In Ayurveda, the raw drugs are classified under various groups known as Gana, which contain substances with comparable qualities that can treat a particular ailment. Ganas are very crucial to making the treatment field simpler and more efficient. Acharya Sushrut made a truly remarkable contribution by classifying the drugs into 37 distinct categories for various ailments. Pippalyadi gana is one of the Ganas described by Acharya Sushruta and is specially mentioned for the management of Pratishyaya. Pratishyaya is one important disease of Urdhwajatrugata Roga. In Ayurveda, Kalka, Kawath, Churna and Vati are the most frequently used form of medication. Vati preparation is considered a secondary Kashaya Kalpana and offers an easy way to administer oral medication. In the present study, 14 drugs from Pippalyadi Gana were selected for Vati preparation and certain of them were used in Kwath form for Bhavna, while the drugs containing volatile oils were utilized in powder form. Vati Kalpana has been prepared according to general method of preparation Vati mentioned in API. Prepared Vati were tested by many parameters including physical and chemical ones, such as weight variation and disintegration time, and chromatographic tests, such as TLC fingerprinting, were used to standardize it. After the testing, all parameters were found to be within API standard limits. This kind of result indicates a good therapeutic property of Vati Kalpana which can be used as oral medication for Pratishyaya.

Background: Ayurveda is a traditional system of medicine that is based on the concept of the three doshas - Vata, Pitha, and Kapha, which govern an individual's physical and mental characteristics. Vata dosha serves as the principle driving force within the body. Pitha plays a crucial role in functions such as digestion, metabolism, generating heat, and producing various forms of energy. Kapha is involved in both anabolic processes and maintaining structural integrity of the body. Vata dosha is again divided in to five – prana, udana, samana, vyana and apana vayu. Among these, vyana vayu is mahajava, most powerful and present throughout the entire body. Aim: This review explores the various aspects of vyanu vayu in context of modern physiology. Materials and methods: A comprehensive search for relevant information on Vyana vayu was done by reviewing classical texts and research articles. Conclusion: Vyana vayu assumes a vital role in the coordination and regulation of a range of physiological processes. It holds significant responsibility in overseeing the proper circulation of nutrients, and information throughout the body, primarily within the circulatory and nervous systems. The comprehension of Vyana vayu in contemporary physiology offers valuable insights into the interrelated nature of bodily functions. Acknowledging its influence may have meaningful implications for health and well-being, potentially enhancing the holistic approach to healthcare.

Ayurveda Acharyas has come after a specific strategy in writing a treatise with respect to grammar and composition. Tantrayuktis, Nyayas, Arthashraya, Tachilya and Kalpanas are the tools that help to realize the complicated Sutras which have been explained in Samhita. Saptakalpanas expounded by Acharya Arunadatta in his Sarvanga Sundara commentary of Astangahrudayam. Saptakalpanas are having specific definitions and roles in understanding and analysing the different situations. The Saptakalpanas elucidate by Acharya Arunadatta are Pradhanasya Kalpana, Pradanena Kalpana, Guna Kalpana, Leshakalpana, Vidyakalpana, Bakshyakalpana and Ajna Kalpana. Ajna Kalpana is nothing but is the command or recommendation of a learned person regarding the code of conduct to which with or without specific reasoning is required. Not only Sadvrutta but in other instances also Acharyas recommended or advised the principles to be followed. In this article the role of Ajna Kalpana in understanding and analyzing the principles is noticed. Likewise many more references to find out in other Samhitas for further research to justify the Ajna kalpana.

Obesity, the name itself causes a social as well as mental trauma for the individuals suffering from it. Although in the initial stage of the disease, the individuals overlook the problem, but later, it becomes the cause of much other upbringing disease. So the question arises, what is the cause of the sudden rise in obesity? Does it relate only to the lifestyle disorder or there is a genetic makeup in an individual, in which the tissues starts proliferating and storing adipose tissue, when it gets any favourable condition and environment. In Ayurveda, the genotype and the phenotype has been explained in the term of Saara (essence). Therefore an attempt has been made to understand the effect of Saara. Materials and Methods: 124 case papers diagnosed with obesity and had undergone any Panchakarma treatment at KLEU Shri BMK Ayurveda hospital, Belgaum, Karnataka, India. Complete before and after assessment of total cholesterol, HDL, LDL, triglyceride, circumference measurement, BMI, weight. Result: In Medosaara population and in Mamsasaara individuals, significant decrease observed in total cholesterol (3.76%) and mid- abdomen circumference (34.97%), and triglyceride (10.3%), LDL (6.52%), Mid-thigh circumference (7.36%), abdominal circumference (7.11%), and BMI (6.73%). Conclusion: The Saara of any individual determines the possible outcome of any disease, irrespective of the treatment the individual is undergoing. Therefore, in this study of obesity, it was concluded that Medosaara individuals has significant weight reduction from the baseline.

