In recent years, REMS 19 of the city of Spinazzola (Bari, Italy) has built an intervention model oriented towards "care" and "taking care" because the guest subjected to a security measure co-participates in the construction and implementation of his own Individual Residential Rehabilitation Program, starting thus the path of discharge at the time of his entry. Targets Evaluate the impact, in terms of psycho-social functioning, of the proposed model that adopts specific EBM interventions, on the therapeutic-rehabilitation path of a group of 7 users diagnosed with personality disorder. A 12-month observational study was conducted on a group of 7 male patients. At the beginning (T0) during (T1) and at the end of the study (T2) the following assessment scales were administered for precise outcomes: BPRS for clinical recovery and psychosocial functioning; VGF for subjective well-being and interpersonal relationships; I GO for autonomy, self-esteem, self-awareness, resocialization and taking care of oneself; SAMI and BHS for positive expectations and confidence in the future. It is possible to note the trend over time of the results obtained during the three administrations of the evaluation scales. In particular, there was a positive change in the identified outcomes, the main targets of the EBM interventions of the proposed intervention model. Conclusions The study highlighted how the proposed intervention model records an improvement in terms of not only clinical but also personal recovery.

Tako-Tsubo Cardiomyopathy (TTC) is an acute syndrome, identified in the early 1990s by Japanese researchers, which mimics an acute myocardial infarction. Cardiomyopathy is transient and begins with a clinical picture similar to that of an acute myocardial infarction. This pathology seems to be related to intense psychological and physical stress with a prevalence in the female sex (95%) in post-menopause. The term Tako-Tsubo means 'octopus trap' in Japanese: the left ventricle takes on a peculiar appearance in systole, similar to a narrow-necked amphora, morphologically identical to the vessel (tsubo) that Japanese fishermen use to catch octopuses (tako). The appearance of the left ventricle (tako-tsubo shape) is due to a 'complete' depletion of cardiac muscle activity (myocardial stunning), a kind of 'stunning' or paralysis of the middle and apical portions of the heart. Coronarography and ventriculography are an essential step in the diagnosis of TTC. The mere demonstration of a coronary tree free of angiographically significant stenosis is not sufficient for the diagnosis of TTC. The other key diagnostic element is, in fact, the demonstration of left ventricular hypokinesia or akinesia. Since left ventricular wall motility disorders change rapidly, ventriculography performed immediately after coronary examination is the gold standard examination to allow the characteristic appearance of the syndrome to be verified and, consequently, to distinguish it from an acute coronary syndrome with uninjured coronary arteries, thus avoiding a diagnostic error.

In December 2019 in China, after a pneumonia outbreak of unknown etiology, a new RNA virus has been identified and called Sars-CoV-2. Sars-CoV-2 induced severe respiratory infections, with global and rapid epidemic diffusion, designated coronavirus disease 2019 (Covid-19). Sars-CoV-2 infection can lead to severe complications, such as acute respiratory distress syndrome (ARDS) with progression to pulmonary fibrosis. Recent clinical studies described that in patients with severe Covid-19, MSC infusions, promote regenerative and reparative effects with anti-inflammatory and anti-fibrotic action. MSCs do not express ACE2 and TMPRSS2, the two main human receptors for host-pathogen interaction, and are not permissive to in vitro Sars-CoV-2 infection, making them suitable for clinical application. The aim of our study was to evaluate the safety and efficacy of MSCs as cellular therapy in ARDS secondary to Sars-CoV-2 in patients undergoing mechanical ventilation, in order to prevent pulmonary fibrosis. MSCs for infusions are thawed at 2x106/ml cellular concentration. The intravenous infusion protocol consists of two doses of third party allogenic MSCs at 1x106/Kg, 15 day apart. From April 2020, six adult patients median age 65 years, median body weight 80 Kg, in mechanical ventilation for ARDS secondary to Sars-CoV-2 infection have been treated. Early or late adverse events were not recorded. Four out six patients showed a significant gas exchange improvement with extubation within seven days from the first infusion. Our results underline the safety and efficacy of MSC infusions for ARDS patients in mechanical ventilation, supporting the need of a phase I/II clinical trial.

