On mechanisms of increased intestinal wall permeability under stress 应激下肠壁通透性增加的机制研究

One of the manifestations of the stress reaction in the colon is an increase in the permeability of its wall due to the development of an inflammatory reaction against the background of changes in neuroendocrine regulation and microbiota state. These processes are accompanied by significant changes in humoral homeostasis and cell activity involved in the development of an inflammatory response in the colon wall. We performed a literature review to analyze and summarize the available research data on the mechanisms of stress-induced changes in the intestine at the molecular, cellular and tissue levels involving the regulatory systems of the body. Scientific information was searched in Web Of Science, Scopus, ScienceDirect, Medline, Russian Science Citation Index (RSCI), as well as in the search engines Google Scholar, PubMed, Semantic Scholar, Taylor & Francis, Wiley Online Library and Bielefeld Academic Search Engine (BASE). Wall permeability has been shown to have a rather complex regulation involving corticotropin-releasing factor, mast cells, dendritic cells, eosinophils, macrophages, Substance P, nerve growth factor, neurotensin, microbiota metabolic factors (serotonin, short-chain fatty acids, indole derivatives and conjugated fatty acids), epigenetic mechanisms, the HES1 (Hairy/Enhancer of Split-1) - GR (glucocorticoid receptor), and the stress-associated polarity signaling pathway. Under stress, there is a change in the functioning of these mechanisms, leading to an increase in the permeability of the intestinal wall. It results in translocation of bacteria from the lumen into the underlying layers which causes activation of the immune response with subsequent development of an inflammatory reaction. The presented data testify to the prospects and validity of the development of methods of correction of stress-induced shifts in the colon by influencing the central and local mechanisms of realization of the stress response and the state of the microbiota.

Pathophysiological aspects of bone tissue regeneration when increasing the width of the jaw alveolar crest with the application of piezosurgical technique 应用压电外科技术增加颌牙槽嵴宽度时骨组织再生的病理生理方面

One of the main problems that impede dental rehabilitation with the use of dental implants is the limited amount of bone tissue in cases of its pronounced atrophy. According to a number of authors, about 50% of patients need additional osteoplastic operations before or at the stage of dental implantation. Objective. The aim of the paper is to assess the effectiveness of piezosurgical apparatus for splitting the alveolar crest of the jaw in the insertion of dental implants. Materials and methods. Forty adult patients (15 men and 25 women) aged 36 to 49 years who had undergone surgery for partial tooth loss on one and/or both jaws were evaluated. All in all, 48 implants were placed against the narrow alveolar crest of the jaws, which was surgically split using microsaws (Group 1- 11 patients), thin dental drills (Group 2 -15 patients) and piezosurgery apparatus (Group 3, 14 patients). The postoperative follow-up period was 4 to 6 months before the orthopaedic treatment. Results. During crestal splitting, the smallest bone reduction was obtained using piezosurgical technique, which was 0.84±0.22 mm, while with microsaws and dental drills, this figure was slightly higher, 0.93±0.21 mm (p≥0.05) and 1.23±019 mm (p≤0.05), respectively. One of the properties of a piezosurgical instrument is the effect on protein molecules, namely, their denaturation. This reduces the number of postoperative complications. Conclusion. The technique of alveolar crest splitting with a piezosurgical instrument works according to the absolutely natural and unique mechanism of bone healing - the mechanism of fracture repair. This technique uses the local bone resource and avoids or significantly reduces the use of foreign bone grafting materials. There is a tendency to a better rate of restoration of the lost width of the alveolar crest of the jaw when using the piezosurgical apparatus, in contrast to the use of microsaws and dental drills.

Comparative phytochemical analysis of alchemilla vulgaris herb infusion and alchemilla mollis herb infusion 金缕草冲剂与金缕草冲剂的植物化学对比分析

The research data revealed the existence of various pharmacological effects of Alchemilla vulgaris herb infusions and confirmed the prospects of introducing these infusions into medical practice. Biologically active substances (BAS) in the Alchemilla herb are of high variability, which adversely affects the phytochemical composition of the infusion and its pharmacological properties. The expansion of the raw material base of Alchemilla is possible due to Alchemilla mollis, which is a micro-specie with a stable chemical composition. Objective: a comparative analysis of the phytochemical composition of infusions of A. mollis and A. vulgaris herbs to assess the possibility of expanding the raw material base of the aerial part due to the cultivated micro-specie A. mollis. Materials and Methods. The objects of the study are A. vulgaris herb infusion (AVI) and A. mollis herb infusion (AMI). The harvesting of raw materials was being produced during the period of mass flowering from cultivated Alchemilla mollis (Buuser) Rothm and wild-growing Alchemilla vulgaris L. s. I. Preliminary qualitative analysis was performed by conventional methods. The content of tannins, polysaccharides, ascorbic acid, and flavonoids were quantified. Results. A preliminary qualitative analysis showed the similarity of the studied infusions, including the presence of phenolic compounds, markers of the genus Alchemilla (caffeic acid, luteolin and quercetin derivatives). The peculiarity of AMI is the predominant content of condensed tannins, which is consistent with the data on the accumulation of this group of biologically active substances in A. mollis herb. Significant higher amounts of tannins (p<0.05) and ascorbic acid (p<0.05) were found in BAS; no differences were found in the amount of polysaccharides and flavonoids. Conclusion. The conducted study indicates the similarity of the chemical composition of the studied infusions, which allows the use of AMI similarly to AVI, and A. mollis herb to expand the raw material base of the herb.
