Introduction : Metabolic associated fatty liver disease (MAFLD), the nomenclature major benefit is the shift towards a diagnosis of inclusion based on the presence of metabolic dysfunction. MAFLD has also been associated with impaired health-related quality of life (HRQOL). Aim : to assess the impact of MAFLD & degree of hepatic steatosis and fibrosis on patients’ HRQOL applying the SF-36 Questionnaire. Methods : This cross sectional study was carried out on 250 adult who were subdivided into 2 groups, Group I (GI) comprised 150 consecutive patients with MAFLD while Group II (GII) comprised 100 apparently healthy subjects. Both studied groups had completed the short form-36 (SF-36) questionnaire. Results : The mean age among cases group was 43.6 ±10.5 years & females were the predominant. There was a highly statistical significance negative correlation between body mass index (BMI) and all domains of SF-36 questionnaire among GI. Scores of all scales of SF-36 were significantly lower in GI compared to GII). Hepatic steatosis grades with in GI according to CAP values were S1(17.3%), S2(37.3%) and S3(45.3). scores of all scales of SF-36 were significantly lower in S3 patients compared to S2 & S1(P value<0.001). Fibrosis stages within GI according to LSM values were F0(44.7%), F1(32.7%), F2(12.7%), F3(6.7%) & F4(3.3%). scores of all scales of SF-36 were significantly lower in F4 patients compared to F3, F2, F1 & F0(P value<0.001). Background: Infraclavicular brachial plexus block is a suitable regional anaesthesia technique for hand, wrist and elbow operations and it is usually performed in conjunction with nerve stimulation. We aimed to compare the onset times of sensory block with equipotential 0.25% bupivacaine and 0.5% bupivacaine. Methods: This prospective double-blinded randomized controlled study was conducted on 60 patients, aged > 18 years, with ASA physical status I–III, and scheduled for upper extremity surgery with infraclavicular brachial plexus block. The participants were randomized into two equal groups; group I in which ICB performed with 0.25%, 20 mL bupivacaine and group II in which ICB performed with 0.5% 20 mL bupivacaine. Results: Regarding the block performance, group II showed earlier onset time and longer duration of sensory and motor blocks compared to group I (P<0.05) and the performance time was significantly shorter in in group II compared to group I (P=0.015). Number of patients required analgesia was significantly lower in group II compared to group I (P=0.003) was significantly lower in group II compared to group I (P=0.003) and the first rescue analgesic requirement was significantly delayed in group II compared to group I (P<0.001). Conclusions: We found that 0.5% bupivacaine showed earlier onset time and longer duration of sensory and motor blocks compared to 0.25% bupivacaine in infraclavicular block. Thus, when a quicker block onset is required, 0.5% bupivacaine is a better choice than 0.25% bupivacaine.

Background: Endoscopic ultrasound elastography (EUS) is an imaging modality that has recently been proposed for the visualization and evaluation of tissue elasticity. This study aimed to assess the role of EUS elastography in the diagnosis of pancreatic mass lesions in comparison to EUS fine needle aspiration (FNA) biopsy and to differentiate between benign and malignant lesions. Methods: This was a cross-sectional study that was conducted on 53 patients are coming to hospital suspected to have pancreatic mass clinically (jaundice, cachexia epigastric pain) or radiological (ultrasound, Computed tomography (CT) and/or Magnetic resonance imaging (MRI) of the abdomen). EUS was done and strain ratio was calculated and fine needle aspiration was done for histopathological examination. Results: Regarding the Pancreatic FL on EUS elastography was hard in 42 (79.25%) patients and soft in 11 (20.75%) patients. Strain ratio of hard Pancreatic FL was significantly higher than soft Pancreatic FL. The type of lesion by histopathological examination, 37(69.81%) patients had adenocarcinoma, 11(20.75%) patients had chronic pancreatitis and 5(9.4%) had neuroendocrinal tumors. EUS-strain ratio can significantly predict the type of Pancreatic FL (malignant or benign) with AUC =0.909, with 100 % sensitivity, 100%specificity, 98%PPV and 93.7%NPV, P value <0.05. There is high concordance between Endoscopic Ultrasound Elastography and biopsy outcomes, as indicated by a Concordance Correlation Coefficient (0.95). Conclusion: FNA biopsy can be replaced by EUS elastography in diagnosis of pancreatic masse and distinction of benign and malignancy

Background: Globally, colorectal cancer poses a significant oncologic challenge, with colonic carcinoma ranking as the third most prevalent cancer worldwide. In Egypt, it represents a noteworthy portion of both male and female cancer cases. The prognosis for colonic carcinoma remains poor due to the lack of early symptoms, emphasizing the need for effective biomarkers to predict prognosis and guide treatment strategies. This study aimed to evaluate SATB2 expression in colonic carcinoma by immunohistochemistry and assess correlation between SATB2 protein and clinicopathological parameters of colonic carcinoma. Methods: This retrospective study was carried out upon 50 cases of colonic adenocarcinoma obtained from the pathology department and early cancer detection unit at Benha Faculty of Medicine during the period of 2017 to 2021. Cases were selected on basis of availability of demographic data (age, sex) and clinicopathological data (Tumor site, tumor size, depth of invasion, lymph node status, grade, TNM stage, lympho-vascular invasion and presence of distant metastasis). Results : There was a highly significant inverse statistical correlation between SATB2 expression and tumor grade, depth of tumor invasion, lymph node metastasis, distant metastasis and tumor stage (P value <0.01). Also, there was a significant inverse statistical correlation between SATB2 expression and presence of lympho-vascular invasion (P value <0.05). Conclusion: SATB2 expression in colonic cancer aligns with key prognostic factors, making it a potential predictor of prognosis. Furthermore, SATB2 expression inversely correlates with tumor aggressiveness, suggesting it may indicate a favorable prognosis, while loss of SATB2 expression may imply a poorer prognosis in colonic cancer.

