Background: Children of all ages experience pain. An explosion in pain research over the last decade has elucidated more clearly our understanding of pain mechanisms, appropriate pain assessment, and the safe applicaton of innovatve pharmacologic and non-pharmacologic strategies to treat pain in adults and children. In additon, we now understand that the negatve consequences of untreated pain may have profound efects on physiologic homeostasis, and in the case of the post-operatve patent, morbidity and mortality may be adversely afected, especially in the pediatric populaton undergoing major surgeries such as cardiothoracic surgeries. Aim of Study: The aim of this study was to compare the efficacy of Parasternal nerve vs Erector Spinae Plane Block in comparison with the control group. Patients and Methods: The study was conducted between June 2022 and January 2023 and was carried at the Cardiot-horacic Surgery Department operation theater. Patients scheduled for elective cardiac surgery for non cyanotic ASD & VSD repair were recruited in this randomized research. 3 Groups

Background: After lung cancer, breast cancer is the most frequent cancer among women and the second greatest cause of cancer death. Aim of Study: The rate and causes of breast conservative surgery conversion to mastectomy in early breast cancer versus down staging after neoadjuvant treatment are identified. Patients and Methods: During this study, 40 females with breast cancer were enrolled and divided into two groups. All patients were subjected to conservative breast surgery and intraoperative frozen section; 20 patients with early breast cancer andanother 20 patients who received neoadjuvant therapy. All patients were subjected to history taking, clinical examination, radiologic investigations, core biopsy, preoperative laboratory investigations and histopathological examination of excised mass. Results: There were no statistically significant differences between study groups as regard rate of conversion to mastectomy, frozen results, post-operative compilations and response and Clipping distribution. Conclusion: The rate of conversion to mastectomy, frozen results, post-operative compilations and response, and clipping distribution were similar in cases of early breast cancer and late stages subjected to neoadjuvant down staging treatment. As a result, all cases of late breast cancer were down staged using neoadjuvant therapy rather than undergoing mastectomy.

Background: Intravenous antibiotic administration is an essential component in the management of patients with cerebrospinal fluid (CSF) infections. However, very low concentrations reach the CSF because of the blood-brain barrier. Local irrigation with antibiotics could be a promising option to enhance the outcomes for such patients. Aim of Study: In this prospective randomized trial, we evaluated whether ventricular irrigation with local antibiotics had a significant benefit in these cases. Patients and Methods: Fifty patients diagnosed with CSF infections were divided into two groups; Group I included 25 patients who had CSF irrigation with local antibiotics, and Group II included the remaining patients who had external ventricular drainage (EVD) only. The main outcomes were the modified Rankin scale (MRS), the hospitalization period, and mortality. Results: Preintervention demographic, clinical, and laboratory criteria were statistically comparable between the two groups. However, patients in the irrigation group showed earlier improvement in their blood leucocytes (41 vs. 71 days), CSF leukocytes (4 vs. 12 days), and CSF proteins (12.9 vs. 27.22 days) compared to the other group. There was a significant shortening in the duration of EVD placement in the irrigation group (18 vs. 60 days in the other group). Moreover, patients in the irrigation group had a lower disability scale. Twelve patients died in the non-irrigation group (48%), which was significantly higher than the mortality rate in the irrigation group (20%). Conclusion: CSF irrigation with local antibiotics is associated with significantly better outcomes in patients with CSF infections.

Background: Reaching the HbA1c target and keeping the euglycemic state are major challenges in the management of individuals with diabetes. Given the high rate of failure of oral antidiabetic drugs (OADs) to control the glycemic state of individuals with type-2 diabetes mellitus (T2DM), other new options have emerged to replace the traditional treatments in such cases. Second-generation basal insulin glarginehas proven its safety and efficacy in improving glycemic control in individuals with uncontrolled T2DM in multiple randomized clinical trials (RCTs). Aim of Study: This study aims to assess the effectiveness and safety of insulin glargine 300U/mL (Gla-300) when added to OADs and titrated to individualized goals in real-world practice. Patient and Methods: The ATOS study was a prospective, observational, international, multicenter study to collect information on 4422 individuals with T2DMinitiating Gla-300 across 18 countries in different geographical regions, and which routinely assessed glycated hemoglobin (HbA1c) at least every 6 months (NCT03703869). Our posthocsub group analysis is a part of this large-scale multinational study that analyzed the data collected from Egypt, one of the countries representing the South African region, to assess the real-world effectiveness of Gla-300 in Egyptian individuals with T2DM. The primary outcomes include an assessment of the effectiveness of Gla-300 in achieving glycemic goals measured by HbA1c after 6 months. The secondary objectives include effectiveness and safety during the extended follow-up period (12 months). Background: MUC4 and CD44 have been addressed as major players in the progression and chemoresistance of several tumors. Aim of Study: We aimed to elucidate the role of MUC4, CD44, and ki67 in meningiomas to detect if anti-cancer agents with mucin-depleting and proteolytic effects could help in overcoming chemoresistance in meningiomas. Material and Methods: Fifty meningioma cases were immunohistochemically tested for CD44 and MUC4. In addition to grading of meningioma, Ki67/MIB 1 labeling index was evaluated. Results: MUC4 and CD44 were expressed in 84% and 100% of our cases respectively. Significant correlation ( p =0.007) was detected between meningioma subtypes and MUC4 intensity being highest in meningothelial and lowest in fibroblastic variant. Moreover, advanced meningioma grades were positively correlated with CD44 intensity ( p <0.001) and Ki67/MIB1 labeling index ( p <0.001). In addition, both CD44 and MUC4 immunohistochemical expression showed a significant positive association with Ki67/MIB 1 labeling index ( p =0.011 and p =0.004) respectively. Conclusion: MUC4 and CD44 are upregulated in advanced meningioma WHO grades II and III and correlated with a higher Ki67/MIB 1 labeling index. Subsequently, they can adversely influence the outcome and recurrence rate in meningioma and can be targeted by an agent with mucolytic and proteolytic effects helping in overcoming the common problem of chemoresistance in aggressive meningiomas.