The infections at work from Covid-19 reported to Inail as of August 31 are 52,209. Information relating to harmful events was obtained from the analysis of the INAIL files and from the certificates of the competent doctor. In the period from April 2020 to January 2021, 72 accidents involving employees of the P.O. Guzzardi di Vittoria, infected with coronavirus (SARS-CoV-2). The beginning of the epidemiological wave among employees was found in a crescendo that goes from the middle of which goes from mid-October 2020 to the end of November 2020, divided as follows: 43% with the beginning of an injury in the second half of October 2020, 33.3% in the first ten days of November, and significant reduction of 15.27% in the first ten days of December to then witness a further reduction in the number of cases in January 2021, equal to 5.5%. The days of illness attributable to the COVID infection gave an average of no. 32 days with a disastrous outcome only by chance. The observational analysis is aimed at studying the incidence of COVID-19 accidents among employees who have served in the COVID assistance modules, specifically U.O. Medicine-COVID, U.O. First Aid, Pre-triage Area, U.O. Radiology, equal to 29 injuries (40.3%). The percentage indicators that emerged from the epidemiological survey, essential for arriving at assessments on containment measures relating to the world of hospital work, made it possible to highlight not a few points for reflection and appropriate, if not due, changes to the work organization.

Lateral elbow tendinopathy is the most common cause of pain in this joint. Although it is also widely known as 'tennis elbow' as it affects 50% of tennis players, especially beginners who learn the one-handed backhand, LET often develops as a work-related condition and therefore constitutes a serious public health problem: only 10% of all affected patients play tennis. The typical clinical presentation is characterized by pain and reduction of strength and function with considerable repercussions on daily activities. Although signs and symptoms of LET are clear, as well as the definition of the diagnosis, to date no ideal treatment has yet emerged; clinicians advocate a conservative approach as the first choice of management and physiotherapy, with its wide range of treatments, is commonly recommended: therapeutic exercise has shown good clinical results and the goal of this review is to evaluate its effectiveness in the management of the aforementioned clinical picture. for the realization of this revision was made reference to the guidelines for reporting systematic reviews PRISMA Statement (PRISMA-P checklist). Therapeutic exercise has been found to be the safest, cheapest and most effective means in the management of lateral elbow tendinopathy; all the included trials have precisely outlined the dosage, type and principle of application in different populations without restrictions of the time of onset of symptoms. Unsupervised isometric exercise demonstrated a considerable reduction in pain and disability in the short term. Eccentric exercise, the most contemplated type, was effective in reducing the painful symptom and increasing the grip strength. Stretching, a valid therapeutic strategy often associated with other modes of intervention, has recorded significant benefits related to painless grip strength, function and VAS, overlapping with the results obtained in the comparison groups. Numerous scientific evidences show that the social skills deficit is one of the main sources of discomfort for a psychotic patient, as it compromises the individual's ability to actively participate in society and to effectively manage different situations, be they personal, family or social. As reported by scientific research, all social behaviors can be learned and then modified thanks to experience and training. The approach centered on the enhancement of social skills, social skills training, includes a range of interventions aimed at acquiring and training the social skills necessary in interpersonal situations to communicate with others in an appropriate and effective way over time. This study therefore aims to detect the usefulness and effectiveness of Social Skills Training (SST) on adaptive social functions and on the personal and social functioning of the subjects examined. The sample is represented by 10 patients with psychotic syndromes belonging to the housing community "Borgo Sociale Regionale Roccabascerana", who participated in SST meetings. The "Social Skills Training for the treatment of schizophrenia" manual (A.S. Belleck, K.T. Mueser, S.Gingerich, J.Agresta) was used for the intervention. In spinal cord injury in the acute phase, pulmonary complications are the most common cause of death and often affect the outcome even during the chronic phase. In the initial acute phase, the person is assisted in ICU and the management of the airways can be guaranteed in two ways: invasive by subjecting the patient to tracheotomy and invasive ventilation, non-invasive by extubating the patient early, supporting him with Non-Invasive Ventilation ( NIV). Both approaches have their advantages and disadvantages. For about 6 years in the Intensive Therapy of the Neuromotor Pole (Sod neuroanesthesia and Reanimation) Careggi hospital in Florence has been strengthened 54 patients with complete or incomplete spinal cord injury, resulting from trauma, have been hospitalized only 23 patients (less than 43%) have needed tracheostomy. Those who did not have the tracheostomy performed NIV in the early stages, and then moved on to more autonomous ventilatory modes. All patients were treated with a mechanical cough assistant. From the retrospective investigation it appears that NIV can be considered a valid alternative to invasive ventilation in the bone marrow patient. However, it is necessary to specify that the desired objectives can only be achieved with solid knowledge of the health professionals involved and above all by close collaboration within the multidisciplinary team. The non-invasive approach has reduced complications for the patient, hospitalization times with a consequent reduction in treatment costs. Vanni, Bucciardini Luca","doi":"10.36017/jahc2203-03","DOIUrl":"https://doi.org/10.36017/jahc2203-03","url":null,"abstract":"In spinal cord injury in the acute phase, pulmonary complications are the most common cause of death and often affect the outcome even during the chronic phase.\u0000In the initial acute phase, the person is assisted in ICU and the management of the airways can be guaranteed in two ways: invasive by subjecting the patient to tracheotomy and invasive ventilation, non-invasive by extubating the patient early, supporting him with Non-Invasive Ventilation ( NIV).\u0000Both approaches have their advantages and disadvantages.\u0000For about 6 years in the Intensive Therapy of the Neuromotor Pole (Sod neuroanesthesia and Reanimation) Careggi hospital in Florence has been strengthened 54 patients with complete or incomplete spinal cord injury, resulting from trauma, have been hospitalized only 23 patients (less than 43%) have needed tracheostomy. Those who did not have the tracheostomy performed NIV in the early stages, and then moved on to more autonomous ventilatory modes. All patients were treated with a mechanical cough assistant.\u0000From the retrospective investigation it appears that NIV can be considered a valid alternative to invasive ventilation in the bone marrow patient. Ingrown toenail (also onychocryptosis) is one of the most frequent nail pathologies in diabetic population. Neurological and vascular complications can increase painful symptoms and suffering of tissue which can lead to ulceration, therefore surgical matricectomy is not recommended. In this research we aim to evaluate the effectiveness of a 40% urea cream treatment associated whit nail acrylic resin brace (ortonixia) versus traditional cut and resin nail’s recostruction. Diameter of nail and NRS scale were used to evaluate change of nail plate and pain before and after treatment. 32 subjects with 52 treated nails were included. About 20 nails of 13 subject were recruited in experimental group. Differential final and initial diameter was 4.67±1,55 mm in experimental group and 3.01±1.26 mm in control group ( p<0.0001) and recurrence rate was 15% and 75% respectively ( p<0.0001). Also days of absence of pain, in experimental group there was a mean of 319.05±113.18 days, instead in control group there was a mean of 208.31±119.84 days (p<0.0002). This statistical analysis shows that effectiveness of new conservative approach is greater than in nail reconstruction method and a valid alternative to surgical treatment in diabetic patients.

