: Background: Breast cancer (BC) is the most common malignant tumor in females worldwide. It is categorized into four main molecular subtypes. Ubiquitin-conjugating enzyme (UBE2C) is essential for the ubiquitin–proteasome system which regulates checkpoints in the cell cycle. Dysregulation of ubiquitination has been associated with different types of cancer. P53 is important tumor suppressor gene. In some cancers mutant p53 proteins not only lose tumor suppressive functions, but also acquire oncogenic activity. Aim: This study aims at evaluation of UBE2C and P53 expression in molecular subtypes of breast cancer and assessment of their role in pathogenesis and tumor progression.

: Background: Vitiligo is a depigmenting skin condition due to the attenuation of melanin in the affected skin area due to specific melanocyte loss. The pathophysiology of vitiligo has been linked to both mitochondrial malfunction and oxidative stress. Dynamin-related protein 1 (DRP1) is a regulator of mitochondrial fission. Uncoupling Protein 2 (UCP2) is an essential mitochondrial protein that regulates cellular energy metabolism. Objectives: This study aimed to assess the gene expression levels of UCP2 and DRP1 in vitiligo patients and correlate those levels with the activity and severity of the disease. Methods: This case-control study included 40 vitiligo patients and 40 age and sex matched healthy controls. Gene expression levels of UCP2 and Drp1 were measured using an enzyme-linked immunosorbent assay. Results: The vitiligo group had significantly higher serum UCP2 and Drp1 gene expression levels (p<0.001 for each) When compared to the control group. UCP2 gene expression showed a significant negative correlation with VIDA score (p<0.001). Drp1 gene expression showed significant negative correlations with age and duration of vitiligo (p=0.002, 0.001 respectively) and a positive correlation with VIDA score (p<0.001). Conclusions: The findings suggest the potential role of dysregulation of Drp1 and UCP2 gene expression in vitiligo. For patients with vitiligo, both markers showed prognostic and diagnostic values.

: Background: Postoperative pain can seriously reduce the quality of life in patients, and acute pain can even trigger chronic pain syndrome. We aimed to compare efficacy of adding PECS block under general anesthesia with preoperative oral multimodal Analgesia (MMA) or alone on postoperative opioid consumption in subjects undergoing breast surgery. Methods: This prospective double-blinded randomized controlled study was conducted on 120 adult women underwent breast surgeries. Patients were randomly allocated into 3 equal groups; group I, only MMA was administered, group II; both PECS and MMA were administered and group III only PECS was administered. All patients were evaluated full clinical assessment and laboratory investigations. In group I, preoperative MMA with oral acetaminophen 975 mg and/or gabapentin 600 mg as preventive analgesia was given within 2 hours prior to surgery. Results: There was an insignificant difference among the studied groups regarding the VAS at all time measurement and perioperative opioid consumption. The incidence of PONV and the need for PONV rescue medication were significantly different among the studied group, being less prevalent in group C followed by group B (P=0.001, 0.001). Conclusions: We found comparable results in the postoperative VAS score and perioperative opioid consumption suggesting that the use of PECS block combined with MMA may not reduce intraoperative and/or postoperative opioid consumption in subjects undergoing elective breast surgery. However, the incidence of PONV was less prevalent in PECS group followed by PECS block combined with MMA.

: Background: The incidence of diabetes mellitus is rapidly increasing, and it frequently causes considerable metabolic disorders and serious consequences. MicroRNA-200b (miR-200b) is a regulator of angiogenesis that has emerged as a diagnostic and predictive tool for certain disorders. Aim: to investigate the association of miR-200b with diabetic retinopathy (DR). Methods: The study included 100 subjects from Benha University Hospital: 40 patients with type 2 diabetes (T2D) without DR, 40 with T2D and DR, and 20 apparently healthy controls. MicroRNA-200b was assessed for all subjects using real time PCR with SYBR Green. Results: Diabetics had lower levels of miR-200b than healthy controls. While, those without DR had even lower miR-200b levels compared to those who did have DR. Additionally, lower miR-200b levels were associated with neuropathy but not with nephropathy. Conclusion: MiR-200b showed promise as a biomarker for both T2D and DR. It perfectly differentiated diabetic patients with and without retinopathy, while offering moderate accuracy for diagnosing diabetes in general. These findings suggest miR-200b may play a risky role in DR among diabetics, and further research is warranted to confirm these results.

