Extramammary Paget’s disease (EPD) is a rare tumor occurs mainly in areas with numerous apocrine and or eccrine sweat glands. We report a 49-year-old woman who had referred to another hospital with the complaint of an erythematous and pruritic plaque on the vulva for 6 months. An incisional biopsy had been taken and pathological result was reported as invasive SCC. In her past medical history, there was a history of total thyroidectomy due to follicular variant of papillary thyroid carcinoma two years ago. The patient had been received radioactive iodine for 3 times, however her Iodine isotope scans were still positive. Then she was visited at our hospital for definite treatment. According to the pathologic report she underwent vulvectomy with inguinal lymph node dissection. Histological examination revealed primary invasive Paget disease of the vulva and there was one inguinal lymph node involvement. Second vision of the initial incisional biopsy was also compatible with primary invasive Paget disease of the vulva. We ruled out the possibility of metastatic deposits of thyroid carcinoma in the vulvar lesion and inguinal lymh node by IHC stains. Accurate classification of Paget's disease is essential for appropriate treatment. IHC stains are necessary to differentiate it from other tumors. The possibility of associated malignancy should be considered. To the best of our knowledge this case is the first case of primary Paget disease of the vulva in association with thyroid carcinoma.

Background: Sickle Cell Anaemia (SCA) is an inherited haemoglobin disorder characterized by shortened red cell life span and altered red blood cell parameters due to the red cell sickling, vaso-occlusion and chronic haemolysis occasioned by the disease. The occurrence of bone pain vaso-occlusive crises may further alter these parameters; an occurrence that highlights on the pathophysiology of SCA and worth noting in the management of affected patients. Aims and Objectives: This study was performed to determine the effect of bone pain vaso-occlusive crises on red blood cell parameters of SCA patients. Study Design: A longitudinal study. Settings: Fifty SCA patients receiving care at a tertiary hospital in Sokoto, Nigeria were consecutively enrolled while in their steady clinical state and followed up till when they re-presented during the bone pain vaso-occlusive crises state. Materials and Methods: Structured proforma was used for clinical data capturing while red blood cell parameters (red blood cell count (RBC), haemoglobin (Hb) concentration, haematocrit (HCT), mean corpuscular volume (MCV), mean corpuscular haemoglobin (MCH), mean corpuscular haemoglobin concentration (MCHC) and red cell distribution width (RDW) were determined via automation using Nortek 3-part haematology analyser. Leishmann stained peripheral blood smears were examined to cross-check morphological details of the red blood cells. Statistics: Data analysis was performed using Statistical Package for the Social Sciences (SPSS) version 21.0 (IBM Corp, Armonk, NY, USA) and further summarized as means ± standard deviations. Comparison of means was performed using paired sample t-test and Anova as appropriate while Pearson’s test was used for correlation analysis. Results were presented in tables while statistical significance was set at p<0.05. Results: Fifty SCA patients were enrolled for the study and consisted of 26 (52%) females and 24 (48%) males. The overall mean age of the study participants was 22.82 ± 6.55 years with the minimum and maximum ages of 18 and 42 respectively. During the bone pain vaso-occlusive crises, 14 (28%), 21 (42%) and 15 (30%) of the patients had mild, moderate and severe bone pains respectively. The red blood cell parameters during the steady and bone pain crises states were RBC count (2.87±0.69 vs. 2.93±0.83; p=0.675); HCT (24.33±5.01 vs. .37±5.61; p=0.373); Hb concentration (8.34±1.53 vs. 7.99±1.54; p=0.228); MCV (87.87±9.13 vs. 82.59±14.64; p=0.021); MCH (29.76±5.61 vs. 30.48±13.54; p =0.729); MCHC (32.89±4.76 vs. 32.10±3.75; p=0.394) and RDW-CV (19.30±6.58 vs. 19.59±5.27; p=0.829) respectively. During bone pain vaso-occlusive crises, the red blood cell parameters based on mild, moderate and severe pains were RBC count (2.82±0.88 vs. 2.78±0.57 and 3.06±0.65; p=0.472); HCT (24.29±6.37 vs. 24.11±5.12 and 24.68±3.53; p=0.947); Hb concentration (8.46±2.01 vs. 8.33±1.57 and 8.24±0.92; p=0.927); MCV (87.60±7.46 vs. 91.03±9.21 and 83.69±9.22, p=0.055);

