Metabolic surgery in patients with diabesity: A review of the historical backgrounds and scoring systems 肥胖症患者的代谢手术:历史背景和评分系统回顾

Diabesity is a modern term that describes the coexistence of adverse health effects of diabetes mellitus and obesity and indicates a causal pathophysiological relationship between the two phenomena. The progression of diabesity leads to a deterioration of multiple organs and systems. Effective intervention for patients with diabesity must include optimal obesity therapy to prevent secondary complications. Metabolic surgery is the most effective and sustainable therapy for severe obesity and the elimination or prevention of many associated diseases, including type 2 diabetes mellitus, hypertension, sleep apnea, heart disease, and certain cancers. This review provides an up-to-date overview of surgical interventions for obesity, particularly the development of metabolic surgery. It evaluates different scoring systems for evidence-based selection of metabolic surgery based on disease severity. We reviewed different predictive scoring systems for better evidence-based selection of the best metabolic surgery for patients with diabesity. We found that medication type, fasting insulin level, and C-peptide influence the outcomes of different types of metabolic surgery and heterogeneous remission rates. There are different predictive scoring systems for evidence-based selection of the best metabolic surgery, either sleeve or mini-bypass, that will ensure the highest chance of diabetes remission. Using the metabolic score calculator is a useful tool to help medical specialists determine the optimal treatment strategy for a particular patient. More research is needed before we can agree on the ideal bariatric procedure that offers the highest chance of remission with the lowest incidence of hypoglycemia.

The nonadherence to prescriptions among type 2 diabetes patients, and its determining factors 2 型糖尿病患者不遵从处方的情况及其决定因素

Aim: The aim is to clarify the prevalence of nonadherence to antidiabetic therapies among type 2 diabetes mellitus (T2DM) patients and identify its causes.Methods: A three-part questionnaire (general background, Knowledge-Attitude-Practice section, and non-adherence reasons) was developed. In total 324 diabetic patients were surveyed via telephone. The evaluation of patient adherence included both direct questions on adherence and a summary of patient responses to non-adherence comments. The analysis was performed on StataCorp Stata 14.2, and included descriptive analysis, simple and multivariate logistic regression.Findings: Among the sociodemographic variables, age group, work level, and alcohol consumption may influence medication adherence. The comorbidity status of patients was also of relevance. Both variables had stronger relationships with adherence to anti-diabetic treatments compared to those with no comorbidities or no additional medicines. A strength of the study is that it addresses various medical diseases and attitudes about them, as well as a wide range of causes for non-adherence to non-diabetic medications.

Antitumor evaluation of amaryllidaceae alkaloids on cancer cell lines: A literature review 金盏花科生物碱对癌细胞株的抗肿瘤评价:文献综述

Amaryllidaceae alkaloids have attracted significant attention in cancer research for their potential antitumor properties. To date, numerous studies have investigated the effects of these alkaloids on cancer cell lines, but a comprehensive synthesis of the existing evidence through a literature review is warranted. This article presents a comprehensive literature review aimed at evaluating the antitumor effects of amaryllidaceae alkaloids on various cancer cell lines and elucidating the underlying mechanisms of action. A search was conducted in major scientific databases, including Google Scholar, PubMed, Scopus, and Web of Science, to identify relevant studies published between 2013 and 2023. Studies that examined the antitumor effects of amaryllidaceae alkaloids on cancer cell lines were included in the review. Data were extracted, analyzed, and synthesized to provide an in-depth overview of the findings. The alkaloids exhibited potent cytotoxicity and suppressed cell proliferation in a concentration-dependent manner. Mechanistic investigations unveiled that amaryllidaceae alkaloids induced apoptosis, cell cycle arrest, and hindered angiogenesis in cancer cells. Moreover, they demonstrated favorable selectivity towards cancer cells, sparing normal cells, suggesting potential as targeted therapies. This literature review consolidates and critically appraises the available evidence on the antitumor effects of amaryllidaceae alkaloids on cancer cell lines. The findings underscore their potential as promising candidates for further exploration in cancer therapeutics. Future preclinical and clinical studies are vital to advance their development and translation into effective anticancer drugs.

