterlagen eine Auswahl zu treffen. So umfaßt unser Thema „Pulmonale Hypertension" jeden Hochdruck im kleinen Kreislauf, unabhängig von seiner Ätiologie. Das Cor pulmonale mit seiner pulmonalen Hypertension auf der Basis von Veränderungen der Lunge und ihrer Gefäße bzw. von Abweichungen der Thoraxform und -wand sowie die primäre pulmonale Hypertension können wir heute nur streifen (KÖNN [1958], GOERTTLER [1965]). Wir wollen uns vorwiegend der pulmonalen Hypertension nach angeborenen Herzund Gefäßfehlern sowie konnatalen und erworbenen Klappenfehlern widmen. Damit ist für unser Thema auch die Ätiologie der pulmonalen Hypertension schon umrissen. Wir stützen uns auf die spezielle Untersuchung von 58 Fällen.

Unter den Zwerchfellhernien stehen nach großen Zusammenstellungen die Hiatushernien an erster Stelle. ADAMS und LOBB beschrieben 1954 eine Sonderform, die besonderes klinisches Interesse verdient, weil sie in der Regel schon auf einer gewöhnlichen Thoraxaufnahme durch eine Aufhellung hinter dem Herzen mit Flüssigkeitsspiegel erkenntlich ist. Abb. 1 zeigt den Befund bei einem 76jährigen Gärtner J. P. (Arch. Nr. 391/49), der 1949 wegen akuter Harnverhaltung in die Chirurgische Klinik aufgenommen werden mußte. Er stand seit längerer Zeit wegen Herzmuskelschwäche mit Ödemen und Atemnot in ärztlicher Behandlung. Der Arzt meldete auch „starke anginöse Herzbeschwerden". Retrospektiv kann man sich natürlich fragen, ob diese Beschwerden nicht durch die große Zwerchfellhernie bedingt waren. Sonst wurden aber keine Klagen vorgebracht, die auf den Magen hingewiesen hätten.

Im Jahre 1865 veröffentlichte KRAUSE in der „Zeitschrift für Rationelle Medizin" die Abbildung eines anomalen Koronargefäßzweiges, der mit der Arteria pulmonalis verbunden war (Abb. 1). KRAUSE hat wahrscheinlich als erster über eine derartige Anomalie berichtet, aber die Folgerungen, die er in bezug auf die Zirkulation in diesem Gefäßgebiet zog, waren nicht ganz richtig. 20 Jahre später, 1885, beschrieb BROOKS in „The Journal of Anatomy and Physiology" zwei Fälle von rechtsseitigen Koronararterien, die von der A. pulmonalis ausgingen und aneurysmaähnliche Anastomosen mit dem gesunden Koronararteriensystem bildeten. BROOKS kommt im Gegensatz zu KRAUSE ZU der Folgerung, daß die Strömung in dem anomalen Gefäß einen arteriovenösen Shunt von der normal verlaufenden Koronararterie zur A. pulmonalis darstellt. Dieser physiologisch richtige Gesichtspunkt in bezug auf die Zirkulation innerhalb anomaler Koronargefäßgebiete wird von EDWARDS im Jahre 1958 in seinem Bericht über die Bedeutung anomaler Koronararterien vertreten. Er hat die Abnormitäten der Koronararterien ihrer Bedeutung nach in drei große Gruppen eingeteilt : Bei Gruppe 1 fehlt eine größere klinische Bedeutung. Die Gruppe 2 behandelt Anomalien, die in bezug auf gewisse kongenitale Vitien als sekundär anzusehen sind, und Gruppe 3 enthält die primären und vom klinischen Gesichtspunkt aus bedeutungsvollen Anomalien. Parmi 89 enfants et adultes atteints de ce qu'il est convenu d'appeler «tumeurs du médiastin» (1956 1965) nous avons identifié 21 tumeurs neurogènes (24%) soit: Neuroblastome 1 (fillette de 1 mois) Ganglio-neuroblastome 1 (fillette de 5 ans) Ganglio-neuromes 10 (9 enfants de 5 à 15 ans et 1 adulte de 32 ans) Neurinomes ou schwannomes dont un kystique 7 (adultes de 26 à 54 ans) Neuro-fibromes 2 (adultes de 29 à 31 ans) 13 tumeurs étaient à droite, 8 à gauche. Le sexe féminin (13 observations) est plus atteint que le sexe masculin (8 observations), sans qu'il soit possible d'expliquer cette prédominance souvent relevée. L'origine nerveuse de certaines tumeurs endothoraciques a été soupçonnée pour la première fois il y a une centaine d'années. Pendant longtemps ces néoplasmes n'ont guère retenu l'attention; puis, les moyens radiologiques d'investigation sont venus en montrer la relative fréquence et ont rendu possible leur dépistage avant l'apparition de troubles cliniques. Grâce aux progrès de la chirurgie, la lumière des scialytiques a jeté un peu de clarté dans la pathologie thoracique; les examens histologiques des pièces d'exérèses ont contribué à l'établissement d'une classification précise de ces tumeurs (BARIÉTY, CORNET, HEIMBURGER, H O F STETTER, etc, etc). I am glad to describe the architecture of the mitral valve, a choice of words that interests me because the term architecture implies much more than "anatomy". I presume that an architect designs a building with its function in mind; he should consider both form and function. As a student I gained the impression that anatomy was concerned essentially with form, and that physiology dealt with function. This is, of course, an unsatisfactory division and a change of thought is seen today in that anatomy now concerns itself with function as much as form and has even introduced a change of name. A department of anatomy is now called a department of human biology or of biological structure etc. The change is significant but it is by no means new. In this very College, although not in this building, William Harvey over 300 years ago gave his lectures on anatomy. If you read these you will perceive at once that he was presenting function as well as form ; physiology as well as anatomy. The Oxford Dictionary gives 1615 as the earliest date on which the term physiology was used. In 1628 William Harvey published his immortal book which he called "Exercitatio anatornica de motu cordis et sanguinis in animalibus", that is "Anatomical studies on the motion of the heart and the blood". It is clear from Harvey's title and from his other writings, and especially from his long series of Lumleian lectures, that he did not differentiate between anatomy and physiology. In his lectures he defines anatomy as "the faculty which through inspection and dissection reveals the uses and actions of the parts". To us this definition includes physiology and, as you know, Harvey's discovery is generally accepted as the beginning of physiology. Obviously in Harvey's time the term "anatomy" had a wider and fuller connotation than it had when I was a medical student. It comprised both anatomy and
