Introduction: Bacteremia and bile duct infection following percutaneous transhepatic biliary drainage (PTBD) are severe complications. Thus, the aim of this study was to investigate the risk factors of mortality and to evaluate the differences between blood and biliary drainage culture positive patients after PTBD. Materials and Methods: This is a retrospective study of 90 patients without any signs of infection prior to the PTBD procedures. Of these patients, we further divided them into three groups based on culture results: 1) first group was only bacteremic, 2) second group was only bile drainage culture-positive, and 3) third group was both bacteremic and bile drainage culture-positive. We compared clinical and laboratory parameters between these three groups and also evaluated the culture results in a high-resistance setting. Results: There were 90 patients who developed bacteremia, bile duct infections or both. In the laboratory findings, bacteremic patients showed significantly higher serum level of alanine aminotransferase (ALT) (p=0.001). Elevated neutrophil counts, lipase levels and carbapenem-resistant isolates were found to be the independent risk factors for 30-day mortality after PTBD. (p=0.01, p=0.01, and p=0.04, respectively). Gram-negative organisms were present in 71.2% of the cases; Escherichia coli was the most common species (28.1%) followed by Enterococcus faecium (18.1%), Klebsiella pneumoniae (15.8%) and Pseudomonas aeruginosa (9.9%). 51 (41.8%) of the cultured gram-negative bacteria were extended spectrum beta-lactamases positive and carbapenem resistance was found in 27 (15.8%) isolates. Conclusion: Elevated ALT level was only difference between three groups. Therefore, bacteremic and bile culture positive patients should be treated with the same care. Elevated neutrophil counts, lipase levels and carbapenem-resistant isolates were the independent risk factors for mortality. Finally, the choice of antibiotic prophylaxis should be reviewed according to the antimicrobial resistance profiles.

Objective: This study aims to investigate the smoking status of teachers, the relationship between smoking and COPD, and teachers' awareness of COPD. Material and Method: 504 teachers participated in this cross-sectional study. The data was obtained through an online questionnaire utilizing a 22-question form designed by the researchers. Results: The mean age of the participants was 38.677.70 years, 47.8% of them were female, and 81.5% worked in public schools. 28% of teachers reported smoking. 24.4% of teachers reported living with a COPD patient, with 86.5% citing smoking as the major risk factor for COPD. 88.3% of participants indicated that avoiding tobacco products is crucial in preventing COPD. The majority of participants (92.7%) provided the correct response when asked about the symptoms of COPD. While 67.7% responded that PFT is used for diagnosis, 50.6% reported that it is a treatable disease, and 25% reported that inhaler medicines are used for treatment. There was a statistically significant distinction between the frequency of hearing about COPD and the specialties of the teachers. It was shown that 91.1% of smokers were aware of COPD, however, no statistical correlation was found between smoking and awareness. Conclusion: This study revealed that the prevalence of smoking among teachers in Sanliurfa was lower than previously reported in the literature. The level of awareness that COPD is a lung disease was found to be higher than in previous studies of a similar sort. It was established that awareness of the risk factors for COPD, its symptoms, preventive strategies, and the fact that COPD is a treatable disease was high. We believe that this awareness will play a crucial role in protecting students from smoking and various smoking-related diseases, such as COPD.

Background: Crush injury damages the nerve, affects its function and causes oxidative stress. Alpha-lipoic acid (ALA) is an antioxidant agent with protective effects on the nerve tissue. In this study, we aimed to investigate the effects of ALA in the treatment of crush sciatic nerve injury in rats. Materials and Methods: Forty rats were divided into five groups. Walking Track Analysis (WTA) was performed in all groups before sacrificing the sciatic nerve. In group I (sham group), the sciatic nerve was exposed but not crushed, whereas in group II (early control group, 24th hour), group III (late control group, 7th day), group IV (early experimental group, 24th hour), and group V (late control group, 7th day), the sciatic nerve was exposed and clipped with an aneurysm clip for 300 seconds. One hour after the crush injury, subjects in groups II and III were given saline (2.5 ml, intraperitoneally), while ALA (100 mg/kg, intraperitoneally) was administered in Groups IV and V. WTA was performed in Groups I, II, and IV at the 24th hour after clipping and was performed in Groups III and V at 7th day after clipping. In all groups, the Sciatic Functional Index (SFI) was calculated after WTA. Following the completion of WTA, sciatic nerve tissue samples were obtained for the measurement of malondialdehyde (MDA), superoxide dismutase (SOD), glutathione peroxidase (GSH-Px) and catalase (CAT) enzyme values. Results: The SFI scores in groups II, III, IV, and V were significantly lower than that of sham group (p˂0.05), while no significant difference was found between groups II and IV and between groups III and V (p>0.05). The CAT values of groups II and IV, the GSH-Px value and MDA value of group IV, and the SOD values of groups II and IV were found to be significantly higher than those of sham group (p<0.05). However, no significant difference was found among groups I, III, and V with regard to CAT, GSH-Px, MDA and SOD values (p>0.05). Conclusion: The results indicated that a single dose of ALA (100 mg/kg) administered intraperitoneally one hour after the sciatic nerve crush injury had no therapeutic efficacy at 24 hours and 7 days after the administration. Further experimental studies are needed to evaluate the effectiveness of ALA applied in several doses rather than a single dose in crush peripheral nerve injury models.

