A 63-year-old man presents to the primary care office with a productive cough, myalgias, fatigue, and a temperature ranging from 100.1 to 101 F for a one-week duration. He has atrial fibrillation, hypertension, benign prostatic hypertrophy, and a history of pancreatic cancer status post partial pancreatectomy and splenectomy three months prior, resulting in uncontrolled, post-surgical diabetes. He is taking amiodarone, carvedilol, hydrochlorothiazide, and metformin. He denies any other recent procedures, hospital admissions, or sick contacts.

IgG4-related sclerosing cholangitis (IgG4-SC) belongs to the spectrum of IgG4-related disease which encompasses many medical conditions. We report a case of a 59-year-old man who presented with clinical and radiologic features of primary sclerosing cholangitis but where hypereosinophilia along with an extremely high blood level of IgG4, liver infiltration with IgG4-positive plasma cell and a spectacular response to corticoids treatment, confirmed the diagnosis of IgG4-SC.

Duplication of left anterior descending (LAD) artery is an uncommon coronary anomaly. Type IV dual LAD is the rarest variety where short LAD arises from left main stem and long LAD arises from right coronary artery or right sinus of Valsalva. I present a case of 60-years-old lady with dilated cardiomyopathy with incidental detection of type IV dual LAD during coronary angiography. The patient was treated conservatively

Background: Bacterial vaginosis (BV), vulvovaginal candidiasis (VVC) and trichomonal vaginitis (TV) are the most common etiologies of infective vaginitis. The Papanicolaou (Pap) smear is a standard screening test for cervical precancerous lesions that can also be useful to identify infective organisms. This study aimed to identify the causativeorganisms for cervico-vaginal infection in liquid-based Pap smears. Methods: This study was a cross sectional study and included 370 liquid based Pap smears with infective organisms. Results: The most prevalent cervico-vaginal infection was BV, which was detected in 259 (70%) of the cases, followed by VVC in 64 (17.3%) and TV in 25 (6.8%) cases.Women aged 30-39 years were mostlyaffected. Conclusion: The present study shows the usefulness of cervical Pap smear in diagnosis of infective vaginitisother than epithelial cell abnormalities.

Acute epididymitis is most commonly infectious in etiology, usually secondary to a urinary tract infection (UTI) or a sexually transmitted infections (like gonorrhea and chlamydia). Enteric gram-negative bacteria (like Escherichia coli) and coagulase-negative Staphylococcus saprophyticus are the most frequently encountered organisms associated with UTIs. Staphylococcus aureus, especially methicillin-resistant Staphylococcus aureus (MRSA), is an uncommon pathogen when considering community associated acute UTI and acute epididymitis. We present an unusual case of a 28-years-old immunocompetent male, who presented with a left scrotal swelling and was found to have acute epididymo-orchitis due to an acute MRSA UTI.

The small butterfly-shaped gland in the neck plays an important role in regulating the functions of the body. It produces hormones that affect the body metabolism. An under-active or overactive thyroid can cause a lot of problems in the body such as cardiovascular, gastrointestinal, muscle, throat and many more diseases. Therefore, it is important to understand main causes of thyroid problems to have a well-functioning body and to generate healthy future.