Complete pachydermoperiostosis: A case report 完全性柏氏畸形:病例报告

Pachydermoperiostosis (PDP), also known as Touraine-Solente-Gole syndrome or primary hypertrophic osteoarthropathy, is a very rare disease mainly characterized by pachyderma, finger clubbing, hyperhidrosis, and periostosis. We reported a case of a patient who suffered from gradual pain and enlargement of both hands and feet for 25 years despite receiving treatment from multiple clinics. He also experienced gradual abnormal changes in his face and scalp. Radiographic images of the lower limbs revealed the presence of subperiosteal bone growth and periosteal hypertrophy. The diagnosis of complete PDP was made late due to its rarity and the unfamiliarity of medical practitioners with the diagnosis. This case highlighted the need for medical practitioners to be aware of rare diseases so that patients may be diagnosed and treated earlier and thereby relieving their anxiety and improve their quality of life.

Outcomes of laparoscopic pyeloplasty and impact of an enhanced recovery protocol 腹腔镜肾盂成形术的效果和强化恢复方案的影响

Purpose: The study aims to analyze the enhanced recovery protocol’s (ERP) effectiveness in a comparative study of elective surgeries for ureteropelvic junction obstruction (UPJO). Methods: The prospective study included 30 patients with UPJO who underwent laparoscopic pyeloplasty in 2018-2021. Results: Postoperative complications developed rarely, and their frequency and severity were comparable. Independent predictors of UPJO recurrence were the spine osteochondrosis >II period (HR 13.97; 95% CI 1.26; 154.8; p=0.032), the concretions self-discharge (HR 28.49; 95% CI 1.78; 455.62; p=0.018), surgical operation duration > 110 minutes (HR 44.7; 95% CI 3.95; 505.4; p=0.002) and previous nephrostomy (HR 1.07; 95% CI 1.02; 1.13; p=0.002). Conclusions: In the surgical treatment of UPJO, it is advisable to use ERPs, as this allows achieving a better treatment quality with comparable results.

What factors shape public knowledge, attitudes, and practices of corticosteroid use in the UAE during the COVID-19 pandemic? 在 COVID-19 大流行期间,阿联酋公众对使用皮质类固醇的认识、态度和做法受哪些因素影响?

Background: Corticosteroid therapy has been associated with detrimental consequences such as cardiovascular problems and immunosuppression. Aims & objectives: This study aimed to assess the public knowledge, attitudes, and practice surrounding the use of corticosteroids in the United Arab Emirates (UAE) during the COVID-19 pandemic. Methods: This was a cross-sectional study carried out among the general population of the UAE from March to July 2022. A self-administered online survey was used, which included questions about participants’ socio-demographics, health status, and knowledge regarding corticosteroids. Eligibility criteria included participants aged>18 years or older and could read and respond to the questionnaire. Results: The study involved 583 participants, where the majority were mostly males living in urban areas. Nearly half had a bachelor’s degree, and 36.9% worked in the health sector. Around 55.0% of the participants had a high level of knowledge about corticosteroids, while more than half had a negative attitude towards them. The most frequently reported reason for using corticosteroids was dermatological diseases. Males with lower education levels, and who had not been infected with COVID-19 had lower odds of having a high level of knowledge about corticosteroids. Furthermore, older participants and those with a lower education level had a more negative attitude towards corticosteroids. Conclusions: This study showed gaps in knowledge and unfavorable attitude toward use of corticosteroids. Aims & objectives: This study aimed to assess the public knowledge, attitudes, and practice surrounding the use of corticosteroids in the United Arab Emirates (UAE) during the COVID-19 pandemic. Methods: This was a cross-sectional study carried out among the general population of the UAE from March to July 2022. A self-administered online survey was used, which included questions about participants’ socio-demographics, health status, and knowledge regarding corticosteroids. Eligibility criteria included participants aged>18 years or older and could read and respond to the questionnaire. Results: The study involved 583 participants, where the majority were mostly males living in urban areas. Nearly half had a bachelor’s degree, and 36.9% worked in the health sector. Around 55.0% of the participants had a high level of knowledge about corticosteroids, while more than half had a negative attitude towards them. The most frequently reported reason for using corticosteroids was dermatological diseases. Males with lower education levels, and who had not been infected with COVID-19 had lower odds of having a high level of knowledge about corticosteroids. Furthermore, older participants and those with a lower education level had a more negative attitude towards corticosteroids. Conclusions: This study showed gaps in knowledge and unfavorable attitude toward use of corticosteroids. Purpose: The study aims to evaluate precisely influence of temperature on the acoustical and physical properties of blood mimicking fluid (BMF) phantom made of polymer material. Materials & methods: Propylene glycol (PG) and polyethylene glycol (PEG) were fabricated to create artificial blood phantom. Acoustic (speed and attenuation) and physical properties (density and viscosity) of BMF were tested at specific temperatures of range 22 °C, 25 °C, 35 °C, 36 °C, and 37 °C using ultrasound signals. Results: At center frequency of 5 MHz, pulse-echo allows to acoustical (speed and attenuation) and physical (density and viscosity) values to be linearly decreased with increasing of temperature. The difference in values of speed, attenuation, density, and viscosity was nuance (±0.01, ±0.001, ±0.005, and ±0.151), respectively. Conclusions: Temperature test of range 22Co-37Co have nuance influence (±0.01, ±0.001, ±0.005, and ±0.151) on acoustical and physical properties when BMF phantom of special material of PG and PEG was used. This nuance influence may be taken in account in quality assurance of ultrasound imaging system.