Objectives: Aim of this study is to analyze the correlation between thiol/disulfide homeostasis (TDH) and papillary thyroid carcinoma (PTCA) and to reveal the possible impact of TDH on the development of PTCA. Material and Methods: Preoperative venous blood sampling was obtained from the patients having applied to our hospital due to euthyroid nodular goiter and detected with suspected malignancy or malignant cytology in FNAB, according to the BETHESDA system. Those whose postoperative histopathological results proved the presence of PTCA were involved in this study. Healthy volunteers who were without any thyroid nodules and thyroid dysfunction. Results: The number of participants in the patient and control groups was 34 and 101, respectively. Compared to the control group, native thiol (μmol/L), total thiol (μmol/L), and native thiol/total thiol × 100 values were ascertained to be lower in the patient group with PTCA; however, these values were not statistically significant. Disulfide, disulfide/native thiol × 100, and disulfide/total thiol × 100 values were detected to be lower in the patient group with PTCA, but not statistically significant. When the patients with PTCA were assessed within themselves, TDH parameters were not associated with the PTCA subtype, the number of foci, the presence of lymphovascular invasion, and the presence of metastatic lap. Conclusion: Considering the parameters of PTCA and TDH, the results were not statistically significant. And besides postoperative histopathological findings such as the PTCA subtype, the number of foci, the presence of lymphovascular invasion, and presence of metastatic lap were not correlated with THD parameters.

Aim: This research was carried out to develop a measurement instrument to determine the innovative behaviors of nurses and midwives. Materials and Methods: This study, which is of a methodological nature was applied with 216 nurses and midwives between 01.06.2021-31.09.2021. The item pool of the scale was created by the researchers with the literature as twenty-three items in a five-point Likert type and submitted to the opinions of twelve specialists. Results: The three-factor scale structure designed by making explanatory and confirmatory factor analysis was confirmed with the validity and reliability. According to the analysis results; the scale consisted of three sub-dimensions (managerial and personal; physical and occupational; organizational and environmental) and 15 items. It was founded that the content validity index of the scale items was 0.468-0.851; factor loads were 0.510-0.871; item-total score correlations were 0.467-0.768, and the Cronbach's alpha reliability coefficient of the sub-dimensions ranged between 0.703-0.876 and it was 0.912 for the whole scale. Conclusion: As a result of the validity and reliability analyzes of the Attitude Scale towards Innovative Behaviors of Nurses and Midwives, it was founded that it can be used as a valid and reliable scale.

Aim: The aim of this study was to determine the anticancer activity of royal jelly (RJ) and Aloe vera (AVE) separately and in combination on human non-small cell lung cancer (A549) and colorectal adenocarcinoma cells (HT29) and to investigate whether the side effects of AVE can be ameliorated by RJ. Materials and Methods: The antiproliferative activity of RJ, AVE, and the combination of AVE and RJ was performed in vitro on A549 and HT29 cells using a Real-Time Cell Analyzer (xCELLigence). To determine the mechanisms underlying the antiproliferative activity, cell apoptosis was performed by flow cytometry using the annexin V-FITC/PI apoptosis detection kit, while cell migration was determined by wound healing assay. Results: Treatment with varying concentrations of AVE, RJ, and the combination of AVE and RJ was concluded to have dose-dependent antiproliferative activity and that the AVE-RJ combination induced 41% of HT29 and 11.79% of A549 early apoptosis (p<0.05). However, the inhibition effect of the combination of AVE and RJ on cancer cell migration was not detected. Conclusion: Taken together, our results showed that the AVE-RJ combination could be a promising therapeutic combination in A549 and HT29 by inducing apoptosis. The obtained results revealed that the AVE (40 µg/mL) and RJ (300 mg/mL) combination in the A549 cell line exhibited earlier antiproliferative activity than the 40 µg/mL AVE. Also, it has been found that the antiproliferative activity of AVE is enhanced when used in combination with RJ. Background: New-onset atrial fibrillation (NOAF) often complicates acute coronary syndromes (ACSs) with adverse short- and long-term consequences. However, the current risk classification model for estimating NOAF during non-ST-segment elevation myocardial infarction (NSTEMI) remains unclear. Some parameters in the CHA2DS2-VASc score are closely related to the development of atrial fibrilation (AF). Methods: This retrospective study was conducted among 670 consecutive NSTEMI patients who applied to our cardiovascular center between June 2020 and June 2022 and underwent percutaneous coronary intervention (PCI). Results: NOAF developed during hospitalization in 55 patients (12.5%). NOAF patients were older and had higher high sensitivity C-reactive protein (hs-CRP), left atrial volume index, Post PCI thrombolysis in myocardial infarction (TIMI) grade <3, CHA2DS2-VASc score, peak troponin I (ng/ml), and SYNTAX score (SS). After univariate logistic regression analysis for the predictors of NOAF development, CHA2DS2-VASc score, post PCI TIMI grade <3, hemoglobin, hsCRP, SS alone were predictors of NOAF, but after multivariate analysis, CHA2DS2-VASc score, post PCI TIMI grade <3, hemoglobin had a determining effect. Conclusions: The CHA2DS2-VASc score can be evaluated as a predictor of NOAF after PCI in NSTEMI. Except for the components of the CHA2DS2-VASc score, post PCI TIMI grade <3 and low hemoglobin levels are independent risk factors for NSTEMI-NOAF.
