Diabetic ketoacidosis is the complicationof Diabetes mellitus, which can result due to extreme hyperglycemia and ketonaemia. This condition is prevalent among patients with untreated diabetes ordrug incompliance. Hyperglycemic hyperosmolar state can clinically be presented with episodes of seizures with high blood sugar levels and absence of ketones and has low incidence rate. A 55 year old male patient presented with episode of seizure, frothing, tongue bite, uprolling of eyes, shortness of breath, pedal edema and high blood sugar levels. Past medical history includes known hypertensive and diabetic since 10 years and is oninj regular insulin with non-adherence to medication. Lab investigations were performed and patient was diagnosed as hyperglycemic diabetic ketoacidosis secondary to drug incompliance. The patient was managed by insulin regimen- regular and basal insulin, antibiotics, antiepileptic drugs and by continuous monitoring of blood sugar levels every 3rd hourly. Diabetic ketoacidosis was recovered and later, there were no episodes of seizures. Creating awareness and proper patient education regarding the disease state, pharmacotherapy and drug compliance plays a vital role in managing the condition and as well as by preventing further complications that might be fatal in some cases.

Erythroderma, a severe form of skin reddening, is often difficult to diagnose and is usually caused by underlying inflammatory skin conditions. In this case report, we present the management of a patient diagnosed with drug-induced erythroderma. The patient developed generalized redness and scaling 21 days after receiving an unknown injection for pain from a local pharmacy. A thorough history and clinical examination were conducted, and drug-induced erythroderma was suspected. The patient was treated with a combination of mid-potency corticosteroids, liquid paraffin, recombinant human epidermal growth factor, and oral antihistamines. Introduction: Retinoblastoma is the most common intraocular tumor in child. Treatment depend on stadification and classification of retinoblastoma. The aim of this study was to describe epidemiological and clinical aspects of retinoblastoma in Madagascar. Method: We report in this retrospective study 44 cases found during 5 years, from January 2015 to December 2019. We have collected information about epidemiological and clinical aspect, delay of diagnosis. Classification of retinoblastoma was done with Reese Ellsworth classification and International Classification of Intra-ocular Retinoblastoma (ICRB). Data was collected with excel software and analysis was done with R-studio 4.2.2 software. The quantitative variables were expressed as an average and the qualitative variables as a proportion and percentage. Result: We had collected 44 cases. The incidence was 8.8 cases per year. Sex ratio was 1.3. Mean age was 40 months. Exophthalmos was found in 35% (n=12) of cases. Leukocoria was found in 24% (n=11) children. The delay time before diagnosis was 13 months. In 97.7% of cases, the retinoblastoma was unilateral. Intraocular form was found in 23 patients (52.3%). Among intraocular form 13 patients (57%) presented with grade E, including one patient with meningeal extension 9 patients (39%) with grade D, 1 patient (4%) in grade B. Extra ocular form was found in 21 patients (47.7%) : 17 (77%) had orbital invasion (stage III), 4 (23%) were metastatic (stage IV). Conclusion: Diagnosis of retinoblastoma was made in advanced stage. Most of patients had extraocular form. Prognosis was worse in Madagascar. Making diagnosis early is necessary to prevent loss of life.

Yunis Varon Syndrome was first discovered by Emilio Yunis and Humberto Varon in the year 1980. It affects both genders in equal number. Most of the infants are with Cleidocranial dysplasia, ectodermal anomalities, distal aphalangia. By the characteristic features which including deformity of the pelvis, dislocation of hips , bone fracture, urinary tract abnormalities, central nervous system abnormalities by this they have reported the condition as Yunis Varon Syndrome. This is an autosomal recessive inherited multisystem disorder due to FIG4 gene mutations, consanguineous marriages, Lysosomal defects, which may leads to improper in the functioning of organs in the infants. Metabolic disorders in which abnormal growth due to some toxic substances in the body. Affected people with this syndrome may experience breathing problems, abnormalities in the skeletal system, congenital heart defects. Genetic testing for mutation can be detected through diagnosis. In some conditions they may also be detected before birth of the baby that is prenatally by ultrasonography.Many of the infants did not survive beyond on year. Genetic counselling will be of benefit for affected individuals and their families.

Ischemic Mitral regurgitation is a leakage of blood backward through the mitral valve each time the left ventricle contraction. Patients are often ill with significant hemodynamic instability (Acute) needs urgent medical treatment. During normal cycle mitral valve (bicuspid valve) opens and closes normally. But in mitral regurgitation mitral valve does not opens and closes properly. The blood from right atrium moves to right ventricle then mitral valve does not closes properly leads to blood ejected back to right atrium which results in pulmonary edema and increases pulmonary hypertension. Ischemic Mitral regurgitation leads to complication of acute myocardial infraction and coronary artery disease

Ectopic Pregnancy also termed as extrauterine pregnancy in which the fertilised egg implantation occurs outside the uterus. Cervical ectopic pregnancy is one of the rarest form with an incidence of less than 0.1%.Cervical ectopic pregnancy is life threatening due to the high risk of bleeding if it is not treated correctly. It can be diagnosed through symptoms, ultra sound imaging and serum Hcg levels. Symptoms include bleeding and pelvic pain. Methotrexate is the drug of choice, but still there is no consensus on the most appropriate treatment for this abnormal pregnancy. In this case we discussed about a 30years old pregnant woman presented with chief complaints of bleeding, white discharge, burning micturition and a known case of diabetes and hypertension.