Tonsillitis is one of the common diseases in pediatric age group. In Ayurveda it can be correlated with Tundikeri, Which has been described under the Mukharoga. Description of Mridweekadi Churna is mentioned in Charaka Samhita in Trimarmiya Aadhyay under Mukha Rogas Chikitsa, and description of Yavagrajadi Vati is mentioned in Yoga Ratnakar in Mukha Roga Chikitsa Aadhyay. According to Acharya Vagbhata the main Dosha involved in Tundikeri is Kapha Dosha. Majority of ingredients of Mridweekadi Churna and Yavagrajadi Vati have Tikta, Kashaya, Laghu, Ruksha Guna, due to this it subsides Kapha Dosha. Therefore, a study was planned to compare the effect of both drug in Tundikeri (tonsillitis). Methods: Children, aged 5 to 16 years, suffering from tonsillitis were registered and categorized under two groups: group- A and group-B. Group A- Patients were treated with Mridweekadi Churna, and Group B- Patients were treated with Yavagrajadi Vati for 10 days and 7 days follow up also done by drug free period. Results: In Group-A, 55% patients were moderately improved and 45% patients showed mild improvements. In Group-B, 15% patients showed marked improvement, 75% patients were moderately improved and 10% patients showed mild improvement. Conclusion: Tonsillitis is an inflammation of the tonsils and it is a common childhood illness. During childhood period there is a natural dominancy of Kapha dosha and Tundikeri- a Kapha Pradhana disease (according to Acharya Vagbhata), hence children are more prone to it. To treat this, drugs should have the Kaphahar properties like Tikta, Kashaya ras and Laghu, Ruksha Guna. In this comparative clinical study both the chosen drugs, Mridweekadi Churna and Yavagrajadi Vati possesses all these properties and also are markedly effective in Tundikeri, but group B Yavagrajadi Vati display better results than Mridweekadi Churna. This study will also help future researchers to evaluate further in this research work.

Obesity is a rapidly increasing global public health concern with a prevalence of 40.3% in India. Obesity is also a significant risk influence for ailments including hypertension, dyslipidaemia, diabetes, coronary heart condition, osteoarthritis, cancer and mental illness. This review investigates the potential role of Triphala in the treatment of obesity and its complications on gut microbiota. Method: We selected relevant articles after searching Scopus, PubMed and Google Scholar using keywords Triphala, Obesity, DM, CVD, and Gut microbiota. Obesity and related complications are considered by definite changes within the human gut microbiome. The gut microbiota of individuals with obesity contains lower proportions of Bacteroidetes and greater amounts of Firmicutes. Gut microbiota modulation and revitalization are emerging as potential obesity prevention and/or treatment strategies via probiotics, prebiotics, synbiotics, or faecal transplants. Research showed that butyrate generating bacteria like Faecalibacterium prausnitzii and Akkermansia muciniphila, were greatest in normal people, but that Bacteroides were halved in diabetes patients. The host metabolic syndrome and cardiovascular risk may be influenced by lipopolysaccharides (LPS) from gut microbiota. A placebo-controlled trial concluded that Triphala includes a hopeful role in reducing weight, circumferential measures and body fat. Triphala's prebiotic effect was studied using in vitro batch cultures, fruit fly, and a simulated model of the human alimentary tract, where Triphala promoted the growth of good bacteria while inhibiting pathogenic species in each model. Clinical research is still under progress on the effect of Triphala on Stool Microbiome Profiles and Inflammation. The action of Triphala in altering the gut microbiota for effective management of obesity and its complications is yet to be studied. Here we recommend the potential action of Triphala in managing obesity and its complications by targeting gut microbiota.

Madhumeha comes under category of Yapya disease. There are very few medicines described in texts for Madhumeha, Svarnamaksika and Shilajita are among of them Rasayana treatment is usually beneficial in yapya diseases. Svarnamaksika is considered as one of the best Rasayana among all Rasayana.[1]  Svarnamaksika improves microcirculation of Rasa dhatu because of having properties like Vyavayi and Laghu. In this case, patient was given Svarnamaksikayoga which contains Svarnamaksika and Salasaradi group which is described in Susruta Samhita. A patient 22-year-old was diagnosed as type 2 diabetes mellitus and after taking 1-year treatment of type 2 DM, she didn’t get any relief and her blood sugar was increasing. Then, she was prescribed insulin when she came for Ayurveda treatment. During first follow-up of 7 days, her blood sugar level was normal so insulin was stopped and she was kept on oral hypoglycemic agent and Ayurveda treatment. After 4 months, her blood sugar level reached to normal level and she was advised to continue Ayurveda treatment only. The collected data of this study suggest that earlier intervention of Ayurveda treatment can reverse the disease process in treatment of insulin dependent diabetes mellitus.
