Studies have shown that patient care outcomes in nursing homes are related to nurse staffing standards/levels, which are affected by profit maximization on adherence to registered nurses and licensed nurse staffing standards. The purpose of this descriptive study was to determine if there was a relationship between adherence to staffing standards and care outcomes in for-profit (FP) and not-for-profit religious-based (NFPRB) nursing homes using the profit maximization theory. Research questions focused on the relationships that profit maximization and nurse staffing standards had on the quality of care outcomes measures and the differences between the nursing homes on these variables. Secondary data were collected from public database and analyzed using the descriptive and inferential statistics, nonparametric tests, and binary logistic regression. Findings showed that profit measures were not related to staffing standards and care outcome measures in the NFPRB. There was a significant relationship between FP nursing homes and standards to care outcomes in FP but not in the NFPRB nursing homes. FP nursing homes did worse than NFPRB on care outcomes. Further research, using qualitative and mixed methodologies, is needed to study the effects of profit measures on nursing home care outcomes. The results of this study can effect positive social change by informing policy makers and healthcare professionals/leaders, and, by reducing adverse events, untimely death, and positively affecting the quality of care and life of the frail and vulnerable older adults residing in nursing homes. Association of Perception of Aging of Senior Citizen to Level of Awareness 老年人对老龄化的认知与认知水平的关系

Background: This study aimed to determine the association of aging perception of senior citizen to level of awareness on their rights, benefits and privileges and living arrangements. Methods: Four hundred (400) senior citizen residents in Barangay Dau were recruited and participated in the study. Frequency, percentage, mean, and standard deviation were used for descriptive while Mann-Whitney U test and Kruskal-Wallis H test were used to determine which among socio-economic variables varies with aging perception. Spearman's rank-order correlation on the other hand was used to determine the relationship between the aging perception and their awareness on their rights, benefits and privileges. Results: The result revealed that most participants have positive control over aging and think that aging have positive impact to their lives. They have less negative emotion toward aging and most of them accepted that they are growing old. Moreover, it was found out that among the socioeconomic factors identified, only gender and living arrangement had variation on aging perception. There is a positive correlation between senior citizen awareness of their rights, benefits and privileges and their belief that aging have positive impact on their life. In addition, there is an inverse relationship between age and Control-Negative indicating that as person grow older, they are more likely perceived that they have positive control over aging.

Aging in Resilient Communities-An Alpine Case Study: The Senior Living Lab Experience 弹性社区的老龄化-高山案例研究:老年生活实验室经验

The Senior Living Lab led by the University of Applied Sciences and Arts in Western Switzerland, provides an interesting case study that sheds insight into user centered approaches to innovation in community healthcare platforms addressing aging. The Senior Living Lab was also an example of nursing leadership, focusing on social innovation from a perspective of care. The participative nature of the qualitative research methods in the field, allowed seniors to have a voice in the co-creation process, generating social innovation solutions within an interdisciplinary research framework. Through focus groups, World Cafe meetings, as well as a film event on hopeful, healthy aging, senior participant’s views were valued. One fundamental message emerged from the dialogical space: seniors don’t want to be stigmatized by a growing form of discrimination referred to as ageism. Synergies reinforced Alpine governance and wellness through collaborative continuums with communal authorities and regional stakeholders, applying participative research methods.

Innovation in Formulation Development for Older People 老年人配方开发创新

In many cases geriatric patients constitute the main users of a medicinal product and not a special population. Even though the heterogeneity of the geriatric patient population is vast with physiological changes occurring with advanced age being diverse, the presence of multi-morbidity, disability and frailty are rather common indicating a special need for age-appropriate formulations. Dosing flexibility, swallowability and overall manageability of drug products are among the issues that have been identified as most important for this age group and thus they should drive the design of therapeutic options for the geriatric population. Pharmaceutical industry is slowly starting to respond to these challenges with an increasing number of innovative geriatric-friendly formulations that have either reached the market or are currently under development. The aim of this article is to discuss the adequacy of the available geriatric-friendly formulations to meet the special needs of this age group, placing particular emphasis on a couple of marketed medicinal products with innovative geriatric-friendly formulation technologies.

Impact of Nutrition and Depression on the Cognitive Functioning of the Elderly in Taiwan 营养与抑郁对台湾老年人认知功能的影响

