. Abstract Background: The second most common kind of neurodegenerative disease is Parkinson's disease (PD)., and there are still significant challenges with its pharmaceutical treatment. Due to its potential therapeutic benefits on motor functions in Parkinson's disease (PD), high-frequency repetitive transcranial magnetic stimulation (HF-rTMS) of the motor cortex is an exciting non-invasive brain stimulation (NIBS) for treating both motor and non-motor symptoms of PD. Objectives: The purpose of this prospective sham-controlled double-blinded study was to quantify the treatable effects of high-frequency rTMS on PD-related motor and non-motor symptoms. Patients and Methods: Between August 2022 and May 2023, 35 participants of both sexes were included in this research after being diagnosed with PD. All patients were attending Minia University Hospital Neurology outpatient clinic. Patients were randomly assigned as 1:1 to either the Real-rTMS group (high-frequency, 5 HZ) or SHAM-rTMS group. Ten consecutive sessions of TMS were performed. The Unified Parkinson's Disease Rating Scale (UPDRS) was used to evaluate all patients both before the first session and again after the tenth session was completed. Results: After 10 consecutive sessions, only the real-rTMS group demonstrated statistically significant improvements in motor section of UPDRS score (UPDRS 3) and overall UPDRS scores. In contrast, the SHAM-group showed no statistically significant changes. Conclusion: These findings suggest that high-frequency rTMS might play a significant role in treating motor manifestations of PD.

. Abstract Background: One of the main causes of end-stage renal disease worldwide is diabetic nephropathy, a catastrophic microvascular sequel of diabetes. The pathophysiology of DN must thus be urgently investigated in order to develop appropriate remedies. MALAT1 (Metastasis Associated Lung Adenocarcinoma Transcript 1) is anticipated to be a novel target for the detection and therapy of diabetic nephropathy. Objectives: This research primarily sought to verify the expression of the circulating lncRNA MALAT1 in peripheral blood mononuclear cells (PBMCs) of diabetic patients, diabetic nephropathy, and healthy controls and assess it's relation to disease related criteria Patients and methods: 50 diabetic volunteers in this trial sought medical attention at the Outpatient Clinic of Endocrinology & DM at Sohag University Hospital, between January2022 and June 2022 and compare them with 25 apparently healthy persons. The expression of the lncRNA Malat1 was measured using quantitative real-time PCR (qPCR) in 25 people with diabetes, 25 people with DKD, and 25 healthy controls. The clinical relevance of the observations was then assessed. Results : As compared to control, LncRNA MALAT1 expression in peripheral blood was substantially higher in the diabetics and DKD groups. Spearman correlation showing significant correlation between RQ and duration of DM as P<0.05, also showing significant correlation between RQ and A/C ratio as P<0.05, there was positive moderate correlation between RQ and HBA1C and showing significant negative correlation between RQ and eGFR as P<0.05. Conclusion : The best technique to identify diabetic nephropathy may be to combine the detection of urine ACR, serum creatinine, and eGFR with diabetes mellitus. LncRNA Malat1 is substantially expressed in DKD patients compared to diabetics and an apparently healthy group.

Background: CBME was introduced in 2019 for medical students with various new teaching learning methods and assessment of competencies using skill certifications, formative and summative assessments. However, imparting knowledge and skills could be challenging with current curriculum due to various factors. Objective: This study was undertaken to understand the progress and various current challenges faced by faculty for appropriate implementation of CBME curriculum. Materials and Methods : A cross sectional study was conducted among teaching faculty from various medical colleges across India. A structured questionnaire was prepared considering various parameters that have been implemented as a part of CBME curriculum. The questionnaire was uploaded as Google forms and the link was sent to medical teachers through electronic platforms. Data was analysed using descriptive and inferential statistics. Results: A total of 165 responses collected from teaching faculties from various medical colleges. 82% of the faculty had undergone FDP programme. 53% of faculty responded that allotted time was sufficient to complete the syllabus, 60% of the teachers felt that there were enough resources for skill certification and CBL was accepted as a more useful learning tool for students by 78% of the faculty followed by SGD (56%), SDL (55%) & AETCOM (55%). Conclusion : Even though faculty supported CBME, regular and frequent FDP, improving student to staff ration and ensuring sufficient resources will help in overcoming the lacunae for successful and uniform nationwide implementation of curriculum.

