L’existence d’un strabisme accommodatif chez les patients hypermétropes peut être responsable d’une ésotropie instable de près qui peut fluctuer au cours de la vie. L’utilisation des verres progressifs chez l’enfant peut alors être une alternative très intéressante et éviter parfois la chirurgie.
The existence of accommodative strabismus in hyperopic patients may be responsible for unstable near esotropia that may fluctuate over the course of life. The use of progressive lenses in children can then be a very interesting alternative and sometimes avoid surgery.

Prolonged use of screens could impact oculomotor and accommodative functions. To characterize this effect, we studied the evolution of these functions in 18 healthy subjects without convergence insufficiency. After one hour of exposure to screens, we observed a significant decrease in accommodative flexibility and the phoria evolved significantly towards esophoria. The presence of visual symptoms also appears to be associated with reduced accommodative flexibility.

Cet article présente le cas d’une femme de 50 ans, amblyope de l’œil gauche et presbyte, souffrant de douleurs cervicales chroniques et de vision floue en vision de près. L’analyse des facteurs visuels, posturaux et environnementaux a permis de proposer des aménagements concrets, améliorant significativement le confort et les capacités fonctionnelles de la patiente au travail. Cette prise en charge clinique illustre l’importance d’une approche globale combinant évaluation visuelle, ajustements optiques et conseils ergonomiques [1].
This article presents the case of a 50-year-old woman with left-eye amblyopia and presbyopia, suffering from chronic neck pain and blurred near vision. The analysis of visual, postural and environmental factors led to concrete adaptations, significantly improving her comfort and functional capacity at work. This clinical management highlights the importance of a comprehensive approach combining visual assessment, optical adjustments and ergonomic guidance.

Le rapport AC/A (accommodation/convergence) est un indicateur précieux dans l’analyse orthoptique des déséquilibres binoculaires. Trop souvent mal interprété ou sous-exploité, il permet pourtant de mieux cerner certaines décompensations et de guider la pose du diagnostic orthoptique et la prise en charge. Cet article propose une clarification des méthodes de calcul, de l’interprétation clinique du rapport AC/A et de ses implications dans la pratique quotidienne, à travers un cas clinique illustrant l’excès de divergence, un exemple moins connu où la thérapeutique n’est pas nécessairement la rééducation.
The CA/A (accommodation/convergence) ratio is a valuable indicator in the orthoptic analysis of binocular disorders. Although it is often misinterpreted or underutilised, it can help to better identify certain decompensations and guide orthoptic diagnosis and management. This article clarifies the methods of calculation, the clinical interpretation of the CA/A ratio and its implications in everyday practice, through a clinical case illustrating excessive divergence, a lesser-known example where treatment does not necessarily involve rehabilitation.

Une patiente de 45 ans présentant une ésophorie-tropie était sous-corrigée en hypermétropie et passe en exotropie lors du changement de correction optique. La prise en charge a reposé non seulement sur l’ajustement de la correction optique, mais aussi sur une prise en charge rééducative, visant à restaurer l’équilibre oculomoteur.

L’accommodation permet, dans des conditions normales et optimales, une vision nette entre le point le plus éloigné de vision nette (le punctum remotum) et le point le plus proche autorisant cette vision nette (le punctum proximum d’accommodation). Cette accommodation est une fonction automatico-réflexe induite par différents éléments comme le flou, la sensation subjective de proximité ou encore la convergence. Dans le cadre d’une demande de fixation rapprochée, en constante augmentation ces dernières décennies, sont apparus des troubles asthénopiques. L’examen minutieux de l’œil et du système oculaire en vision éloignée ne suffit plus. Il est impératif de savoir mener un examen complet et exhaustif de toutes les composantes sollicitées en vision rapprochée pour assurer une prise en soin avisée de ces troubles asthénopiques.

This function is a reflexive, automatic process triggered by several factors, including blur, the subjective perception of proximity, and convergence. With the steadily increasing visual demand for near tasks in recent decades, related asthenopics symptoms have become more prevalent. À thorough examination of the eyes and ocular system at distance is no longer sufficient. It is now essential to perform a comprehensive and detailed assessment of all components involved in near vision to provide effective and informed care for these asthenopic disorders.
