Convalescent plasma therapy in the management of COVID-19 patients and its effect on morbidity and mortality 恢复期血浆治疗在COVID-19患者管理中的作用及其对发病率和死亡率的影响

BACKGROUND: Since the beginning of the COVID-19 pandemic, there have been preliminary studies worldwide, hoping to shed some light on the efficacy of convalescent plasma therapy (CPT) in the management and recovery of SARS-CoV-2-infected patients. There is a dearth of such studies, especially from one of the world's second-worst hit country-India. This study aims to determine the effects of CPT on the clinical and biochemical profile, and overall mortality of patients suffering from moderate-to-severe SARS-CoV-2 infection. MATERIALS AND METHODS: After random selection of inpatients with COVID-19 disease, patients were matched for various characteristics and allotted into cases and control groups. This was done keeping a 1:2 ratio for cases and controls. Cerebral palsy (CP) therapy was administered to only cases, and these groups were compared. P < 0.05 was considered statistically significant. RESULTS AND CONCLUSIONS: Totally, 141 patients – 47 cases and 94 controls – were compared for the efficacy of CPT on clinical and biochemical profiles along with mortality benefit. It was found that CP therapy provided no significant morbidity and mortality benefit.

Out-of-pocket expenditure on maternal health care in two backward districts of Karnataka: A cross-sectional study 卡纳塔克邦两个落后地区的产妇保健自付费用:一项横断面研究

BACKGROUND: Maternal health is an important indicator of a woman's health and society. Safe motherhood is still a nightmare in many communities. Medical services related to maternal health are almost free in India, yet many families face significant out-of-pocket (OOP) expenditure, especially in poor communities in the rural areas, leading to more poverty and exclusion. This study has the aim of revealing OOP expenditure on maternal health care in rural areas. METHODS: A community-based cross-sectional study was conducted among 386 randomly selected mothers who had delivered a baby in the past 2 years in two backward districts of Karnataka using a mixed method. Univariate, bivariate, and t-tests were used for the analysis. RESULTS: The findings revealed that poor education and low income have a significant association with OOP expenditure. Getting laboratory services, purchasing medicine from the market, informal payments to the hospital staff, and other indirect costs are the major components of catastrophic expenditure for the poor mothers. CONCLUSION: It is concluded that there is strong evidence to support the high burden of maternal health-care spending. More comprehensive schemes are needed in the government's maternal health-care services to reduce out-of-pocket expenses.

Saving the pulpless teeth: Approaches to conserve the tooth with severe coronal destruction: A case series 保存无髓牙:保存严重冠状破坏牙齿的方法:一个病例系列

Endodontically treated teeth become fragile unless optimally restored. Treatment planning of these teeth should take into account the remaining coronal tooth structure as well as the functional requirements. A successful treatment of these teeth with significant tooth structure loss requires not only an effective endodontic therapy but also prosthodontic rehabilitation of the tooth. Simple and successful procedures can be viable treatment options for conserving the endodontically treated teeth with severe coronal tooth structure loss. Hence, the aim of this case series was to present different approaches to restore endodontically treated teeth with minimal coronal structure.

Prevalence of dry eye in postmenopausal women at a tertiary level hospital 某三级医院绝经后妇女干眼症的流行情况

BACKGROUND: Dry eye disease is a multifactorial ocular surface disease that causes symptoms of vague ocular pain, discomfort, grittiness, burning sensation, itching, and redness in the eyes. It is one of the most common disorders of the eye which is often underrecognized and can significantly affect the quality of life of patients. OBJECTIVES: The objective of this study is to assess how often postmenopausal women have dry eyes and to evaluate the relationship between dry eye disease and age in postmenopausal women. MATERIALS AND METHODS: During 3 months, postmenopausal women who attended the ophthalmology outpatient department at a tertiary level hospital participated in a hospital-based cross-sectional study. Four hundred postmenopausal women were conveniently selected as the sample. To identify the existence of dry eyes, Schirmer's test was performed. RESULTS: The study population's average age was 59.09 ± 8.46 years. In the research, the average age of menopause onset was 47.68 ± 2.67 years. Dry eye condition was found to be 39.75% prevalent. The vast majority of individuals with dry eye illness had moderate dry eyes (18.25%). Dry eyes became more common with age, which was statistically significant. CONCLUSION: The findings of the current study have shown that many postmenopausal women experience dry eye disease. The study emphasizes the value of routine eye examination for postmenopausal women to identify dry eye problems early, receive the proper treatment, and help these women improve their quality of life.

Vertigo in pediatric age group: Our experiences 小儿眩晕:我们的经验

BACKGROUND: Vertigo is a morbid symptom among children. However, children rarely experience vertigo, a clinical condition that has received less research than the adult group. OBJECTIVE: This study aimed to evaluate vertigo among pediatric age group on the basis of the clinical and neurotological profiles. MATERIALS AND METHODS: There were 78 children manifesting with giddiness or vertigo, who attended vertigo clinic at a tertiary care teaching hospital between March 2019 and April 2022. RESULTS: Of 78 children, 30 (38.46%) were boys and 48 (61.53%) were girls. In this study, the average age of the participating children was 11.8 years. Of the 78 children, 10 (12.82%) children were of preschool age, 22 (28.20%) children were of elementary school age, and 46 (58.97%) children were in the adolescent age group. The most common cause of the vertigo was vestibular migraine in pediatric age (30.76%). The most common associated symptom with vertigo was nausea and vomiting. CONCLUSION: There are various etiologies for vertigo. The most common etiology for vertigo in children is vestibular migraine. Children may show same types of clinical presentations. The treatment of vertigo in children should be individualized as per the diagnosis.

Clinical profile and radiological pattern in follow-up study of patients treated for moderate-to-severe COVID-19 pneumonia 中重度COVID-19肺炎患者临床特征及影像学随访研究

The COVID-19 pandemic is the modern-day apocalypse that the world has experienced, with a death toll of 60.1 lakh by 2022. Although many patients recovered from the disease, a major part of the population has been crippled with the sequelae that occurred with the infection. One such complication is the development of pulmonary fibrosis, especially among those who recovered from moderate-to-severe COVID-19 infection that has been reported in several studies. Albeit adequate and early usage of antivirals, antibiotics, and steroids, and the development of lung fibrosis was inevitable in certain patients. Here, we report four cases, who recovered from COVID-19 infection but developed pulmonary fibrosis along the course of illness. They were followed for 6 months, along with treatment with antifibrotics, and were assessed for improvement, both clinically and radiologically. While three among the four patients showed resolution in their follow-up CT scans, one patient's fibrosis worsened with progression to interstitial lung disease.

Although many patients recovered from the disease, a major part of the population has been crippled with the sequelae that occurred with the infection. One such complication is the development of pulmonary fibrosis, especially among those who recovered from moderate-to-severe COVID-19 infection that has been reported in several studies. Albeit adequate and early usage of antivirals, antibiotics, and steroids, and the development of lung fibrosis was inevitable in certain patients. Here, we report four cases, who recovered from COVID-19 infection but developed pulmonary fibrosis along the course of illness. They were followed for 6 months, along with treatment with antifibrotics, and were assessed for improvement, both clinically and radiologically.