Background : Garlic is this ancient medicinal plant which has a wide range of uses, including fighting against microbes, viruses, fungi, protozoa, and helminths. Allicin is the active ingredient in garlic and responsible for its antiparasitic effects. Aim of work : the current research was conducted to demonstrate the possible curative and preventative benefits of allicin on both adult and juvenile S. mansoni worms. Methods: For 96 hours, immature and adult S. mansoni worms were cultured with varying doses of allicin (25, 50, 100, and 200 µg/ml). Their motor activity and death rate were evaluated every 24 hours using light microscopy. The ultrastructural effects of allicin on worms were assessed by scanning electron microscopy. The researchers used 40 male Swiss albino mice strain CD1, divided into four groups with 10 animals in each group. Allicin was administered to three groups at varied intervals after they were infected with S. mansoni cercariae. Parasitological studies, including worm burden, oogram pattern, and tissue egg load, were used to assess the efficacy of allicin on day 54 after infection. The results: revealed a high significant difference in S. mansoni immature worms when incubated with allicin at doses of 50, 100, and 200µg/ml. Also incubating adult worms of S. mansoni with allicin at concentrations of 100µg/ml and 200µg/ml were significant. In vivo, there was non-significant impact on worm load with high significant rise in dead egg percentage. Conclusion: These data suggest that allicin is effective against S. mansoni worms in vitro, but with little impact in vivo.

Conclusion: These data suggest that allicin is effective against S. mansoni worms in vitro, but with little impact in vivo.","PeriodicalId":503219,"journal":{"name":"Benha Medical Journal","volume":"105 25","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-07-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141667171","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

: Background: Chronic smoking's impact on ocular health remains an area of concern, particularly its effect on macular perfusion. This study aimed to evaluate the influence of long-term smoking on macular perfusion using Optical Coherence Tomography Angiography (OCTA). Methods : A prospective, randomized clinical trial was conducted at Benha University Hospital's ophthalmology division from January 2020 to December 2021. Forty participants were divided into smoking (Group 1) and control (Group 2) groups. Exclusion criteria included systemic and ocular diseases, refractive errors, retinal vasculopathies, ocular surgeries, or media opacity. OCTA assessed macular perfusion, while other ophthalmic parameters were measured. Results: The smoking group exhibited a higher smoking index (490.00±325.64) and duration (17.30±7.37 years) compared to controls. Despite comparable visual acuity and intraocular pressure between groups, smokers demonstrated significantly lower superficial and deep layer densities across various macular regions (p < 0.001). The foveal avascular zone (FAZ) area was notably larger in smokers (p < 0.001), indicating compromised perfusion. Conclusions: DCP's is more vulnerable to oxidative damage and poor perfusion brought by cigarette smoking, which led to a considerable drop in VD of DCP compared to SCP. In addition, bigger research examining the long-term effect of smoking on retinal microcirculation in the macula may help clarify the significance of smoking as a risk factor for systemic vascular diseases.

: Background: Breast carcinoma is the most common malignant tumor and the leading cause of carcinoma death in women. Aim and objectives: to determine the expression of FOXA1 in breast carcinoma, to correlate the expression of FOXA1 with hormonal receptors (ER and PR), Her2neu, Ki67and pathological response to chemotherapy. Patient and method: This retrospective study was carried on 100 controlled, selected cases included seven cases with bilateral breast cancer, eighty-six cases IDC, five cases ILC, ductal carcinoma insitu (DCIS) was three cases, medullary carcinoma three case, two cases papillary and one case tubular. Fifty-six cases of the total cases with positive axillary lymph nodes metastasis. It was performed on formalin-fixed paraffin-embedded tissue blocks of specimens, collected from Pathology Department, Faculty of Medicine, Benha University, from the years (2013-2019). Result: Concerning the pathological response was found that, FOXA1 was positive in forty-four (44) out of fifty (50) cases with complete pathological response, twenty-seven (27) out of thirty-seven (37) cases with partial pathological response and seven (7) cases out of thirteen (13) cases with no pathological response. While for (ER), (89) cases are positive out of them (74) case FOXA1 positive, and (PR), (80) cases positive, FOXA1 positivity (68) out of them, however Her-2neu negative cases were (48) from them (41) case are FOXA1 positive. For Ki-67, fourteen (14) cases show low scoring, while (31) cases has intermediate score and (55) cases has high scoring, FOXA1 positivity out of them (14, 31, 33 cases respectively). Conclusion: FOXA1 is a marker of good prognosis in breast cancer as FOXA1 expression associated with complete pathological response, positive hormone receptors and low-intermediate Ki67.
