Hospital-at-home units are proliferating under the Medicare waiver, issued during the public health emergency and extended into 2024, which allows hospitals to be paid the same for care delivered at a patient’s home as in an inpatient setting. Leaders in the hospital-at-home movement find the at-home units require significant resources, but they generate high satisfaction from clinicians and patients alike and reduce the need to build more inpatient facilities.

The access medical director role in the ambulatory setting is emerging in health systems. Characteristics of the role include collaborative, patient-centered, systems thinker, and technical proficiency in data and qualitative skills; however, training opportunities are limited. This study aimed to create an evidence-and expertise-informed framework to identify elements of the emerging clinician leadership role in ambulatory access in U.S. health systems and to identify the prevalence of clinician leaders assigned to the leadership role in ambulatory access in U.S. health systems.

Mentorship is a professional relationship between two individuals where one individual has considerably more experience than the other and is willing to provide time, guidance, and feedback to improve the skills and experience of another. As physicians, we are fortunate throughout our clinical careers to have received graduated guidance from senior residents, fellows, attending physicians, and consultants that improved the way we deliver care. As physician leaders, we may find that identifying a mentor who is willing to take the time to improve our skills in leadership and management is more difficult.

What is the best method for eliciting physician input and feedback and establishing bidirectional communication to address factors contributing to burnout, enhance a sense of professional community, and provide mental health support? Trained physician facilitators conducted small-group, physician-only discussions (PODs) that led to organized, actionable items and directly informed directions of departmental initiatives to reduce burnout.

The industry progressively changes under a constellation of influences, and yet, the patient-physician relationship continues to be a pillar of the healthcare delivery system. Physicians at all levels can (and should) recognize the opportunity offered by this current focus on helping others retain confidence in physicians and their ability to provide leadership. What is relatively difficult, however, is being able to describe the value and ROI of physician leadership.