Objective: To study a rare case of Rhabdomyosarcoma. Methods: Observational descriptive study of a case, a 3-year-old female child, presenting to our OPD. She was thoroughly examined and radiological investigations (HRCT Temporal bone and MRI Brain) were done. The Neurosurgery department proceeded with a biopsy of the lesion by left temporal bone craniotomy through subtemporal approach. Results: Histopathology suggested a Rhabdomyosarcoma of anaplastic type. The patient then underwent 6 cycles of Chemoradiation. Conclusion: The awareness that Rhabdomyosarcoma of petrous apex can present as Gradenigo syndrome along with 7th nerve palsy is important for the early diagnosis and its treatment.

Objective- Although oxytocin is widely used in labor and childbirth, questions remain regarding the pharmacokinetics (PK), pharmacodynamics (PD) and dose of oxytocin in pregnant people as well as the potential effects on the fetus and newborn. The objective of the current study was to use published studies to investigate the PK/PD and dose of oxytocin in pregnancy, examine the placental transfer of oxytocin through the development of a physiologically-based PK (PBPK) model, and postulate whether the model predicts the effect(s) of oxytocin on fetuses and newborn infants. Methods- A literature review of articles related to oxytocin in the areas of PK/PD, placental transfer and its influence on newborns were reviewed and summarized. A PBPK model for oxytocin was developed to predict maternal-fetal transfer. Results- Previous studies have demonstrated a positive relationship between the plasma oxytocin concentration and uterine contractions during labor. An ex vivo study demonstrated maternal to fetal and fetal to maternal oxytocin transfer, but a clinical study investigating oxytocin concentrations in umbilical blood could not confirm these results. Previous studies have suggested that a high concentration of oxytocin is related to fetal bradycardia and possibly autism in the child. Conclusion- The current study developed a PBPK model to measure the placental transfer of oxytocin and its potential for adverse effects on the fetus and developing child. Further research is needed to determine oxytocin’s efficacy and safety in the maternal population and in their fetuses and developing children, and modeling techniques such as PBPK will contribute to that understanding.

Since the emergence of the novel coronavirus epidemic, online teaching has begun to be widely carried out in various universities, and has been widely recognized by teachers and students. This article discusses the online teaching method of histology and embryology in Zhongshan Medical College of Sun Yat-sen University, analyzes the advantages and disadvantages of online teaching, and proposes improvement measures for online teaching in order to provide reference for better online teaching in universities.

This article aims to explore the effect of the intangible cultural heritage co-creation model on the psychological healing of special children. Through the review and analysis of relevant literature, this paper believes that the intangible cultural heritage co-creation model has positive effects on the psychological healing of special children, and can help them relieve anxiety, enhance self-confidence, and improve social skills, etc. At the same time, this paper also analyzes the problems existing in the implementation of the intangible cultural heritage co-creation model, and proposes corresponding solutions.

Objective:The role of Eintraoperative joint organ removal or transit abdominal surgery recovery after surgery (ERAS) in reducing hospital stay time and perioperative hospitalization costs have been proven. However, most studies have been carried out in areas rich in medical resources. In this study, we evaluated the safety and efficacy of ERAS compared with traditional perioperative period care for laparoscopic cholecystectomy(LC) in areas poor in medical resources in China. Methods:A randomized controlled trial on laparoscopic cholecystectomy was conducted at The People's Hospital ofFengqing from July 2020 to March 2021. Patients were randomly divided into an ERAS group and a traditional care group. The main outcome was the length of hospital stay after surgery. The secondary outcomes included hospitalization costs and visual analogue scale (VAS) scores for postoperative pain, first exhaust time, and first semi-liquid diet time after surgery. The ERAS group was provided with perioperative education, nutritional support, preoperative 2 h oral carbohydrates, restricted rehydration, intraoperative insulation, no drainage tubes, wound hemp, early mobilization postoperative nausea and vomiting (PONV) prevention and multi-modal analgesia, early activity. Results: The study was conducted on a total of 140 patients with 70 patients each in the ERAS group and traditional care groups. In the ERAS group, Thelength of hospital stay after surgery was shorter (4.06 vs.4.61 days, P<0.05), the average hospitalization cost (CNY 7900 vs. 8470, P<0.05),and VAS score (3.10 vs. 3.57 points, compared to the eraser group P<0.001) was lower, the first exhaust time was shorter (12.2 vs. 14.2 hours, P<0.001),thefirst semi-liquid diet time after surgery was shorter(14.5 vs. 16.8 hours, P<0.001), and the incidence of major postoperative complications was lower (bile leakage, postoperative bleeding, bile duct damage) (0% vs. 12.9%, P<0.05). No significant difference in surgery time (57.8 vs. 54.6 min, P>0.05) was observed between the two groups. Conclusion: Even in areas poor in medical resources in China, the use of the ERAS concept for LC surgery can reduce the length of hospital stay, cut down the cost of hospitalization, and lessen the pain of patients after surgery and accelerate postoperative recovery.