The current study was carried out to study the impacts of milk butter from Iraqi-bred cows fed on a diet mixed with fenugreek seed on the lipid profile of Wistar ratsin Al-Diwaniyah City, Iraq. The experiment contained the recruitment of 10 lactating locally-bred cows (LLBCs) fed with 12.5gm/animal/day for 24months of soaked fenugreek seeds (SFSs), SFS group. As a cow control (CC) group, 10 LLBCs were used with no treatment. After the end of this experiment, the milk was utilized to produce butter. Then, the SFS-originated butter (SFSB) was orally-administered to 10 rats at 0.08mg/kg daily for 30 days, SFSB group. For the rat control (RC) group, 10 rats were orally-administered with CC-originated butter (CCB) at 0.08mg/kg daily for 30 days, CCB group. After the end of the rat experiment, the body weight and serum profile of lipids of rats for both SFSB and CCB groups were measured. The result showed that the oral supply of the butter from SFS cows induced potentially (p˂0.05) increased in the HDL levels in the SFSB rats. In addition, the LDL and VLDL from the same rats revealed no significant (p˃0.05) changes after using the butter originated from the cows treated with SFS. Significant (p˂0.05) elevations were recorded in the levels of TC, TG, and weights in the rats treated with the butter produced from the SFS cow milk. The study concludes that feeding cows with fenugreek seeds has successful impacts in increasing the levels of HDL “good lipid” and decreasing the levels of LDL and VLDL “bad lipids” in the rats treated orally with butter produced from these cows.

Pregnancy is the period where a woman undergoes several physical and hormonal changes in accordance to the growth of the fetus. Wide alteration in the levels of hormones from the normal values affect the state of gestation and the growth of the baby. Especially the most commonly encountered disorders like pregnancy induced thyroid disorder effects the neuro development of the baby whereas gestational diabetes makes mode of delivery complex. Hormonal imbalance can be investigated by haematological findings like thyroid stimulating hormone (TSH), T3 and T4 tests to identify thyroid disorders whereas HbA1C, Random blood sugar(RBS), fasting random blood sugar(FRBS) , post prandial blood sugar (PPBS) and oral glucose tolerance test (OGTT) to screen gestational diabetes. Apart form these there are some uncommon hormonal pregnancy disorders like cushing syndrome, conns syndrome, congenital adrenal hyperplasia, adrenal insufficiency. These complexes can be rectified up to certain extent by diagnosing them before hand thereby implementing life style changes, dietary changes and taking medications. Therefore, prior screening of the diseases is important in supress further complications in pregnancy and to have healthy progeny.

Hormonal imbalance can be investigated by haematological findings like thyroid stimulating hormone (TSH), T3 and T4 tests to identify thyroid disorders whereas HbA1C, Random blood sugar(RBS), fasting random blood sugar(FRBS) , post prandial blood sugar (PPBS) and oral glucose tolerance test (OGTT) to screen gestational diabetes. Apart form these there are some uncommon hormonal pregnancy disorders like cushing syndrome, conns syndrome, congenital adrenal hyperplasia, adrenal insufficiency. These complexes can be rectified up to certain extent by diagnosing them before hand thereby implementing life style changes, dietary changes and taking medications. The traditional medicine, Cassia alata, also known as Ketepeng Cina in Indonesia and Gelenggang in Malaysia, has been used to cure a variety of illnesses particularly skin conditions. Additionally, it has been suggested that C. alata may have antifungal, anticancer, anti-inflammatory, and antioxidant properties. Alatinon, alanonal, flavones, flavonols, and flavonoid glycosides, as well as -sitosterol-D-glucoside, are among the isolated metabolite substances from C. alata. Mostly from the leaves, chemicals have been isolated. The secondary metabolites from other plant components such as the seed, flower, and bark, which are also said to have strong antibacterial and antifungal activity, need to be further identified. Due to its biological activity and secondary metabolites, this plant has been demonstrated to have pharmacological activities against selected diseases.

Introduction: Trigger Thumb is a condition that is caused by a thickened A-1 pulley, often secondary to a stenosed tendon sheath. There are several treatment options for trigger thumb, yet the use of prolotherapy to treat Trigger Thumb has not been widely discussed in the literature. Therefore, this study aims to present the case of Bilateral Trigger Thumbs which were treated using Prolotherapy. Case: A 49-year-old female patient was admitted to the hospital with a complaint of inability to flex his both left & right thumbs two months ago. On physical examination, there was pain and swelling at the base, palmar side of his both thumbs, tender in consistency, and warm. There was a pain in daily activities using his both hands. There wasn’t a history of trauma. The range of motion of both thumbs was decreased. The prolotherapy was given to the right thumb, yet the left thumb was treated by lidocaine injection only. After several treatments using prolotherapy, the right thumb showed better results compared to the left by increasing ROM, decreasing pain level and swelling, and the improving function of the right thumb. Conclusion: In this case, the right thumb, which was treated with prolotherapy, has shown better results compared to the left thumb which was treated with medication only. We suggest prolotherapy to treat trigger fingers for the other patients in our hospital.
