Spectrum of thyroid dysfunctions among hospitalized patients with non-critically ill coronavirus disease 2019: A cross-sectional study 2019年非危重型冠状病毒病住院患者甲状腺功能障碍谱:一项横断面研究

Background: Patients with coronavirus disease 2019 (COVID-19) particularly critically ill ones may present with different types of thyroid abnormalities. However, data regarding thyroid function tests (TFTs) among noncritical patients with COVID-19 are scarce. This study aimed to assess thyroid functions and their associations with the severity of illness among non-critically ill hospitalized patients with COVID-19.Methods: This cross-sectional study assessed TFTs in 87 (aged 18-65 years) RT-PCR-confirmed COVID-19 patients admitted to a tertiary-care hospital in Bangladesh. Diagnosis of non–critical illness and severity (mild, moderate, and severe) were defined by WHO’s interim guidance. Patients having known thyroid dysfunctions or taking drugs that may affect thyroid functions were excluded from the study. Serum TSH, FT4, and FT3 were measured by chemiluminescent immunoassay.Results: Majority of the patients (72%) had normal thyroid function. Among the abnormalities, the highest frequency was isolated hyperthyroxinemia (12.6%) and the rest were subclinical hypothyroidism (6.9%), subclinical thyrotoxicosis (4.6%), thyrotoxicosis (2.3%), isolated tri-iodothyroninemia (1.1%), and hypothyroidism (1.1%).  Serum TSH, FT4, and FT3 levels were similar across the spectrum of noncritical illness. No significant correlation was found between the inflammatory markers (C-reactive protein, ferritin, and D-dimer) and TSH levels.Conclusions: More than one-fourth of non-critically ill hospitalized patients with COVID-19 presented with a spectrum of thyroid abnormalities with isolated hyperthyroxinemia being the most common. Mortality causes in goldsmiths of Bangladesh: Findings from verbal autopsy 孟加拉金匠的死亡原因:尸检结果

Background: Information on the mortality causes of goldsmiths in Bangladesh is limited. This study aimed to find out the mortality causes in a selected group of goldsmiths.Methods: A World Health Organization recommended questionnaire was adapted to conduct verbal autopsy of 20 deceased goldsmiths. Death-related information was gathered from the family members present during deceased’s illness preceding death. The mortality causes were determined by the interview outcomes and medical records-review.Results: The mean age of the goldsmiths at death was 59.2± 9.3 years. Among then, 70.0% were smokers and 50.0% were alcohol consumers. Cardiovascular diseases (CVD) were the most common immediate and underlying cause of death (60.0% and 45.0%, respectively).Conclusion: The life expectancy of goldsmiths was much lower than the average life expectancy of Bangladeshi population, where CVD was the primary cause of death. Smoking and alcohol consumption were prevalent among them. Awareness about healthy lifestyles should be prioritized for highly CVD prevention among the goldsmiths. Bangabandhu Sheikh Mujib Medical University Journal 2023;16(2): 87-90

Assessment of health-related quality of life of COVID-19 patients during follow-up 新冠肺炎患者随访期间健康相关生活质量评估

Background: The COVID-19 pandemic has had a significant impact on the quality of life of individuals worldwide. The aim of this study was to assess the health-related quality of life (HRQOL) and its associated factors among COVID-19 patients discharged from hospital.Methods: This facility-based cross-sectional study was conducted among laboratory confirmed COVID-19 patients who were discharged from medicine department of Bangabandhu Sheikh Mujib Medical University and did their first-month follow-up from April to September 2021. A validated Bengali version of International Quality of Life Assessment, Short form – 36 (SF-36) questionnaire was used to assess the HRQOL status. Univariate and multivariate analyses were done for identifying factors associated with low HRQOL.Result: Out of 225 patients, the mean (standard deviation) age was 36.9 (14.2) years and four in every ten patients were women. Out of eight subgroups, almost all scored averagely 79 out of 100. Older age, rural residence and severe to critical disease was reported as factors for low physical and mental component summary of SF-36.Conclusion: COVID-19 patients with older age, rural residence and having severe to critical disease condition need to be addressed critically as they have a higher chance of having low HRQOL status after COVID-19 infection.Bangabandhu Sheikh Mujib Medical University Journal 2023;16(2): 91-95

