The Squeezed Heart-A Case Report 挤压心脏——一例报告

Pompe’s disease is an autosomal recessive disorder caused by inherited deficiency of α-1,4-glucosidase (acid maltase), a lysosomal enzyme. Patients usually die in the first year of life from cardio-respiratory failure due to massive left ventricular hypertrophy. We report a case of 3-month-old boy presented with fatal infantile onset Pompe’s disease.

Appropriate Use of Statins in The Elderly Population 他汀类药物在老年人群中的合理应用

Dyslipidemia as a risk factor in the elderly and its influence on morbidity and mortality remains a topic, at least, controversial for this population range. Statins, due to their LDL-c reducing effect, can improve other conditions that contribute to the morbidity and mortality of elderly patients. These pleitropic effects are of vital importance in the anti-atherosclerotic effect of statins. Pleitropic properties may also be involved in the non-cardiovascular benefits of statins, recently described. The similarity of pathological processes between middle-aged individuals and elderly people over 65 years of age, together with epidemiological data, confer that high cholesterol levels lead to a high risk in the elderly. Angiographic studies show that, even with advanced circulatory disease, the elderly respond to cholesterol-reducing therapy, with reduced risk. Dyslipidemia plays a key role in the progression of coronary artery disease and is an important predictor of coronary artery disease related events in elderly individuals. Statins are widely accepted as effective therapy for secondary prevention of cardiovascular disease. Although studies are limited, such benefits should also be extended to elderly patients. Major surgery in a Jehovah's Witness patient represents a challenge for the anesthetic and surgical team not only because of the ethical dilemma, but also because it creates a major obstacle to the rapid control of hemorrhage in a surgical setting. We present a case report of a pancreatoduodenectomy in a Jehovah's Witness patient who refused to be transfused with the use of the principles of bloodless surgery and maintenance of cell savage as a backup, if necessary. The advances in our understanding of hemostatic agents and available techniques for managing patient bleeding represent options for avoiding blood transfusion in this patient population and all patients undergoing major surgery

Clinical Decision Making: A Comparison Study between Greece and Slovakia 临床决策:希腊和斯洛伐克的比较研究

Background: Clinical decisions are a nurses' daily clinical practice skill that needs to be constantly evolving in order to provide quality patient care. Purpose: To investigate and compare the clinical decisions made by nurses in Greece and Slovakia. Materials and Methods: Q-methodology was used and eight modified scenarios were given to the nursing staff. Using power analysis, a total of 100 nurses that worked in public hospitals in Greece (nG = 50) and Slovakia (nS = 50) successfully completed the study. Results: In the scenarios with more medical interventions, Greek nurses showed higher scores in the scenarios of tachycardia, bradycardia and cardiopulmonary resuscitation. In contrast, Slovak nurses scored higher individual scores in all four scenarios (anxiety - difficulty sleeping –dyspnea - vomiting) with more nursing interventions (p<0.05). Conclusions: Greek nurses overall had higher individual scores in the scenarios with more medical interventions, probably due to the medically-centered philosophy of nurses, the lack of protocols and autonomy to provide patient care.