Effectiveness of magnetic therapy versus exercise in elderly patients with chronic mechanical neck pain: A randomized clinical trial 磁疗与运动疗法对老年慢性机械性颈痛患者的疗效对比:随机临床试验

Background: Mechanical neck pain (MNP) affects all age groups, it has a substantial physical, psychological, and economic impact, especially in elderly. This study aimed to evaluate effect of addition of magnetic therapy (MT) to exercise versus exercise therapy alone on neck pain (NP), proprioception, and balance in elderly patients with chronic MNP. Materials & Methods: 50 elderly patients with chronic MNP were assigned to magnetic group (25 patients who practiced exercise program for 35-40 minutes/session three sessions per week for four weeks in addition exposure to MT for 20 minutes each session three sessions per week for four weeks) and control group (25 patients who practiced only the exercise program). The severity of pain, cervical joint position sense (JPS) and balance performance were measured by using the visual analogue scale (VAS) goniometer and the both single-leg stance time (SLST), and timed up and go (TUG) tests, respectively. ANOVA and Independent t-tests were used to determine significant differences within and between two groups for normal variables, whereas the Mann-Whitney and Kruskal-Wallis tests were used for not normal variables. Results: The results show significant improvements in VAS, SLST (with open and closed eyes), TUG, and JPS after interventions in both groups, p-value <0.05, while addition of MT to exercise program induced only significant differences in VAS, SLST (with open & closed eyes), TUG, except JPS, p-value = 0.002, 0.047 & 0.037, 0.001, and 0.928 respectively, in favor to the magnetic group. Conclusions: Multi-dimensional exercise program is effective in relieving NP and improving balance in elderly with MNP, addition of MT to exercise program could induce superior effects only on pain and balance for elderly patients with MNP.

Background: One of the greatest difficulties of the hearing impaired is understanding speech, especially in the presence of competitive noise. Speech perception issues are a common difficulty for those who have hearing loss. Poor speech perception is the most frequent cause of seeking hearing treatment. The most extensively used treatment to enhance speech perception is hearing aids or other assistive devices. Aim: To evaluate the effect of hearing aid methods on the speech perception of patients with hearing loss. Methods: We conducted our study based on PRISMA recommendations and guidelines. We searched Cochrane, PubMed, Scopus, and Web of Science for relevant clinical trials and prospective observational studies. Clinical trials were assessed according to Cochrane’s risk of bias tool, while observational studies were evaluated according to the National Heart, Lung, and Blood Institute. We involved the following outcomes: speech recognition thresholds, word recognition score (WRS) in noise, and WRS in quiet. Results: Six studies met the eligibility criteria for our meta-analysis. We found that hearing aids can cause an increase in WRS in noise (mean difference [MD]=18.32 [3.08, 33.55], p=0.02) and WRS in quiet (MD=45.13 [7.79, 82.46], p=0.02). In addition, hearing aids lead to a decrease in speech recognition thresholds (MD=45.13 [7.79, 82.46], p=0.02). Conclusions: Hearing aids are an effective treatment for patients with hearing loss. They can significantly improve speech perception (WRS) in quiet and noisy environments and provide other benefits, such as improved communication, increased safety, and better cognitive function.