Background: According to the Alzheimer's Disease International (ADI), the regional distribution of new dementia cases worldwide in 2015 shows an increased proportion of new cases arising in Asia, the Americas and Africa. Moreover, an average of one new case of dementia every three seconds is estimated, with prevalence increasing with age. Nutritional disorder is a risk factor in reversible dementia, and literature also points out that the prevalence of depression in dementia patients is 22%, rendering depression a likely risk factor for future dementia. Therefore, this study mainly aimed to explore the relationship between nutrition and depression and cognitive function, and hopes that the findings can be used as a reference for providing medical care to the elderly. Methods: This study used a cross-sectional survey to study the elderly in northern Taiwan. A total of 245 subjects were selected using convenience sampling, and a questionnaire survey was administered. The research tool was a structured questionnaire constructed by the researchers, and the results were analyzed using the SPSS 20.0 statistical software. Results: A total of 245 elderly people over the age of 65 completed the questionnaire survey. According to the Pfeiffer's Short Portable Mental State Questionnaire (SPMSQ), 233 respondents (95.1%) showed normal cognitive functioning, and 12 respondents (4.9%) showed mild to serious cognitive impairment. The mean age of the subjects was 72.82 years old, with a standard deviation of ± 7.80. 92 (37.6%) of the subjects self-reported normal health condition; average total cognitive score was 9.13 (± 1.37); the average number of chronic diseases was 0.98 (± 1.04); the average nutritional score was 25.23 (± 3.71); and the average total depression score was 1.56 (± 0.65). Most of the respondents were women, had university education or above, were non-smokers, “thought about regularly exercising 3 times a week for 30 minutes each” and “agreed to regularly do trans theoretical exercises”. However, these variables were not significantly correlated to cognitive functioning. The relationship between age and cognitive functioning was also statistically insignificant. The number of chronic diseases showed significant negative correlation, indicating that subjects with more chronic diseases showed lower cognitive scores. Total cognitive score showed significant positive correlation to nutritional status, indicating that the better the nutritional condition of the elderly, the better their cognitive functioning. Total cognitive score negatively correlated to total depression score, indicating that those with higher depression score had poorer cognitive functioning. Conclusion: This study showed that nutritional status, depression and chronic diseases were important variables that affect cognitive functioning. Iranian Older Peoples Emotional Well-being in Long-term Care 伊朗老年人在长期护理中的情绪健康

This paper investigates elderly people’s emotional well-being in long-term care environments. It is to explore how architecture influences their well-being in a place. While the literature on elderly people’s well-being in a place is growing, moving to long-term care environments still maintains as a challenge for majority of older people. This unwanted relocation in old age might cause psychological issues for older people. In order to have more spirituality as a key factor of emotional well-being, older people need to have a successful aging, but how do they age successfully? There are different approaches to successful aging, but this paper will mainly focus on Erikson’s psychosocial development model and Tornstam’s theory of gerotranscendence, as two of the main approaches to this issue. The population of Iran is getting old because of improvements in health system and the boost in the birth rate in 1980s, causing a wave in the population pyramid of Iran. Possibility of staying at home will probably decrease due to a number of reasons, such as: inappropriate level of safety in personal houses and lack of reliable 24 h in-place caregivers. This paper is looking for the way, in which older people’s well-being gets increased in nursing homes and how architecture can facilitate this process. As an early stage of an on-going research, this paper discusses the existing literature on the effects of nursing homes on elderly people’s emotional well-being with a particular emphasis on Iranian aging population.

The Strategy and Policy Development of Population Towards the Demographic Bonus 2035 in Kupang District-Indonesia 面向2035年印尼古邦地区人口红利的人口战略与政策发展

Background: Population concerns the number, structure, age, sex, religion, birth, marriage, pregnancy, death, distribution, mobility and quality in the economic, social and cultural. Residents if less managed wisely lead to security disturbances at the national and regional levels. Therefore, comprehensive management touch to the population problem is needed.Purpose: Produce document as the direction of population development strategy and policy in Kupang district to demographic bonus of 2035.Method: Combination of qualitative and quantitative research. Data were obtained through in-depth interview, FGD, Central Biro Statistic publication. 35 people were FGD participants. Implementation of research was scheduled in 60 working days.Results: The formulation strategy and development policy of Kupang District with target; population 525,706 people, growth rate 1.85% per year, TFR of 2, NRR 1.12, CBR 22.40, CPR 90%, CDR 7.03/1000 population, IMR 19.30/1000 live birth, MMR 175/100.000 live birth; Life Expectancy Rate 77.30 years, increasing labor quality, decreasing the number illegal labor abroad. Fertility field policy; availability of demographic information, delaying marriage age, increasing acceptors coverage, continuity of old and new acceptors services, increasing adolescents and couples of childbearing age, provision and distribution of contraceptives, strengthening family economy, enhancing partnerships. Field Mortality; Decrease in maternal mortality during pregnancy, Decrease infant and child mortality, reduce CDR, improve quality of life clean and healthy. Mobility; redistribution of population among subdistricts, equality of development and improvement labor skills.Conclusion: These strategies and policies are used as direction and solution the problem population development in Kupang District UC.

Japan and Its Rapid Ageing Society: Does e-health Technology Provide a Solution? 日本及其快速老龄化社会:电子医疗技术是否提供了解决方案?

Japan can be called a rapidly ageing society. In 2016, one-quarter of the Japanese society was aged 65 and over. Against this backdrop, in this article I argue that a rapidly ageing society like Japan will rely heavily on technological solutions to ensure adequate care and a decent standard of living for elderly people. Thus, the country is predestined as a case study considering the relationship between ageing, professional care, e-health technology and other assistive devices. I use examples from Telehealth networks and monitoring sensor systems to shed some light on these recent and growing developments in the field of e-health technologies.

Genetic and Environmental Risk Factors of Alzheimer's Disease 阿尔茨海默病的遗传和环境危险因素

Alzheimer’s is one of the most common neurodegenerative disease generally found in the form of dementia in elderly population. Advanced age is still considered as most important risk factor for this disease. A complicated array of molecular events has been implicated behind this disease. As AD is chronic neurodegenerative disease and etiology is still unclear, familial AD accounts only 5% of the disease. Then it is more important to look for other factors that play great role in the onset of the disease. Thus, this paper reviews the role of genetics, different environmental, oxidative stress factors and its mechanistic insights underlying the pathogenesis of Alzheimer’s disease.