Efficacy of expressed breast milk alone or in combination with paracetamol in reducing pain during retinopathy of prematurity screening: A randomized controlled trial 纯母乳或与扑热息痛联合应用在早产儿视网膜病变筛查中减轻疼痛的疗效:一项随机对照试验

Background: The aim of this study was to determine the efficacy of expressed breast milk (EBM) alone or in combination with oral paracetamol for pain reduction during retinopathy of prematurity (ROP) screening. Methods: A randomized control trial was conducted in two departments of Bangabandhu Sheikh Mujib Medical University. A total of 60 preterm neonates who underwent ROP screening were randomized into three equal groups. Group A got nesting and swaddling as per institutional protocol (control). Group B received 2 ml EBM two minutes prior to the ROP screening and Group C received 15 mg/kg syrup paracetamol 30 minutes prior to the ROP screening and EBM like Group B. Premature infant pain profile (PIPP) scores was used prior, during and 2 minutes after ROP screening procedure.Results: The three groups were similar in terms of baseline characteristics. The mean (standard deviation) PIPP scores during the procedure were 16.4 (1.1), 15.0 (1.8) and 13.4 (1.8) in control, EBM, and EBM with paracetamol groups respectively. The PIPP scores were significantly lower in the EBM and EBM with paracetamol groups during the procedure compared to control group. In the EBM and EBM with paracetamol groups, the mean difference in PIPP scores (between before and during the procedure) was also substantially lower.Conclusion: Breast milk alone or in combination with paracetamol can reduce significant pain during ROP screening than control group.Bangabandhu Sheikh Mujib Medical University Journal 2023;16(2): 106-110

Polya partial gastrectomy for megaduodenum: A case report 多发性胃部分切除术1例

Megaduodenum is an extremely rare condition in children presenting with features of duodenal obstruction. It may be associated with bands and adhesions. However, when the obstruction is chronic and functional, mere excision of the band and adhesionolysis is not curative. Some form of bypass procedure is required for its successful treatment. Here we are describing polya partial gastrectomy in a 13-year-old girl suffering from megadoudenum for its successful treatment. Bangabandhu Sheikh Mujib Medical University Journal 2023;16(1): 70-72

Background: Vincristine is an anticancer agent administered to all children with acute lymphoblastic leukemia (ALL), and peripheral neuropathy is the major dose-limiting toxicity of this therapy. As cure rates of childhood ALL exceeds 80%, therefore treatment-related toxicities need to be reduced. Thus, the aim of this study was to determine the prevalence and risk factors of vincristine-induced peripheral neuropathy (VIPN) in children with ALL undergoing induction chemotherapy.Methods: A case-control study was conducted from September 2017 to August 2018 in the Department of Paediatric Haematology and Oncology at Bangabandhu Sheikh Mujib Medical University, Dhaka, Bangladesh. Eighty newly diagnosed ALL and 35 acute myeloid leukemia (AML) cases aged 5 to 17 years with no pre-existing neurological abnormality were recruited. To assess the peripheral neuropathy, we used pediatric-modified total neuropathy score and National Cancer Institute- Common Terminology Criteria for Adverse Events (NCI-CTCAE), version-04 grade.Results: Among ALL patients, 29.2% developed peripheral neuropathy compared to 10% in AML control group (P=0.04). Higher proportion (57.1%) of peripheral neuropathy was found in age below 10 years (P<0.001). There was no significant association of peripheral neuropathy with sex and body mass index of the patients.Conclusion: Almost 3 in 10 patients developed VIPN during the induction therapy which is significantly higher in age below 10 years compared to ≥ 10 years.  Bangabandhu Sheikh Mujib Medical University Journal 2023;16(1): 02-07