Background: Color Doppler is an important, noninvasive, non-ionizing imaging modality for assessment of vascular system of most parts of the body "Neck, limbs, hepatic vasculature …. etc.". It is commonly used to assess patient either before medical or surgical treatment but less common to be used intraoperative. Color Doppler assessment of hepatic vasculature in the daily work is commonly used to assess patency of portal vein and hepatic artery and to assess the hemodynamics of hepatic blood flow. Intraoperative assessment of hepatic vasculature integrity is not a common use of Doppler. It can help the surgeon to complete his operation on the usual steps with confirming non injury of hepatic vasculature. Case Report: We present a case who presented to Sohag University Hospital outpatients clinics by repeated right hypochondrial pain. Ultrasound was done and revealed that he has chronic calcular cholecystitis with a stone obstructing cystic duct the surgeon decided to do cholecystectomy. During the operation a sudden profuse bleeding at the site of hepatic hilum occurs, the surgeon doubt that a major vessel "Hepatic artery or portal vein was injured ", so he asked for urgent color Doppler assessment of hepatic vasculature integrity. Conclusion: Color Doppler examination is an important tool for assessment of vascular system of different organs including liver. It can be used intraoperatively to confirm integrity of blood vessels and exclusion occurrence of iatrogenic complication during the operation itself, guiding the surgeon to make a suitable decision during the same setting and avoiding re-exploration of the patient.

Background : Arabs have been reported to exhibit a high percentage of resistance to the COVID-19 vaccine; these rates were comparatively higher than the global rate. The low level of public acceptance of COVID-19 vaccinations is most likely a result of misconceptions. According to reports from Egypt and many other Arab countries, the main obstacles to mass immunization campaigns are worry over severe post-vaccination side effects and incorrect information regarding the COVID-19 vaccine. Objectives : The current study's objectives were to assess the level of COVID-19 vaccination acceptability and identify factors that influence it among hospital attendants in Qena City. Patients and methods: 450 people who were at least 18 years old participated in a cross-sectional study. A standardized questionnaire was used to collect the data. The participants were split into two categories: those who decided to receive the COVID-19 vaccine and those who decided not to. Results: Our study's subjects had a 50.9% acceptance rate for the COVID-19 vaccination. Age, marital status, the belief that immunity will develop after receiving the COVID-19 vaccination, and trust in the vaccine's safety and efficacy were the key determinants of COVID-19 vaccine acceptance. Conclusion: Low levels of COVID-19 vaccination acceptability have been observed among the subjects in our study. Confidence in the safety of the vaccine was the main predictor of COVID-19 vaccine adoption. Therefore, there is a need for intervention that focuses on providing the general population with enough knowledge about the safety and effectiveness of COVID-19 vaccines.

Background: since antiquity post-partum haemorrhage (PPH) has been a terrible event for obstetricians. It accounts for 34% of maternal deaths in Africa and developed countries. It complicates 6% of cesarean delivery (CD) and still raising and correlates with increased frequency of caesarean sections, many uterotonic agents have successfully used alone or in combinations for prevention of such catastrophic, but the most effective medication is still up for controversy. Objective: To compare the efficacy and safety of temporary uterine packing combined with topical tranexamic acid (TA) as adjunct for reducing blood loss following an elective cesarean delivery with intravenous tranexamic acid and placebo in women who have at least one risk factor for postpartum hemorrhage. Patients and methods: A double-blind, randomized clinical trial (: NCT03706339) conducted on 450 pregnant women at term (38 – 40 weeks) gestation scheduled for elective cesarean delivery, who were assigned to either intravenous TA, topical TA, or placebo(saline). The main outcome measures were blood loss at and 6 hours after cesarean delivery, the need for any additional oxytoxic drugs, and TA-related side effects. Results: There was a significant decrease in the intraoperative blood loss and total blood loss in both topical TA and IV TA groups compared to placebo group (p=0.0001,0.0001,0.0001,0.0001). Also, the need to extra uterotonics was significant decrease in IV TA group, 9 (6%) patients compared to 33 (22%) patients in placebo group, and 24 (16%) patients in topical TA group, (p=0.0001 and 0.006) respectively. Finally, operative time, hospital stay, postoperative hemoglobin, and post-operative complication showed no significant difference between the three groups (P= 0.276, 0.126, 0.853, 0.955, 1.00, 1.00) respectively. Conclusion: IV TA and temporary uterine packing combined with topical tranexamic acid is more effective than placebo in reducing total blood loss during and after cesarean delivery in women who have at least one risk factor for postpartum hemorrhage, but IV TA more effective.

Objective: To compare the efficacy and safety of temporary uterine packing combined with topical tranexamic acid (TA) as adjunct for reducing blood loss following an elective cesarean delivery with intravenous tranexamic acid and placebo in women who have at least one risk factor for postpartum hemorrhage. Patients and methods: A double-blind, randomized clinical trial (: NCT03706339) conducted on 450 pregnant women at term (38 – 40 weeks) gestation scheduled for elective cesarean delivery, who were assigned to either intravenous TA, topical TA, or placebo(saline).
