Does motivation for practising mindfulness moderate the effects of mindfulness training on stressed adults? 练习正念的动机会减缓正念训练对压力过大的成年人的影响吗?

Background

Mindfulness-based intervention (MBI) has been associated with positive health outcomes in adults; however, there are important gaps in our knowledge of factors that influence which individuals are more likely to engage with and benefit from these interventions. Research on other types of behavioural interventions suggests that motivation changes over the course of intervention and may moderate attendance and therapeutic outcomes. This study investigated changes in motivation for practising mindfulness across an MBI and associations between baseline motivation and outcomes, to gain a preliminary understanding of the role of motivation in MBI adherence and outcomes.

Methods

In a sample of adults (N = 80) who received an MBI, we characterised change in motivation from pre- to postintervention and tested whether baseline motivation predicted change in attendance and outcomes.

Results

We found that some, but not all, dimensions of motivation for practising mindfulness increased and also were associated with attendance and pre–post change stress and emotion regulation.

Conclusions

Our findings suggest that there may be potential for optimising MBI through attention to individual differences in motivation and change in motivation over time. The findings justify research with larger sample sizes, control groups and repeated measurements to test the temporal ordering of changes in these outcomes.

Objective

This study aimed to explore clients' lived experiences of being deeply moved or touched by something that the therapist said or did and to investigate to what extent this relates to the phenomenon of Kama Muta (being moved by love). Kama Muta is described as a social-relational emotion, which seems to play a central role in connecting to others and is tied to emotional processes. It appears that research on Kama Muta is still in its infancy; while it has been explored and analysed within the context of social psychology and nature, there has been scarce analysis and research to date on the specific implications of Kama Muta for therapy; however, it seems to represent an appropriate way to think about the small things phenomenon proposed for this research. The purpose of this research was to explore whether ‘being moved by the “small things”’ in therapy matters to clients and what impact it has on the processes of change. It aimed at advancing our comprehension about which factors, outside the typical therapeutic treatment, may help strengthen the therapist–client relationship. Therefore, the author explores how clients are ‘moved’ in therapy by the small things, whether those experiences can be classified as Kama Muta and the implications of this for the therapeutic field.

Method

Seven participants, five students in counselling and psychotherapy and two fully accredited psychotherapists, took part in qualitative semi-structured interviews about their experiences as clients of being deeply moved or touched during therapy. Interview transcripts were subjected to thematic analysis. For reasons of ethics approval, the author selected participants who either were on a path to become therapists or were fully accredited. The interviewer asked participants to recall events retrospectively; thus, the interviewees were not clients who were currently undergoing treatment. The author used scripts to open and close the interviews, thus sharing all relevant information about the study with the interviewees, including informed consent and confidentiality. The same script with open-ended questions was used with all seven participants. All participants were informed of the purpose of the research beforehand and had the opportunity to ask questions and share additional thoughts throughout the process. The interviews were recorded using a digital voice recorder and transcribed using a transcribing software.

Results

Coding schemes were inductively developed exclusively from the data gathered from the interviews. The author maintained a rigorous reflexivity practice throughout, and data were analysed and valida

A pilot RCT of an online mindfulness-based cognitive intervention for chemsex 基于正念的化性在线认知干预试验性研究

Objectives

Chemsex is a term used to describe sex acts using psychoactive substances to enhance the sexual experience/arousal and predominates among men who have sex with men (MSM). Chemsex drugs predominantly include γ-hydroxybutyric acid (GHB) and analogues, methamphetamine, mephedrone and erectile dysfunction agents (e.g., Sildenafil). Mindfulness-based cognitive therapy (MBCT) combines cognitive therapy and mindfulness and has been shown to help support both sexual functioning and methamphetamine use. However, limited clinical trials have evaluated the use of cognitive mindfulness for chemsex in MSM. The present study assessed the effectiveness of an online mindfulness-based cognitive intervention (MBCI) among 29 MSM aged 18–30 years engaged in chemsex.

Design and Setting

A mixed methods approach consisted of a randomised waitlist-controlled trial and a content analysis reviewing participants' feedback on MBCI. The design of the intervention was based on a behavioural taxonomy mapped to selected behaviour change techniques (BCTs) to support the reliability of MBCI in its delivery.

Main Outcome Measures

Participants completed assessments measuring levels of chemsex, mindfulness, sexual self-efficacy and well-being, taken at Weeks 0, 8 and 12 (follow-up).

