De l’aide médicale à mourir au Québec : une analyse du contexte et de la pratique infirmière en soins palliatifs. 从魁北克的医疗援助到死亡:姑息治疗的背景和护理实践分析。

Context : Quebec's "medical aid in dying" (MAID) is a medical intervention mostly practiced in palliative care units. MAID results from a deep revolution within Quebec's ethics regarding end-of-life care. However, there is a lack of empirical studies regarding nursing practice within the context of MAID.Aim : To conduct a literature review of legal and scientific data, as well as data from the media, related to the patient experience of nursing practices in palliative care for patients in Quebec who have requested MAID.Methodology : Burn, Grove, and Sutherland's methodology allowed us to extract and analyze six empirical studies from the CINAHL database. We also analyzed 17 media articles and one legal paper that documents the legalization of MAID in Quebec.Results : There has been a strict law governing MAID since 2015, but some major social events and issues have recently allowed for the expansion of the practice. The literature review allowed us to identify two themes : 1). the nature of nursing practice within MAID, and 2). the need for nursing education that addresses the complex aspects of MAID.Discussion and conclusion : We discuss the effects of the domination of the biomedical narrative on MAID and the collapse of Quebec's health system, which both negatively impact nursing practice in the context of MAID.

The place of scientific knowledge in the teaching of technical skills in initial nursing training: a descriptive qualitative study 初级护理培训中科学知识在技术技能教学中的地位:一项描述性定性研究

Context: Universitarization and the nursing education reference system encourage the use of scientific knowledge and the integration of evidence into clinical practice. Students in initial training are trained in technical gestures. On what knowledge are they based?

Objective: The main objective of this study is to identify the knowledge utilised by nurses and trainers to teach technical nursing procedures.

Method: A descriptive qualitative study was carried out using semi-directed interviews with 16 participants.

Results: The teaching of nursing skills is mainly based on local or authoritative knowledge. Trainers and nurses who display scientific knowledge update their practices and education more easily. As in health care services, the lack of knowledge and training in evidence-based medicine and the use databases, as well as the lack of English language skills, are the major obstacles to teaching technical procedures based on scientific knowledge.

Discussion: The use of scientific data in the learning of technical procedures during initial training could legitimate the knowledge taught, develop the students’ critical thinking and encourage their autonomy in the face of protocol injunctions and service habits.

Evaluating the French version of an online training course for a pediatric pain assessment scale 评估儿童疼痛评估量表在线培训课程的法语版本

Context: There is no French-language training to educate nurses on the use of the Face, Legs, Activity, Cry, Consolability (FLACC) pain scale, whose scores guide the treatment of pediatric pain.

Aims: The aim of this study was to evaluate a French online training program for the FLACC scale offered to Francophone undergraduate nursing students.

Methods: Online training was offered to nursing students enrolled in a pediatric nursing course. Participants completed online questionnaires pre- and post-training to assess their perception of their knowledge and confidence, the accuracy of their pain assessment scores, as well as the usefulness and user-friendliness of the training.

Results: The FLACC online training improved students’ perceived knowledge (p = 0.0004) and confidence (p = 0.0053) in the FLACC pediatric pain scale. Students’ accuracy of severe pain assessment scores significantly improved (p = 0.0159) and slightly improved for moderate pain (p = 0.6363). However, accuracy for mild pain assessment was slightly decreased post-training (p = 0.7686).

Discussion: An improvement of the quality of videos linked to mild pain, and the quantity of videos for all levels of pain, is required for this study to be replicated among a larger sample.

Conclusion: The online training fills the gap in nurses’ lack of knowledge about the use of the FLACC pain scale and improves access to quality training in French.

Training in “reasoned handling care”: (Re)discovering meaning in work “理性处理护理”培训:(重新)发现工作的意义

Context: Patient handling has had adverse health effects on nursing staff for many years. In order to prevent these effects, training programs using a demonstration-based pedagogy, aiming at promoting “good” handling practices, are often proposed.

Goals: To develop a training program that enhances learners’ reasoning abilities and allows them to formulate situation-appropriate handling practices that emancipate the professional and respect the patient’s autonomy and well-being.

Method: A training program targeting nurses and nursing students was designed and implemented. Learners were requested to design the simulation scenarios themselves. The training sessions were recorded and analyzed.

