Intravascular tumor extension of a thyroid cancer is a rare but life-threatening condition. We report herein the case of a 66-year-old female patient with follicular thyroid carcinoma and an internal jugular vein (IJV) tumor thrombus. The patient had undergone a thyroidectomy at our institution and presented with left-sided swelling of face and neck. An MRI showed a thrombus within the left IJV, a finding confirmed by US. During surgery, a left IJV tumor thrombus was identified and excised followed by venous reconstruction. The pathological examination confirmed a follicular thyroid carcinoma. The patient received postoperative treatment with 150 mCi of 131I, followed by thyroid-stimulating hormone suppression therapy with thyroxine. Although the fibrous capsule prevents the tumor’s invasion into the surrounding structures, the presence of a tumor thrombus carries a poor prognosis.

Myasthenia gravis (MG) is a rare autoimmune disease and the most common neuromuscular junction disorder. The most common symptoms are diplopia, ptosis, and fluctuating weakness of ocular and limb muscles. Here we report a case of MG after COVID-19 vaccination. A 37-year-old Iranian lady presented with generalized weakness, diplopia, and ptosis after three days from taking the second dose of COVID-19 vaccine. Symptoms began three days after taking the second dose of Sinopharm BIBP COVID-19 vaccine (BBIBP-CorV) and worsened within a week. Her past medical history included migraine-type headaches and gastric reflux. She was consuming Rizatriptan and Nortriptyline for her migraine headache and Pantoprazole, Domperidone, and Sertraline for stomach reflux. She had a family history of Essential tremor in her father, uncle, and grandfather. Antibody against the acetylcholine receptor (AChR-Ab) was more than eight times compared to the normal range. The pattern of repetitive nerve stimulation was suggestive for MG. Treatment with corticosteroid, azathioprine, and pyridostigmin led to amelioration of symptoms. Follow-up testing revealed AChR-Ab returned to the normal range. There are some case reports in that a chronic autoimmune disorder flared up after the COVID1-9 infection or its vaccination. There are several reports of MG exacerbations, acute respiratory distress syndrome, higher ICU admission, and mortality rate and worsening neurological symptoms after COVID-19 infection. There is still debate about the association between neuromuscular disorders and COVID-19 infection and vaccination. This matter is more doubtful for myasthenia gravis.

A persistent increase in morbidity indicators has been connected to the global epidemic of obesity; it is regarded as a societal issue & poses significant health dangers. Synthetic medications and surgery are examples of therapeutic approaches; both have high prices and potentially dangerous side effects. Plant-based drugs provide an alternative strategy. Traditional uses of medicinal plants are one of the options for treating obesity, and they help with the development and research of obesity phytotherapy. In this review, we provided information regarding plant species along with their mode of action used for the effective treatment of obesity. It was revealed that the species in the catalogue had anti-obesity properties, including actions to slow down the absorption of fat, reduce enzymatic activity, mediate lipid levels, and boost the effects of lipolysis, which were primarily linked to phenolic compounds. For species with phenolic compounds in their chemical components, more thorough botanical, chemical, pre-clinical, and clinical research are especially required. The safe therapy for obesity may involve taking standardized medicinal plant extracts. The effectiveness of some plant combinations for medical purposes, however, may be reduced, and they may also have unanticipated negative effects.

In various industries, the importance and evolution of process safety leadership and culture have been stressed by several studies. Nevertheless, the palm oil milling industry remains an understudied domain. This paper undertakes a critical review of the most recent developments in the palm oil milling process and unit operations that trigger concerns about process safety to leaders in the palm oil milling industry with guidelines about the requisite process safety leadership and culture to be understood with a view to enhancing safety outcomes. In addition, the paper explores the principle of 3C for explicating constant accidents along with the four-level safety culture in the palm oil milling industry. To do so, the author presents case studies of two Malaysian key palm oil companies.

Femoral artery Pseudoaneurysm was caused at the puncture site of the right groin after diagnostic coronary angiography. Manual compression for 48 hours was administrated but failed then balloon tamponade was attempted for hemostasis at the aneurysmal site but hemostasis was not achieved. Next, using contralateral anterograde approach was use to cross the wire from the neck of the pseudoaneurysm and ipsilateral retrograde approach by direct puncture of the pseudoaneurysm was tried. A 0.014-inch guidewire was crossed from the neck of the pseudoaneurysm to the external iliac artery. The wire was replaced with a 0.035-inch guidewire. A star close device was inserted and was deployed at the neck of the pseudoaneurysm. Final angiography revealed completion of hemostasis.

Psychosis is associated with schizophrenia, which is primarily associated with severe impairments. And it has an impact on how other aspects of our lives, including work, family, friends, and education, operate (WHO, 2022). A person's behavior and brain are both affected by substance use disorder. It results in an inability to manage the use of a drug or medication, whether it is legal or illicit. The patient, who was diagnosed with a substance use disorder similar to schizophrenia, was the subject of this qualitative investigation. This study adopted a single-case study design. The data were collected by using a mental status examination questionnaire, and the data were analyzed by content analysis. The study participant was a 22-year-old male who has been educated in logistics management and is self-employed. The participant was an inpatient at a private mental health center. This study found that mental and behavioural disorders due to multiple drug use or other psychoactive substances can be managed through cognitive-behavioural therapy, aversion therapy, family therapy, outpatient rehabilitation, inpatient rehabilitation, and pharmacotherapy. This study also provides an insight into how schizophrenia compares to substance use disorder in terms of symptoms, causal factors, and management plans.