Results

Participants reported lower levels of chemsex and higher levels of cognitive mindfulness, sexual self-efficacy and well-being postintervention and at the 12-week follow-up. The content analysis feedback responses yielded favourable outcomes, suggesting that participants developed a sense of self-compassion, confidence and positive identity.

Conclusions

The MBCI effectively reduced chemsex; however, this study warrants replication with a larger, more diverse group of participants.

Background

Marital distress is a risk factor for a panoply of mental and physical health disorders. One of the causes of marital distress that is increasingly gaining relevance is attachment injuries in the marital relationship. Emerging from the confluence of adult attachment theory and emotionally focussed couples therapy, attachment injuries refer to tears in the relationship bond caused by abandonment or betrayal in the marriage, especially at times of critical need when attachment needs are salient.

Objective

The objective of this study was to investigate how therapists in India conceptualise and assess attachment injuries in marital relationships.

Method

A qualitative design was used, whereby in-depth interviews were conducted with 13 therapists and the transcripts were analysed using thematic analysis.

Results

The analysis resulted in two overarching themes. The first was ‘Attachment Injuries are a Common Phenomenon in Couples undergoing Marital Distress’, which included themes related to markers of attachment injuries, injurious events, the gendered nature of attachment injury experiences and the impact of attachment injuries. The second overarching theme was ‘Therapists Utilize Diverse Methods for the Crucial Process of Identification of Attachment Injuries’, which included themes related to the methods used for assessment, the significance of attachment lens, the role of early experiences and the utility of identifying attachment injuries.

Conclusion

The results highlighted how attachment injuries, originating from various injurious events, are often a component of marital distress in couples accessing therapy. The findings also pointed towards the need for therapists to develop competency in assessing and addressing attachment injuries, irrespective of their primary therapy orientation.

Background

Almost 2 million people in the UK live with sight loss, and this number is predicted to double by 2050. There is increasing recognition of the need to provide counselling services to people living with sight loss.

Aim

The aim of this research was to assess whether there is an unmet need for counselling services for people living with sight loss in North Wales and to estimate the social value and budget impact of the potential roll-out of counselling services to this population.

Method

Between December 2020 and May 2021, a telephone survey was completed by 192 people living with sight loss in North Wales, UK. The survey instrument was a 19-item questionnaire, which included the Short Warwick–Edinburgh Mental Well-being Scale (SWEMWBS). Using data from the SWEMWBS and published statistics, a social return on investment (SROI) and budget impact analysis were conducted.

Results

Most respondents (84%, n = 161) had never been offered counselling in relation to sight loss. Thirty-nine per cent (n = 74) stated that they would use a new counselling service if it were available. Fifty-one per cent (n = 97) indicated that counselling could be especially beneficial for people who have recently been registered or diagnosed with sight loss (the timely window). Of those who completed the SWEMWBS, approximately 25% reported low mental well-being or probable depression. This commentary offers a critical reflection on the current status of evidence-based psychological treatments for psychological distress in dementia, with a particular emphasis on acceptance and commitment therapy (ACT).

Purpose

The paper hypothesises several explanations, such as ethical dilemmas with recruitment and consent, and possible preconceptions surrounding the nature, scope and likelihood of therapeutic change in this population, for a lack of interventional research on people with dementia.

Implications

We highlight some recent developments in psychological therapy with people with dementia that are raising more questions and generating new outcome research.

Objectives

There is evidence of a significant role for therapeutic alliance (TA) in psycho-oncology care. This review aims to provide an overview by systematically identifying key characteristics of TA in this setting and exploring related concepts.

Methods

PubMed, PsycInfo and Web of Science were systematically searched for empirical studies on adult cancer patients receiving psycho-oncological care considering TA. Quality assessment was undertaken using the Mixed Methods Appraisal Tool. A narrative synthesis was conducted.

Results

Of the 4647 identified publications, 24 quantitative, qualitative and mixed-methods studies were included. Characteristics of studies, interventions and samples were heterogenic. Patients consistently reported high TA scores. Favourable associations of TA with distress and well-being and mixed results for further outcomes and compliance were detected. Associations between the patient's and therapist's characteristics yielded mixed results. Qualitative evidence pointed to a unique nature of TA in psycho-oncology, for example, patients experiencing it in the context of other patient–provider relationships occurring in the context of the disease.