Results: The scenarios designed by learners were pedagogically and subjectively relevant. Interactions during the different phases—design, simulation, and debriefing—allowed learners to assess acceptable and adapted solutions and to discuss their own practice, considering patient handling as a care activity in its own right.

Conclusion: The training program appears as an efficient alternative to classic training programs. It develops learners’ reasoning abilities and restores meaning to nursing work.

Introduction : Life expectancy in psychiatric patients is reduced by 15 to 20 years due to the presence of comorbidities and the lifestyle of these patients. Metabolic syndrome, also a side effect of antipsychotic drugs, is an aggravating factor of this phenomenon, which is why health promotion is essential. The objective of this study is to describe the knowledge, attitudes, and practices of psychiatric nurses with regard to metabolic syndrome, using N. Pender's health promotion model as a framework.Methodology : A multicenter descriptive study using a « knowledge, attitude, practice » survey was conducted in 16 psychiatric departments in the Ile-de-France region. 70 nurses were interviewed via online questionnaires.Results : The results show partial knowledge. A gap exists between attitudes and nursing practices. The nurses' actions mainly involve counseling. The type of nursing knowledge is discussed, as well as the need for reflection on the nurse-patient relationship. Nursing interventions need to be further developed to better manage metabolic syndrome.Conclusion : Training is needed to carry out preventive measures and promote health more effectively.

Environmental diseases are increasing globally and the sources of contamination are multiple: indoor and outdoor air, food, cosmetics, drinking and irrigation water.Endocrine-disrupting chemicals (EDCs) are among the causes of the increasing presence of chronic diseases. There are periods of vulnerability—in particular during pregnancy, childhood, and puberty—when these substances can modify the functioning and establishment of major functions such as brain development, the various endocrine systems, the gonads, and the regulation of fatty tissue.Recent studies show the general population's widespread exposure to endocrine disruptors, either persistent (accumulating over time) or non-persistent (but with permanent exposure). However, the consideration of EDCs in clinical practice remains marginal, even though doing so could reduce the risk of diseases linked to environmental factors and positively influence the natural evolution of a chronic disease when it is diagnosed. The aim of this article is to clarify the concepts associated with the exposome and to list the main scientific data attesting to the effects of endocrine disruptors on health in order to draw up clinical, teaching, and research perspectives.

Personal resources used by nurses enrolled in an advanced practice curriculum during the COVID-19 pandemic in France: Sequential mixed research 法国COVID-19大流行期间参加高级实践课程的护士使用的个人资源:顺序混合研究

Introduction: At the beginning of 2020, a public health emergency was declared in France following the emergence of the SARS-CoV-2 pandemic.

Context: Nurses involved in an advanced practice mobilized their specific skills during this health crisis by drawing on the available resources.

Objectives: To analyze personal resources used by those nurses in the process of skill mobilization during the pandemic.

Method: Sequential mixed research, first quantitative and then qualitative, concerning nurses or students enrolled in an advanced practice curriculum.

Results: Our analysis highlighted two groups of activity for those nurses: frontline workers against COVID-19 or coordination. Coordination seemed to present more opportunities to use the specific skills of advance practice nursing.

Discussion: Adjustment strategies for stress (coping and hardiness) boost nurses’ commitment to their professional practice. Disciplinary knowledge contributes to the enrichment of the knowledge necessary for the mobilization of skills.

Conclusion: Future research should explore resources and the mobilization of skills in the implementation of advance practice nursing.

Bereaved parents’ experience of support groups following perinatal bereavement: a thematic synthesis 丧亲父母在围产期丧亲后参加支助小组的经验:专题综合

Support groups play an essential role in caring for parents experiencing perinatal death and bereavement. However, there is very little data on the effectiveness of these services on their well-being. To better understand the experience of bereaved parents using support group services, a qualitative systematic review was conducted using Thomas and Harden’s method. Seven databases (MEDLINE, CINAHL, Scopus, PsycARTICLES, Cairn, Érudit, and Repère) were searched in October 2017 with an update in June 2021. A total of 16 studies were included and critically evaluated. The thematic synthesis made it possible to generate four analytical themes to describe parents’ experience according to the notion of spaces. The first theme refers to external space, i.e., the format of the support groups (virtual or face-to-face), and the other three themes reflect internal spaces, i.e., the functions of the groups (support, expression, and learning). This thematic synthesis confirms that participating in support groups benefits bereaved parents. It also offers possible solutions to improve the structure and accessibility of support groups.