Background: The quality and effectiveness of therapeutic rehabilitative interventions represent a landmark in the field of mental health. The respect of human rights is strictly linked to health in general, and these two aspects depend on how mental health services guarantee the treatment pathway. Methods: The focus of this study is on the international success of Open Dialogue (OD), once designed for those affected by schizophrenia, since it questions the traditional structure of mental health services. From the study of theoretical principles and the scientific evidences of the benefits of dialogic approach on Services, the core of this contribution consists of how much dialogue can be found inside Services themselves. The field of work was characterised by the operators of two MHCs: Modena Polo Est and Pavullo. The inquiry focused on operators’ behaviours and dispositions towards OD, regardless of the occupation of the workers, but selecting those with direct experience of Open Dialogue. Results: Results highlight that dialogic approach is, in most cases, already adopted by operators, and that it positively affects their dispositions. Conclusion: Despite arduous to undertake, it can be inferred that tolerating uncertainty in its practical application and the reject of a medical-centred approach are extremely necessary. Indeed, not only are they coherent with the dialogic approach, but they also bring improvement aside from the selected approach.

The purpose of our study is to evaluate the T2 weighted sequence with multishot radial sampling (Radial Acquisition Multi-shot) also known as Multivane sequence in Philips Healthcare, introduced by J.Pipe with the aim to minimize motion artifacts in Magnetic Resonance Imaging (MRI). In the field of prostate MRI the Multivane sequence is useful in non-cooperating patients and/or patients who, due to their clinical conditions (such as glaucoma, arrhythmia, and severe benign prostatic hypertrophy), have not been receiving intravenous antispasmodic agents, administered for limiting the motility of intestinal loops, particularly rectum. The Multivane sequence is based on the collection of data throughout parallel multiple lines in periodic rotation around the center of k-space and advanced mathematical reconstruction. As the data at the center of k-space (low frequency) containing signals with maximum amplitude will be continuously sampled, this trajectory will provide an excellent contrast-noise ratio (CNR) and spatial resolution, without motion artifacts responsible of "blurring" in the final image. Specifically, each given point of the periphery of the k-space will be sampled by a certain line and the next one and so on, and for the final image reconstruction, once multiple data will be estimated, different algorithms will be used to compensate for motion artifacts. In this study we compared the Radial Acquisition Multishot TSE Multivane (Philips Healthcare) with the classic T2W TSE sequences with linear Cartesian sampling. Multivane sequences have proven to be superior and therefore of greater utility compared to sequences with linear Cartesian data sampling, in patients who can not receiving spasmolytic agents.
