Objective: to examine the morphofunctional status dynamics of female athletes who had COVID-19 during training in the middle altitude. Materials and methods. The study of the morphofunctional status was conducted in the beginning and at the end of training camps in Kislovodsk at an altitude of 1240 meters. It included 78 elite female athletes during the preparatory period of the training process. We have identified the initial level and dynamics of the functional state of the cardiovascular and respiratory systems, heart rate variability and body composition. We have also carried out a questionnaire of the state and collected medical background about the COVID-19. Results. According to the data obtained, training in the middle attitude did not significantly influence the morphofunctional status of female athletes, both who had and who did not have COVID-19. All indicators were within physiological standards. The functional state dynamics of athletes was mostly connected to the response to mountain hypoxia, which in higher degree is shown in athletes with the third type of the heart rate autonomic regulation. The athletes who were ill with COVID-19 were slightly different (within normal limits) from those who were not in terms of body composition indicators, expiratory volume and rate, as well as blood pressure. The systemic analysis has demonstrated that the factor-based structure of variables of the morphofunctional status of female athletes who were ill with COVID is described with an increased correlation connection between structural and functional indicators, stress of the body’s adaptation. Conclusion. Despite the seemingly favorable morphofunctional status of female athletes, who had COVID-19, in terms of systemic connections it is still characterized with an emergency transitional process of adaptation, during which complication of various diseases is possible. It should be noted by working doctors in sports medicine and coaches working with athletes, especially in the middle altitude conditions.

Objective is to study the relationship between clinical, laboratory and radiological manifestations of alveolar-bronchiolar dysfunction with the production of interleukin-20 in patients with community-acquired pneumonia. Materials and methods. We examined 60 patients of both sexes aged 18 to 45 years with bacterial community-acquired pneumonia in the first 3 days of the disease, as well as 15 practically healthy individuals. The material of the study was venous blood, in the serum of which the concentration of interleukins (IL) was determined: IL-1β, -2, -4, -8, -10, -17A, -20, -28A, -33, tumor necrosis factor-alpha (TNFα), interferon-gamma (IFNγ), Clara cell protein (CCP), surfactant protein D (SP-D), prostacyclin (PgI2), soluble form of the Fas-receptor (sFas), and its ligand (sFasL), leukotriene D4 (LTD4), thromboxane A2 (TA2). Results. The development of pneumonia is accompanied by an increase in the production of the studied mediators, which is more pronounced in patients with a severe course of the disease. An increase in the concentration of IL-20 from the minimum to the maximum level is accompanied by a significant proportional decrease in the concentration of IL-1β, TNFα, IL-33, TA2, LTD4, Fas, FasL. A high level of IL-20 was associated with a decrease in the concentration of CCP, indicating a weakening of the alveolar-bronchiolar dysfunction and a decrease in the intensity of the inflammatory reaction in the lung tissue. It was also found that a high level of IL-20 was associated with increased production of PgI2, IFNγ, IL-1RA, IL-17A, IL-8, IL-4, IL-28A, IL-10, SLPI. Conclusion. The results of the study indicate the important role of IL-20 in the development of inflammation of the lower respiratory tract, which consists in regulating the activity of the acute phase response, modulating the production of prostacyclin and leukotrienes, determining the limitation of alveolar-bronchiolar dysfunction in patients with pneumonia, as well as the restriction of proapoptogenic influences that determine the decrease in volumes. tissue and cellular damage in such patients.