The ERAS group was provided with perioperative education, nutritional support, preoperative 2 h oral carbohydrates, restricted rehydration, intraoperative insulation, no drainage tubes, wound hemp, early mobilization postoperative nausea and vomiting (PONV) prevention and multi-modal analgesia, early activity. Results: The study was conducted on a total of 140 patients with 70 patients each in the ERAS group and traditional care groups. In the ERAS group, Thelength of hospital stay after surgery was shorter (4.06 vs.4.61 days, P<0.05), the average hospitalization cost (CNY 7900 vs. 8470, P<0.05),and VAS score (3.10 vs. 3.57 points, compared to the eraser group P<0.001) was lower, the first exhaust time was shorter (12.2 vs. 14.2 hours, P<0.001),thefirst semi-liquid diet time after surgery was shorter(14.5 vs. 16.8 hours, P<0.001), and the incidence of major postoperative complications was lower (bile leakage, postoperative bleeding, bile duct damage) (0% vs. 12.9%, P<0.05). No significant difference in surgery time (57.8 vs. 54.6 min, P>0.05) was observed between the two groups. To give full play to lab potential and fulfill the unique feature of training talents for valuing practice and seeking innovation, an intelligent, efficient, and open information management platform has been built in the experimental teaching center of life sciences in Northwestern Polytechnical University. This study constructs an intelligent, efficient and open source information management platform. The platform is composed of five modules: comprehensive lab management system, instrument and equipment management system, reagents and consumables management system, laboratory opening platform, and laboratory teaching management system. The use of the management platform has solved the problems of miscellaneous teaching data, insufficient sharing of teaching resources, conflict of course arrangement and low lab open utilization under the traditional management mode. It also stimulates the enthusiasm and initiative of students in innovation and practice, perfects the management mechanism of making teachers play a leading role in experimental teaching, and improves the work efficiency of laboratory managers.

Objective: This study sought to examine whether the inclusion of self-efficacy theory could increase the effectiveness of awake prone positioning in COVID-19 patients. Method: A total of 80 COVID-19 patients admitted to the respiratory department of our hospital between December 2022 and February 2023 were randomly allocated into either the intervention group (n=40) or the control group (n=40). The control group received routine respiratory medicine treatment and underwent awake prone positioning, whereas the intervention group received additional self-efficacy theory intervention alongside awake prone positioning. The patients’ oxygen saturation level (SpO2) and respiratory rate (RR) were closely monitored at three different time points: before awake prone positioning, 30 minutes and 1 hour after awake prone positioning. Furthermore, the patients’ Modified Medical Research Council (mMRC) dyspnea scale and self-efficacy scores were evaluated on the first, second, and third days of awake prone positioning. The cumulative duration of awake prone positioning and patient satisfaction were recorded on the third day of the intervention. Results: The data indicate that the intervention group demonstrated significant improvements in both SpO2 and RR compared to the control group, both at 1 hour and at the end of awake prone positioning (P<0.05). Moreover, the intervention group experienced a substantial reduction in the dyspnea index compared to the control group on the second and third day (P<0.05). The intervention group also reported higher self-efficacy scores, cumulative duration of awake prone positioning ventilation on the third day, and satisfaction levels compared to the control group on the first, second, and third day (P<0.05). Conclusions: The incorporation of self-efficacy theory to awake prone positioning improves the short-term outcomes of COVID-19 patients. This patient-centered approach has the potential to increase patients’ self-efficacy and satisfaction levels, emphasizing the importance of incorporating patient-centered care in healthcare practices.