: Background: Dislocation after revision total hip arthroplasty (THA) continues to be one of the most common and concerning complications after the procedure. Dual-mobility (DM) acetabular components decrease the risk of post-operative instability also in high-risk patients, both in primary and revision hip arthroplasty. This study aimed to evaluate the outcome of DM acetabular cup (DMC) for instability in THA. Methods: This prospective study included 20 patients with Hip abductor insufficiency underwent total hip replacement using dual mobility cup. Complete local examination of the involved hip joint and radiological evaluation to ensure precise templating The Harris hip score (HHS) is used for clinical evaluation of patients at 6 weeks, 3months, 6 months, and the last follow-up. Standard radiographs are taken for all patients at subsequent follow-up examinations to examine component position or migration, osteolysis and loosening, and union of transfemoral osteotomy if used. Results: Regarding the prothesis, Cementless cup-Cementless stem was used in 3 (15.0%) patients, Cemented cup-Cementless stem was used in 9 (45.0%) patients and Cemented cup-Cemented stem was used in 8 (40.0%) patients. Harris hip score is improved over time postoperatively. HHS was significantly higher after 6 wks., after 6 months and after 1 years compared to preoperative HHS. Conclusion: the use of DMC has been found to be an effective technique for preventing hip arthroplasty instability. The clinical outcomes achieved with DMC were quite satisfactory, and the incidence of complications was significantly lower compared to other techniques.

: Background: Deficiency of vitamin D has become a global public health problem, with nearly 1 billion people worldwide being in a state of vitamin D insufficiency or deficiency. The aim of this work was to evaluate the role of hypovitaminosis D in diabetes-related anemia of chronic disease and value of its replacement in response to therapy. Methods: This randomized control trial included patients with diabetes-related anemia of chronic disease), the least number is 318 patients. The three groups were randomized equally; group 1 was with diabetes-related anemia of chronic disease) with normal 25 hydroxy vitamin D and received treatment for anemia, group 2 was with diabetes-related anemia of chronic disease with low level of 25 hydroxy vitamin D, and group 3 was with diabetes, but no anemia then follows up for hemoglobin concentration. Those Patients were informed about using vitamin D in treatment (benefits and side effects) in addition to treatment of specific anemia, then we assessed anemia after three months. Results: The study found no significant difference in age, sex, or type of diabetes mellitus (DM) between groups. However, individuals with anemia, particularly those with low vitamin D levels, had lower serum iron and TIBC levels and higher ferritin levels. Vitamin D levels were negatively correlated with ferritin and positively correlated with iron and TIBC. HbA1C levels had negative correlations with iron, TIBC, and WBCs. Conclusion: The current study suggested that there was close relationship between vitamin D deficiency and anemia in diabetic patients.

Background: Understanding the implications of maternal oxygenation during elective cesarean sections with spinal anesthesia on neonatal outcomes is crucial for optimizing perinatal care. This study aimed to determine the effect of maternal oxygenation on neonatal outcome in full term elective cesarean section under spinal anesthesia. Methods: This randomized controlled trial, conducted from June to December 2022 at Benha University Hospital, included pregnant patients beyond 37 weeks of gestation undergoing elective cesarean section. Participants were divided into two groups: Group A received oxygen supplementation, while Group B did not. Various assessments, including maternal and neonatal parameters, were conducted. Results : Group A exhibited significantly higher maternal oxygen levels (100 ± 0 vs. 95 ± 1, P < 0.001). Neonates in Group A demonstrated lower umbilical vein carbon dioxide (35.3 ± 6.4 vs. 39.4 ± 7.8, P = 0.013) and bicarbonate levels (19.3 ± 2.7 vs. 21.6 ± 5.3, P = 0.019) but higher oxygen levels (34 ± 6 vs. 22 ± 4, P < 0.001). Apgar scores were higher in Group A at 1 minute (median = 9 vs. 8, P = 0.023), 10 minutes (median = 10 vs. 9, P < 0.001), and 20 minutes. Group A neonates also had significantly higher oxygen levels at 1, 3, 5, and 10 minutes post-delivery but there was no statistically significant difference in admission rate. Conclusion: Maternal oxygenation during elective cesarean sections may positively influences neonatal outcomes, as evidenced by improved Apgar scores and higher neonatal oxygen levels but no effect on admission rate.

: Background: Postdural puncture headache (PDPH) commonly arises following a lumbar puncture. It is typically managed through conservative measures or an epidural blood patch (EBP), but each has limitations. This trial aimed to compare the efficacy and safety of pregabalin versus EBP in the management of PDPH. Methods: This randomized controlled trial was conducted on 75 cases with confirmed diagnosis of PDPH. Patients were equally randomized into three groups: Group A (pregabalin group) received oral pregabalin treatment administered at 150 mg daily, with 75 mg given every 12 hours. Group B (EBP group) received EBP and Group C (control group) received conservative treatment. Results: Complete relief of headache was experienced by 13 (52%) patients in group A, 18 (72%) patients in group B, and 5 (20%) patients in group C, with significant distinctions observed among the examined groups (P<0.001). The VAS scores, severity of headache, and frequency of attacks demonstrated a significant improvement at 12h, 24h,36h, and 48h post-treatment within all groups A, B, and C (P<0.001) compared to baseline. Group A was comparable at most time points during follow-up with EBP and better than conservative treatment in terms of VAS scores, severity of headache and frequency of attacks. Conclusions: Pregabalin was comparable at most time points during follow-up with EBP and better than conservative treatment in terms of pain, impact on daily activities and frequency of attacks. Being a non-invasive route, oral pregabalin showed lower back pain than EBP, making it a promising drug for the management of PDPH.