Fasting with diabetes during Ramadan: An updated review 斋月期间糖尿病禁食:最新综述

Introduction: In reality, many Muslims have an intense spiritual urge to participate in fasting, even those who could get an exemption. There are many cultural variations in the Muslim population in different parts of the world, even in the same country with different social and family backgrounds. Thus, persons with diabetes should seek medical advice before planning to fast or proceed to fast during Ramadan.Materials: This was a narrative, non-systematic review of the international literature from the major medical online database (PubMed and Google Scholar) in 2023. The term “Ramadan fasting” and “Diabetes” was used to search, and the relevant literature was captured and narrated in a concise thematic account.Results: Practically, there are certain risks associated with fasting for patients with diabetes, which might exacerbate their existing illnesses. Along with the globally increasing prevalence of diabetes, there is an increased number of the population who intend to or fast. The article captured a wide array of topics related to Ramadan fasting by patients with diabetes, including its epidemiology, risk stratification, lifestyle modification, assessment of safety and efficacy profiles of pharmacotherapies, hypo or hyperglycemia, and the impact of diabetes education.Conclusion: Fasting with diabetes poses a range of risks and thus pre-Ramadan risk stratification is essential. Particular attention should focus on the patient’s current risk factors, complications, comorbidities, and the roles of newer antihyperglycemic medicines and advanced technology for safer fasting practices to mitigate the risks of hypoglycemia and or hyperglycemic crises.Bangabandhu Sheikh Mujib Medical University Journal 2023;16(1): 41-57

Change of lipid profile in children with acute lymphoblastic leukemia due to induction chemotherapy in a tertiary care hospital of Bangladesh 孟加拉国一家三级医院诱导化疗引起的急性淋巴细胞白血病患儿脂质谱变化

Background: Acute lymphoblastic leukemia (ALL) is the most common childhood malignancy. In the Department of Paediatric Haematology and Oncology of Bangabandhu Sheikh Mujib Medical University (BSMMU), 58% of ALL cases were recorded among 455 newly diagnosed malignancy patients in a single year. Studies found that remarkable hypertriglyceridemia occurs with L-asparaginase therapy and steroid. This study was done to evaluate the changes of serum total cholesterol, triglycerides (TG), high density lipoprotein (HDL), and low-density lipoprotein (LDL) during and after induction chemotherapy in children with ALL.Methods: This prospective observational study was performed in the Department of Pediatric Hematology and Oncology of BSMMU from March-November 2013. Newly diagnosed acute lymphoblastic leukemia patients aged 3-15 years were included in this study after having written consent from the parents of the participants to participate in the study and enrolled for the treatment of ALL (according to modified UKALL 2003 protocol).Results: Total cholesterol, TG, HDL, and LDL changed significantly due to induction therapy. Serum total cholesterol and LDL decreased after completion of L-asparaginse in comparison to before induction, increased significantly after completion of induction in comparison to after completion of L-asparaginase (P=0.001), and increased significantly after induction in relation to before induction therapy (P=0.003). TG decreased significantly (P=0.033) after completion of L-asparaginase than before induction but increased after completion of induction. HDL increased after completion of L-asparaginase and after induction significantly (P=0.001). LDL decreased after completion of L asparaginase which was significant (P=0.005).Conclusion: After induction chemotherapy, total cholesterol, HDL and LDL level increased and TG level decreased among ALL patients.Bangabandhu Sheikh Mujib Medical University Journal 2023;16(1): 35-40

Hemophilia in a newborn without family history: A case report 无家族史新生儿血友病1例报告

Haemophilia is the most frequent diagnosed inborn clotting factor deficiency in newborn. Studies reveal that majority of hemophilia A cases are due to an inversion of the long arm of X-chromosome. Newborns with hemophilia are at risk of intracranial and extracranial hemorrhage, and other bleeding complications. Diagnostic criteria for haemophilia include confirmation of a factor activity level below 40% of normal (below 0.40 IU/mL), or a hemophilia gene mutation. Haemophilia typically result in an isolated prolongation of the activated partial thromboplastin time. However, the definitive diagnosis requires measurement of factors VIII and IX levels. Here, we describe a newborn with cephalhematoma gastrointestinal bleeding that is the manifestation of neonatal haemophilia with antenatally diagnosed duodenal atresia and there is no positive family history. This case shows a rare hemophilia presentation reflecting the importance of coagulation studies when faced with unexplained bleeding.Bangabandhu Sheikh Mujib Medical University Journal 2023;16(1): 62-65