Conclusion

The review displays a unique synopsis of a heterogenic, quickly developing research field. First hints towards the efficacy and particularity of TA in psycho-oncology are delivered. Results encourage further focus on TA, both in psycho-oncological research and in clinical practice.

Objective

This study aimed to investigate the interest in online and in-person psychological support services in Italy during the COVID-19 pandemic, stimulated by the implementation of the government-issued ‘bonus psicologo’ for psychotherapy sessions.

Methods and Measures

Public interest was measured using data from Google Trends, a resource that offers open access to actual search requests, within the time frame of 2 July 2017 to 13 November 2022. These data were stratified for Italian regions to explore regional differences. We conducted interrupted time series analyses to assess changes in interest before, during and after the lockdown.

Results

Results indicated a significant increase in interest in online psychological support during the lockdown, which was sustained over time and was positively correlated with the implementation of the ‘bonus psicologo’ (p < .001). Interest in in-person support decreased during the lockdown but began to rise post-lockdown (p = .028), with no notable impact from the economic incentive. High interest in online support continued in certain regions even 1 year post-lockdown, warranting further analysis.

Conclusion

The study underscores a heightened post-pandemic interest in psychological support, both online and in person, in comparison with pre-pandemic levels. Google Trends emerges as a valuable tool for monitoring public interest during specific events, though the findings must be interpreted with care, given the study's time frame and reliance on relative search volume as a measure. Future research is needed to validate these findings and to compare them against epidemiological data.

Background

The past two decades have seen expanding interest in the application of motivational interviewing (MI) with couples. Thus far, the literature detailing the development of MI with couples has focused primarily on elucidating counselling processes and techniques or on evaluating intervention effects. To date, no published studies have examined client perceptions of the counselling process in MI with couples.

Methods

This study involved qualitative analysis of 55 sessions from 24 couples (48 individuals) who received short (three-session) courses of MI with couples in the context of a randomised controlled trial. All participants identified as cisgender male and were 18 or older. In each couple, at least one partner was aged 18–29, HIV negative and reported drug use in the past 30 days.

Results

Analysis revealed that client couples attributed the benefits of MI with couples to expectancy-related effects (merely structuring time to talk) and common factors (the presence of the counsellor as a non-judgmental observer). Clients also conveyed appreciation for techniques and activities that enhanced dyadic functioning through communication skill building and the identification of shared values. Finally, clients appreciated the opportunity to discuss drug use and sexual behaviour in an integrated way.

Discussion

Findings indicated the utility of counsellor-delivered behavioural interventions. They also align with recent formulations of the counsellor's role in MI with couples and the introduction of facilitating dyadic functioning as a distinct process. Finally, they highlight the potential value of training counsellors to address the intersection of drug use, sexual risk and sexual pleasure in sessions. These dimensions of practice receive inconsistent attention across counselling training programme curricula.

Interaction between multisensory information and emotional activation in video-facilitated psychotherapy 视频辅助心理疗法中多感官信息与情绪激活之间的相互作用

Background

Emotional activation is regarded as central in psychotherapy. We have developed a method called VideoTalk, in which patients video themselves at home according to the therapist's instructions, and the videos are watched together in the therapy session. The aim of the study was to find out whether watching and listening to a video in psychotherapy readily activates the patient's emotions.

Methods

The video material was analysed by theory-based content analysis using the interacting cognitive subsystems (ICS) theory. The ICS theory suggests a link between sensory information and emotions and describes two levels of meaning: propositional and implicational. The implicational level gains information from the propositional level and directly from perceptions and is central to the activation of emotions. Five patients participated in our schema therapy-based intervention. Our material included videos of 10 therapy sessions, in which the patient and the therapist watched a video made by the patient at home in a state of helplessness. Watching the video in the session was performed in parts, and between watching periods, there were observation phases consisting of discussion about what the patient had seen and heard on the video and how it affected them. Our data included 38 observation phases, and in 35 of these, the patient verbalised an emotion after watching the video. The implicational level was involved in almost all observation phases in which the patient verbalised emotions.

Results

Our findings are in line with the hypothesis that added multisensory information via video enhances input to the implicational level and therefore emotional activation in psychotherapy. This is a possible mechanism by which the use of VideoTalk could facilitate the psychotherapy process.