Objective: to study the structure of medical prescriptions for antibacterial drugs used in patients with various types of infection of the bronchopulmonary system in the conditions of typical practice in Kursk (Russian Federation) and Chisinau (Republic of Moldova). Materials and methods. In order to assess the structure of prescribing various classes of antibacterial agents used to treat patients with infections of the respiratory system, a survey of doctors from medical organizations in Kursk and Chisinau was conducted in the period from October 2019. to January 2020 as a one-time descriptive study. As model diseases are taken: acute bronchitis with purulent sputum, exacerbation of chronic obstructive pulmonary disease with purulent sputum, non-severe community-acquired pneumonia. Results. The leaders in prescription for acute bronchitis with purulent sputum were cephalosporins: in Kursk, their share was 24% (cefixime was most often prescribed), and in Chisinau, 26.8% (leaders were cefixime and ceftriaxone). In patients with exacerbation of chronic obstructive pulmonary disease and purulent sputum, doctors from both cohorts also more often recommended cephalosporins: 29.4% and 36.1% for Kursk and Chisinau, respectively; among them, cefixime, ceftriaxone and cefotaxime were leaders in Kursk, while ceftriaxone, cefotaxime and cefuroxime axetil were the priorities in Chisinau. For non-severe community-acquired pneumonia, doctors in Kursk preferred macrolides (more often - azithromycin and clarithromycin), inhibitor-protected penicillins (more often - amoxicillin/clavulanate) and cephalosporins (mainly of the 3rd generation), in Chisinau, cephalosporins (mainly of the 3rd generation) and penicillins (including inhibitor-protected) were more often prescribed, the proportion of macrolides was significantly less. Conclusion. Doctors from Chisinau and Kursk implement pharmacotherapy for this group of patients in accordance with modern clinical guidelines; at the same time, cases of irrational use of certain medicines are recorded.

Objective: to study arterial stiffness in patients with acute coronary syndrome without ST elevation, who have hypertension (AH) and stage 2-3A chronic kidney disease (CKD) and to assess the ability of angiotensin-converting enzyme inhibitor perindopril and angiotensin receptor antagonist losartan to reduce arterial stiffness in these patients. Materials and Methods. We studied 44 patients with ACS without ST elevation combined with CKD stage 2-3A, AH (the 1st group). The comparison groups were the ACS without ST segment elevation, AH patients with normal renal function (the 2nd group, n=27) and the 3rd group (n=44) of patients with chronic CHD, AH and CKD. Group 1 patients were divided into 2 subgroups taking perindopril or losartan. The parameters of vascular wall stiffness (pulse wave velocity (PWV), cardio-ankle vascular index (CAVI), ankle brachial index (ABI), aortic augmentation index (AI), central systolic and pulse aortic pressure, peripheral blood pressure (BP), estimated glomerular filtration rate (GFR) were assessed.) Results. The patients with ACS without ST elevation combined with 2-3A stages of CKD and AH had a significantly higher cPAP, AI, PWV, and CAVI than the patients of the 2nd group. During 3 months of complex therapy with perindopril, a decrease in PWV, cSAP, cPAP, AI was observed. There were no significant differences in the effects of perindopril and losartan on peripheral and central blood pressure, on renal function, on arterial stiffness parameters. Conclusion. Patients with AH and CKD stage 2-3A have more pronounced arterial stiffness compared to similar patients with normal GFR. Antihypertensive therapy with perindopril and losartan allows to reach target levels of peripheral BP, significantly reduce central aortic pressure and improve elastic properties of the arterial vascular wall.

A comparative study of the water-soluble polysaccharide complex and pectin substances of first and second year shoots of common raspberry. Objective - comparative study and sorption activity of polysaccharides of common raspberry shoots. Materials and methods. The samples for the research were collected in a dark coniferous forest on the territory of the Ilyinsky district of Perm Region. The content of polysaccharide fractions was determined by the modified Dreywood anthrone- sulfurous method. A water-soluble polysaccharide complex and pectin substances were sequentially obtained from different parts of the shoots. The sorption activity was determined by the ability of the substances to bind methylene blue, which is a marker for most medical sorbents. The acute toxicity of the obtained polysaccharide fractions was examined according to Prozorovsky V.V. Results. The shoots of the first and second year of common raspberry accumulate more pectin substances than water-soluble polysaccharides. Leaves of raspberry contain more polysaccharides compared to leafless parts. As a result of acute toxicity study, it was found that all studied substances can be classified as low-toxic (LD50>5,000 mg/kg). It was found that water-soluble polysaccharide complex obtained from leafless parts of the shoot of the first and second year shows sorption activity reliably higher than that of the reference preparation (Polyphepan). Sorption activity of pectin substances obtained from leaves of the first and second year shoots also exceeds that of the reference preparation. The highest sorption activity is shown by pectin substances of the second year, and the lowest sorption activity by pectin substances of leafless raspberry shoots. Conclusion. The obtained substances are promising for creating modern sorbents based on polysaccharide fractions of the first and second year shoots of common raspberry. Taking into account the simplicity in harvesting and high sorption activity of polysaccharides of the leafless part of the raspberry shoot, not only leaves but also leafless parts of the shoot should be harvested.
